      <title>Caso cl&#237;nico</title>
      <p>Homem de 41 anos procurou o servi&#231;o de urg&#234;ncia por febre ligeira e les&#245;es cut&#226;neas em agravamento desde h&#225; 48 horas. O paciente negou o uso de drogas, alergias conhecidas, hist&#243;rico pessoal ou familiar de doen&#231;a relevante, assim como contexto epidemiol&#243;gico suspeito.</p>
      <p>O paciente apresentou-se febril (38,3&#186;) e a inspe&#231;&#227;o revelou p&#225;pulas e placas eritematosas, pseudovesiculadas e dolorosas na nuca, pesco&#231;o, ombros e bra&#231;os, assim como n&#243;dulos subcut&#226;neos eritematosos e dolorosos (tipo eritema nodoso) nas pernas ( <xref ref-type="fig" rid="f01">Figura 1</xref> ). Restante exame objetivo sem altera&#231;&#245;es. O estudo laboratorial mostrou leucocitose ligeira (10800/uL) com 81,4% de neutr&#243;filos, velocidade de sedimenta&#231;&#227;o 89 mm/h (valor normal (N) &lt;10) e prote&#237;na-C reactiva (PCR) 128,5 mg/L (N&lt;5,0). Os eletr&#243;litos e os testes de fun&#231;&#227;o hep&#225;tica e renal eram normais.</p>

      <p>Poucas horas ap&#243;s a admiss&#227;o, o paciente manifestou desconforto tor&#225;cico ligeiro em repouso. O eletrocardiograma mostrou ritmo sinusal a 58 por minuto com bloqueio atrioventricular do 1&#186; grau e bloqueio incompleto do ramo direito. A troponina I (TnI) foi 1,89ng/mL (N&lt;0,05) e aumentou para 10,82ng/mL ap&#243;s seis horas. O segundo eletrocardiograma mostrou-se sobrepon&#237;vel. O ecocardiograma transtor&#225;cico (ETT) foi normal e demonstrou fra&#231;&#227;o de eje&#231;&#227;o ventricular esquerda preservada (FEVE; 53% pelo m&#233;todo Simpson biplano) sem altera&#231;&#245;es da cin&#233;tica segmentar. Contudo, o strain longitudinal global (SLG) mostrou-se reduzido, especialmente &#224; conta dos segmentos m&#233;dio-basais, com os apicais relativamente poupados ( <xref ref-type="fig" rid="f02">Figura 2.A</xref> ). A angiografia coronariana excluiu doen&#231;a arterial coronariana (DAC) obstrutiva.</p>

      </p>
      <p>O paciente foi internado com o diagn&#243;stico presuntivo de dermatose neutrof&#237;lica aguda febril. No segundo dia (D2), foi realizada bi&#243;psia cut&#226;nea e iniciada prednisolona (PDN) oral 1mg/kg/dia tendo em conta a persist&#234;ncia da febre e das les&#245;es cut&#226;neas assim como o aumento da PCR (242 mg/L). Apesar do al&#237;vio total da dor tor&#225;cica, a TnI subiu para 15,01 ng/mL no D2. Ap&#243;s o in&#237;cio de PDN, o paciente permaneceu apir&#233;tico e os marcadores de inflama&#231;&#227;o e de necrose mioc&#225;rdica regrediram. Testes laboratoriais complementares (proteinograma eletrofor&#233;tico, testes de autoimunidade, horm&#244;nios tireoidianos, hemoculturas, serologias) revelaram-se normais. A an&#225;lise histol&#243;gica da pele mostrou edema subepitelial, infiltrado inflamat&#243;rio na derme com predom&#237;nio polimorfonuclear, e aus&#234;ncia de vasculite ( <xref ref-type="fig" rid="f03">Figura 3</xref> ). Desse modo, os crit&#233;rios para o diagn&#243;stico de SS<xref ref-type="bibr" rid="B3">3</xref> foram satisfeitos e considerou-se elevada probabilidade de comprometimento cardiovascular na forma de miocardite aguda (MA).<xref ref-type="bibr" rid="B3">3</xref> At&#233; D9, o paciente permaneceu apir&#233;tico, as les&#245;es cut&#226;neas quase cicatrizaram, e os n&#237;veis de PCR e TnI diminu&#237;ram (9,9 mg/L e 0,32 ng/dL respectivamente). Recebeu alta hospitalar sob PDN em redu&#231;&#227;o progressiva.</p>

