  <body>
    <p>A cardiomiopatia hipertr&#243;fica (CMH) &#233; um dist&#250;rbio gen&#233;tico caracterizada pela hipertrofia inexplicada do ventr&#237;culo esquerdo, comumente assim&#233;trico com maiores espessamentos do septo interventricular basal. A obstru&#231;&#227;o da via de sa&#237;da do ventr&#237;culo esquerdo est&#225; presente em repouso em cerca de um ter&#231;o dos pacientes e pode ser provocada em outro ter&#231;o. As caracter&#237;sticas histol&#243;gicas da CMH incluem hipertrofia e desordem dos mi&#243;citos, al&#233;m de fibrose intersticial. A hipertrofia tamb&#233;m &#233; frequentemente associada &#224; disfun&#231;&#227;o diast&#243;lica do ventr&#237;culo esquerdo.</p>
    <p>O primeiro caso de CMH foi descrito por Henri Liouville em 1869 no Gazette Medecine Paris. Em 1907, o Dr. A. Schmincke, patologista alem&#227;o, descreveu dois cora&#231;&#245;es com hipertrofia ventricular esquerda; ambos vieram de mulheres na casa dos cinquenta anos. Levy e von Glahn, em 1944, da Universidade da Col&#244;mbia, em Nova York, publicaram uma s&#233;rie de casos que se assemelham a CMH. Em 1949, William Evans, um cardiologista de Londres, descreveu a ocorr&#234;ncia familiar de hipertrofia card&#237;aca em uma s&#233;rie de pacientes semelhantes aos descritos no artigo de Levy e von Glahn. Dr. Eugene Braunwald e Dr. Andrew Glenn Morrow publicaram uma s&#233;rie de trabalhos onde detalharam aspectos cl&#237;nicos e hemodin&#226;micos desta doen&#231;a permitindo estabelecer objetivos terap&#234;uticos.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B5">5</xref></p>
    <p>Na maioria dos pacientes a CMH tem um curso relativamente benigno. Por&#233;m a CMH tamb&#233;m &#233; uma causa importante de morte s&#250;bita (MS), particularmente em adolescentes e adultos jovens, com risco de 0,5 a 2% ao ano, sendo a causa mais frequente de MS em adolescentes.<xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B7">7</xref> A preven&#231;&#227;o de eventos de MS atrav&#233;s de implante de um dispositivo de implantado parece &#243;bvia, uma vez que as op&#231;&#245;es de tratamento cl&#237;nico para preven&#231;&#227;o de arritmia grave fatal n&#227;o podem ser oferecida pelo tratamento farmacol&#243;gico de forma confi&#225;vel. Como uma terapia de elevado custo e n&#227;o isenta de eventos adversos (infec&#231;&#227;o e choque inapropriado) tornou-se mandat&#243;rio estabelecer quais grupos teriam benef&#237;cio com a indica&#231;&#227;o de terapia com desfibrilador implantado.</p>
    <p>Entre os escores de avalia&#231;&#227;o de risco de MS para definir os pacientes de maior benef&#237;cio, ganharam destaques a calculadora de risco da AHA publicada em 2011<xref ref-type="bibr" rid="B8">8</xref> e a calculadora de risco para avalia&#231;&#227;o de MS pela Sociedade Europeia de Cardiologia em 2015.<xref ref-type="bibr" rid="B9">9</xref> Com metodologias diferentes ambas se propunham auxiliar o cl&#237;nico a identificar um grupo de pacientes de maior benef&#237;cio.</p>
    <p>Um classificador ideal deveria ser simples, com poucos crit&#233;rios, cada crit&#233;rio sendo de f&#225;cil interpreta&#231;&#227;o, reproduz&#237;vel com base na pr&#225;tica existente, altamente sens&#237;vel, de alto valor preditivo negativo, capaz de reduzir riscos com menor custo poss&#237;vel. Ainda que atenda a todos os quesitos acima, temos que levar em considera&#231;&#227;o que os modelos progn&#243;sticos s&#227;o desenvolvidos para serem aplicados em novos pacientes, que podem vir de diferentes centros, de diferentes ra&#231;as, h&#225;bitos, morbidades distintas e com os mais diferentes microbiomas. Portanto, novos pacientes s&#227;o comumente referidos como diferentes, mas semelhantes aos pacientes usados para desenvolver os modelos. Quando uma nova popula&#231;&#227;o de pacientes pode ser considerada semelhante (suficiente) &#224; popula&#231;&#227;o em desenvolvimento para justificar a valida&#231;&#227;o e, eventualmente, a aplica&#231;&#227;o de um modelo? A resposta a esta pergunta &#233; totalmente dependente de registros m&#233;dicos que possam revalidar ferramentas que usamos em nossa pr&#225;tica.<xref ref-type="bibr" rid="B10">10</xref></p>
    <p>O trabalho em quest&#227;o<xref ref-type="bibr" rid="B11">11</xref> vai al&#233;m e avalia de forma prospectiva uma coorte de pacientes aplicando as ferramentas mais usadas na atualidade para validar qual delas seria de maior acur&#225;cia em nossa popula&#231;&#227;o, assim como identifica pontos fortes e fragilidades de nossa capacidade de avaliar e predizer eventos futuros. A import&#226;ncia do trabalho vai al&#233;m do tema, uma vez que demonstra a necessidade de obtermos registros, permitindo que a comunidade cientifica seja capaz de revalidar os mais distintos escores cl&#237;nicos em nossa pr&#225;tica. Com enorme impacto em efic&#225;cia (ao reduzir ocorr&#234;ncia de mortes s&#250;bitas) assim como de efici&#234;ncia (ao permitir que possamos alocar recursos para contemplar os pacientes com maior chance de benef&#237;cio).</p>

