        <sec>
          <title>Fundamento</title>
          <p> A atividade f&#237;sica, em diferentes dimens&#245;es (ocupacional, dom&#233;stica e de lazer), pode ter papel relevante sobre a hipertens&#227;o arterial (HA). Praticar atividade f&#237;sica apenas no lazer, ou isoladamente nas outras dimens&#245;es, pode ser insuficiente para o controle efetivo da HA.</p>
        </sec>
        <sec>
          <title>Objetivo</title>
          <p> Analisar o efeito isolado e cumulativo das dimens&#245;es da atividade f&#237;sica sobre a ocorr&#234;ncia de hipertens&#227;o arterial em trabalhadores.</p>
        </sec>

          <title>M&#233;todos</title>
          <p> Estudo transversal com 1.070 trabalhadores de Limpeza Urbana e Ind&#250;stria de Cal&#231;ados, na Bahia, utilizando question&#225;rio aplicado por entrevistador acerca de aspectos sociodemogr&#225;ficos, ocupacionais, de estilo de vida e morbidade hipertensiva.</p>

          <title>Foram medidos</title>
          <p> peso, altura, circunfer&#234;ncia abdominal e press&#227;o arterial. Caso de HA: press&#227;o arterial sist&#243;lica &#8805;140 ou diast&#243;lica &#8805;90, ou tratamento regular de Hipertens&#227;o. Foram investigadas as dimens&#245;es ocupacional, dom&#233;stica e de lazer da atividade f&#237;sica. Realizou-se an&#225;lise multivariada com Regress&#227;o de Cox para estudos transversais.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> A preval&#234;ncia de HA foi de 24%, sendo 37% dentre pacientes com idades entre 35 e 44 anos, e 51% entre 45 e 54 anos. O modelo multivariado evidenciou que ativos em uma dimens&#227;o ou em nenhuma tiveram 62% mais HA, e um valor 25% mais elevado de HA foi observado dentre aqueles trabalhadores ativos em duas das tr&#234;s dimens&#245;es, na compara&#231;&#227;o com os ativos nas tr&#234;s dimens&#245;es. Sexo masculino, maior idade (&gt; 31 anos) e excesso de peso se associaram &#224; HA, com raz&#245;es de preval&#234;ncia de 1,62, 2,10 e 2,26, respectivamente.</p>
        </sec>
        <sec>
          <title>Conclus&#245;es</title>
          <p> Verificou-se efeito cumulativo das dimens&#245;es da atividade f&#237;sica sobre a ocorr&#234;ncia de HA. Classificar sujeitos ativos no trabalho ou no ambiente dom&#233;stico como inativos ao se basear apenas na dimens&#227;o do lazer pode implicar em erro metodol&#243;gico. <title>Introdu&#231;&#227;o</title>
      <p>Hipertens&#227;o arterial (HA) &#233; uma doen&#231;a cr&#244;nica definida por n&#237;veis press&#243;ricos maiores ou iguais a 140/90 mmHg, representando um dos fatores de risco para doen&#231;as cardiovasculares. Por outro lado, sabe-se que a atividade f&#237;sica (AF) ausente ou insuficiente &#233; risco modific&#225;vel nesse agravo.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref></p>
      <p>Todo movimento produzido pela musculatura esquel&#233;tica que gere energia maior que o estado de repouso &#233; AF. Considera-se ativo o indiv&#237;duo que pratica no m&#237;nimo 30 minutos de AF com intensidade moderada, cinco dias por semana. Isso n&#227;o inclui apenas o lazer, ao contr&#225;rio, a literatura ressalta a import&#226;ncia de se abordar a AF em diferentes dimens&#245;es: lazer (AFL), ocupacional (AFO), dom&#233;stica (AFD) e de deslocamento.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref></p>
      <p>O Brasil, signat&#225;rio do Plano de Preven&#231;&#227;o das Doen&#231;as Cr&#244;nicas n&#227;o Transmiss&#237;veis da Organiza&#231;&#227;o Mundial da Sa&#250;de, comprometeu-se com a redu&#231;&#227;o relativa de 25% da preval&#234;ncia de HA e 10% de AF insuficiente, at&#233; o ano de 2025. Esse desafio explicita a relev&#226;ncia da HA como problema de sa&#250;de.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref></p>
      <p>Embora a associa&#231;&#227;o negativa entre AF e Hipertens&#227;o seja conhecida - mais ativos fisicamente t&#234;m menor preval&#234;ncia de HA -, h&#225; recentemente o interesse pelo papel das dimens&#245;es da AF. Nesse sentido, evid&#234;ncias acerca das dimens&#245;es t&#234;m sido mostradas: indiv&#237;duos ativos no trabalho, lazer e em pr&#225;ticas esportivas apresentam menor preval&#234;ncia de HA;<xref ref-type="bibr" rid="B7">7</xref> ademais, a insufici&#234;ncia de AFO e AFL aumenta significantemente o risco de HA.<xref ref-type="bibr" rid="B8">8</xref> Entretanto, as atividades f&#237;sicas ocupacionais, consideradas pesadas, tamb&#233;m t&#234;m sido associadas &#224; maior preval&#234;ncia de hipertens&#227;o. No entanto, a associa&#231;&#227;o positiva da AFO mais alta com a hipertens&#227;o tem sido atribu&#237;da, por alguns, ao papel de confundidores n&#227;o avaliados, dentre eles, as demandas psicol&#243;gicas.<xref ref-type="bibr" rid="B9">9</xref>-<xref ref-type="bibr" rid="B11">11</xref> Al&#233;m disso, tem sido discutido o chamado &#8220;paradoxo da AF&#8221;, segundo o qual os benef&#237;cios da atividade no lazer poderiam ser minimizados pela alta AFO.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B14">14</xref> Portanto, h&#225; conflu&#234;ncias e lacunas acerca do papel das diferentes dimens&#245;es da AF para a ocorr&#234;ncia de HA.</p>
      <p>Assim, o objetivo deste estudo foi verificar como se d&#225; a associa&#231;&#227;o das dimens&#245;es da AF (AFO, AFL e AFD) isoladamente e de forma cumulativa na ocorr&#234;ncia de HA.</p>