      <title>Discuss&#227;o</title>
      <p>Os autores apresentam um caso no qual o diagn&#243;stico de SS foi estabelecido pela presen&#231;a de 2 crit&#233;rios major e dois minor dos propostos por Driesch.<xref ref-type="bibr" rid="B4">4</xref> O tipo idiop&#225;tico foi assumido devido &#224; aus&#234;ncia de patologia maligna e do uso pr&#233;vio de quaisquer f&#225;rmacos. As manifesta&#231;&#245;es extracut&#226;neas podem ocorrer, particularmente em associa&#231;&#227;o com malignidade.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> O comprometimento cardiovascular &#233; extremamente raro e, &#224; luz do nosso conhecimento, apenas dois casos de miocardite foram reportados na variante idiop&#225;tica.<xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref> Ambas as manifesta&#231;&#245;es tipicamente respondem aos corticosteroides.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>No caso descrito, o desconforto tor&#225;cico transit&#243;rio associado &#224; eleva&#231;&#227;o da TnI levantou a suspeita de comprometimento cardiovascular. Quer a MA como o infarto agudo do mioc&#225;rdio est&#227;o descritos como manisfesta&#231;&#245;es cardiovasculares.<xref ref-type="bibr" rid="B2">2</xref> A angiografia coronariana, que se mant&#233;m como o gold standard para o diagn&#243;stico de DAC<xref ref-type="bibr" rid="B7">7</xref> ou para sua exclus&#227;o na suspeita de MA,<xref ref-type="bibr" rid="B8">8</xref> revelou-se normal. Existe alguma evid&#234;ncia de que a ecocardiografia bidimensional speckle tracking (2D-EST) pode ajudar a suportar o diagn&#243;stico de MA uma vez que o strain longitudinal se correlaciona com a presen&#231;a de fibrose e edema na RMC e de infiltra&#231;&#227;o linfocit&#225;ria na bi&#243;psia endomioc&#225;rdica (BEM).<xref ref-type="bibr" rid="B9">9</xref> - <xref ref-type="bibr" rid="B12">12</xref> No caso relatado, a presen&#231;a de SLG reduzido sobretudo &#224; custa dos segmentos m&#233;dio-basais, ao inv&#233;s dos m&#233;dio-apicais (padr&#227;o t&#237;pico de DAC significativa),<xref ref-type="bibr" rid="B13">13</xref> e a r&#225;pida resposta aos corticosteroides refor&#231;aram a probabilidade de MA. Devido &#224; estabilidade cl&#237;nica e &#224;s conhecidas limita&#231;&#245;es da BEM, foi realizada RMC que sugeriu este diagn&#243;stico de acordo com os crit&#233;rios Lake-Louise.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B12">12</xref> De fato, a RMC emergiu como ferramenta diagn&#243;stica n&#227;o invasiva e h&#225; evid&#234;ncia crescente de que novas t&#233;cnicas, como o T1 e T2 mapping , podem melhorar a acuidade diagn&#243;stica para MA e tamb&#233;m ajudar na monitoriza&#231;&#227;o da doen&#231;a.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref> Adicionalmente, o SLG normalizou tr&#234;s meses ap&#243;s o tratamento, enquanto o paciente permanecia assintom&#225;tico.</p>

