    <p>Um menino de 3 anos e 5 meses de idade com hist&#243;rico de doen&#231;a de Kawasaki diagnosticada aos 6 meses de idade e infarto do mioc&#225;rdio sem supradesnivelamento do segmento ST (IAMSSST) foi submetido &#224; cirurgia de revasculariza&#231;&#227;o do mioc&#225;rdio (CRM) devido a um grande defeito de perfus&#227;o e disfun&#231;&#227;o sist&#243;lica moderada na Tomografia computadorizada por emiss&#227;o de f&#243;ton &#250;nico (SPECT) (<xref ref-type="fig" rid="f01">Figura 1A</xref>) e uma angiocoronariografia mostrando um aneurisma gigante parcialmente trombosado na art&#233;ria coron&#225;ria direita (ACD) e uma oclus&#227;o completa da art&#233;ria descendente anterior (ADA) proximal esquerda (<xref ref-type="fig" rid="f01">Figuras 1B-C</xref> ) Foi realizado enxerto da art&#233;ria mam&#225;ria interna esquerda (AMIE) &#224; ADA e anastomose livre da art&#233;ria mam&#225;ria interna direita (AMID) para a art&#233;ria descendente posterior (ADP) da ACD.</p>

    <p>Tr&#234;s meses depois, o SPECT mostrou 16% de isquemia do ventr&#237;culo esquerdo (VE). A angiocoronariografia revelou les&#227;o grave na anastomose distal da AMID para a ADP e oclus&#227;o completa na AMIE distal (Figura Suplementar S1). Decidiu-se por realizar a interven&#231;&#227;o percut&#226;nea ad hoc. Um cateter guia 5F JR3.5 foi utilizado para o engate seletivo da AMID. Um fio Runthrough (Terumo Corporation, T&#243;quio, Jap&#227;o) foi avan&#231;ado e a angioplastia com bal&#227;o simples (ABS) foi realizada com bal&#245;es semi-complacentes de 1,25 x 12 e 1,5 x 15, obtendo um resultado favor&#225;vel (<xref ref-type="fig" rid="f02">Figura 2-A</xref>). Posteriormente, o guia JR3.5 foi posicionado na subcl&#225;via esquerda, a partir da qual o fio Runthrough foi avan&#231;ado at&#233; a AMIE, seguido por um microcateter 1.8F Finecross (Terumo Corporation, T&#243;quio, Jap&#227;o), que permitiu atravessar toda a oclus&#227;o. Posteriormente, devido &#224; incapacidade de engate seletivo da AMIE, todas as inje&#231;&#245;es do meio de contraste foram realizadas atrav&#233;s do micro-cateter.</p>

    <p>V&#225;rias angioplastias com bal&#227;o resultaram em estenose residual significativa, portanto, um stent eluidor de Zotarolimus de 2,0 x 12 foi implantado com sucesso (<xref ref-type="fig" rid="f02">Figura 2-B</xref>). <p>A 3 years and 5 months old boy with a history of Kawasaki disease diagnosed at 6 months of age and a previous Non-ST myocardial infarction (NSTEMI) underwent Coronary Artery Bypass Graft (CABG) surgery due to a large perfusion defect and moderate systolic dysfunction on Single-photon emission computed tomography (SPECT) (<xref ref-type="fig" rid="f01001">Figure 1A</xref>) and a coronary angiogram showing a partially thrombosed giant aneurysm on the Right coronary artery (RCA) and a complete occlusion of the proximal Left anterior descending artery (LAD) (<xref ref-type="fig" rid="f01001">Figures 1B-C</xref>). A Left internal mammary artery (LIMA) graft to LAD and a free Right internal mammary artery (RIMA) to Posterior descending artery (PDA) anastomosis of RCA was performed.</p>

      </p>
      <p>Three months later, SPECT showed a 16% left ventricle (LV) ischemia. Coronary angiography revealed a severe lesion at the RIMA to PDA distal anastomosis and a complete occlusion at the distal LIMA (Supplemental Figure S1). Ad-hoc percutaneous intervention was decided. A 5F JR3.5 guiding catheter was used to selectively engage the RIMA. A Runthrough wire (Terumo Corporation, Tokyo, Japan) was advanced and Plain old balloon angioplasty (POBA) was performed with 1.25 x 12 and 1.5 x 15 semi-compliant balloons, achieving a favorable result (<xref ref-type="fig" rid="f02001">Figure 2-A</xref>). Subsequently, the JR3.5 guide was positioned in the left subclavian, from which the Runthrough wire was advanced into the LIMA, followed by a 1.8F Finecross microcatheter (Terumo Corporation, Tokyo, Japan), which allowed crossing the entire occlusion. Thereafter, due to the inability to selectively engage the LIMA, all contrast media injections were performed through the microcatheter.</p>

      <p>Several balloon angioplasties have resulted in significant residual stenosis, thus a 2.0 x12 Zotarolimus-eluting stent was successfully deployed (<xref ref-type="fig" rid="f02001">Figure 2-B</xref>). At a 3-month follow-up, SPECT showed no significant ischemia, confirming optimal post-procedural results (<xref ref-type="fig" rid="f03001">Figure 3-C</xref>).</p>

      <p>The risks of major adverse cardiovascular events can reach up to 48% in pediatric patients with giant coronary aneurysms due to Kawasaki disease.<xref ref-type="bibr" rid="B1">1</xref> Percutaneous coronary intervention (PCI), CABG and systemic thrombolysis have been used,<xref ref-type="bibr" rid="B2">2</xref> yet no reports of re-intervention after CABG are available. PCI in this setting is complex due to technical difficulties, limited experience and the need to adapt adult devices to small children. Multidisciplinary assessment and intervention (encompassing adult and pediatric specialists) are key for procedural success.</p>