        <title>Delineamento e popula&#231;&#227;o do estudo</title>
        <p>Estudo de corte transversal com trabalhadores da Ind&#250;stria de Cal&#231;ados (n=446) e trabalhadores da Limpeza Urbana (n=624), totalizando 1.070 participantes. No conjunto dos dois inqu&#233;ritos, a taxa de resposta foi de 97%. O plano amostral na Ind&#250;stria de Cal&#231;ados foi de uma amostra aleat&#243;ria estratificada proporcional ao sexo e n&#250;mero de trabalhadores de cada uma das duas f&#225;bricas no interior da Bahia; na Limpeza Urbana fez-se um censo na empresa municipal em Salvador. A coleta dos dados foi realizada em 2012 e 2010, respectivamente. Como crit&#233;rio de inclus&#227;o, todos os trabalhadores entrevistados deveriam estar empregados durante a coleta de dados, que foi conduzida por equipe treinada, composta por profissionais da sa&#250;de, seguran&#231;a e acad&#234;micos do curso de Fisioterapia. Todos estavam cientes da necessidade de esclarecer d&#250;vidas sobre as perguntas, para obterem respostas as mais fidedignas poss&#237;veis. As entrevistas foram realizadas em cada empresa participante, durante o dia de trabalho regular, em local reservado, assegurando a privacidade dos trabalhadores.</p>

        <title>Question&#225;rio</title>
        <p>O question&#225;rio aplicado pelo entrevistador incluiu perguntas sobre: fatores sociodemogr&#225;ficos, caracter&#237;sticas do trabalho e h&#225;bitos de vida. Fatores sociodemogr&#225;ficos: sexo, idade, situa&#231;&#227;o conjugal, escolaridade, cor ou ra&#231;a, e ter ou n&#227;o filhos. Caracter&#237;sticas do trabalho: demandas f&#237;sicas, avaliadas atrav&#233;s do manuseio de cargas: levantar, empurrar e puxar; aspectos psicossociais do trabalho, medidos por meio do Job Content Questionnaire (JCQ), instrumento que mede as dimens&#245;es do Modelo Demanda-Controle;<xref ref-type="bibr" rid="B15">15</xref> hor&#225;rio de trabalho e tempo de trabalho na empresa estudada. H&#225;bitos de vida: tabagismo atual ou pregresso e frequ&#234;ncia de uso de bebida alco&#243;lica. Foram realizadas medidas diretas da PA, peso, altura e circunfer&#234;ncia de cintura.</p>

        <p>A vari&#225;vel dependente &#233; a HA, definida por duas medidas da PA: a primeira, no in&#237;cio da entrevista, com os participantes sentados durante cinco minutos antes da aplica&#231;&#227;o do question&#225;rio e, a segunda, ao final do preenchimento do question&#225;rio aplicado pelo entrevistador, com intervalo m&#233;dio de 20 minutos entre essas tomadas. Foi utilizado um esfigmoman&#244;metro aneroide devidamente calibrado e um estetosc&#243;pio. As medidas foram realizadas conforme recomenda&#231;&#245;es da 6&#170; Diretriz Brasileira de Hipertens&#227;o Arterial. Foram considerados casos de hipertens&#227;o arterial aqueles com press&#227;o arterial sist&#243;lica &#8805; 140 e ou press&#227;o arterial diast&#243;lica &#8805; 90, bem como indiv&#237;duos em tratamento regular para a hipertens&#227;o. As informa&#231;&#245;es foram dicotomizadas, classificando os indiv&#237;duos como aqueles que apresentavam ou n&#227;o um fen&#244;meno hipertensivo.</p>

        <title>Vari&#225;vel independente principal</title>
        <p>A vari&#225;vel independente principal &#233; a AF em tr&#234;s dimens&#245;es: ocupacional (AFO), dom&#233;stica (AFD) e de lazer (AFL).</p>
        <p>A AFO foi obtida por meio do autorrelato do trabalhador sobre suas demandas f&#237;sicas ocupacionais de manuseio de cargas: levantar, empurrar e puxar, utilizando-se uma escala de dura&#231;&#227;o de 0 a 5, onde 0 representou a aus&#234;ncia de exposi&#231;&#227;o (nunca) e 5, a exposi&#231;&#227;o m&#225;xima (o tempo todo). As tr&#234;s vari&#225;veis foram somadas e esse valor dividido por tr&#234;s, obtendo-se a m&#233;dia dos escores. Os que atingiram escore &#8804; 2 (menos ativos) foram os considerados expostos e os n&#227;o expostos tiveram escore 3 at&#233; 5 (mais ativos).</p>
        <p>Em rela&#231;&#227;o &#224; AFL, foi perguntado: &#8220;O que voc&#234; faz quando n&#227;o est&#225; trabalhando na empresa ou em casa?&#8221;, com quatro op&#231;&#245;es de resposta: (I) Conversa com os parentes ou amigos, l&#234; jornal ou revista, v&#234; televis&#227;o e vai ao culto (ou missa); (II) caminha, pesca, cuida da horta ou do quintal; (III) corre, faz gin&#225;stica, nada, joga bola e anda de bicicleta; (IV) treina para competi&#231;&#245;es. Consideraram-se indiv&#237;duos com respostas &#8220;I&#8221; e &#8220;II&#8221;, (portanto, com AFL mais baixa), os expostos; e os que responderam positivamente &#224;s afirmativas &#8220;III&#8221; e &#8220;IV&#8221; foram os considerados ativos no lazer, ou seja, os n&#227;o-expostos.</p>
        <p>Sobre o dom&#237;nio da AFD, perguntou-se sobre horas semanais dedicadas ao trabalho dom&#233;stico. A classifica&#231;&#227;o empregada foi a de expostos e n&#227;o expostos, aus&#234;ncia de horas dedicadas aos trabalhos dom&#233;sticos e presen&#231;a, respectivamente.</p>
        <p>Na an&#225;lise multivariada, utilizou-se a vari&#225;vel principal AF, reunindo as tr&#234;s dimens&#245;es em tr&#234;s estratos. Os trabalhadores que tinham alta AF nas tr&#234;s dimens&#245;es, ou seja, ativos no lazer, no trabalho e em casa, foram considerados os n&#227;o expostos, e dois estratos de exposi&#231;&#227;o foram utilizados: o primeiro deles incluiu indiv&#237;duos ativos em duas dimens&#245;es e, ao segundo estrato, foram incorporados indiv&#237;duos inativos ou ativos em apenas uma dimens&#227;o.</p>