      <title>Mensagens finais</title>
      <p>O caso que apresentamos d&#225; &#234;nfase &#224; import&#226;ncia de reconhecer a SS como uma rara mas poss&#237;vel causa de doen&#231;a cardiovascular, entidade que deve ser precocemente identificada de modo a iniciar tratamento adequado.</p>
      <p>No caso presente, o diagn&#243;stico de MA foi altamente sugerido por modalidades imagiol&#243;gicas n&#227;o invasivas ap&#243;s a exclus&#227;o de DAC obstrutiva. &#192; luz do nosso conhecimento, essa foi a primeira vez que a RMC foi usada para avaliar comprometimento mioc&#225;rdico num doente com SS e tamb&#233;m a primeira em que a an&#225;lise por 2D-EST foi usada na monitoriza&#231;&#227;o evolutiva. Ambas as manifesta&#231;&#245;es, cut&#226;neas e cardiovasculares, regrediram completamente ap&#243;s tratamento com corticosteroides.</p>

        <title>Introduction</title>
        <p>Sweet Syndrome is an acute febrile neutrophilic dermatosis characterized by an association of fever, neutrophilia, tender erythematous skin lesions (papules, nodules, and plaques), as well as pathologic findings consisting predominantly of mature neutrophils typically located in the upper dermis.<xref ref-type="bibr" rid="B1">1</xref> It is a rare condition with a worldwide distribution which can present as one of three main clinical types: idiopathic, malignancy-associated, or drug-induced.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B3">3</xref> Extracutaneous manifestations may occur but cardiovascular involvement is extremely rare.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref></p>

        <title>Case presentation</title>
        <p>A previously healthy 41-year-old male presents to the emergency department with a 48-hour history of mild fever and worsening widespread skin lesions. He denied recent drug intake, known allergies, relevant personal or familial diseases, as well as suspicious epidemiological context.</p>
        <p>The patient was febrile (38.3&#186;C) and heart rate, blood pressure, and oxygen saturation were all normal. The chest and abdominal examination were both unremarkable. Skin examination revealed painful pseudovesiculate, erythematous papules, and plaques on the nape, neck, shoulders, and arms, as well as painful hyperpigmented subcutaneous nodules ( erythema nodosum-like ) on the legs ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ). Blood tests showed slight leucocytosis (10800/uL) with 81.4% of neutrophils, erythrocyte sedimentation rate was 89mm/h (normal value (NV) &lt;10) and C-reactive protein (CRP) 128.5mg/L (NV&lt;5.0). Electrolytes, renal and hepatic profiles were normal.</p>
        <p>

        </p>
        <p>A few hours later, the patient complained of transient chest discomfort at rest. The electrocardiogram showed sinus rhythm at 58 per minute with first-degree atrioventricular block plus incomplete right bundle branch block. Troponin I (TnI) was 1.89ng/mL (NV&lt;0.05) and raised up to 10.82ng/mL six hours later. The repeated electrocardiogram was identical to the previous one. Transthoracic echocardiogram (TTE) was normal, demonstrating preserved left ventricular ejection fraction (LVEF; 53% Simpson&#8217;s biplane method) with no major wall motion abnormalities. However, global longitudinal peak systolic strain (GLPSS) was reduced, especially at the expense of the mid-basal segments being the apex relatively spared ( <xref ref-type="fig" rid="f02001">Figure 2.A</xref> ). Coronary angiogram excluded obstructive coronary artery disease (CAD).</p>

        </p>
        <p>The patient was admitted to the internal medicine ward with the presumptive diagnosis of acute febrile neutrophilic dermatosis. On the second day (D2), skin biopsy was performed and oral prednisolone (PDN) 1mg/Kg/day was initiated, taking into consideration the persistence of both fever and skin lesions as well as the increase of the CRP value (242mg/L). Despite no chest discomfort relapse, TnI reached a peak level of 15.01ng/mL on D2. After initiating PDN, the patient remained afebrile, and both systemic inflammatory and myocardial injury biomarkers started to decrease. Complementary laboratorial tests (such as electrophoretic proteinogram, autoimmunity screening, thyroid hormones, blood cultures and serology tests) were normal. The histological skin analysis revealed subepithelial oedema, dermal inflammatory infiltrate with polymorphonuclear predominance and absence of vasculitis ( <xref ref-type="fig" rid="f03001">Figure 3</xref> ). Based on this information, diagnostic criteria were fulfilled for SS<xref ref-type="bibr" rid="B3">3</xref> and a high likelihood for cardiovascular involvement presenting as acute myocarditis (AM)<xref ref-type="bibr" rid="B3">3</xref> was considered. On D9, there had been no fever relapse, skin lesions were mostly healed, and CRP and TnI levels almost normalized (9.9mg/L and 0.32ng/dL respectively). The patient was discharged on a tapering corticosteroid regimen.</p>
        <p>

