  <body>
    <p>Come&#231;o este texto sem falar de exerc&#237;cio. Quero frisar que portadores de doen&#231;as cr&#244;nicas como hipertens&#227;o, insufici&#234;ncia card&#237;aca, diferentes tipos de miocardiopatias e algumas formas de arritmias est&#227;o sob risco aumentado para desfechos fatais ap&#243;s contra&#237;rem o SARS-COV-2 e desenvolverem a infec&#231;&#227;o denominada COVID-19. Da mesma forma, quem realizou revasculariza&#231;&#227;o mec&#226;nica ou cir&#250;rgica, portadores de pr&#243;teses valvares (biol&#243;gicas ou met&#225;licas), de dispositivos como marca-passo ou desfibrilador implantado (entre outros), assim como aqueles com algumas formas de cardiopatias cong&#234;nitas, tamb&#233;m fazem parte de um grupo de maior risco.<xref ref-type="bibr" rid="B1">1</xref></p>
    <p>Ponto n&#250;mero um: Os pacientes acima s&#227;o definitivamente de maior risco e eles fazem parte do grupo de risco em qualquer idade.*</p>
    <p>*Observa&#231;&#227;o: Assim como os cardiopatas, os idosos, os imunossuprimidos e os portadores de algumas doen&#231;as cr&#244;nicas (como o diabetes e as doen&#231;as pulmonares), tamb&#233;m t&#234;m risco aumentado ao serem infectados.</p>
    <list list-type="bullet">
      <list-item>
        <p>Mas a pergunta que n&#227;o quer calar: E o exerc&#237;cio nos cardiopatas e na popula&#231;&#227;o geral, como devemos proceder? &#201; consenso que a pr&#225;tica do exerc&#237;cio f&#237;sico regular deve continuar, mesmo no isolamento em casa, sendo essa uma posi&#231;&#227;o clara entre cardiologistas, m&#233;dicos do exerc&#237;cio e demais especialistas. Claro que por um lado poder&#225; parecer mais dif&#237;cil, mas por outro, a maior disponibilidade de tempo para muitos acabar&#225; com a desculpa do &#8220;n&#227;o tenho tempo&#8221;. A semana continua tendo 168 horas. Ser&#225; que n&#227;o d&#225; para dedicar cinco per&#237;odos de meia hora, pelo menos, para fazer algum tipo de exerc&#237;cio dentro de casa?</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero dois: Exercitar-se dentro de sua casa &#233; indicado pelos dois motivos seguintes: a) respeita o crit&#233;rio principal de cuidados contra a infec&#231;&#227;o pelo SARS-COV-2 que causa a COVID-19; b) faz bem para sa&#250;de.</p>
    <list list-type="bullet">
      <list-item>
        <p>No entanto, surge outra pergunta: Se me sinto bem, por que n&#227;o posso me exercitar na rua, nas pra&#231;as ou nas academias dos condom&#237;nios? Se pensarmos de forma individual, sair &#224; rua ou correr na esteira do condom&#237;nio n&#227;o parece ser problem&#225;tico. Agora, o pensamento social e epidemiol&#243;gico vai na dire&#231;&#227;o de que a chance de aglomera&#231;&#227;o aumenta e isso vai de encontro ao que &#233; preconizado pelos especialistas. Imaginem se todos decidissem ir ao cal&#231;ad&#227;o situado na beira de uma cidade que tenha praia ou a um parque dar uma corridinha ao mesmo tempo&#8230; Seria a gl&#243;ria&#8230; para o v&#237;rus!!!</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero tr&#234;s: Na rela&#231;&#227;o risco-benef&#237;cio os especialistas recomendam que n&#227;o se saia de casa para fazer exerc&#237;cio neste momento pand&#234;mico.</p>
    <list list-type="bullet">
      <list-item>
        <p>Outra pergunta que me parece importante: E se eu estiver com algum sinal e/ou sintoma, devo me exercitar? N&#227;o!!! O exerc&#237;cio n&#227;o deve ser realizado em vig&#234;ncia de sinais e/ou sintomas, seja nesta ou em qualquer outra situa&#231;&#227;o de infec&#231;&#227;o. O melhor a se fazer &#233; repousar at&#233; a plena recupera&#231;&#227;o.</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero quatro: Se sintom&#225;tico, n&#227;o fazer exerc&#237;cio, nem mesmo dentro de casa. Ap&#243;s recuperar-se, a volta a pr&#225;tica passa a ser indicada.</p>
    <list list-type="bullet">
      <list-item>
