          <title>M&#233;todos</title>
          <p> Estudo transversal, retrospectivo e anal&#237;tico. Revisamos prontu&#225;rios m&#233;dicos de pacientes com diabetes mellitus, dislipidemia, e pr&#233;-hipertens&#227;o ou hipertens&#227;o, que realizaram medida de press&#227;o arterial central (PAC) utilizando o Mobil-O-Graph&#174;, e doppler de car&#243;tida ou ecocardiografia tr&#234;s meses antes ou ap&#243;s a medida da PAC. An&#225;lise estat&#237;stica realizada por correla&#231;&#227;o de Pearson ou de Spearman, an&#225;lise de regress&#227;o m&#250;ltipla e de regress&#227;o bivariada, e teste t (independente) ou de Mann-Whitney. Um p&lt;0,05 indicou signific&#226;ncia estat&#237;stica.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> Prontu&#225;rios de 355 pacientes foram avaliados, 56,1 &#177;14,8 anos, 51% homens. A VOP correlacionou-se com espessuras da &#237;ntima m&#233;dia (EIM) das car&#243;tidas (r=0,310) do septo do ventr&#237;culo esquerdo (r=0,191) e da parede posterior do ventr&#237;culo esquerdo (r=0.215), e com di&#226;metro do &#225;trio esquerdo (r=0,181). A EIM associou-se com VOP ajustada por idade e press&#227;o sist&#243;lica perif&#233;rica (p=0,0004); uma EIM maior que 1mm aumentou em 3,94 vezes a chance de se apresentar VOP acima de 10m/s. A VOP foi significativamente maior em indiv&#237;duos com hipertrofia do ventr&#237;culo esquerdo (p=0,0001), EIM &gt; 1 mm (p=0,006), placa de car&#243;tida (p=0,0001), estenose &#8805; 50% (p=0,003), e les&#245;es de &#243;rg&#227;os-alvo (p=0,0001).</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p> A VOP correlacionou-se com a EIM e com par&#226;metros ecocardiogr&#225;ficos, e se associou independentemente com EIM. Essa associa&#231;&#227;o foi mais forte em pacientes com hipertrofia do ventr&#237;culo esquerdo, EIM aumentada, placa de car&#243;tida, estenose &#8805; 50%, e les&#245;es de &#243;rg&#227;os-alvo.       <title>Introdu&#231;&#227;o</title>
      <p>A alta preval&#234;ncia e a elevada mortalidade das doen&#231;as cardiovasculares (DCVs) destacam a urgente necessidade de se implementar ferramentas para melhor estratifica&#231;&#227;o de risco cardiovascular, identificar os pacientes em alto risco, e diagnosticar e tratar precocemente as doen&#231;as. Uma dessas ferramentas s&#227;o os biomarcadores cardiovasculares, os quais conseguem detectar as DCVs em sua fase subcl&#237;nica, com boa acur&#225;cia, melhorando, assim, a preven&#231;&#227;o de eventos e o cen&#225;rio epidemiol&#243;gico.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref></p>
      <p>Alguns dos principais biomarcadores relacionados &#224; estrutura e &#224; fun&#231;&#227;o vascular s&#227;o a espessura da &#237;ntima-m&#233;dia (EIM), presen&#231;a de placas na art&#233;ria car&#243;tida, velocidade de onda de pulso (VOP), e o &#237;ndice tornozelo-braquial (ITB).<xref ref-type="bibr" rid="B2">2</xref> Al&#233;m disso, outros biomarcadores cardiovasculares s&#227;o usados para identificar les&#245;es de &#243;rg&#227;os-alvo (LOA), tais como hipertrofia do ventr&#237;culo esquerdo, n&#237;veis elevados de creatinina s&#233;rica, excre&#231;&#227;o aumentada de albumina, e taxa de filtra&#231;&#227;o glomerular reduzida.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref></p>
      <p>A VOP, um marcador de dano vascular utilizado na avalia&#231;&#227;o de rigidez arterial, &#233; considerada um forte marcador independente de LOA e eventos adversos.<xref ref-type="bibr" rid="B5">5</xref> A VOP tamb&#233;m &#233; um preditor de mortalidade por todas as causas, indicando o risco real do paciente.<xref ref-type="bibr" rid="B6">6</xref> O aumento de um metro por segundo (1m/s) na VOP leva a um aumento de 14% no risco de eventos adversos e de 15% no risco cardiovascular e mortalidade por todas as causas.<xref ref-type="bibr" rid="B6">6</xref> Entre suas vantagens, a VOP &#233; um m&#233;todo n&#227;o invasivo, f&#225;cil, de custo relativamente baixo, e amplamente validado,<xref ref-type="bibr" rid="B2">2</xref> com valores de refer&#234;ncia claramente estabelecidos.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref> Apesar dessas evid&#234;ncias, a VOP continua subutilizada na pr&#225;tica cl&#237;nica, e poucos estudos analisaram sua rela&#231;&#227;o com outros biomarcadores, especialmente utilizando o m&#233;todo oscilom&#233;trico. Assim, o objetivo deste estudo foi investigar a rela&#231;&#227;o entre a VOP e outros biomarcadores das altera&#231;&#245;es estruturais cardiovasculares em pacientes com fatores de risco cardiovascular.</p>