        </p>
        <p>Four days later, the patient presented completely asymptomatic with no skin lesions and both systemic inflammatory and myocardial injury biomarkers have normalized. A cardiovascular magnetic resonance (CMR) was scheduled six days after discharge and showed findings suggestive of myocarditis ( <xref ref-type="fig" rid="f04001">Figure 4</xref> ). Both LVEF and GLPSS improved up to 63% and -22.4%, respectively, three months after the initial assessment ( <xref ref-type="fig" rid="f02001">Figure 2.B</xref> ). The patient did not wish to undergo a second CMR study.</p>

        <title>Discussion</title>
        <p>We present a case where the diagnosis of SS was established as two major and three minor Driesch criteria were identified.<xref ref-type="bibr" rid="B4">4</xref> The idiopathic type was assumed since no recent drug intake was reported and no signs of malignant disease were present. Extracutaneous manifestations may occur, particularly in association with malignancy.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> Cardiovascular involvement is extremely rare and up to this date, only two cases of myocarditis have been reported in the idiopathic type, to our best knowledge.<xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref> Both manifestations typically respond well to corticosteroids.<xref ref-type="bibr" rid="B1">1</xref></p>
        <p>In this patient, the presence of transient chest discomfort associated with TnI elevation raised the suspicion of cardiovascular involvement. Both AM and acute myocardial infarction have been previously described as cardiovascular manifestations.<xref ref-type="bibr" rid="B2">2</xref> Coronary angiogram remains the gold standard for the diagnosis of CAD<xref ref-type="bibr" rid="B7">7</xref> or for its exclusion in case of suspected AM<xref ref-type="bibr" rid="B8">8</xref> and was normal in this case. There is some evidence that two-dimensional speckle tracking echocardiography (2D-STE) may help support the diagnosis of AM since GLPSS correlates with the presence of fibrosis and oedema on CMR and with lymphocytic infiltrates on endomyocardial biopsy (EMB).<xref ref-type="bibr" rid="B9">9</xref> - <xref ref-type="bibr" rid="B12">12</xref> In our case, the presence of GLPSS reduction mainly at the expense of the mid-basal segments, instead of the mid-apical segments (typical pattern of significant CAD),<xref ref-type="bibr" rid="B13">13</xref> and the prompt response to corticosteroids raised the likelihood of AM. Due to clinical stability and the well-known limitations of EMB, a CMR was later performed and suggested this diagnosis according to the Lake-Louise criteria.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B12">12</xref> In fact, CMR has emerged as a useful non-invasive diagnostic tool and there is growing evidence that novel techniques, such as T1 and T2 mapping, may improve its diagnostic accuracy for myocarditis and help monitor disease evolution.<xref ref-type="bibr" rid="B8">8</xref> , <xref ref-type="bibr" rid="B14">14</xref> , <xref ref-type="bibr" rid="B15">15</xref> Additionally, GLPSS normalized three months after treatment, while the patient remained asymptomatic.</p>

        <title>Final messages</title>
        <p>The case we present emphasizes the importance of acknowledging SS as a rare yet plausible cause of cardiovascular disease, and one that should be early recognized in order to start adequate treatment.</p>
        <p>In this case, the diagnosis of AM was highly suggested by the combination of non-invasive imaging modalities after CAD exclusion. To our best knowledge, this was the first time CMR was used to assess myocardial involvement in a patient with SS and also the first to report the use of 2D-STE for evolution monitoring. Both cutaneous and cardiovascular manifestations completely regressed after corticosteroid treatment.</p>