        <p>Outro ponto relevante diz respeito &#224; hist&#243;ria pr&#233;via de exerc&#237;cio. Que bom que todos fossem fisicamente ativos e a inatividade fosse algo muito pontual. Alguns pensam que por serem ativos ou atl&#233;ticos est&#227;o imunes ao contagio pelo SARS-COV-2. Nada disso!!! Atletas de alta performance j&#225; contra&#237;ram COVID-19 e convalesceram ou convalescem, alguns at&#233; tendo sido internados.</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero cinco: Indiv&#237;duos ativos ou atletas devem tomar os mesmos cuidados do resto da popula&#231;&#227;o, estando sob risco de contrair a doen&#231;a como qualquer outro ser humano.</p>
    <list list-type="bullet">
      <list-item>
        <p>Muitos de n&#243;s tivemos uma inf&#226;ncia rica no que diz respeito &#224; atividade f&#237;sica. Brincamos de bambol&#234;, cabra cega, sapata (amarelinha) e de stop. Pulamos corda, brincamos de esconder, fizemos cabo de guerra, entre outras brincadeiras que ficaram na mem&#243;ria, que evocam um tempo sem pandemia, mas que inexoravelmente n&#227;o voltar&#225; mais. Nesse cen&#225;rio, por que n&#227;o aproveitar o tempo com seus filhos; repetindo: aproveitar o tempo com seus filhos, algo inimagin&#225;vel para muitos at&#233; dias atr&#225;s!!! Arraste-os da cama e do sof&#225;, incentive-os a sair um pouco do WhatsApp, dos jogos e do Netflix. V&#225; brincar!!!</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero seis:De volta para o passado . Incentive seus filhos a terem um comportamento menos sedent&#225;rio. O ideal, segundo a Organiza&#231;&#227;o Mundial de Sa&#250;de, &#233; que o adulto dispense 150 minutos e os mais jovens se exercitem pelo menos 300 minutos por semana.<xref ref-type="bibr" rid="B2">2</xref> Em suma, integra&#231;&#227;o familiar em prol da sa&#250;de da pr&#243;pria fam&#237;lia.</p>
    <list list-type="bullet">
      <list-item>
        <p>Nem toda atividade f&#237;sica &#233; um exerc&#237;cio, mas todo exerc&#237;cio &#233; uma forma de atividade f&#237;sica. Os exemplos s&#227;o in&#250;meros: varrer, passar, limpar e/ou aspirar a casa, jardinagem, trabalho na horta, subir e descer escadas (tomar cuidado se as escadas de um pr&#233;dio estiverem sendo usadas por outros moradores ao mesmo tempo), entre outros.<xref ref-type="bibr" rid="B2">2</xref></p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero sete: Intensificar a atividade f&#237;sica dentro de casa no dia a dia. Voc&#234; e seus familiares ter&#227;o uma vida mais ativa, saud&#225;vel e &#250;til.</p>
    <list list-type="bullet">
      <list-item>
        <p>Atividade sexual &#233; uma forma de exerc&#237;cio/atividade f&#237;sica. Para muitos a mais prazerosa dentre todas elas. Em tempos do bin&#244;mio isolamento-coronav&#237;rus essa &#233; uma preocupa&#231;&#227;o generalizada. Com certeza n&#227;o dispomos de dados cient&#237;ficos, mas a l&#243;gica nos faz pensar que o distanciamento deva prevalecer ao desejo. Que o bom senso deva ser maior do que o princ&#237;pio do prazer.</p>
      </list-item>
    </list>
    <p>Ponto n&#250;mero oito: Infelizmente, se recomenda que a atividade sexual que envolve abra&#231;os, car&#237;cias, beijos e penetra&#231;&#227;o deva ser evitada (no cen&#225;rio da atividade sexual ocasional, &#233; claro). A masturba&#231;&#227;o, o uso de vibradores e de brinquedos er&#243;ticos n&#227;o tem contraindica&#231;&#227;o. Se estes forem compartilhados, a indica&#231;&#227;o da lavagem com &#225;gua e sab&#227;o e/ou o uso do &#225;lcool e gel 70% me parece ser indicado (dedu&#231;&#227;o meramente intuitiva, pois n&#227;o existem estudos testando qualquer hip&#243;tese nesse particular).</p>
    <list list-type="bullet">
      <list-item>