        <p>De setembro de 2012 a mar&#231;o de 2017, foram realizadas 660 medidas da press&#227;o arterial central (PAC). Entre essas avalia&#231;&#245;es, 131 pacientes realizaram o exame duas vezes ou mais, totalizando 169 avalia&#231;&#245;es repetidas. Assim, a popula&#231;&#227;o do estudo foi composta por 491 pacientes que realizaram avalia&#231;&#245;es da PAC.</p>
        <p>O c&#225;lculo da amostra baseou-se em um erro de 5% e n&#237;vel de confian&#231;a de 95%, que indicou um tamanho amostral m&#237;nimo de 216 pacientes. A amostra do estudo consistiu em 355 pacientes brasileiros encaminhados a uma cl&#237;nica de cardiologia para avalia&#231;&#227;o da PAC ( <xref ref-type="fig" rid="f01">Figura 1</xref> ).</p>

Foram aplicados os seguintes crit&#233;rios de exclus&#227;o: idade inferior a 18 anos, aus&#234;ncia dos seguintes diagn&#243;sticos: diabetes mellitus (DM), dislipidemia (DLP), pr&#233;-hipertens&#227;o (PH) ou hipertens&#227;o (HT); aus&#234;ncia de Doppler de car&#243;tida ou de ecocardiografia nos tr&#234;s meses antecedentes ou posteriores &#224; medida de PAC ( <xref ref-type="fig" rid="f01">Figura 1</xref> ).</p>
        <p>Os diagn&#243;sticos de todos os pacientes foram obtidos dos prontu&#225;rios m&#233;dicos e, quando esses n&#227;o estavam dispon&#237;veis, os seguintes crit&#233;rios diagn&#243;sticos foram usados &#8211; glicemia de jejum &gt; 125 mg/dL ou uso de hipoglicemiantes para DM, n&#237;veis de triglicer&#237;deos &gt; 150 mg/dL e de lipoprote&#237;na de baixa densidade (LDL) &gt; 100mg/dL e/ou de lipoprote&#237;na de alta densidade (HDL) &lt; 40 mg/dL e/ou uso atual de estatinas para dislipidemia. Indiv&#237;duos com press&#227;o arterial sist&#243;lica (PAS) perif&#233;rica entre 121 e 139 mmHg e press&#227;o arterial diast&#243;lica (PAD) entre 81 e 89 mmHg, medidas durante a avalia&#231;&#227;o da PAC, foram classificados como pr&#233;-hipertensos, e aqueles com press&#227;o arterial igual ou superior a 140/90 mmHg foram classificados como hipertensos.<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>Dados sobre as seguintes vari&#225;veis foram coletadas dos prontu&#225;rios m&#233;dicos: sexo (feminino ou masculino), tabagismo (sim ou n&#227;o), e estado civil (com ou sem parceiro/a). Al&#233;m dos resultados dos exames de imagem, resultados do Doppler de car&#243;tida e/ou ecocardiografia conduzidos nos tr&#234;s meses antes e ap&#243;s o exame de PAC foram analisados. Quando esses exames eram realizados mais de uma vez nesse per&#237;odo, os resultados do &#250;ltimo teste antes da medida da PAC foram considerados para an&#225;lise.</p>

Ap&#243;s analisar e corrigir as inconsist&#234;ncias, os dados foram exportados para o programa Statistical Package for Social Science (SPSS), vers&#227;o 18.0. O teste de Kolmogorov-Smirnov foi aplicado, e se realizou uma an&#225;lise descritiva dos dados. A an&#225;lise estat&#237;stica foi realizada com base na distribui&#231;&#227;o dos dados, utilizando testes param&#233;tricos e n&#227;o param&#233;tricos. Os dados num&#233;ricos foram descritos como m&#233;dia e desvio padr&#227;o ou m&#233;dia e intervalo interquartil, dependendo da distribui&#231;&#227;o dos dados. As vari&#225;veis categ&#243;ricas foram apresentadas em frequ&#234;ncia absoluta e relativa. O coeficiente de correla&#231;&#227;o de Pearson ou a correla&#231;&#227;o de Spearman foram usados para avaliar a correla&#231;&#227;o da VOP com os resultados obtidos no Doppler de car&#243;tida e ecocardiograma. <title>Discuss&#227;o</title>
      <p>No presente estudo, a VOP correlacionou-se com todos os biomarcadores avaliados, e se associou com EIM, mesmo ap&#243;s ajuste para idade e press&#227;o sist&#243;lica perif&#233;rica. A chance de apresentar VOP acima de 10 m/s foi 3,94 vezes maior na presen&#231;a de EIM maior que 1mm. A VOP apresentou um aumento linear com a presen&#231;a e tamanho da placa de ateroma, e com a presen&#231;a de LOA. Esses resultados est&#227;o de acordo com os de estudos publicados anteriormente,<xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B20">20</xref> , <xref ref-type="bibr" rid="B21">21</xref> e refor&#231;am o valor desse biomarcador e sua capacidade de identificar precocemente les&#245;es cardiovasculares, al&#233;m de seu excelente custo-benef&#237;cio.</p>
      <p>A correla&#231;&#227;o da VOP com par&#226;metros ecocardiogr&#225;ficos encontrada no presente estudo pode ser explicada pelo fato de que a rigidez arterial aumenta a PAS, o que causa um retorno precoce das ondas de pulso na s&#237;stole em vez de na di&#225;stole, e aumento da p&#243;s-carga do ventr&#237;culo esquerdo. Esse aumento de carga imposto sobre o mioc&#225;rdio promove hipertrofia card&#237;aca, e consequente hipertrofia ventricular.<xref ref-type="bibr" rid="B22">22</xref> - <xref ref-type="bibr" rid="B24">24</xref></p>
