      Caro Editor,
    </p>
    <p>Em dezembro de 2019, a China reportou &#224; Organiza&#231;&#227;o Mundial da Sa&#250;de (OMS) um surto de pneumonia na cidade de Wuhan, capital da prov&#237;ncia de Hubei. Poucos dias depois, o agente causador foi identificado, o severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>A doen&#231;a, batizada de coronavirus disease 2019 (COVID-19), rapidamente se espalhou pelos pa&#237;ses e, em 11 de mar&#231;o de 2020, a OMS declarou pandemia global.<xref ref-type="bibr" rid="B1">1</xref> No Brasil, o primeiro caso foi confirmado em 26 de fevereiro e a primeira morte em 17 de mar&#231;o, em S&#227;o Paulo. Em 09 de abril, o Brasil somava 155.000 casos confirmados e 10.000 mortes pela doen&#231;a.<xref ref-type="bibr" rid="B2">2</xref></p>
    <p>Os recentes estudos j&#225; apontam a dupla rela&#231;&#227;o entre o aparelho circulat&#243;rio e a COVID-19:<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref> i. o v&#237;rus pode causar altera&#231;&#245;es cardiovasculares, como arritmias, les&#227;o card&#237;aca aguda, miocardite, dentre outras; ii. a presen&#231;a de doen&#231;as do aparelho circulat&#243;rio eleva o risco de agravamento e mortalidade pela COVID-19. Essa rela&#231;&#227;o tem sido motivo de preocupa&#231;&#227;o por parte de cl&#237;nicos e cientistas. Partindo deste pressuposto, este estudo objetivou descrever o perfil cl&#237;nico e epidemiol&#243;gico de &#243;bitos por COVID-19 que tinham doen&#231;as do aparelho circulat&#243;rio previamente.</p>
    <p>Trata-se de um estudo observacional transversal envolvendo 197 &#243;bitos por COVID-19 ocorridos em Pernambuco que tinham pelo menos uma doen&#231;a do aparelho circulat&#243;rio previamente. Foram analisadas as seguintes vari&#225;veis: sexo, faixa et&#225;ria, sinais/sintomas, comorbidades e fatores risco e tempo entre os primeiros sintomas e o &#243;bito. Os dados foram obtidos da p&#225;gina eletr&#244;nica de monitoramento da COVID-19 do estado (https://dados.seplag.pe.gov.br/apps/corona.html) em 07 de maio de 2020. Ap&#243;s a coleta, o banco de dados passou por ajustes das vari&#225;veis para a subsequente an&#225;lise. Neste estudo, foi utilizada apenas a estat&#237;stica descritiva (frequ&#234;ncia absoluta, frequ&#234;ncia relativa, m&#233;dia e desvio padr&#227;o) com o aux&#237;lio do software SPSS vers&#227;o 24.0 (IBM Corporation). Por utilizar dados de dom&#237;nio p&#250;blico, este estudo dispensou a aprova&#231;&#227;o pelo Comit&#234; de &#201;tica em Pesquisa.</p>
    <p>Em 07 de maio de 2020, o estado de Pernambuco j&#225; havia registrado 9.325 casos e 749 &#243;bitos em decorr&#234;ncia da COVID-19. Desses &#243;bitos, 293 (39,11%) possu&#237;am o campo &#8220; comorbidades &#8221; preenchido (12 informavam que os pacientes n&#227;o possu&#237;am comorbidades e 281 citando as comorbidades). Dos 281 com comorbidades relatadas, 197 apresentavam pelo menos uma doen&#231;a do aparelho circulat&#243;rio, o que correspondeu a 70,10% dos indiv&#237;duos com comorbidades relatadas e a 26,30% de todos os &#243;bitos.</p>
    <p>Observou-se predom&#237;nio do sexo feminino (53,3%; n = 105) e de indiv&#237;duos com 50 anos ou mais (92,3%; n = 182). Os seguintes quatro sinais/sintomas apresentaram frequ&#234;ncia superior a 50%: dispneia (80,7%; n = 159), tosse (72,1%; n = 142), febre (67,0%; n = 132) e satura&#231;&#227;o de oxig&#234;nio &lt; 95% (58,9%; n = 116) ( <xref ref-type="table" rid="t1">Tabela 1</xref> ).</p>

    <p>Quanto &#224; presen&#231;a de comorbidades, 78,7% (n = 155) apresentavam duas ou mais comorbidades, sendo pelo menos uma relacionada ao aparelho circulat&#243;rio. Dentre elas, a hipertens&#227;o arterial sist&#234;mica foi observada em 82,7% (n = 163) dos indiv&#237;duos e a cardiopatia n&#227;o especificada em 25,9% (n = 51) dos indiv&#237;duos ( <xref ref-type="table" rid="t1">Tabela 1</xref> ).</p>
    <p>Al&#233;m do comprometimento do aparelho circulat&#243;rio, as doen&#231;as e fatores de risco mais comuns na popula&#231;&#227;o estudada foram diabetes mellitus (53,8%; n = 106), obesidade (11,2%; n = 22), doen&#231;a renal cr&#244;nica (10,7%; n = 21), acidente vascular cerebral pr&#233;vio (8,1%; n = 16), tabagismo (7,6%; n = 15), doen&#231;a pulmonar obstrutiva cr&#244;nica (4,6%; n = 9) e c&#226;ncer (4,1%; n = 8). O tempo m&#233;dio (em dias) entre o in&#237;cio dos sintomas e o &#243;bito foi de 9,7 &#177; 7,8. Salienta-se que dos 197 pacientes inclu&#237;dos neste estudo, em 10 deles n&#227;o foi poss&#237;vel calcular o tempo entre o in&#237;cio dos primeiros sintomas e o &#243;bito ( <xref ref-type="table" rid="t1">Tabela 1</xref> ).</p>