        <p>Leitores dos Arquivos, certamente poderiam ser elencados outros v&#225;rios pontos no que tange esse tema, mas me parece que no final destas poucas linhas, o que vale &#233; o refor&#231;o de que a inatividade f&#237;sica &#233; um inimigo nefasto. Seus efeitos n&#227;o costumam se manifestar de forma aguda como s&#227;o os provocados pelo SARS-COV-2, os quais devem ser combatidos com medidas emergenciais de sa&#250;de embasadas nas melhores evid&#234;ncias cient&#237;ficas. Por sua vez, ao afetar negativamente os sistemas cardiovascular, respirat&#243;rio, metab&#243;lico e muscular, assim como o sistema imune, a inatividade f&#237;sica tamb&#233;m &#233; muito prejudicial. Em suma, os oito pontos acima foram escritos para que voc&#234; reflita e pense que hoje temos de viver nos moldes impostos por essa nova ordem global, restritiva, mas n&#227;o limitante.</p>

    <body>
      <p>I will begin this text without speaking about exercise. I want to emphasize that individuals with chronic diseases, such as hypertension, heart failure, different types of myocardiopathy, and some forms of arrhythmia are at an increased risk of fatal outcomes after contracting SARS-COV-2 and developing the infection denominated COVID-19. In the same manner, those who have undergone mechanical or surgical revascularization, individuals with prosthetic valves (whether biological or metallic), pacemakers, implanted defibrillators, or other devices, as well as those with some forms of congenital heart disease are also part of a higher risk group.<xref ref-type="bibr" rid="B1">1</xref></p>
      <p>First point: The patients described above are definitely at a higher risk and they are part of the risk group regardless of age.*</p>
      <p>*Note: Like individuals with heart disease, the elderly, immunosuppressed patients, and individuals with some chronic diseases, such as diabetes and pulmonary disease, are also at an increased risk of being infected.</p>
      <list list-type="bullet">
        <list-item>
          <p>This raises the question, &#8220;Regarding exercise in individuals with heart disease and in the general population, how should we proceed? There is a consensus that regular practice of physical exercise should continue, even in isolation at home; this position is clear among cardiologists, exercise physicians, and other specialists. Naturally, on one hand this may seem difficult, but, on the other, the increase in available time will do away with the excuse that &#8220;I don&#8217;t have time.&#8221; The week continues to be made up of 168 hours. Is it not possible to dedicate five half-hour periods, at least, to some type of exercise at home?</p>
        </list-item>
      </list>
      <p>Second point: Exercising at home is indicated for the following two reasons: a) It follows the main criterion of care against SARS-COV-2 infection that causes COVID-19; b) it is healthy.</p>
      <list list-type="bullet">
        <list-item>
          <p>This, however, brings up another question: If I feel well, why can&#8217;t I exercise in the street, in public squares, in gyms, or in condominiums? If we think in individual terms, going out or running on the condominium treadmill does not appear to be problematic. However, social and epidemiological reasoning indicates that the chance of there being a crowd increases and this goes against specialists&#8217; recommendations. Imagine if everyone decided to go to the sidewalk next to the beach or to the park to jog at the same time&#8230; It would be wonderful&#8230; for the virus!!!</p>
        </list-item>
      </list>
      <p>Third point: In the risk-benefit ratio, specialists recommend not leaving home to exercise during this pandemic moment.</p>
      <list list-type="bullet">
        <list-item>
          <p>Another question seems important to me: If I have signs and/or symptoms, should I exercise? No!!! Exercise should not be practiced in the presence of signs and/or symptoms, neither in this nor in any other situation of infection. The best thing to do is rest until you have fully recovered.</p>
        </list-item>
      </list>
      <p>Fourth point: If you are symptomatic, do not exercise, not even at home. After you have recovered, return to practicing as indicated.</p>
      <list list-type="bullet">
        <list-item>