      <p>A HVE, a qual pode ser identificada pelo aumento na espessura da parede do ventr&#237;culo esquerdo no ecocardiograma, correlaciona-se com a VOP, e valores de VOP s&#227;o significativamente maiores em indiv&#237;duos com HVE.<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> A sobrecarga sobre o ventr&#237;culo esquerdo &#233; uma das principais causas de eventos cardiovasculares relacionados com a PAC.<xref ref-type="bibr" rid="B25">25</xref></p>
      <p>Muitos estudos apresentam n&#227;o s&#243; uma correla&#231;&#227;o<xref ref-type="bibr" rid="B26">26</xref> - <xref ref-type="bibr" rid="B28">28</xref> como tamb&#233;m uma associa&#231;&#227;o entre rigidez arterial e HVE.<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B29">29</xref> - <xref ref-type="bibr" rid="B32">32</xref> Portanto, a rigidez arterial aumentada pode ser usada como preditor de HVE, contribuindo para a preven&#231;&#227;o e diagn&#243;stico dessa condi&#231;&#227;o.<xref ref-type="bibr" rid="B23">23</xref></p>
      <p>Em nosso estudo, n&#227;o observamos uma associa&#231;&#227;o independente da VOP com ESVE, EPPVE ou DAE, possivelmente por n&#227;o termos realizado uma an&#225;lise de associa&#231;&#227;o entre hipertrofia e VOP como nos estudos citados, mas sim entre par&#226;metros ecocardiogr&#225;ficos e VOP. Ainda, em um dos estudos citados,<xref ref-type="bibr" rid="B22">22</xref> foram utilizados achados eletrocardiogr&#225;ficos e n&#227;o resultados ecocardiogr&#225;ficos, e a maioria dos estudos realizou essa an&#225;lise de associa&#231;&#227;o com base no &#237;ndice da massa ventricular esquerda.<xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B28">28</xref> , <xref ref-type="bibr" rid="B30">30</xref> , <xref ref-type="bibr" rid="B32">32</xref></p>
      <p>A rela&#231;&#227;o entre o aumento da rigidez arterial e o aumento da EIM pode ser explicada pela fisiopatologia da rigidez arterial, que engloba mudan&#231;as na matriz extracelular da camada m&#233;dia (t&#250;nica m&#233;dia), incluindo quebra de elastina, dep&#243;sito de col&#225;geno, e reticula&#231;&#227;o.<xref ref-type="bibr" rid="B24">24</xref> , <xref ref-type="bibr" rid="B33">33</xref> Tais altera&#231;&#245;es morfol&#243;gicas tamb&#233;m est&#227;o relacionadas com envelhecimento vascular.<xref ref-type="bibr" rid="B34">34</xref></p>

      <p>A correla&#231;&#227;o<xref ref-type="bibr" rid="B38">38</xref> e a associa&#231;&#227;o da EIM<xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B39">39</xref> com a VOP tamb&#233;m foram previamente relatadas na popula&#231;&#227;o idosa.</p>
      <p>A avalia&#231;&#227;o da EIM e da VOP pode aumentar a reclassifica&#231;&#227;o de risco, e esses biomarcadores podem ser utilizados na identifica&#231;&#227;o de LOA.<xref ref-type="bibr" rid="B40">40</xref> A combina&#231;&#227;o desses biomarcadores aumenta o poder preditivo de eventos cardiovasculares em idosos, fornecendo novas informa&#231;&#245;es cl&#237;nicas importantes.<xref ref-type="bibr" rid="B41">41</xref></p>

      <p>A avalia&#231;&#227;o combinada de EIM e presen&#231;a de placas melhora a predi&#231;&#227;o de risco cardiovascular, e a avalia&#231;&#227;o quantitativa de placas aumenta ainda mais a sensibilidade preditiva.<xref ref-type="bibr" rid="B43">43</xref> Ainda, a VOP na car&#243;tida femoral e o n&#250;mero de placas de ateroma est&#227;o associados de maneira significativa e independente com morte cardiovascular, e pode melhorar a identifica&#231;&#227;o de indiv&#237;duos em alto risco cardiovascular.<xref ref-type="bibr" rid="B44">44</xref></p>
      <p>Al&#233;m das associa&#231;&#245;es entre VOP e biomarcadores, a diferen&#231;a significativa na VOP encontrada entre indiv&#237;duos com e sem LOA destaca a capacidade da VOP em detectar a les&#227;o precocemente. A rigidez arterial &#233; um preditor independente de mortalidade tanto para diab&#233;ticos como para a popula&#231;&#227;o em geral, e est&#225; relacionada com desenvolvimento e progress&#227;o de LOA.<xref ref-type="bibr" rid="B45">45</xref></p>
      <p>A rigidez arterial, avaliada pela VOP, associa-se independentemente com a presen&#231;a de LOA subcl&#237;nica, incluindo calcifica&#231;&#227;o da art&#233;ria coron&#225;ria, &#237;ndice tornozelo-braquial reduzido (doen&#231;a arterial perif&#233;rica), e hiperintensidade da subst&#226;ncia branca (doen&#231;a arterial cerebral).<xref ref-type="bibr" rid="B46">46</xref></p>
