  <body>
    <p>A insufici&#234;ncia card&#237;aca (IC) constitui problema cl&#237;nico de extrema import&#226;ncia devido &#224; sua alta preval&#234;ncia e &#224; gravidade das manifesta&#231;&#245;es cl&#237;nicas.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> O aumento da longevidade da popula&#231;&#227;o tem elevado o n&#250;mero de hospitaliza&#231;&#245;es e &#243;bitos por IC. &#218;ltimos dados divulgados pelo DATASUS (2020) mostram que ocorrem 26.482 mortes por IC.<xref ref-type="bibr" rid="B3">3</xref></p>
    <p>A caracter&#237;stica multifatorial da s&#237;ndrome exige que a escolha do tratamento seja realizada por equipe multiprofissional e atenta para as recomenda&#231;&#245;es propostas nas diretrizes de tratamento da IC. O acompanhamento e monitoriza&#231;&#227;o dos pacientes s&#227;o essenciais para a melhora dos desfechos cl&#237;nicos.<xref ref-type="bibr" rid="B4">4</xref> - <xref ref-type="bibr" rid="B6">6</xref></p>
    <p>O controle da frequ&#234;ncia card&#237;aca (FC) &#233; considerado alvo terap&#234;utico no tratamento da IC, visto que FC elevada &#233; marcador de eventos em IC.<xref ref-type="bibr" rid="B4">4</xref> Diversos estudos evidenciaram a import&#226;ncia do controle da FC para melhorar os desfechos como risco de morte e admiss&#227;o hospitalar por IC.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref></p>
    <p>Portanto, estudos observacionais que monitoram pacientes com IC s&#227;o fundamentais para apresentar um panorama da efetividade dos tratamentos na IC. Nesse sentido, estudo realizado por Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> teve o objetivo de avaliar se os pacientes acompanhados em ambulat&#243;rio de cardiologia tem sua FC controlada e recebem prescri&#231;&#227;o de medica&#231;&#227;o para IC adequada segundo as diretrizes.<xref ref-type="bibr" rid="B9">9</xref></p>
    <p>Em um recente estudo global foi observado que, apesar de satisfat&#243;ria ader&#234;ncia &#224;s recomenda&#231;&#245;es das diretrizes de tratamento da IC, &#233; observada subdosagem dos medicamentos recomendados. S&#227;o necess&#225;rias a&#231;&#245;es para melhorar essa situa&#231;&#227;o, pois altas doses das terapias recomendadas s&#227;o associadas &#224; redu&#231;&#227;o da mortalidade em estudos observacionais de IC.<xref ref-type="bibr" rid="B10">10</xref></p>
    <p>Em seu estudo, Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> ressaltam que a redu&#231;&#227;o da FC com os betabloqueadores n&#227;o &#233; igual para todos os pacientes. Levantando, dessa forma, o importante questionamento do que seria mais importante para determinar uma boa evolu&#231;&#227;o dos pacientes com IC, a dose alvo do betabloqueador ou a redu&#231;&#227;o da FC?</p>
    <p>O estudo avaliou 171 pacientes e mostraram dados muito relevantes sobre a efic&#225;cia do tratamento da IC sobre o controle da FC. Apesar de o betabloqueador ter sido prescrito em doses elevadas para 98,8% dos pacientes, uma grande parcela (40,9%) apresentou FC maior que 70 batimentos por minuto. Os autores observaram tamb&#233;m que outras classes de medicamentos recomendados para tratamento da IC, como inibidores da enzima conversora de angiotensina, bloqueadores do receptor da angiotensina e diur&#233;ticos, estavam sendo prescritos adequadamente.<xref ref-type="bibr" rid="B9">9</xref></p>
    <p>Dessa forma, considerando o impacto da FC no progn&#243;stico de pacientes com IC, o estudo conduzido por Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> sugere que outras medidas precisam ser adotadas, al&#233;m da prescri&#231;&#227;o das doses alvo dos medicamentos indicados pelas diretrizes, para manter o controle da FC em pacientes com IC.</p>

      <p>Heart failure (HF) is an important public health issue due to its high prevalence and the severity of clinical manifestations.<xref ref-type="bibr" rid="B1">1</xref> , <xref ref-type="bibr" rid="B2">2</xref> The increment in population longevity has increased the number of hospitalizations and deaths due to HF. Recent data by DATASUS (2020) show the occurrence of 26,482 deaths from HF.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>This syndrome has a multifactorial characteristic and its treatment requires a multidisciplinary team to meet the recommendations proposed in the guidelines for the treatment of HF. Follow-up and monitoring of patients are essential for improving the clinical outcomes.<xref ref-type="bibr" rid="B4">4</xref> - <xref ref-type="bibr" rid="B6">6</xref></p>
      <p>Heart rate (HR) is considered a therapeutic target in the treatment of HF, since elevated HR is a marker of events in HF.<xref ref-type="bibr" rid="B4">4</xref> Several studies have shown the importance of HR control to improve outcomes such as risk of death and hospital admission by HF.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref></p>
      <p>Therefore, observational studies are essential to investigate the impact of the treatment of patients with HF. In this sense, a study carried out by Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> aimed to assess whether patients monitored in outpatient settings have their HR controlled and receive drug prescriptions according to the guidelines for the treatment of HF.<xref ref-type="bibr" rid="B9">9</xref></p>
      <p>A recent global study showed that despite satisfactory adherence to the recommendations of guidelines for the treatment of HF, an under-dosage of the recommended drugs is observed. Actions are needed to improve this situation, since high doses of recommended therapies are associated with reduced mortality in observational studies of patients with HF.<xref ref-type="bibr" rid="B10">10</xref></p>
      <p>In their study, Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> mentioned that beta-blockers do not have the same effect reducing HR for all patients. Thus, the authors raised an important question of what would be more important to determine a good outcome for patients with HF, the target dose of the beta-blocker or the reduction in HR?</p>
      <p>The study evaluated 171 patients in outpatient settings and showed very relevant data on the effectiveness of the treatment of HF on HR control. Although beta-blockers were prescribed in high doses for 98.8% of patients, a large portion (40.9%) had HR greater than 70 beats per minute. The authors also observed that other classes of drugs recommended for the treatment of HF, such as angiotensin-converting enzyme inhibitors, angiotensin receptor blockers and diuretics, were properly prescribed.<xref ref-type="bibr" rid="B9">9</xref></p>
      <p>Thus, considering the impact of HR on the prognosis of patients with HF, the study conducted by Cardoso et al.<xref ref-type="bibr" rid="B9">9</xref> suggests that other actions in addition to the prescription of the recommended drug therapy need to be adopted to ensure the control of HR in patients with HF.</p>