          <p>Another relevant point deals with previous history of exercise. It would be great if everyone were physically active and inactivity were a minor issue. Some people think that because they are active or athletic they are immune to SARS-COV-2. Not at all!!! High performance athletes have already contracted COVID-19 and recovered or are in recovery; some have even been hospitalized.</p>
        </list-item>
      </list>
      <p>Fifth point: Active or athletic individuals should follow the same precautions as the rest of the population, as they are at the same risk of contracting the disease as any other human being.</p>
      <list list-type="bullet">
        <list-item>
          <p>Many of us had childhoods filled with physical activity. We played hula hoop, jump rope, hopscotch, and hide and seek, as well as other games that we still remember and that remind us a time with no pandemics but that will inexorably never return. Faced with the current scenario, why not take advantage of this time with your children; reiterating: make the most of this time with your children, which was something many of us could not imagine was possible a few weeks ago!!! Drag them out of bed or off the sofa; encourage them to give WhatsApp, games, and Netflix a break. Go play!!!</p>
        </list-item>
      </list>
      <p>Sixth point:Return to the past . Encourage your children to be less sedentary. According to the World Health Organization, it is ideal that adults dedicate 150 minutes to exercise weekly and that younger individuals exercise for at least 300 minutes weekly.<xref ref-type="bibr" rid="B2">2</xref> In short, integration of the family promotes the family&#8217;s own health.</p>
      <list list-type="bullet">
        <list-item>
          <p>Not all physical activity is exercise, but all exercise is a form of physical activity. There are countless examples, sweeping, ironing, cleaning and/or vacuuming, gardening, working in the yard, going up and down the stairs (with precaution if the stairs in an apartment building are being used by other residents at the same time), and others.<xref ref-type="bibr" rid="B2">2</xref></p>
        </list-item>
      </list>
      <p>Seventh point: Intensify physical activity at home on a daily-to-day basis. You and your family will have a more active, healthier, and more productive lifestyle.</p>
      <list list-type="bullet">
        <list-item>
          <p>Sexual activity is a form of physical exercise/activity. For many, it is the most pleasurable one. In times of isolation-coronavirus, this is a general concern. We certainly do not have scientific data, but logic leads us to think that distancing should come before desire, that common sense should be more prevalent than the pleasure principle.</p>
        </list-item>
      </list>
      <p>Eighth point: Unfortunately, it is recommended that sexual activity involving hugging, touching, kissing, and penetration be avoided (in the scenario of occasional sexual activity, of course). Masturbation, the use of vibrators and erotic toys is not contraindicated. If objects are to be shared, it seems to me that they should be washed with water and soap and/or 70% alcohol gel (this is merely an intuitive deduction, given that there are no studies testing hypotheses on this subject).</p>
      <list list-type="bullet">
        <list-item>
          <p>Readers of the Archives, regarding this theme, it would certainly be possible to list several other points, but it seems to me that at the end of these indications what is most important is to emphasize that physical inactivity in a treacherous enemy. Its effects typically do not manifest in an acute manner as is the case with SARS-COV-2, whose effects need to be combated with emergency health measures based on the best scientific evidence. In turn, by negatively affecting the cardiovascular, respiratory, metabolic, muscular, and immune systems, physical inactivity is also very harmful. In conclusion, the previous eight points were written in order to provoke reflection that at this moment we must live within the confines imposed by this new global order that is restrictive, but not limiting.</p>