      <p>Quando a LOA est&#225; presente, mas n&#227;o &#233; identificada, muitos pacientes s&#227;o erroneamente classificados como em risco baixo a m&#233;dio, quando na verdade est&#227;o em um risco cardiovascular alto.<xref ref-type="bibr" rid="B47">47</xref></p>
      <p>As ferramentas diagn&#243;sticas devem ser aprimoradas e estabelecidas para a identifica&#231;&#227;o precoce de um risco aumentado, para prevenir o in&#237;cio de LOA e suas complica&#231;&#245;es. A identifica&#231;&#227;o apropriada de indiv&#237;duos com baixo risco &#233; igualmente importante para evitar tratamentos desnecess&#225;rios e seus efeitos colaterais.<xref ref-type="bibr" rid="B48">48</xref> O uso de biomarcadores vasculares &#233; um m&#233;todo custo-efetivo, com valor agregado, na melhoria da identifica&#231;&#227;o de indiv&#237;duos em alto risco, facilitando, assim, a preven&#231;&#227;o de DCV.<xref ref-type="bibr" rid="B44">44</xref></p>
      <p>As limita&#231;&#245;es deste estudo incluem: (1) quando o diagn&#243;stico de diabetes mellitus, dislipidemia e hipertens&#227;o arterial n&#227;o estava dispon&#237;vel nos prontu&#225;rios m&#233;dicos, o diagn&#243;stico foi feito durante o estudo, de maneira ad hoc , o que pode ter subestimado ou superestimado as frequ&#234;ncias dessas doen&#231;as. (2) Algumas vari&#225;veis de exposi&#231;&#227;o tamb&#233;m estavam ausentes nos prontu&#225;rios m&#233;dicos. (3) Ainda, n&#227;o podemos assegurar que todos os pacientes foram submetidos ao Doppler e &#224; ecocardiografia no mesmo local e com o mesmo avaliador. Hipertrofia n&#227;o pode ser detectada pelo &#237;ndice da massa ventricular, uma vez que essa informa&#231;&#227;o tamb&#233;m n&#227;o estava dispon&#237;vel nos prontu&#225;rios.</p>
      <p>Os prontu&#225;rios m&#233;dicos e os crit&#233;rios diagn&#243;sticos foram avaliados com rigor cient&#237;fico, e os dados foram revisados por dois pesquisadores, com verifica&#231;&#227;o cruzada. Todos esses procedimentos devem validar nossos achados.</p>
      <p>O presente estudo destaca a import&#226;ncia do uso da VOP para a detec&#231;&#227;o precoce de rigidez arterial e LOA, com foco no aumento da EIM, presen&#231;a de placas de car&#243;tida, e HVE. Em geral, a VOP pode otimizar a estratifica&#231;&#227;o do risco cardiovascular para facilitar a interven&#231;&#227;o precoce e prevenir DCV e suas complica&#231;&#245;es.</p>

    <sec sec-type="conclusions">
      <title>Conclus&#245;es</title>
      <p>A VOP correlacionou-se significativamente com a EIM e com par&#226;metros ecocardiogr&#225;ficos, e se associou com a EIM. Uma EIM maior que 1mm aumentou a chance de se apresentar uma VOP maior que 10m/s em 3,94 vezes. A VOP foi maior nos indiv&#237;duos com HVE, EIM maior que 1mm, com estenose igual ou maior que 50%, e pacientes com LOA.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p>The relationship between pulse wave velocity (PWV) and biomarkers of structural changes of the left ventricle and carotid arteries remains poorly understood.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>To investigate the relationship between PWV and these biomarkers.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>This was an analytical, retrospective, cross-sectional study. Medical records of patients with diabetes mellitus, dyslipidemia, and pre-hypertension or hypertension, who underwent central blood pressure (CBP) measurement using Mobil-O-Graph&#174;, and carotid doppler or echocardiography three months before and after the CBPM were analyzed. Statistical analysis was performed using Pearson or Spearman correlation, linear bivariate and multiple regression analysis, and the t test (independent) or Mann-Whitney test. A p &lt;0.05 indicated statistical significance.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>Medical records of 355 patients were analyzed, mean age 56.1 (&#177;14.8) years, 51% male. PWV was correlated with intima-media thickness (IMT) of carotids (r=0.310) and left ventricular septal thickness (r=0.191), left ventricular posterior wall thickness (r=0.215), and left atrial diameter (r=0.181). IMT was associated with PWV adjusted by age and peripheral systolic pressure (p=0.0004); IMT greater than 1 mm increased the chance of having PWV above 10 m/s by 3.94 times. PWV was significantly higher in individuals with left ventricular hypertrophy (p=0.0001), IMT &gt; 1 mm (p=0.006), carotid plaque (p=0.0001), stenosis &#8805; 50% (p=0.003), and target-organ damage (p=0.0001).</p>