    <p>O perfil evidenciado neste estudo est&#225; em conson&#226;ncia com o observado em outras partes do mundo.<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref> No entanto, tr&#234;s aspectos chamam a aten&#231;&#227;o: i. a elevada propor&#231;&#227;o de indiv&#237;duos com m&#250;ltiplas comorbidades (78,7% dos pacientes possu&#237;am duas ou mais doen&#231;as/fatores de risco de base), ii. a ampla variedade de doen&#231;as/fatores de risco observado e iii. o estado cl&#237;nico em que indiv&#237;duos chegaram ao atendimento hospitalar (fun&#231;&#227;o respirat&#243;ria comprometida).</p>
    <p>O somat&#243;rio de comorbidades/fatores de risco em uma &#250;nica pessoa pode elevar o risco de mortalidade pela COVID-19, embora ainda n&#227;o existam estimativas precisas destes riscos. Em estudo conduzido em um hospital na cidade de Wuhan, China, envolvendo 416 pacientes internados com COVID-19, 44 (10,6%) e 22 (5,3%) tinham doen&#231;a card&#237;aca coronariana e doen&#231;a cerebrovascular, respectivamente. Outras comorbidades tamb&#233;m foram observadas, como insufici&#234;ncia card&#237;aca cr&#244;nica (4,1%; n = 17), insufici&#234;ncia renal cr&#244;nica (3,4%; n = 14), doen&#231;a pulmonar obstrutiva cr&#244;nica (2,9%; n = 12) e c&#226;ncer (2,2%; n = 9), assim como foi observado em nossa investiga&#231;&#227;o. A mortalidade foi maior nos indiv&#237;duos com inj&#250;ria card&#237;aca (51,2% no grupo com inj&#250;ria vs. 4,5% no grupo sem inj&#250;ria) e doen&#231;as preexistentes foram fatores associados &#224; maior mortalidade.<xref ref-type="bibr" rid="B4">4</xref></p>
    <p>A associa&#231;&#227;o entre v&#225;rias comorbidade/fatores de risco pode explicar o quadro de comprometimento respirat&#243;rio no momento da admiss&#227;o, com dispneia e satura&#231;&#227;o de oxig&#234;nio &lt; 95%, o que indica grave comprometimento pulmonar desses pacientes. A &#237;ntima rela&#231;&#227;o funcional entre os sistemas cardiovascular (duplamente comprometido pela doen&#231;a de base e pela infec&#231;&#227;o por SARS-CoV-2) e pulmonar (inj&#250;ria pulmonar acentuada) deve ser valorizada no processo de cuidado dos pacientes com COVID-19 que apresentam doen&#231;as do aparelho circulat&#243;rio.<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref></p>
    <p>Por fim, destacamos a necessidade de adotar e/ou fortalecer mecanismos que reduzam a contamina&#231;&#227;o de indiv&#237;duos com doen&#231;as do aparelho circulat&#243;rio pela COVID-19. Para aqueles j&#225; contaminados, o diagn&#243;stico precoce e o monitoramento do quadro cl&#237;nico devem ser rigorosamente observados, de modo a evitar o agravamento e a morte desses indiv&#237;duos.</p>

        Dear Editor,
      </p>
      <p>On December 31, 2019, China notified the World Health Organization (WHO) of an outbreak of pneumonia in the city of Wuhan, capital of the Hubei province. A few days later, the causative agent, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was identified.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>The disease, named coronavirus disease 2019 (COVID-19), rapidly spread throughout countries worldwide, and, on March 11, 2020, the WHO declared a global pandemic.<xref ref-type="bibr" rid="B1">1</xref> In Brazil, the first case was confirmed on February 26, and the first death occurred on March 17, in S&#227;o Paulo. On April 9, Brazil had a total of 155,000 confirmed cases and 10,000 deaths from the disease.<xref ref-type="bibr" rid="B2">2</xref></p>
      <p>Recent studies have already indicated the following double relationship between the circulatory system and COVID-19:<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref> i. The virus may cause cardiovascular changes, such as arrhythmias, acute cardiac injury, myocarditis, and others, and ii. The presence of circulatory system diseases increases the risk of worsening and mortality due to the disease. This relationship has been cause for concern on the part of clinical physicians and scientists. Based on this assumption, this study aimed to describe the clinical and epidemiological profile of deaths due to COVID-19 in patients who had prior circulatory system diseases.</p>
