        <sec>
          <title>Fundamentos</title>
          <p> A resist&#234;ncia &#224; insulina (RI) &#233; um dist&#250;rbio importante em crian&#231;as obesas, pois est&#225; intimamente relacionado a doen&#231;as cardiovasculares. O tecido adiposo epic&#225;rdico (TAE) desempenha um papel no desenvolvimento da RI devido a mol&#233;culas bioativas secretadas, sendo que o processo inflamat&#243;rio dessas mol&#233;culas pode causar atraso eletromec&#226;nico atrial (AEA).</p>

          <title>Objetivo</title>
          <p> O objetivo do nosso estudo foi determinar a rela&#231;&#227;o entre o TAE e o AEA com a RI em crian&#231;as obesas.</p>
        </sec>
        <sec>
          <title>M&#233;todos</title>
          <p> O estudo incluiu 94 pacientes obesos. A IR foi calculada usando o Modelo de Avalia&#231;&#227;o da Homeostase da Resist&#234;ncia &#224; Insulina (HOMA-IR) e definida como HOMA-IR maior que o percentil 90 em uma curva de percentil espec&#237;fica para idade e sexo. Os pacientes foram divididos em dois grupos de acordo com sua RI. Todos os pacientes foram submetidos a exames ecocardiogr&#225;ficos. A signific&#226;ncia estat&#237;stica foi estabelecida como valor de &lt; 0,05 bicaudal.</p>
        </sec>
        <sec>
          <title>Resultados</title>
          <p> A TAE encontrava-se significativamente maior no grupo RI (p &lt; 0,001). O valor de corte ideal para que o TAE previsse a RI foi &gt; 3,85 mm, com especificidade de 92,5% e sensibilidade de 68,5% (p = 0,002). No modelo de regress&#227;o log&#237;stica multivariada, o TAE (OR = 1.256, IC de 95%: 1.016&#8211;1.53, p = 0.035) esteve associado &#224; RI ap&#243;s o ajuste para as vari&#225;veis estatisticamente significativas na an&#225;lise univariada. O AEA inter e intra-atrial mostrou-se significativamente prolongado no grupo RI em compara&#231;&#227;o com o grupo sem RI (p &lt; 0,010; p = 0,032, respectivamente).</p>
        </sec>
        <sec>
          <title>Conclus&#227;o</title>
          <p> No nosso estudo, revelamos que o TAE esteve positivamente correlacionada com a RI e foi preditor independente de RI. (Arq Bras Cardiol. 2020; [online].ahead print, PP.0-0)</p>

        <title>Popula&#231;&#227;o de estudo</title>
        <p>Para este estudo prospectivo e transversal, foram inclu&#237;dos 94 pacientes obesos com idades entre 8 e 18 anos, admitidos no ambulat&#243;rio de Endocrinologia Pedi&#225;trica da Universidade Kahramanmara&#351; S&#252;t&#231;&#252;imam entre agosto de 2018 e fevereiro de 2019. A equipe de enfermagem do ambulat&#243;rio realizou todas as medidas antropom&#233;tricas, como peso e altura, com os pacientes vestindo apenas roupas &#237;ntimas. O &#237;ndice de massa corporal (IMC) foi calculado dividindo-se o peso em quilogramas pela altura ao quadrado em metros. A obesidade foi definida como IMC maior que o percentil 95 em uma curva de percentil espec&#237;fica para idade e sexo. Valores acima do percentil 99 foram definidos como obesidade m&#243;rbida.<xref ref-type="bibr" rid="B19">19</xref></p>
        <p>A resist&#234;ncia &#224; insulina de todos os pacientes foi calculada e categorizada com base na resist&#234;ncia &#224; insulina. A resist&#234;ncia &#224; insulina foi calculada usando o Modelo de Avalia&#231;&#227;o da Homeostase da Resist&#234;ncia &#224; Insulina (HOMA-IR) (glicose plasm&#225;tica em jejum (mmol/L) &#215; insulina plasm&#225;tica em jejum (mU/L)/22,5) e definida como HOMA-IR maior que o percentil 90 em curva percentil espec&#237;fica para idade e sexo.<xref ref-type="bibr" rid="B20">20</xref></p>
        <p>Pacientes com DM, S&#237;ndrome de Cushing, resist&#234;ncia &#224; insulina conhecida; os que usam medicamentos para resist&#234;ncia &#224; insulina; os que apresentam hipoglicemia; com dist&#250;rbios metab&#243;licos cardiovasculares e hep&#225;ticos conhecidos; e aqueles com janelas ac&#250;sticas ruins para ecocardiografia foram exclu&#237;dos. Os dados demogr&#225;ficos e laboratoriais dos pacientes foram registrados. Todos os pacientes foram submetidos a ecocardiografia transtor&#225;cica padr&#227;o, incluindo Doppler tecidual e exames ecocardiogr&#225;ficos realizados por um cardiologista especializado. O mesmo cardiologista avaliou os resultados de ETT pr&#233;-alta de 20 pacientes selecionados aleatoriamente para avaliar a reprodutibilidade da espessura do TAE e dos par&#226;metros do Doppler tecidual para o atraso eletromec&#226;nico atrial. Utilizando o m&#233;todo de Bland-Altman, a diferen&#231;a m&#233;dia em termos de intraobserva&#231;&#227;o foi de 3,8% (0,23 &#177; 0,54%), indicando boa reprodutibilidade.</p>

        <title>Ecocardiograma</title>