        </sec>
        <sec>
          <title>Conclusion</title>
          <p>PWV was correlated with IMT and echocardiographic parameters, and independently associated with IMT. This association was stronger in individuals with left ventricular hypertrophy, increased IMT, carotid plaque, stenosis &#8805; 50%, and target organ damage. <title>Introduction</title>
        <p>The high prevalence and mortality of cardiovascular diseases (CVD) highlights the urgent need to implement tools to better stratify cardiovascular risks, to identify patients at high risk, and to diagnose and treat CVD in early stages. One of these tools are cardiovascular biomarkers, which can detect CVD in a subclinical phase with good accuracy, thereby improving the prevention of events and the epidemiological scenario.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref></p>
        <p>Some of the main biomarkers related to vascular structure and function are intima-media thickness (IMT), the presence of carotid plaques, pulse wave velocity (PWV), and the ankle-brachial index (ABI).<xref ref-type="bibr" rid="B2">2</xref> In addition, other cardiovascular biomarkers are used to identify target-organ damage (TOD), such as left ventricular hypertrophy, elevated serum creatinine levels, increased albumin excretion, and reduced glomerular filtration rate.<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref></p>
        <p>PWV, a vascular damage biomarker used to assess arterial stiffness, is considered a strong and independent marker of TOD and adverse events.<xref ref-type="bibr" rid="B5">5</xref> PWV is also a predictor of all-cause mortality, indicating the patient&#8217;s actual risk.<xref ref-type="bibr" rid="B6">6</xref> Each one meter per second rise in PWV leads to an increases by 14% in the risk of adverse events and by 15% in the cardiovascular risk and all-cause mortality.<xref ref-type="bibr" rid="B6">6</xref> Among its advantages, PWV is non-invasive, easily performed, relatively inexpensive, and widely validated method<xref ref-type="bibr" rid="B2">2</xref> with clearly established reference values.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref> Despite this evidence, PWV remains underused in clinical practice, and few studies have analyzed its relationship with other biomarkers, specially using oscilometric method. Thus, the objective of this study was to investigate the relationship between PWV and other biomarkers of cardiovascular structural changes in patients with cardiovascular risk factors.</p>

        <sec>
          <title>Participants</title>
          <p>From September 2012 to March 2017, 660 central blood pressure (CBP) measurements were performed. Among these evaluations, 131 patients performed the examination two times or more, for a total of 169 repeated evaluations. Therefore, the study population consisted of 491 patients that underwent CBP measurement to restratify patients considered as low or intermediate cardiovascular risk.</p>
          <p>The sample was calculated considering a 5% error and a 95% confidence level, indicating a minimum sample of 216 patients. Finally, the study sample consisted of 355 Brazilian patients referred to cardiology clinic for CBP measurements ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ).</p>

          <title>Study Design and Procedures</title>
          <p>This analytical, retrospective, cross-sectional study was performed by analysis of medical records and test reports. Data were first collected from the medical records contained in the institutional archives. The following exclusion criteria were applied: age younger than 18 years; absence of the following diagnoses: diabetes mellitus (DM), dyslipidemia (DLP), pre-hypertension (PH) or hypertension (HT); absence of a carotid Doppler or an echocardiography in the three months before and after CBP measurement ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ).</p>
          <p>Then, the diagnoses of all patients were retrieved from the medical records; when the diagnoses were not available, the diagnostic criteria were used &#8211; fasting blood glucose levels &gt; 125 mg/dL or use of hypoglycemic drugs for DM; triglyceride levels &gt; 150 mg/dL and low-density lipoprotein (LDL) &gt; 100 mg/dL and/or high-density lipoprotein (HDL) &lt; 40 mg/dL and/or current use of statins were considered dyslipidemic. Individuals with peripheral systolic blood pressure (SBP) ranging from 121 to 139 mmHg and DBP ranging from 81 to 89 mmHg, obtained during CPB measurement procedures, were classified as pre-hypertensive and those with blood pressure equal to or higher than 140/90 mmHg were classified as hypertensive.<xref ref-type="bibr" rid="B4">4</xref></p>