        <title>Covari&#225;veis</title>
        <p>Para a vari&#225;vel idade, adotou-se, como exposto, aqueles trabalhadores com idades acima da mediana. Cor ou ra&#231;a foi estratificada em &#8220;n&#227;o preta&#8221; que inclu&#237;a branca, parda, amarela e ind&#237;gena, classificados como n&#227;o-expostos, e os indiv&#237;duos de cor ou ra&#231;a &#8220;preta&#8221; foram classificados como expostos. Quanto &#224; escolaridade, os expostos foram os que n&#227;o tinham o Ensino M&#233;dio completo. <p>Todas as vari&#225;veis passaram por uma etapa descritiva e suas frequ&#234;ncias foram verificadas. Nas vari&#225;veis cont&#237;nuas, foram feitas medidas de tend&#234;ncia central, de dispers&#227;o e posi&#231;&#227;o. C&#225;lculos das raz&#245;es de preval&#234;ncia para a HA, segundo as vari&#225;veis independentes, foram conduzidas inicialmente na etapa univariada da an&#225;lise. Na an&#225;lise multivariada, as covari&#225;veis entraram nos modelos por blocos, tendo como vari&#225;vel independente principal a AF com tr&#234;s estratos: ativos nas tr&#234;s dimens&#245;es, ativos em duas, e inativos ou ativos em uma dimens&#227;o. <p>Ao todo, 1.070 trabalhadores foram inclu&#237;dos no estudo, sendo que 842 (78,7%) eram homens. A preval&#234;ncia de HA foi de 24%. Trabalhadores que n&#227;o conclu&#237;ram o ensino m&#233;dio correspondiam a 46% da popula&#231;&#227;o, 82% tinham parceiros est&#225;veis e 63% possu&#237;am jornada di&#225;ria de trabalho com hor&#225;rio de turno. Os que fumam ou j&#225; fumaram representaram 26%, e 42% da popula&#231;&#227;o consumia bebida alco&#243;lica mais de uma vez por semana. Excesso de peso esteve presente em 43% dos participantes. Nas vari&#225;veis da AF, 47% e 61% da popula&#231;&#227;o tinham baixa AFO e baixa AFL, respectivamente; 28% n&#227;o faziam AFD. Idade e IMC apresentaram a maior for&#231;a de associa&#231;&#227;o com a HA na etapa univariada (RP=2,9 e RP =2,8, respectivamente). Tamb&#233;m apresentaram forte associa&#231;&#227;o: sexo (RP=1,95), AFD (RP=1,62) e tipo de jornada (RP=1,60) (<xref ref-type="table" rid="t1">Tabela 1</xref>).</p>