      <p>This is a cross-sectional observational study involving 197 patients who died from COVID-19 in Pernambuco, all of whom had at least one prior circulatory system disease. The following were analyzed: sex, age range, signs/symptoms, comorbidities and risk factors, and time from first symptoms to death. Data were obtained from the state&#8217;s COVID-19 monitoring webpage (https://dados.seplag.pe.gov.br/apps/corona.html) on May 7, 2020. After collection, the database underwent adjustment of variables for subsequent analysis. In this study, only descriptive statistics (absolute frequency, relative frequency, mean, and standard deviation) were used, with the assistance of SPSS software, version 24.0 (IBM Corporation). Given that this study used public domain data, Research Ethics Committee approval was waived.</p>
      <p>On May 7, 2020, the state of Pernambuco had registered 9,325 cases and 749 deaths due to COVID-19. Of these deaths, 293 (39.11%) had the field &#8220;comorbidities&#8221; filled out (12 stated that the patients had no comorbidities, and 281 cited the comorbidities). Of the 281 comorbidities reported, 197 presented at least one circulatory system disease, accounting for 70.10% of individuals with reported comorbidities and 26.30% of all deaths.</p>
      <p>Predominance was observed in women (53.3%; n = 105) and patients 50 years or older (92.3%; n = 182). The following four signs/symptoms had frequency above 50%: dyspnea (80.7%; n = 159), cough (72.1%; n = 142), fever (67.0%; n = 132), and oxygen saturation &lt; 95% (58.9%; n = 116) ( <xref ref-type="table" rid="t1001">Table 1</xref> ).</p>

      </p>
      <p>With respect to the presence of comorbidities, 78.7% (n = 155) had two or more, at least one of which was related to the circulatory system. Systemic arterial hypertension was observed in 82.7% (n = 163) of individuals, and non-specified heart disease was observed in 25.9% (n = 51) ( <xref ref-type="table" rid="t1001">Table 1</xref> ).</p>
      <p>In addition to involvement of the circulatory system, the most common diseases and risk factors in the study population were diabetes mellitus (53.8%; n = 106), obesity (11.2%; n = 22), chronic renal disease (10.7%; n = 21), prior stroke (8.1%; n = 16), tobacco use (7.6%; n = 15), chronic obstructive pulmonary disease (4.6%; n = 9), and cancer (4.1%; n = 8). Average time from onset of symptoms to death was 9.7 &#177; 7.8 days. It is worth underscoring that, for 10 of the 197 patients included in this study, it was not possible to calculate time from onset of symptoms to death ( <xref ref-type="table" rid="t1001">Table 1</xref> ).</p>
      <p>The profile shown in this study is in agreement with that observed in other parts of the world.<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref> Nonetheless, the following three aspects warrant attention: i. the elevated proportion of individuals with multiple comorbidities (78.7% had two or more diseases/baseline risk factors), ii. the wide variety of diseases/risk factors observed, and iii. the clinical status in which individuals arrived at the hospital (compromised respiratory function).</p>
      <p>The sum of comorbidities/risk factors in a single person may elevate the risk of mortality from COVID-19, although there are still not any precise estimates of these risks. In a study involving 416 hospitalized patients with COVID-19, carried out in a hospital in Wuhan, China, 44 (10.6%) and 22 (5.3%) had coronary heart disease and cerebrovascular disease, respectively. The also observed other comorbidities, such as chronic heart failure (4.1%; n = 17), chronic renal failure (3.4%; n = 14), chronic obstructive pulmonary disease (2.9%; n = 12), and cancer (2.2%; n = 9), which were also observed in our study. Mortality was higher in individuals with heart injury (51.2% in the group with injury vs. 4.5% in the group without injury), and preexisting diseases were factors associated with higher mortality.<xref ref-type="bibr" rid="B4">4</xref></p>
      <p>The association between several comorbidities/risk factors may explain the compromised respiratory condition upon admission, with dyspnea and oxygen saturation &lt; 95%, which indicates severely compromised pulmonary function in these patients. The close functional relationship between the cardiovascular (doubly affected by the baseline disease and by SARS-CoV-2 infection) and pulmonary systems (accentuated pulmonary injury) should be prioritized during the process of caring for patients with COVID-19 who have circulatory system disease.<xref ref-type="bibr" rid="B3">3</xref> - <xref ref-type="bibr" rid="B5">5</xref></p>
      <p>Finally, we underline the need to adopt and/or strengthen mechanisms that reduce contamination of individuals with circulatory system diseases by COVID-19. For those who are already contaminated, early diagnosis and monitoring of clinical picture should be rigorously followed, in order to avoid worsening and death in these individuals.</p>