        <p>Os exames ecocardiogr&#225;ficos transtor&#225;cicos foram realizados por ecocardiografistas especializados, cegos &#224;s informa&#231;&#245;es cl&#237;nicas dos pacientes, com o sistema de ultrassonografia card&#237;aca Vivid 7&#174; (GE VingMed Ultrasound AS; Horten, Noruega) usando sondas de 2,5 a 5 MHz. As imagens ecocardiogr&#225;ficas foram realizadas nas posi&#231;&#245;es lateral e supina esquerda. Os exames ecocardiogr&#225;ficos Doppler 2D, modo M, pulsado e fluxo colorido foram feitos em todos os pacientes. Eixos paraesternais longos e curtos, janelas apicais e subcostais foram utilizadas para obter tra&#231;ados Doppler e imagens bidimensionais. Foram quantificados os di&#226;metros atriais esquerdo e direito, os di&#226;metros sist&#243;lico final e diast&#243;lico final do ventr&#237;culo esquerdo, bem como as espessuras das paredes posterior e septal do ventr&#237;culo esquerdo na di&#225;stole. Os volumes do &#225;trio esquerdo foram quantificados com a fra&#231;&#227;o de eje&#231;&#227;o (FE) do VE pelo m&#233;todo disco com a regra de Simpson. A fun&#231;&#227;o diast&#243;lica VE foi quantificada pelas velocidades do fluxo transmitral, ou seja, E de pico (diast&#243;lica inicial), A de pico (diast&#243;lica final), raz&#227;o E/A, bem como o tempo de desacelera&#231;&#227;o da onda E (TD) e tempo de relaxamento isovolum&#233;trico (TRIV).</p>
        <p>A avalia&#231;&#227;o ecocardiogr&#225;fica e a quantifica&#231;&#227;o da gordura epic&#225;rdica foram realizadas atrav&#233;s da identifica&#231;&#227;o do espa&#231;o livre de eco entre o revestimento externo do mioc&#225;rdio e a camada visceral do peric&#225;rdio. Sua mensura&#231;&#227;o foi realizada perpendicularmente &#224; parede livre do ventr&#237;culo direito na janela paraesternal do eixo longo. O n&#237;vel da medida foi no ventr&#237;culo m&#233;dio e o tempo foi ajustado para a di&#225;stole final, com uma m&#233;dia de tr&#234;s ciclos card&#237;acos. Para alinhar o feixe de ultrassom perpendicularmente &#224; parede livre do ventr&#237;culo direito, o anel a&#243;rtico foi aceito como marco anat&#244;mico.<xref ref-type="bibr" rid="B21">21</xref></p>

        <p>Todas as an&#225;lises estat&#237;sticas foram realizadas com o software SPSS 14 (SPSS Inc., Chicago, IL, USA). A signific&#226;ncia estat&#237;stica foi estabelecida como valor de &lt; 0,05 bicaudal. As vari&#225;veis categ&#243;ricas foram expressas por n&#250;mero e percentuais, e as cont&#237;nuas como m&#233;dia &#177; desvio padr&#227;o (DP) ou mediana e intervalos interquart&#237;licos (IIQ), conforme sua normalidade de distribui&#231;&#227;o. A suposi&#231;&#227;o de normalidade dos dados foi determinada pelo teste de Kolmogorov Smirnov. O teste t da amostra independente e o teste U de Mann-Whitney foram usados para comparar os dados entre os grupos. O teste do qui-quadrado foi usado para compara&#231;&#227;o das vari&#225;veis categ&#243;ricas. As an&#225;lises de correla&#231;&#227;o foram realizadas usando o teste de correla&#231;&#227;o de Pearson no caso de vari&#225;veis normalmente distribu&#237;das e o teste de correla&#231;&#227;o de Spearman para vari&#225;veis n&#227;o-normalmente distribu&#237;das. Determinou-se o ponto de corte ideal para que o TAE previsse a RI usando a an&#225;lise da curva caracter&#237;stica operador-receptor (ROC) MedCalc (v12.7.8). A an&#225;lise foi feita determinando-se a &#225;rea sob a curva (AUC) com intervalo de confian&#231;a de 95%. O melhor valor de corte para o TAE foi determinado pelo c&#225;lculo da maior soma de sensibilidade e especificidade-1. As vari&#225;veis RI e outras vari&#225;veis dispon&#237;veis foram analisadas quanto &#224; correla&#231;&#227;o pela an&#225;lise univariada. Vari&#225;veis com correla&#231;&#227;o significativa na an&#225;lise univariada foram inseridas no modelo de regress&#227;o log&#237;stica multivariada com o m&#233;todo backward stepwise , juntamente com outros fatores de confus&#227;o em potencial para determinar preditores independentes de RI.</p>

      <title>Resultados</title>
      <p>Os pacientes inscritos foram agrupados em dois grupos com base na presen&#231;a de resist&#234;ncia &#224; insulina. Quarenta pacientes apresentaram resist&#234;ncia &#224; insulina e 54 pacientes n&#227;o apresentaram resist&#234;ncia. Ambos os grupos tiveram distribui&#231;&#227;o semelhante por idade e sexo. (p = 0,102, p = 0,069, respectivamente). Entre as medidas antropom&#233;tricas, peso, altura e IMC mostraram-se significativamente maiores em pacientes com resist&#234;ncia &#224; insulina. Al&#233;m disso, as medidas de press&#227;o arterial diast&#243;lica e sist&#243;lica estiveram significativamente maiores no grupo com RI. Uma compara&#231;&#227;o dos par&#226;metros laboratoriais revelou que o grupo com RI apresentava n&#237;veis s&#233;ricos de insulina e glicose significativamente mais elevados (p &lt; 0,001, p = 0,002, respectivamente). Os demais par&#226;metros laboratoriais foram semelhantes entre os grupos ( <xref ref-type="table" rid="t1">Tabela 1</xref> ). Entre as medidas ecocardiogr&#225;ficas padr&#227;o, a espessura do TAE estava significativamente elevada no grupo com RI (p = 0,004). Outros par&#226;metros