      <title>Discuss&#227;o</title>
      <p>O ac&#250;mulo de AF nas dimens&#245;es da AFO, AFD e AFL pode resultar em melhorias da HA. Ser ativo em uma ou nenhuma das tr&#234;s dimens&#245;es da AF demonstrou maior associa&#231;&#227;o positiva com o desfecho: 62% mais HA comparados &#224;queles ativos nas tr&#234;s dimens&#245;es. Esses achados contribuem com a literatura sobre o papel da AF insuficiente como fator de risco modific&#225;vel para HA, e coloca em quest&#227;o o papel isolado da AFL como estrat&#233;gia para o controle desse agravo.</p>
      <p>No que diz respeito &#224; cor ou ra&#231;a, a forte miscigena&#231;&#227;o da popula&#231;&#227;o na Bahia pode estabelecer limites para investigar sua associa&#231;&#227;o com a HA. Conforme descrito por outros autores,<xref ref-type="bibr" rid="B20">20</xref> a prov&#225;vel homogeneiza&#231;&#227;o dos grupos, pela forte miscigena&#231;&#227;o, deve ter um papel no resultado que revelou preval&#234;ncias de HA praticamente iguais entre os grupos &#8220;pretos&#8221; e &#8220;n&#227;o pretos&#8221;, nessa popula&#231;&#227;o.<xref ref-type="bibr" rid="B21">21</xref>,<xref ref-type="bibr" rid="B22">22</xref> Em conson&#226;ncia com essa hip&#243;tese explicativa, um estudo multic&#234;ntrico sobre HA que, al&#233;m de servidores da Universidade Federal da Bahia, incluiu servidores de mais cinco institui&#231;&#245;es do Brasil, de diferentes estados da Federa&#231;&#227;o,<xref ref-type="bibr" rid="B23">23</xref> encontrou forte associa&#231;&#227;o dessa vari&#225;vel com a HA, cujas preval&#234;ncias s&#227;o maiores em pretos, seguidos dos pardos, sendo a menor preval&#234;ncia observada entre os brancos.</p>
      <p>A associa&#231;&#227;o da HA com idade e sexo foi bem consistente com a literatura.<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B23">23</xref>-<xref ref-type="bibr" rid="B26">26</xref> Essa popula&#231;&#227;o &#233; composta por 79% de homens jovens, com 32 anos de idade m&#233;dia. &#201; visto que, nessa fase da vida, os homens apresentam mais HA quando comparados &#224;s mulheres.<xref ref-type="bibr" rid="B1">1</xref> Estudos que revelam maior preval&#234;ncia entre mulheres s&#227;o baseados em autorrelato ou popula&#231;&#245;es em faixas et&#225;rias mais elevadas, nas quais o sexo feminino supera o masculino com maior preval&#234;ncia.<xref ref-type="bibr" rid="B27">27</xref> Quanto aos estudos baseados em autorrelato,<xref ref-type="bibr" rid="B1">1</xref> a predomin&#226;ncia de HA entre mulheres pode ser decorrente da maior busca de servi&#231;o por essas e, consequentemente, maior conhecimento do estado hipertensivo.</p>
      <p>No Brasil, em 2016, a preval&#234;ncia de HA aumentou com a idade, sendo de 19,1% naqueles indiv&#237;duos entre 35 e 44 anos.<xref ref-type="bibr" rid="B1">1</xref> Comparando com os resultados do presente estudo, viu-se que, dentre indiv&#237;duos na faixa et&#225;ria de 35 a 44 anos, a preval&#234;ncia de HA &#233; de 37%. Portanto, entre os aqui estudados, a HA foi mais alta do que a m&#233;dia nacional. Em compara&#231;&#227;o com Chor et al.,<xref ref-type="bibr" rid="B23">23</xref> que estudaram servidores de universidades, os estratos por idade deste estudo com oper&#225;rios apresentaram preval&#234;ncias mais altas.</p>
      <p>Oper&#225;rios com excesso de peso tiveram 2,3 vezes maior preval&#234;ncia de HA quando comparados com aqueles sem excesso de peso. Esse resultado &#233; consistente com outros estudos, demonstrando associa&#231;&#227;o da HA com maior densidade corporal.<xref ref-type="bibr" rid="B28">28</xref>,<xref ref-type="bibr" rid="B29">29</xref> As demais vari&#225;veis que se associaram na etapa univariada reduziram muito suas associa&#231;&#245;es com HA na an&#225;lise multivariada.</p>
      <p>A vari&#225;vel independente principal associou-se consistentemente com a HA na an&#225;lise multivariada, constatando-se efeito cumulativo da baixa AF nas tr&#234;s dimens&#245;es sobre a preval&#234;ncia da HA.</p>
      <p>Explorou-se a associa&#231;&#227;o independente entre atividade f&#237;sica ocupacional (AFO) e HA por meio de an&#225;lises que contemplaram AFO tanto com escala de dura&#231;&#227;o quanto de intensidade do manuseio de carga. Os trabalhadores mais ativos nessa dimens&#227;o foram mais protegidos para HA, mesmo com ajustes para idade, sexo e excesso de peso. A motiva&#231;&#227;o para essas an&#225;lises com a AFO baseou-se na recente discuss&#227;o da literatura acerca do &#8220;efeito paradoxal&#8221; da AFO sobre a press&#227;o arterial.<xref ref-type="bibr" rid="B30">30</xref> Segundo pesquisadores, indiv&#237;duos submetidos &#224; alta AFO apresentariam maior risco para n&#237;veis press&#243;ricos elevados.<xref ref-type="bibr" rid="B14">14</xref>,<xref ref-type="bibr" rid="B31">31</xref> &#201; poss&#237;vel que ocupa&#231;&#245;es que se caracterizam por recrutamento de grandes grupos musculares, a exemplo da coleta de lixo, embora possam determinar fadiga f&#237;sica, n&#227;o parecem representar associa&#231;&#227;o com HA, como proposto no efeito paradoxal. No presente estudo, o trabalho ativo foi sempre protetor para esse agravo, particularmente, quando associado &#224; AFD e AFL.</p>
      <p>Embora discuta-se a AFL como um dos principais tratamentos n&#227;o farmacol&#243;gicos contra a HA,<xref ref-type="bibr" rid="B32">32</xref>-<xref ref-type="bibr" rid="B34">34</xref> de acordo com o presente estudo, a AFL isolada pode n&#227;o suprir a insufici&#234;ncia de AF para o melhor controle da HA: ser ativo em apenas uma dimens&#227;o, e inativo nas duas demais dimens&#245;es, apresentou associa&#231;&#227;o positiva com a HA.</p>
      <p>Poucas pesquisas exploram a AFD de forma isolada e resultados com essa dimens&#227;o parecem n&#227;o mostrar associa&#231;&#227;o com HA. Por&#233;m, quando combinada com a pr&#225;tica de AF nas outras dimens&#245;es, a AFD pode produzir efeitos ben&#233;ficos no controle da HA.</p>
      <p>Embora no presente estudo tenha sido observado efeito cumulativo da baixa AF nas diferentes dimens&#245;es para a ocorr&#234;ncia da HA, n&#227;o foi encontrada intera&#231;&#227;o entre essas dimens&#245;es (nem no modelo aditivo). Fato semelhante foi observado em outro estudo, que tamb&#233;m revelou efeito cumulativo das diferentes dimens&#245;es, mas n&#227;o intera&#231;&#227;o entre essas.<xref ref-type="bibr" rid="B7">7</xref> An&#225;lise de intera&#231;&#227;o para explorar o comportamento sedent&#225;rio e ou tempo de inatividade em frente &#224; TV na determina&#231;&#227;o de doen&#231;as cr&#244;nicas em adultos parece uma abordagem promissora.<xref ref-type="bibr" rid="B35">35</xref></p>
      <p>Al&#233;m do ac&#250;mulo de AF nas diversas dimens&#245;es ser um fator de prote&#231;&#227;o, o ac&#250;mulo durante os v&#225;rios est&#225;gios da vida pode melhorar ainda mais os indicadores de sa&#250;de.<xref ref-type="bibr" rid="B36">36</xref></p>
      <p>Neste estudo, a AF foi obtida pelo autorrelato, e n&#227;o medida de forma direta, atrav&#233;s de monitores card&#237;acos, aceler&#244;metros, ped&#244;metros ou frequenc&#237;metros. Apesar da tend&#234;ncia em se considerar que medidas diretas sejam mais v&#225;lidas do que o autorrelato, essas t&#234;m sido questionadas enquanto padr&#227;o-ouro na Epidemiologia Ocupacional. Devido ao seu custo e exig&#234;ncias de calibra&#231;&#227;o de instrumentos, medidas diretas s&#227;o obtidas, em geral, durante pequenas amostras de tempo e, no caso da AFO, em uma pequena amostra da jornada de trabalho. Isto &#233; particularmente relevante, limitando a validade das medidas diretas, em estudos com popula&#231;&#245;es cujas atividades ocupacionais apresentem grande variabilidade durante a jornada de trabalho e, nesse caso, a amostra de tempo da jornada no qual se fazem as medidas diretas n&#227;o &#233; representativa do tempo total de trabalho.<xref ref-type="bibr" rid="B37">37</xref></p>
      <p>No delineamento do estudo de corte transversal, o principal vi&#233;s &#233; o de preval&#234;ncia e, portanto, n&#227;o se pode descartar a causalidade reversa, ou seja, n&#227;o se pode afirmar rigorosamente se a maior preval&#234;ncia de HA dos menos ativos &#233; decorrente do estilo de vida sedent&#225;rio ou se eles j&#225; eram hipertensos antes de passarem a ter uma vida mais sedent&#225;ria. Por&#233;m, como a HA &#233; um agravo cr&#244;nico, cuja terap&#234;utica assegura seu controle, mas n&#227;o a cura, &#233; poss&#237;vel que esse tipo de vi&#233;s esteja minimizado, sendo o corte transversal uma op&#231;&#227;o adequada de delineamento de estudo. A popula&#231;&#227;o do estudo &#233; composta predominantemente por jovens trabalhadores, mas os resultados obtidos e descritos de preval&#234;ncia de hipertens&#227;o para as diferentes faixas et&#225;rias permitem compara&#231;&#227;o com outras popula&#231;&#245;es, desde que observada cada faixa de idade.</p>
    </sec>
    <sec sec-type="conclusions">
      <title>Conclus&#227;o</title>
      <p>Neste estudo, evidenciou-se que o menor volume de movimento nas diversas dimens&#245;es da AF foi um fator associado &#224; HA nessa popula&#231;&#227;o. Visto que outros estudos demonstram linha semelhante de evid&#234;ncias, os resultados do presente estudo podem ser considerados v&#225;lidos. A partir destes, considerou-se que a investiga&#231;&#227;o da AF, fator modific&#225;vel na ocorr&#234;ncia da HA, n&#227;o pode prescindir da abordagem das suas diferentes dimens&#245;es, sob pena de implicar em erro metodol&#243;gico ao classificar sujeitos ativos no trabalho ou no ambiente dom&#233;stico como inativos, ao se basear apenas na dimens&#227;o do lazer. Devem ser planejadas interven&#231;&#245;es no ambiente ocupacional, objetivando ouvir dos trabalhadores o significado da HA para eles pr&#243;prios, dando acesso aos resultados de pesquisas, a fim de estimular mudan&#231;as de h&#225;bitos e a amplia&#231;&#227;o do protagonismo dos trabalhadores na defini&#231;&#227;o do lazer ativo, al&#233;m de dar visibilidade &#224; import&#226;ncia da AFO. Novas investiga&#231;&#245;es devem aprimorar o estudo da AF, a fim de trazer novas evid&#234;ncias sobre seu papel na HA.</p>