          <p>Data on the following other variables were collected from the medical records: sex (female or male), tobacco smoking (yes or no) and marital status (with partner or without partner). In addition to the results of imaging tests, results of carotid Doppler and/or echocardiogram studies conducted in the three months before or after the CBP examination were analyzed. When those studies were performed more than once in this period, results of the last test before the CBP measurement was considered for analysis.</p>

        <sec>
          <title>Central Blood Pressure Measurement</title>
          <p>CBP was measured non-invasively using the validated oscilometric Mobil-O-Graph NG (IEM, Stolberg, Germany) with inbuilt ARCSolver algorithm.<xref ref-type="bibr" rid="B10">10</xref> All the CBPM procedures were performed by the same person, always between 1 p.m and 2 p.m. The measurements were made using triple pulse wave analysis and calibration MAD-c2 (mean arterial diastolic blood pressure).<xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B10">10</xref></p>
          <p>Chronological age was calculated as the difference between the date of birth and the date of the CBP measurement. Weight (kg) and height (m) were used for body mass index calculation (using Quetelet formula)<xref ref-type="bibr" rid="B11">11</xref> and its subsequent classification.<xref ref-type="bibr" rid="B12">12</xref> Peripheral SBP (SBPp), peripheral DBP (DBPp), central SBP (SBPc), augmentation index (AIx), and PWV were also analyzed.<xref ref-type="bibr" rid="B13">13</xref> All patients were instructed not to smoke or drink coffee before the test.</p>

          <title>Carotid Doppler and Echocardiogram</title>
          <p>Imaging examinations were performed at different imaging centers, determined by patient&#8217;s choice. Those performed at the cardiology clinic where data collection was performed, were conducted using the Philips HD 11 ultrasound machine.</p>
          <p>The carotid Doppler was performed following the American<xref ref-type="bibr" rid="B14">14</xref> and European<xref ref-type="bibr" rid="B15">15</xref> consensus guidelines, and the highest values obtained from the left and right common carotid arteries were considered for statistical analysis purposes.</p>
          <p>Echocardiographic parameters were assessed by two-dimensional transthoracic echocardiography,<xref ref-type="bibr" rid="B16">16</xref> measuring the left ventricular septal thickness (LVST), the left ventricular posterior wall thickness (LVPWT), and the left atrial diameter (LAD).</p>

          <title>Target-organ Damage</title>
          <p>The identification of TOD was based on the presence of increased IMT,<xref ref-type="bibr" rid="B17">17</xref> atheroma plaques in the carotid Doppler,<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref> left ventricular hypertrophy (LVH) in the echocardiogram,<xref ref-type="bibr" rid="B18">18</xref> and increased arterial stiffness identified by a PWV higher than 10 m/s<xref ref-type="bibr" rid="B3">3</xref> , <xref ref-type="bibr" rid="B4">4</xref> ( <xref ref-type="fig" rid="f02001">Figure 2</xref> ).</p>

          <title>Statistical Analysis</title>
          <p>Data were collected and scanned in duplicate by two researchers, using Epidata software, version 3.1. After assessing and correcting inconsistencies, the data were exported to the Statistical Package for Social Science (SPSS), version 18.0. The Kolmogorov-Smirnov test was applied, and a descriptive data analysis was performed. Statistical analysis was performed based on data distribution, using parametric and non-parametrical tests. Numeric data were described as mean and standard deviation or median and interquartile range, depending on data distribution. Categorical variables were presented with absolute and relative frequencies. The Pearson product-moment correlation or Spearman&#8217;s rank-order correlation were used to assess the correlation of PWV with the results of the carotid Doppler and the echocardiogram. Correlations were classified in weak (0 &lt; r &lt;0.30), moderate (0.30 &#8804; r &lt; 0.60), strong (0.60 &#8804; r &lt; 0.90) and very strong (0.90 &#8804; r &lt; 1).<xref ref-type="bibr" rid="B19">19</xref></p>