ecocardiogr&#225;ficos e laboratoriais padr&#227;o foram semelhantes entre os dois grupos. A <xref ref-type="table" rid="t2">Tabela 2</xref> apresenta atrasos eletromec&#226;nicos atriais registrados a partir de diferentes segmentos anulares. A PA lateral e septal esteve significativamente mais elevada no grupo com RI (62,2 &#177; 8,3 vs. 56,6 &#177; 8,4, p = 0,002; 46,1 &#177; 6,1 vs. 42,7 &#177; 5,9, p = 0,019, respectivamente). A PA tric&#250;spide mostrou-se semelhante entre os grupos. AEA inter e intra-atrial encontrava-se significativamente prolongado no grupo com RI em compara&#231;&#227;o com o grupo sem resist&#234;ncia &#224; insulina (23 (18&#8211;30) vs. 19,5(15&#8211;23,5), p &lt; 0,010; 9,5 (6,2&#8211;10,0) vs. 6 (4&#8211;9,2), p =0,032 respectivamente).</p>

      <title>Discuss&#227;o</title>
      <p>O presente estudo investigou a rela&#231;&#227;o entre tecido adiposo epic&#225;rdico e resist&#234;ncia &#224; insulina em crian&#231;as obesas. Demonstrou-se que o tecido adiposo epic&#225;rdico est&#225; positivamente correlacionado &#224; RI, sendo preditor independente da RI.</p>
      <p>A RI denota uma condi&#231;&#227;o de insensibilidade relativa dos tecidos perif&#233;ricos (por exemplo: m&#250;sculo, f&#237;gado e tecido adiposo) aos efeitos do horm&#244;nio. A RI desempenha um papel fundamental no desenvolvimento e progress&#227;o de fatores de risco cardiometab&#243;licos que, em associa&#231;&#227;o &#224; obesidade, devido aos efeitos lipol&#237;ticos dos adip&#243;citos, levam a grandes quantidades de &#225;cidos graxos livres e a altera&#231;&#245;es na secre&#231;&#227;o de adipocinas, ambas envolvidas na modula&#231;&#227;o de sensibilidade &#224; insulina.<xref ref-type="bibr" rid="B20">20</xref> - <xref ref-type="bibr" rid="B22">22</xref> Embora a preval&#234;ncia de RI seja vari&#225;vel entre os obesos, Gabato et al. encontraram o &#237;ndice de 29,1% em seu estudo.<xref ref-type="bibr" rid="B23">23</xref> Em diversos outros estudos, esse &#237;ndice mostrou-se superior a 50%.<xref ref-type="bibr" rid="B24">24</xref> - <xref ref-type="bibr" rid="B27">27</xref> Em nosso estudo, a taxa de RI foi de 43%. O motivo dessa diferencia&#231;&#227;o pode ser explicado pelo uso do valor constante do HOMA-IR em outros estudos, mas, em nosso estudo, usamos os valores percentuais do HOMA-IR de acordo com a idade e o sexo.</p>
      <p>O HOMA-IR &#233; uma estimativa aproximada da RI, com base na rela&#231;&#227;o entre glicemia de jejum e n&#237;veis de insulina, com valores mais elevados de HOMA-IR representando uma RI mais severa.<xref ref-type="bibr" rid="B10">10</xref> Valores mais elevados de RI e HOMA-IR aumentam o risco cardiometab&#243;lico. N&#227;o h&#225; evid&#234;ncias de associa&#231;&#227;o entre medidas de RI e FA incidente.<xref ref-type="bibr" rid="B28">28</xref> Muitos estudos mostraram que a RI est&#225; intimamente relacionada &#224;s fun&#231;&#245;es atriais.<xref ref-type="bibr" rid="B29">29</xref> , <xref ref-type="bibr" rid="B30">30</xref> Em nosso estudo, observou-se que os valores de HOMA-IR se correlacionaram positivamente com os par&#226;metros do Doppler tecidual atrial, indicando fun&#231;&#227;o atrial em crian&#231;as obesas e eleva&#231;&#227;o da condu&#231;&#227;o atrial tecidual no grupo com RI.</p>
      <p>A obesidade causa prolongamento do tempo de condu&#231;&#227;o eletromec&#226;nica por diversos mecanismos, como inflama&#231;&#227;o adiposa na parede atrial, aumento da atividade do sistema nervoso simp&#225;tico, aumento do processo inflamat&#243;rio, desregula&#231;&#227;o da adipocinina e ativa&#231;&#227;o de vias de sinaliza&#231;&#227;o pr&#243;-fibr&#243;tica. Demonstrou-se que o prolongamento da condu&#231;&#227;o eletromec&#226;nica &#233; propenso a fibrila&#231;&#227;o atrial.<xref ref-type="bibr" rid="B31">31</xref> A RI, frequentemente associada &#224; obesidade, afeta as fun&#231;&#245;es atriais devido &#224; inflama&#231;&#227;o subcl&#237;nica existente. Em nosso estudo, tanto o tempo de condu&#231;&#227;o intra-atrial quanto o tempo de condu&#231;&#227;o interatrial estiveram maiores em crian&#231;as obesas com resist&#234;ncia &#224; insulina, de acordo com a literatura, em compara&#231;&#227;o com o grupo sem RI. Isso pode ser explicado pelo processo inflamat&#243;rio associado &#224; resist&#234;ncia &#224; insulina e pelo atraso na transmiss&#227;o desse processo inflamat&#243;rio no tecido atrial. &#192; luz dessas informa&#231;&#245;es, pode-se dizer que crian&#231;as obesas com resist&#234;ncia &#224; insulina podem ser mais propensas a fibrila&#231;&#227;o atrial.</p>