        <sec>
          <title>Background</title>
          <p> Physical activity, each type in its own manner, whether occupational, domestic or leisure, can play a significant role regarding high blood pressure (HBP). However, practicing physical activity only at leisure time, or in specific situations, can be insufficient to achieve the effective control of HBP.</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p> To analyze the isolated and cumulative effect of different types of physical activity and the prevalence of HBP among workers.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p> A cross-sectional study with 1,070 Urban Cleaning and Footwear Industry workers in Bahia, who answered a survey, conducted by an interviewer on sociodemographic, occupational, lifestyle and hypertensive morbidity aspects. Weight, height, waist circumference and blood pressure were measured. Case of HBP: Systolic blood pressure &#8805;140 or diastolic blood pressure &#8805;90, or regular treatment for HBP. The occupational, domestic and leisure aspects of Physical Activity were studied. A multivariate analysis with Cox Regression was performed for cross-sectional studies.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p> The prevalence of HBP was 24%, being 37% among workers aged between 35-44 years, and 51% among workers aged between 45-54 years. The multivariate model showed that workers who were active in one form of physical activity only or no forms had 62% higher BP levels and that these levels were 25% higher among workers who were active in two out of three physical activity forms. Being a male, being older (&gt; 31 years old) and being overweight were characteristics associated with HBP, with prevalence ratios of 1.62, 2.10 and 2.26, respectively.</p>
        </sec>
        <sec>
          <title>Conclusions</title>
          <p> There was a cumulative effect of the form of physical activity on the occurrence of HBP. Classifying active subjects at work or at home as inactive persons by relying only on the leisure form can lead to methodological errors. (Arq Bras Cardiol. 2020; 114(5):755-761)</p>