          <p>The association between PWV and the other biomarkers (IMT, LVST, LVPWT, LAD) was assessed by linear bivariate regression analysis and those variables with p&lt;0,020 were used in multiple regression analysis. All assumptions were met for the application of linear regression analysis. PWV was compared by IMT size, with the presence or not of LVH, with the presence or not of plaque, with plaque size, and with the presence or not of TOD using the t test for independent samples or Mann-Whitney test. Values of p&lt;0.05 were considered statistically significant.</p>

        <title>Discussion</title>
        <p>In the present study, PWV was correlated with all biomarkers evaluated, and associated with IMT even when adjusted for age and peripheral systolic pressure. The chance of having PWV above 10 m/s increases by 3.94 times in the presence of IMT greater than 1 mm. PWV have had a linear increment with the presence and size of atheroma plaque and with the presence of TOD. These findings are in accordance with previously published studies,<xref ref-type="bibr" rid="B2">2</xref> , <xref ref-type="bibr" rid="B20">20</xref> , <xref ref-type="bibr" rid="B21">21</xref> and reinforce the value of this biomarker and its ability to identify early cardiovascular damage, in addition to its excellent cost-effectiveness.</p>
        <p>The correlation of PWV with echocardiographic parameters observed in the present study may be explained by the fact that arterial stiffness increases the SBP, and, consequently, the reflected wave returns early and arrives in systole instead of diastole, increasing the post-load of the left ventricle. This increased workload imposed on the myocardium promotes cardiac myocyte hypertrophy, resulting in ventricular hypertrophy.<xref ref-type="bibr" rid="B22">22</xref> - <xref ref-type="bibr" rid="B24">24</xref></p>
        <p>LVH, which may be identified by an increase in left ventricular wall thickness on echocardiogram, is correlated with PWV, and PWV values are significantly higher in individuals with LVH.<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> The increase in the load imposed on the left ventricle is one of the main causes of cardiovascular events related to CBP.<xref ref-type="bibr" rid="B25">25</xref></p>
        <p>Many studies show not only a correlation<xref ref-type="bibr" rid="B26">26</xref> - <xref ref-type="bibr" rid="B28">28</xref> but also an association between arterial stiffness and LVH.<xref ref-type="bibr" rid="B22">22</xref> , <xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B29">29</xref> - <xref ref-type="bibr" rid="B32">32</xref> Therefore, increased arterial stiffness may be used as a predictor of LVH, contributing to the prevention and diagnosis of this condition.<xref ref-type="bibr" rid="B23">23</xref></p>
        <p>In our study, PWV was not independently associated with LVST, LVPWT, or LAD, perhaps because the association analysis was not performed between hypertrophy itself and PWV, as in the studies cited, but rather between echocardiographic parameters and PWV. Furthermore, one of the studies cited<xref ref-type="bibr" rid="B22">22</xref> used electrocardiographic and not echocardiographic findings, and most studies performed this association analysis based on the left ventricular mass index.<xref ref-type="bibr" rid="B23">23</xref> , <xref ref-type="bibr" rid="B28">28</xref> , <xref ref-type="bibr" rid="B30">30</xref> , <xref ref-type="bibr" rid="B32">32</xref></p>
        <p>The relationship between the increases in arterial stiffness and the increases in IMT can be explained by the pathophysiology of arterial stiffness, which encompasses changes in the extracellular matrix of the middle layer (tunica media), including elastin breakdown, collagen deposition, and reticulation.<xref ref-type="bibr" rid="B24">24</xref> , <xref ref-type="bibr" rid="B33">33</xref> Those morphological changes are also related to vascular aging.<xref ref-type="bibr" rid="B34">34</xref></p>
        <p>Increased IMT is also associated with the presence of risk factors for arteriosclerosis; and age, arterial blood pressure, serum lipids, and fasting blood glucose levels are all independent predictors of carotid atherosclerosis.<xref ref-type="bibr" rid="B35">35</xref> Increased IMT is one of the first subclinical manifestations of arteriosclerosis.<xref ref-type="bibr" rid="B36">36</xref> The presence of multiple cardiovascular risk factors is independently associated with increases in IMT and decreases in arterial compliance.<xref ref-type="bibr" rid="B37">37</xref></p>
        <p>The correlation<xref ref-type="bibr" rid="B38">38</xref> and association of IMT with PWV was also previously reported in an elderly population.<xref ref-type="bibr" rid="B38">38</xref> , <xref ref-type="bibr" rid="B39">39</xref></p>