      <p>A gordura epic&#225;rdica &#233; um ac&#250;mulo visceral de gordura que apresenta a maioria das propriedades fisiopatol&#243;gicas de outros tecidos adiposos viscerais, como deposi&#231;&#227;o lip&#237;dica e libera&#231;&#227;o de horm&#244;nios, citocinas e quimiocinas, causando tamb&#233;m inflama&#231;&#227;o local.<xref ref-type="bibr" rid="B32">32</xref> - <xref ref-type="bibr" rid="B35">35</xref> Demonstrou-se que a distribui&#231;&#227;o de gordura corporal, particularmente a distribui&#231;&#227;o de gordura abdominal, est&#225; correlacionada com o tecido adiposo epic&#225;rdico.<xref ref-type="bibr" rid="B33">33</xref> Portanto, a rela&#231;&#227;o entre a espessura do tecido adiposo epic&#225;rdico obtida na ecocardiografia e diversas condi&#231;&#245;es patol&#243;gicas, como a s&#237;ndrome metab&#243;lica, doen&#231;a arterial coronariana, hiperlipidemia, eleva&#231;&#227;o da press&#227;o arterial e RI foram estudados em pacientes obesos adultos e pedi&#225;tricos. O tecido adiposo epic&#225;rdico causa o desenvolvimento e/ou agravamento da RI, aumentando os &#225;cidos graxos livres, TNF, IL1, IL6, causando libera&#231;&#227;o de resistina e diminui&#231;&#227;o dos n&#237;veis de adiponectina.<xref ref-type="bibr" rid="B36">36</xref> Diversos estudos examinaram a rela&#231;&#227;o entre o tecido adiposo epic&#225;rdico e a RI, demonstrando uma correla&#231;&#227;o entre tecido adiposo epic&#225;rdico e a IMC em adultos obesos.<xref ref-type="bibr" rid="B37">37</xref> - <xref ref-type="bibr" rid="B39">39</xref> Abac&#305; et al.<xref ref-type="bibr" rid="B40">40</xref> mostraram correla&#231;&#227;o significativa com o tecido adiposo epic&#225;rdico em crian&#231;as obesas. <xref ref-type="bibr" rid="B40">40</xref> De acordo com a literatura dispon&#237;vel, nosso estudo demonstrou uma correla&#231;&#227;o entre o IMC e o tecido adiposo epic&#225;rdico. Ishorbagy et al.<xref ref-type="bibr" rid="B41">41</xref> relataram tecido adiposo epic&#225;rdico maior em pacientes obesos em compara&#231;&#227;o com controles saud&#225;veis, embora n&#227;o tenham apresentado predi&#231;&#227;o da s&#237;ndrome metab&#243;lica.<xref ref-type="bibr" rid="B41">41</xref> Da mesma forma, Abac&#305; et al.<xref ref-type="bibr" rid="B40">40</xref> sugeriram que o tecido adiposo epic&#225;rdico n&#227;o conseguiu prever a RI em crian&#231;as obesas.<xref ref-type="bibr" rid="B40">40</xref> Por outro lado, descobrimos que o tecido adiposo epic&#225;rdico era preditor independente de RI. A causa dessa discrep&#226;ncia &#233; que nosso estudo foi um estudo de caso-controle aninhado que incluiu apenas pacientes em vez de controles saud&#225;veis. Outro motivo importante foi que, em nosso estudo, a RI foi medida utilizando-se percentis determinados de acordo com idade e sexo.</p>
      <p>A rela&#231;&#227;o entre press&#227;o arterial e RI foi demonstrada em muitos estudos.<xref ref-type="bibr" rid="B42">42</xref> , <xref ref-type="bibr" rid="B43">43</xref> Em nosso estudo, descobrimos que a press&#227;o arterial sist&#243;lica &#233; preditor independente de RI. Isso pode ser devido ao aumento do tecido adiposo no corpo, que desempenha um papel importante na RI e na inflama&#231;&#227;o subcl&#237;nica causada por citocinas inflamat&#243;rias, como IL-6, IL-1 e TFN-alfa, secretadas nesse tecido adiposo. A inflama&#231;&#227;o subcl&#237;nica pode prejudicar a fun&#231;&#227;o endotelial e aumentar a press&#227;o sangu&#237;nea, diminuindo a libera&#231;&#227;o de NO. Outro mecanismo poss&#237;vel &#233; que pode haver ativa&#231;&#227;o simp&#225;tica do processo inflamat&#243;rio da obesidade e relacionado &#224; obesidade.<xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B44">44</xref> - <xref ref-type="bibr" rid="B46">46</xref></p>
      <p>Nosso estudo teve algumas limita&#231;&#245;es: sua principal limita&#231;&#227;o foi o tamanho relativamente pequeno da amostra. O TAE ecocardiogr&#225;fico &#233; uma medida linear e, portanto, pode n&#227;o avaliar o volume total de gordura epic&#225;rdica que varia em v&#225;rios locais do mioc&#225;rdio. Como o TAE &#233; um tecido metabolicamente ativo, citocinas inflamat&#243;rias e marcadores inflamat&#243;rios podem ser investigados em estudos futuros. A aus&#234;ncia de medi&#231;&#227;o da circunfer&#234;ncia abdominal foi outra limita&#231;&#227;o, impedindo a determina&#231;&#227;o de uma rela&#231;&#227;o entre a circunfer&#234;ncia abdominal e gordura epic&#225;rdica.</p>
    </sec>
    <sec sec-type="conclusions">
      <title>Conclus&#227;o</title>
      <p>Em conclus&#227;o, o tecido adiposo epic&#225;rdico &#233; um par&#226;metro barato e de f&#225;cil acesso, que pode ser facilmente medido pela ecocardiografia, e usado para identificar a resist&#234;ncia &#224; insulina em crian&#231;as. Como o atraso eletromec&#226;nico atrial mostrou-se elevado em crian&#231;as obesas com resist&#234;ncia &#224; insulina, ele deve ser acompanhado de perto para avaliar a fibrila&#231;&#227;o atrial.</p>

        <title>Abstract</title>
        <sec>
          <title>Background</title>
          <p>Insulin resistance (IR) is an important disorder in obese children because it is closely related to cardiovascular diseases. Epicardial adipose tissue (EAT) plays a role in the development of IR due to secreted bioactive molecules, and the inflammatory process of these molecules may cause atrial electromechanical delay (EMD).</p>