    <body>
      <p>Hypertrophic cardiomyopathy (HCM) is a genetic disorder characterized by commonly asymmetric unexplained left ventricular hypertrophy, with greater thickening of the basal interventricular septum. Left ventricular outflow tract obstruction is present at rest in around one third of patients and can be caused in another third. The histological features of HCM include hypertrophy and myocyte disorder, in addition to interstitial fibrosis. Hypertrophy is also often associated with left ventricular diastolic dysfunction.</p>
      <p>The first case of HCM was described by Henri Liouville in 1869 in the Gazette Medecine Paris. In 1907, Dr. A. Schmincke, a German pathologist, described two hearts with left ventricular hypertrophy; both were seen in women in their fifth decade of life. Levy and von Glahn, in 1944, from the University of Colombia, in New York, published a series of cases that resembled HCM. In 1949, William Evans, a cardiologist from London, described the familial occurrence of cardiac hypertrophy in a series of patients similar to those described in the article by Levy and von Glahn. Dr. Eugene Braunwald and Dr. Andrew Glenn Morrow published a series of studies where they detailed the clinical and hemodynamic aspects of this disease, allowing the establishment of therapeutic objectives.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B5">5</xref></p>
      <p>HCM has a relatively benign course in the majority of patients. However, HCM is also an important cause of sudden death (SD), particularly in adolescents and young adults, with a risk of 0.5 to 2% per year, being the most frequent cause of SD in adolescents.<xref ref-type="bibr" rid="B6">6</xref> , <xref ref-type="bibr" rid="B7">7</xref> The prevention of SD events through a device implantation seem obvious, since the clinical treatment options for the prevention of severe fatal arrhythmia cannot be reliably offered by pharmacological treatment. As a high-cost therapy and one that is not exempt from adverse events (infection and inappropriate shock), it became mandatory to establish which groups would benefit from the indication of therapy with an implanted defibrillator.</p>
      <p>Among the SD risk assessment scores to define which patients would have the greatest benefit, the AHA risk calculator published in 2011<xref ref-type="bibr" rid="B8">8</xref> and the risk calculator for assessing SD by the European Society of Cardiology in 2015 were highlighted.<xref ref-type="bibr" rid="B9">9</xref> With different methodologies, both proposed to assist the clinician in identifying a group of patients that would have the greatest benefit.</p>
      <p>An ideal classification system should be simple, have few criteria, with each criterion being easy to interpret, be reproducible based on existing practice, highly sensitive, with high negative predictive value, capable of reducing risks at the lowest possible cost. Although it meets all the above requirements, we have to consider that the prognostic models are developed to be applied to new patients, who may come from different centers, have different ethnicities, habits, different morbidities and with the most different microbiomes. Therefore, new patients are commonly referred to as different, but similar to the patients who were used to develop the models. When can a new population of patients be considered (sufficiently) similar to the developing population to justify the validation and, possibly, the application of a model? The answer to this question is totally dependent on medical records that can revalidate tools used in our practice.<xref ref-type="bibr" rid="B10">10</xref></p>
      <p>The present study<xref ref-type="bibr" rid="B11">11</xref> goes further and prospectively evaluates a cohort of patients using the tools most frequently used at present to validate which of them would be the most accurate in our population, and also identifies the strengths and weaknesses of our capacity to assess and predict future events. The importance of the study goes beyond the topic, since it demonstrates the need to obtain records, allowing the scientific community to be able to revalidate the most distinct clinical scores in our practice, with a huge impact on efficacy (by reducing the occurrence of sudden deaths) as well as efficiency (by allowing us to allocate resources to include patients with the greatest chance of benefit).</p>