        <title>Introduction</title>
        <p>High blood pressure (HBP) is a chronic disease defined by blood pressure levels greater than or equal to 140/90 mmHg and represents one of the risk factors for cardiovascular diseases. On the other hand, it is known that insufficient or lack of physical activity (PA) is a modifiable risk factor for this condition.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref></p>
        <p>Every movement produced by skeletal muscles that generates energy greater than the resting state is considered PA. An active person is an individual who practices at least 30 minutes of moderate-intensity PA five days a week. This does not include leisure only; on the contrary, the literature emphasizes the importance of approaching PA in different forms: leisure (PA-L), occupational (PA-O), domestic (PA-D) and displacement.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref></p>
        <p>Brazil, a signatory to the World Health Organization&#8217;s Plan for the Prevention of Noncommunicable Chronic Diseases, committed to a 25% relative reduction in the prevalence of HBP and a 10% reduction in insufficient PA by the year 2025. This challenge highlights the relevance of HBP as a health problem.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref></p>
        <p>Although a negative association between PA and HBP has been observed - more physically active people have lower HBP prevalence - there is a recent interest in the role of PA forms. In this sense, evidence about the forms has shown that individuals who are active at their work, leisure time and also practice sports have lower prevalence of HBP;<xref ref-type="bibr" rid="B7">7</xref> in addition, the lack of PA-O and PA-L significantly increases the risk of HBP.<xref ref-type="bibr" rid="B8">8</xref> However, occupational physical activities, considered strenuous, have also been associated with higher prevalence of hypertension, but this positive association of higher PA-O with hypertension has been attributed by some researchers to the role of unvalued confounders, such as psychological demands.<xref ref-type="bibr" rid="B9">9</xref> -<xref ref-type="bibr" rid="B11">11</xref> In addition, the so-called &#8220;PA paradox&#8221; has been discussed, according to which the benefits of leisure activity could be minimized by high levels of PA-O.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B14">14</xref> Therefore, there are confluences and gaps regarding the role of the different PA forms on the occurrence of HBP.</p>
        <p>Thus, the objective of this study was to verify how forms of PA (PA-O, PA-L and PA-D) are associated with the occurrence of HBP, both alone and cumulatively.</p>

          <title>Study design and population</title>
          <p>A cross-sectional study with Footwear Industry workers (n=446) and Urban Cleaning workers (n=624), totaling 1,070 subjects. In both survey clusters, the response rate was 97%. The sampling plan in the Footwear Industry was a stratified random sample proportional to the gender and number of workers from each of the two factories in the countryside of Bahia; in Urban Cleaning, a census was carried out at the municipal company in Salvador. Data collection was carried out in 2012 and 2010. As an inclusion criterion, all workers interviewed should be employed during data collection, which was conducted by a trained team of health and safety professionals and students of the physical therapy program. Everyone was aware of the need to clarify any aspects about the questions in order to obtain the most reliable answers possible. The interviews were carried out in each participating company, during regular working days, in a reserved place, ensuring the workers&#8217; privacy.</p>
        </sec>
        <sec>
          <title>Survey</title>
          <p>The survey carried out by the interviewer included questions about sociodemographic factors, job characteristics and life habits. The sociodemographic factors asked were gender, age, marital status, schooling, color or race, and if the interviewed worker had any children. Job characteristics: physical demands, evaluated through the handling of loads (lifting, pushing and pulling); psychosocial aspects of work, measured using the Job Content Questionnaire (JCQ), an instrument that measures the forms of the Demand-Control Model;<xref ref-type="bibr" rid="B15">15</xref> working hours and time of service in the company studied. Life habits: current or previous smoker and frequency of alcohol use. Direct measurements of Blood Pressure (BP), weight, height and waist circumference were performed.</p>
        </sec>
        <sec>
          <title>Dependent variable</title>
          <p>The dependent variable is HBP, defined by two measures of BP: the first at the beginning of the interview, with subjects sitting for five minutes before the start of the survey, and the second upon completion of the survey, as carried out by the interviewer, with a mean interval of 20 minutes between these measurements. A duly calibrated aneroid sphygmomanometer and stethoscope were used. The measurements were performed according to the recommendations of the Sixth Brazilian Guidelines on High Blood Pressure. Cases of hypertension were considered as those with systolic blood pressure &#8805; 140 and/or diastolic blood pressure &#8805; 90, and subjects undergoing regular treatment for hypertension. The information was divided into two groups, classifying subjects as those who did and those who did not show a hypertensive condition.</p>
        </sec>
        <sec>
          <title>Main independent variable</title>
          <p>The main independent variable is PA in three forms: occupational (PA-O), domestic (PA-D) and leisure (PA-L).</p>
          <p>The level of PA-O was obtained through the worker&#8217;s self-report on his physical demands of load handling (lifting, pushing and pulling), using a 0-5 scale, where 0 meant no exposure (never) and 5 meant maximum exposure (all the time). The three variables were summed, with the resulting value divided by three, to calculate the mean of the scores. Those workers who reached score &#8804; 2 (less active) were considered exposed, whereas the non-exposed ones had scores ranging from 3 to 5 (more active).</p>
          <p>For the PA-L, the following question was asked, &#8220;What do you do when you&#8217;re not working at home or at work?&#8221;, with four answer options: (I) Talk to family or friends, read the newspaper or a magazine, watch TV, go to church; (II) Hike, fish, maintain the garden or the yard; (III) Run, exercise, swim, play a ball game, ride a bicycle; (IV) Train for a competition. Individuals with &#8220;I&#8221; and &#8220;II&#8221; responses were considered, therefore, to have lower PA-L levels, i.e., the exposed ones; and those who responded positively to &#8220;III&#8221; and &#8220;IV&#8221; were considered active in leisure, i.e., the non-exposed ones.</p>
          <p>In the PA-D domain, workers were questioned about weekly hours devoted to domestic work. They were classified as exposed and non-exposed, absence and presence of hours dedicated to housework, respectively.</p>
          <p>In the multivariate analysis, the main variable PA was used, subdividing the three forms into three strata. Workers who had high PA levels in the three forms, i.e., active at leisure, at work and at home, were considered non-exposed, and two strata of exposure were used: the first one included active individuals in two forms; and the second, individuals who were inactive or active in only one form.</p>
        </sec>
        <sec>
          <title>Covariables</title>
          <p>For the age variable, those workers who were older than the median age were considered to be exposed. Color or race was stratified into &#8220;non-black&#8221;, which included White, Brown, Asian and Indigenous, classified as non-exposed, whereas Black individuals were classified as exposed. As for schooling, the exposed ones were those who had not completed High School. For marital status, those who had a stable partner were exposed, even if they were not formally married.</p>
          <p>The variable &#8220;type of workday&#8221; consists of rotating shifts (exposed groups) and business hours (non-exposed groups). For the psychosocial aspects of work, psychological demand and job control questions, from the JCQ, were used. Exposed individuals were those who had high demand and low control over their work. For the time of service variable, those exposed were the ones with values above the median.</p>
          <p>An individual who smokes or used to smoke was considered exposed. For &#8220;alcohol use frequency&#8221;, those who consumed alcohol more than once a week were classified as exposed.</p>
          <p>The anthropometric indicator used to verify the association with HBP in the multivariate analysis was Body Mass Index (BMI), calculated from subjects&#8217; direct height and weight measurements. Excess weight was considered present above the cut-off point of &#8805; 25 kg/m<xref ref-type="bibr" rid="B2">2</xref>, and those were the exposed ones, including, therefore, those who were overweight and obese.<xref ref-type="bibr" rid="B16">16</xref></p>
        </sec>
        <sec>
          <title>Statistical analysis</title>
          <p>All variables went through a descriptive analysis and their frequencies were verified. For continuous variables, measures of central tendency, dispersion and position were provided. The HBP prevalence ratio, according to the independent variables, were was initially calculated in the univariate analysis. In the multivariate analysis, the covariables entered the block models, and PA was considered as a main independent variable with three strata: active in the three forms, active in two forms, and inactive or active in one form. The reference group consisted of active individuals in the three PA forms. The selection of covariables for entry into the multivariate models was based on the theoretical and biological plausibility, and on the gross association strength of the univariate analysis. The Cox regression model was used for cross-sectional studies.<xref ref-type="bibr" rid="B17">17</xref> Interaction was tested using an additive model of the three PA forms for the occurrence of HBP.</p>
          <p>The study population, consisting of a pool of surveys, is not random. Thus, the analyses did not use statistical inference; on the contrary, the final model in the multivariate analysis is presented with all the selected variables, with their respective association measures (Prevalence Ratios). For this reason, they are all kept in the final model. Thus, in this study, procedures compatible with the non-random nature of the population investigated were used, according to an extensive literature on the subject.<xref ref-type="bibr" rid="B18">18</xref>,<xref ref-type="bibr" rid="B19">19</xref> All analyses were done using the Statistical Package for the Social Sciences (SPSS) software, version 24.0.</p>