        </sec>
        <sec>
          <title>Objective</title>
          <p>The objective of our study was to determine the relationship between EAT and EMD with IR in obese children.</p>
        </sec>
        <sec>
          <title>Methods</title>
          <p>Ninety-four obese patients were included in the study. IR was calculated using the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) and defined as HOMA-IR greater than the 90thpercentile in an age- and sex-specific percentile curve. Patients were divided into two groups according to their IR. All patients underwent echocardiographic examinations. Statistical significance was set to a two-sided p-value &lt; 0.05.</p>
        </sec>
        <sec>
          <title>Results</title>
          <p>EAT was significantly higher in the IR group (p &lt; 0.001). The optimal cut-off value for EAT to predict IR was found to be &gt; 3.85 mm, with 92.5% specificity and 68.5% sensitivity (p = 0.002). In the multivariate logistic regression model, EAT (OR = 1.256, 95% CI: 1.016&#8211;1.53, p = 0.035) was also associated with IR after adjustment for variables found to be statistically significant in univariate analysis. Inter- and intra-atrial EMD was significantly prolonged in the IR group compared to the group without IR (p &lt; 0.010; p = 0.032 respectively). Conclusion: In our study, we revealed that EAT was positively correlated with IR and was an independent predictor of IR. (Arq Bras Cardiol. 2020; [online].ahead print, PP.0-0)</p>

        <title>Introduction</title>
        <p>Obesity is a major health problem worldwide due to its growing prevalence and early development in life.<xref ref-type="bibr" rid="B1">1</xref> - <xref ref-type="bibr" rid="B3">3</xref> The number of overweight people tends to progressively increase in both developed and developing countries, and the proportion of obese people is around one third of the normal adolescents population.<xref ref-type="bibr" rid="B4">4</xref> - <xref ref-type="bibr" rid="B6">6</xref> As a result, complications of obesity, such as metabolic syndrome, type 2 diabetes mellitus (DM), cardiovascular disorders, respiratory disorders, and psychosocial problems tend to increase.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref></p>
        <p>Obesity is typically associated with insulin resistance (IR) and glucose metabolism disorders. Adipose tissue stored in subcutaneous and visceral tissues plays an important role in the development of IR via the active proteins it secretes.<xref ref-type="bibr" rid="B9">9</xref> The distribution of this adipose tissue is equally important, with intra-abdominal fat accumulation being closely linked to IR.<xref ref-type="bibr" rid="B10">10</xref> Additionally, it is already known that subcutaneous fat tissue is correlated to IR whether or not DM is present.<xref ref-type="bibr" rid="B11">11</xref> , <xref ref-type="bibr" rid="B12">12</xref> Recent studies have demonstrated that extra-abdominal visceral fat deposits like mediastinal and epicardial adipose tissue (EAT) are also related to IR.<xref ref-type="bibr" rid="B9">9</xref> , <xref ref-type="bibr" rid="B13">13</xref> , <xref ref-type="bibr" rid="B14">14</xref> The association between obesity-dependent insulin resistance and EAT has not been fully explained.</p>
        <p>Childhood obesity is an important risk factor for atrial fibrillation whereas structural remodeling is very important.<xref ref-type="bibr" rid="B15">15</xref> In many studies, this close relationship was investigated with electromechanical delay (EMD), which is one of these echocardiographic markers defined as the temporal delay between the detected onset of electrical activity and the realization of force in the myocardium. EMD is an indicator of atrial conduction heterogeneity and can also be obtained easily by tissue Doppler imaging (TDI).<xref ref-type="bibr" rid="B16">16</xref> In addition, it has been demonstrated that EMD is prolonged in diseases associated with insulin resistance.<xref ref-type="bibr" rid="B17">17</xref> , <xref ref-type="bibr" rid="B18">18</xref> However, the relationship between electromechanical delay and insulin resistance in obese patients has not been studied.</p>
        <p>The aim of our study was to determine the relationship between EAT and insulin resistance. In addition, the relationship between insulin resistance and electromechanical delay was investigated.</p>

        <title>Methods</title>
        <sec>
          <title>Study Population</title>