        <p>The assessment of IMT and PWV can improve cardiovascular risk reclassification, and these biomarkers may be used to identify subclinical TOD.<xref ref-type="bibr" rid="B40">40</xref> The combination of these biomarkers increases the predictive power of cardiovascular events among elderly people, providing additional and important clinical information.<xref ref-type="bibr" rid="B41">41</xref></p>
        <p>Significantly higher PWV values were also identified in individuals with stenosis equal to or greater than 50% in the present study. Higher PWV values were also significantly associated with the presence of carotid plaques.<xref ref-type="bibr" rid="B36">36</xref> Furthermore, decreased carotid elasticity is associated with the presence of plaques and the risk of stroke.<xref ref-type="bibr" rid="B42">42</xref></p>
        <p>The combined assessment of IMT and the presence of plaques improves the prediction of cardiovascular risk, and the quantitative evaluation of plaques further increases the predictive sensitivity.<xref ref-type="bibr" rid="B43">43</xref> Moreover, femoral carotid PWV and the number of atheroma plaques are significantly and independently associated with cardiovascular death and can improve the identification of individuals at high cardiovascular risk.<xref ref-type="bibr" rid="B44">44</xref></p>
        <p>In addition to the associations between PWV and biomarkers, the significant difference in PWV found between study subjects with and without TOD highlights the capacity of PWV to early identify the damage. Arterial stiffness is an independent predictor of mortality for both diabetics and the general population, and it is related to TOD development and progression.<xref ref-type="bibr" rid="B45">45</xref></p>
        <p>Arterial stiffness, assessed by PWV, is independently associated with the presence of subclinical TOD, including coronary artery calcification, reduced ankle-brachial index (peripheral arterial disease), and white matter hyperintensity (cerebral arterial disease).<xref ref-type="bibr" rid="B46">46</xref></p>
        <p>When TOD is present and is not identified, many patients are wrongly classified as medium-to-low risk when they are actually at high cardiovascular risk.<xref ref-type="bibr" rid="B47">47</xref></p>
        <p>Diagnostic tools must be improved and established for the early identification of increased risk, to prevent the onset of TOD and its complications. The appropriate identification of low-risk individuals is equally as important to avoid unnecessary treatments and their concomitant side effects.<xref ref-type="bibr" rid="B48">48</xref> The use of vascular biomarkers is a cost-effective, value-added method of improving the identification of individuals at high cardiovascular risk, thereby facilitating the prevention of CVD.<xref ref-type="bibr" rid="B44">44</xref></p>
        <p>The limitations of this study are as follows: when diagnosis of diabetes mellitus, dyslipidemia, and arterial hypertension was not available in the medical records, the diagnosis was made during the study, on an ad hoc basis, which may have under- or overestimated the frequencies of those diseases. Some exposure variables were missing from the medical records. In addition, we cannot ensure that all patients underwent carotid Doppler and echocardiography at the same clinic and with the same evaluator. Hypertrophy could not be detected by the ventricular mass index, since this information was not available in the medical records.</p>
        <p>The medical records and diagnostic criteria were reviewed with scientific rigor, and the data were reviewed by two researchers and subsequently crosschecked. All these procedures should suffice to validate the findings.</p>
        <p>The present study highlights the importance of using PWV for the early detection of arterial stiffness and TOD, focusing on increases in IMT, the presence of carotid plaques, and LVH. Overall, the use of PWV may optimize the stratification of cardiovascular risk to facilitate early intervention and prevent CVD and its complications.</p>

        <title>Conclusions</title>
        <p>PWV was significantly correlated with IMT and echocardiographic parameters and was associated with IMT. Furthermore, an IMT greater than 1 mm increased the chance of having PWV above 10 m/s by 3.94 times. PWV was significantly higher in individuals with LVH, IMT greater than 1 mm, in those with stenosis equal to or greater than 50%, and in those with TOD.</p>