          <title>Biases</title>
          <p>In order to minimize the effect of healthy workers, selected workers who were on medical leave were invited to participate, except when the leave was maternity-related or was due to an injury allegedly unrelated to occupational exposure. In this case, the next one on the list would be selected instead. Information biases were minimized with clarifications being provided to workers. The surveys would be under the responsibility of the Federal University of Bahia, so that it would be impossible for both companies and managers to access individual information.</p>

        <p>A total of 1,070 workers were studied, of which 842 (78.7%) were men. The prevalence of HBP was 24%. Workers who did not complete High School made up 46% of the population, 82% had stable partners, 63% had daily working hours with shift schedules. Those who smoke or have smoked accounted for 26%, and 42% of the population consumed alcohol more than once a week. Overweight was present in 43% of the subjects. In PA variables, 47% and 61% of the population had low PA-O and PA-L levels, respectively; 28% were not involved in PA-D. Age and BMI had the greatest factor associated with HBP in the univariate analysis (PR = 2.9 and PR = 2.8, respectively). The following also had strong association: gender (PR = 1.95), PA-D (PR = 1.62) and type of workday (PR = 1.60) (<xref ref-type="table" rid="t1001">Table 1</xref>).</p>

The frequency of HBP for each one of the PA strata was: 16% for individuals who were active in the three forms, 39% for those who were active in two forms, and 45% for individuals who were inactive or active in only one form (data not shown).</p>
        <p><xref ref-type="table" rid="t2001">Table 2</xref> shows the multivariate analysis, with the main independent variable, PA, in its combinations. Model 1 shows the gross association between PA and HBP. In model 2, the main independent variable is adjusted by sociodemographic variables. And in model 3, an additional adjustment is made for lifestyle variables, when an adjustment of 11% in the main association is observed. The entry of occupational variables in model 4 practically did not change the PR of the main association. Active individuals in two of the three forms, and inactive or active individuals in only one form of PA had a PR of 1.25 and 1.62, respectively. Overweight (PR: 2.26), age &gt; 31 years (PR: 2.10) and male gender (PR: 1.62) covariables maintained their association with HBP in the final model. No interaction was found in the additive model between the three forms of PA for the occurrence of HBP.</p>