          <p>For this prospective and cross-sectional study, 94 obese patients aged 8&#8211;18 years admitted to the Kahramanmara&#351; S&#252;t&#231;&#252;imam University Pediatric Endocrinology outpatient clinic between August 2018 and February 2019 were included. An outpatient clinic nurse performed all anthropometric measurements, including weight and height with the patients wearing underwear only. Body mass index (BMI) was calculated by dividing body weight into kilograms by the square of height in meter. Obesity was defined as BMI greater than the 95thpercentile in an age- and sex-specific percentile curve. A value above the 99thpercentile was defined as morbid obesity.<xref ref-type="bibr" rid="B19">19</xref></p>
          <p>All patients&#8217; insulin resistance was calculated, and they were categorized on the basis of insulin resistance. Insulin resistance was calculated using the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) (fasting plasma glucose (mmol/L) &#215; fasting plasma insulin (mU/L)/22.5) and defined as HOMA-IR greater than the 90thpercentile in an age- and sex-specific percentile curve.<xref ref-type="bibr" rid="B20">20</xref></p>
          <p>Patients with DM, Cushing Syndrome, known insulin resistance; those using drugs for insulin resistance; those having hypoglycemia; those with known metabolic cardiovascular and hepatic disorders; and those with poor acoustic windows for echocardiography were excluded. Demographic and laboratory data of the patients were recorded. All patients underwent standard transthoracic echocardiography including tissue Doppler examination and echocardiographic examinations were performed by an expert cardiologist. The same cardiologist evaluated pre-discharge TTE results of 20 randomly selected patients to assess the reproducibility of EAT thickness and tissue Doppler parameters for atrial electromechanical delay. Using the Bland-Altman method, the mean difference in terms of intra-observation was 3.8% (0.23 &#177; 0.54%), indicating good reproducibility.</p>
        </sec>
        <sec>
          <title>Echocardiography</title>
          <p>Transthoracic echocardiographic examinations were performed by expert echocardiographers who were blind to the patients&#8217; clinical information with the Vivid 7&#174;cardiac ultrasonography system (GE VingMed Ultrasound AS; Horten, Norway) using 2.5- to 5-MHz probes. The echocardiographic images were taken in left lateral and supine positions and 2D, M-mode, pulsed, and color flow Doppler echocardiography examinations were performed in every patient. Parasternal long and short axes, apical and subcostal windows were used to obtain Doppler tracings and two-dimensional images. Left and right atrial diameters left ventricular end-systolic and end-diastolic diameters, as well as the posterior and septal wall thicknesses of the left ventricle at diastole, were quantified. Left atrial volumes were quantified with the disc method LV ejection fraction (EF) with the Simpson&#8217;s rule. LV diastolic function was quantified using mitral inflow velocities, i.e. peak E (early diastolic), peak A (late diastolic), E/A ratio as well as E-wave (DT) deceleration time and isovolumic relaxation time (IVRT).</p>
          <p>Echocardiographic assessment and quantification of epicardial fat were done by identifying the echo-free space between the outer lining of the myocardium and the visceral layer of pericardium. Its measurement was made perpendicular to the free wall of the right ventricle in the parasternal long-axis window. The measurement level was at the mid ventricle and the timing was set to end-diastole, with an average of 3 cardiac cycles being taken. To align the ultrasound beam perpendicular to the right ventricular free wall, aortic annulus was accepted as the anatomic landmark.<xref ref-type="bibr" rid="B21">21</xref></p>

          <title>Tissue Doppler Echocardiography (TDE)</title>
          <p>The pulsed Doppler sample volume was placed at the level of LV lateral mitral annulus, septal mitral annulus, and RV tricuspid annulus from an apical four-chamber view. The time interval from the onset of the P-wave on surface ECG to the beginning of the late diastolic wave (Am), which is called PA, was taken from the lateral mitral annulus (lateral PA), septal mitral annulus (septal PA), and RV tricuspid annulus (tricuspid PA). The difference between septal PA and tricuspid PA (septal PA &#8722; tricuspid PA) was identified as an intra-atrial electromechanical delay while the difference between lateral PA and tricuspid PA (lateral PA &#8722; tricuspid PA) was identified as inter-atrial electromechanical delay.<xref ref-type="bibr" rid="B16">16</xref></p>

          <title>Statistical Analysis</title>
          <p>All statistical analyses were performed using the SPSS version 14 (SPSS Inc., Chicago, IL, USA) software package. Statistical significance was set to a two-sided p-value &lt; 0.05. Categorical variables were expressed as number and percentage while continuous variables as mean&#177;standard deviation (SD) or median and interquartile ranges (IQR), depending on their normality of distribution. The normality assumption of the data was determined using the Kolmogorov Smirnov test. The independent sample t-test and Mann&#8211;Whitney U test were used to compare the groups&#8217; means. The Chi-square test was used to compare categorical data. Correlation analyses were performed using the Pearson correlation test for normally distributed variables and Spearman correlation test for non-normally distributed variables. An optimal cut-off point was determined for EAT to predict IR using the receiver operator characteristic (ROC) curve analysis MedCalc (v12.7.8). This was accomplished by determining the area under the curve (AUC) with 95% confidence interval. The best cutoff value for EAT was determined by calculating the highest sum of sensitivity and specificity-1. The IR and available variables were analyzed for correlation using the univariate analysis. Variables with significant correlation in the univariate analysis were entered in the multivariate logistic regression model using the backward stepwise method along with other potential confounders to determine independent predictors of IR.</p>

        <title>Results</title>
        <p>Enrolled patients were divided into two groups based on the presence of insulin resistance. Forty patients had insulin resistance and 54 patients did not. Both groups had similar age and gender distribution. (p = 0.102, p = 0.069, respectively). Among the anthropometric measurements, weight, height, and BMI were significantly greater in patients with insulin resistance. Additionally, diastolic and systolic blood pressure measurements were significantly greater in the IR group. A comparison of laboratory parameters revealed that the IR group had significantly higher serum insulin and glucose levels (p &lt; 0.001, p = 0.002, respectively). The other laboratory parameters were similar between the groups ( <xref ref-type="table" rid="t1001">Table 1</xref> ). Among standard echocardiographic measurements, EAT thickness was significantly increased in the group with IR (p = 0.004). Other standard echocardiographic and laboratory parameters were similar between the two groups. Atrial electromechanical delays recorded from different annular segments are given in <xref ref-type="table" rid="t2001">Table 2</xref> . Lateral and septal PA was significantly higher in the IR group (62.2 &#177; 8.3 vs. 56.6 &#177; 8.4, p = 0.002; 46.1 &#177; 6.1 vs. 42.7 &#177; 5.9, p = 0.019 respectively). Tricuspid PA was similar between the groups. Inter- and intra-atrial EMD was significantly prolonged in the IR group compared to the group without insulin resistance (23 (18&#8211;30) vs. 19.5 (15&#8211;23.5), p &lt; 0.010; 9.5 (6.2&#8211;10.0) vs. 6 (4&#8211;9.2), p = 0.032, respectively)</p>

        <title>Discussion</title>
        <p>This study investigated the relationship between epicardial adipose tissue and insulin resistance among obese children. It has been shown that epicardial adipose tissue is positively correlated to IR and an independent predictor of IR.</p>
        <p>IR denotes a condition of relative insensitivity of peripheral tissues (e.g. muscle, liver, and adipose tissue) to the effects of the hormone. IR plays a pivotal role in the development and progression of cardio-metabolic risk factors that, in association with obesity, due to lipolytic effects of adipocytes, leading to large amounts of free fatty acids and impaired secretion of adipokines, both involved in the modulation of insulin sensitivity.<xref ref-type="bibr" rid="B20">20</xref> - <xref ref-type="bibr" rid="B22">22</xref> Although the prevalence of IR is variable among obese patients, Gabato et al. reported it to be 29.1% in their study.<xref ref-type="bibr" rid="B23">23</xref> In many other studies, this rate has been shown to be over 50%.<xref ref-type="bibr" rid="B24">24</xref> - <xref ref-type="bibr" rid="B27">27</xref> In our study, the rate of IR was found to be 43%. The reason for this difference can be explained by the use of constant HOMA-IR value in other studies, but, in our study, we used HOMA-IR percentile values according to age and gender.</p>
        <p>The HOMA-IR is a proxy estimate of IR based upon the relationship between fasting glucose and insulin levels, with higher values of HOMA-IR representing more severe IR.<xref ref-type="bibr" rid="B10">10</xref> Increased IR and HOMA-IR values increase cardio-metabolic risk. There is no evidence of an association between IR measures and incident AF.<xref ref-type="bibr" rid="B28">28</xref> Many studies have shown that IR is closely related to atrial functions.<xref ref-type="bibr" rid="B29">29</xref> , <xref ref-type="bibr" rid="B30">30</xref> In our study, it was observed that HOMA-IR values were positively correlated with atrial tissue Doppler parameters which are indicative of atrial function in obese children and tissue atrial conduction was increased in the IR group.</p>