        <title>Discussion</title>
        <p>PA accumulation in PA-O, PA-D and PA-L forms can lead to HBP improvements. Being active in one or none of the three forms of PA demonstrated a greater positive association with the outcome: 62% more HBP compared to those who are active in the three forms. These findings contribute to the literature on the role of insufficient PA as a modifiable risk factor for HBP and calls into question the isolated role of PA-L as a strategy to control this condition.</p>
        <p>Regarding color or race, the strong miscegenation of the population in Bahia can represent a limitation to investigate its association with HBP. As described by other authors,<xref ref-type="bibr" rid="B20">20</xref> the probable homogenization of ethnical groups, due to strong miscegenation, should play a role in the result, which revealed an almost equal prevalence of HBP among the &#8220;black&#8221; and &#8220;non-black&#8221; groups in this population.<xref ref-type="bibr" rid="B21">21</xref>,<xref ref-type="bibr" rid="B22">22</xref> According to this explanatory hypothesis, a multicenter HBP study which, in addition to including servers from the Federal University of Bahia also included servers from five other institutions in different states in Brazil,<xref ref-type="bibr" rid="B23">23</xref> found a strong association of this variable with HBP, whose prevalence was higher among Black people, followed by Brown people, with the lowest prevalence among Caucasians.</p>
        <p>The association of HBP with age and gender was very consistent with the literature.<xref ref-type="bibr" rid="B11">11</xref>,<xref ref-type="bibr" rid="B23">23</xref>-<xref ref-type="bibr" rid="B26">26</xref> The studied population was composed of 79% young men, with a mean age of 32 years. Men in this stage of life are seen to have higher prevalence of HBP than women.<xref ref-type="bibr" rid="B1">1</xref> Studies that show a higher prevalence among women are based on self-report or populations in higher age groups, in which the female gender shows higher values than the male gender with greater prevalence.<xref ref-type="bibr" rid="B27">27</xref> As for studies based on self-reporting,<xref ref-type="bibr" rid="B1">1</xref> the predominance of HBP among women may be due to the fact that women are more likely than men to seek out health care and, consequently, greater knowledge of their hypertensive condition is available.</p>
        <p>In Brazil, in 2016, the prevalence of HBP was increasing with age, and among those aged between 35 and 44 years, the prevalence was 19.1%.<xref ref-type="bibr" rid="B1">1</xref> In a comparison with the results of this study, a prevalence of 37% was found in the age group of 35 to 44 years; therefore, among those studied here, HBP was higher than the national average. In comparison with Chor et al.,<xref ref-type="bibr" rid="B23">23</xref> who studied university servers, the age strata of this study with workers had higher prevalence rates.</p>
        <p>Overweight workers had 2.3 times the prevalence of HBP compared to those who were not overweight. This result is consistent with other studies, demonstrating the association of HBP with greater body density.<xref ref-type="bibr" rid="B28">28</xref>,<xref ref-type="bibr" rid="B29">29</xref> The other variables that were associated in the univariate stage had their associations with HBP greatly reduced in the multivariate analysis.</p>
        <p>The main independent variable was consistently associated with HBP in the multivariate analysis, showing a cumulative effect of low PA in the three forms regarding the prevalence of HBP.</p>
        <p>The independent association between occupational physical activity (PA-O) and HBP was explored through analyses that included PA-O, both with a duration scale and an intensity scale of load handling. The most active workers in this form were more effectively protected against HBP, even with age, gender and overweight adjustments. Motivation for these analyses with PA-O levels was based on the recent literature discussion about the &#8220;paradoxical effect&#8221; of PA-O on blood pressure.<xref ref-type="bibr" rid="B30">30</xref> According to researchers, individuals subjected to high PA-O levels would have a higher risk of HBP.<xref ref-type="bibr" rid="B14">14</xref>,<xref ref-type="bibr" rid="B31">31</xref> It is possible that occupations that demand the use of large muscle groups, such as garbage collection, although they may exhibit physical fatigue, do not seem to show association with HA, as proposed in the paradoxical effect. In this study, active work was always protective against this condition, particularly when associated with PA-D and PA-L.</p>
        <p>Although PA-L is deemed to be one of the main non-pharmacological treatments for HBP,<xref ref-type="bibr" rid="B32">32</xref>-<xref ref-type="bibr" rid="B34">34</xref> according to the present study, PA-L alone may not compensate for insufficient PA for the purposes of effective HBP control. Being active in only one form and inactive in the other two forms showed a positive association with HBP.</p>
        <p>Few studies explore PA-D alone. The data on this form of PA do not seem to show an association with HBP. Nevertheless, when combined with the practice of PA in other forms, PA-D can produce beneficial effects on HBP control.</p>
        <p>Although the present study showed the cumulative effect of low PA in its different forms as a risk factor for HBP, no interaction was found between these forms (not even in the additive model). A similar fact was observed in another study, which also revealed a cumulative effect of different forms of PA, but no interaction among them.<xref ref-type="bibr" rid="B7">7</xref> Interaction analysis to explore sedentary behavior and/or time spent in front of the TV in determining chronic diseases in adults seems to be a promising approach.<xref ref-type="bibr" rid="B35">35</xref></p>
        <p>In addition to PA accumulation in the different forms being a protective factor, accumulation during the various stages of life can further improve health indicators.<xref ref-type="bibr" rid="B36">36</xref></p>
        <p>In this study, PA levels were obtained from self-report and not measured directly through cardiac monitors, accelerometers, pedometers and frequency meters. Despite the tendency to consider direct measures more valid than self-reporting, the first have been questioned as the gold standard in Occupational Epidemiology. Due to their cost and instrument calibration requirements, direct measurements are usually obtained from short time sampling and, in the case of PA-O, during a small sample of the workday schedule. This is particularly relevant in studies with populations whose occupational activities involve high variability over the course of a workday, since this factor may limit the validity of direct measurements. In other words, direct measurements that sample a small proportion of the workday may not be representative of the whole working day.<xref ref-type="bibr" rid="B37">37</xref></p>
        <p>In the cross-sectional study design, the main bias is the prevalence. Therefore, reverse causality cannot be ruled out, i.e., it is not possible to determine whether the higher prevalence of HBP among less active individuals is due to a sedentary life or whether they were already hypertensive before moving on to a more sedentary life. However, since HBP is a chronic disease, treatment can ensure its control, but not its cure. Thus, this type of bias can be minimized, and cross-section may be an appropriate option for study design. The study population is predominantly composed of young workers, but the results obtained and described for the prevalence of HBP among the different age groups allow for comparisons with other populations, provided that each age group is observed.</p>

        <title>Conclusion</title>
        <p>This study showed that lower activity levels in the different forms of PA was a risk factor associated with HBP in this population. Since other studies demonstrate a similar line of evidence, the results of this study can be considered valid. From these studies, it was considered that the investigation of PA, a modifiable factor in the occurrence of HBP, cannot waive its different forms, otherwise it would imply a methodological error when classifying subjects who are active at work or at home as inactive individuals, based on the form of leisure. Interventions should be planned in the occupational environment, so workers have the opportunity to express what HBP means for themselves, and get access to study results in order to stimulate changes in habits and to increase the role of workers in electing active leisure activities and highlighting the importance of PA-O. New research should enhance the study of PA in order to bring new evidence on its role in HBP.</p>