        <p>Obesity causes prolongation of electromechanical conduction time by many mechanisms such as fat inflammation on the atrial wall, increase in sympathetic nervous system activity, increased inflammatory process, adipokinin dysregulation and activation of pro-fibrotic signaling pathways. Electromechanical conduction prolongation has been shown to be prone to atrial fibrillation.<xref ref-type="bibr" rid="B31">31</xref> IR, which is frequently associated with obesity, has an effect on atrial functions due to existing subclinical inflammation. In our study, both intra- and inter-atrial conduction time was found to be higher in obese children with insulin resistance, according to the literature, compared to the non-IR group. This may be explained by the inflammatory process associated with insulin resistance and by the delayed transmission of this inflammatory process on the atrial tissue. In the light of this information, it can be said that obese children who have insulin resistance may be more prone to atrial fibrillation.</p>
        <p>Epicardial fat is a visceral fat accumulation that has most of the pathophysiological properties of other visceral adipose tissues, like lipid deposition and release of hormones, cytokines, and chemokines; and it also causes local inflammation.<xref ref-type="bibr" rid="B32">32</xref> - <xref ref-type="bibr" rid="B35">35</xref> It has been shown that body fat distribution, particularly abdominal fat distribution, is correlated with epicardial adipose tissue.<xref ref-type="bibr" rid="B33">33</xref> Hence, the relationship between epicardial adipose tissue thickness obtained from echocardiography and a number of pathological conditions such as metabolic syndrome, coronary artery disease, hyperlipidemia, blood pressure elevation, and IR has been studied in obese adult and pediatric patients. Epicardial adipose tissue causes the development and/or worsening of IR by increasing free fatty acids, TNF, IL1, IL6, and resistin release and decreasing adiponectin levels.<xref ref-type="bibr" rid="B36">36</xref> Several studies that examined the relationship between epicardial adipose tissue and IR demonstrated a correlation between epicardial adipose tissue and BMI in obese adults.<xref ref-type="bibr" rid="B37">37</xref> - <xref ref-type="bibr" rid="B39">39</xref> Abac&#305; et al. showed a significant correlation with epicardial fatty tissue among obese children.<xref ref-type="bibr" rid="B40">40</xref> In line with the literature available, our study demonstrated a correlation between BMI and epicardial fatty tissue. Ishorbagy et al. reported that epicardial fatty tissue was larger in amount in obese patients than in healthy controls, although it did not predict metabolic syndrome.<xref ref-type="bibr" rid="B41">41</xref> Similarly, Abac&#305; et al. suggested that epicardial fatty tissue failed to predict IR among obese children.<xref ref-type="bibr" rid="B40">40</xref> On the other hand, we found that epicardial fatty tissue was an independent predictor of IR. The cause of this discrepancy is that our study was a nested case-control study that only included patients instead of healthy controls. Another important reason was that in our study, IR was taken using determined percentiles according to age and gender.</p>
        <p>The relationship between arterial blood pressure and IR has been shown in many studies<xref ref-type="bibr" rid="B42">42</xref> , <xref ref-type="bibr" rid="B43">43</xref> In our study, we found that systolic blood pressure is an independent predictor of IR. This may be due to increased fat tissue in the body that plays an important role in IR and subclinical inflammation caused by inflammatory cytokines, such as IL-6, IL-1 and TFN-alpha secreted from this adipose tissue. Subclinical inflammation can both impair endothelial function and increase blood pressure by decreasing NO release. Another possible mechanism is: there may be sympathetic activation of both the obesity and the obesity-related inflammatory process.<xref ref-type="bibr" rid="B36">36</xref> , <xref ref-type="bibr" rid="B44">44</xref> -47</p>
        <p>Our study had some limitations: its main limitation was the relatively small sample size. Echocardiographic EAT is a linear measurement. Thus, it may not assess the total epicardial fat volume that varies at several myocardial locations. As a result of EAT, metabolically active tissue, inflammatory cytokines and inflammatory markers could be investigated in future studies. The absence of waist circumference measurement was another limitation, precluding the determination of a relationship between waist circumference and epicardial fat.</p>
      </sec>
      <sec sec-type="conclusions">
        <title>Conclusion</title>
        <p>In conclusion, epicardial adipose tissue is a cheap, easily accessible parameter that can be easily measured with echocardiography and used to identify insulin resistance among children. Since atrial electromechanical delay increased in obese children with insulin resistance, it should be followed closely for atrial fibrillation.</p>
