    <p>A insufici&#234;ncia card&#237;aca (IC) &#233; uma entidade complexa e geralmente com progn&#243;stico reservado, apontada como uma doen&#231;a cardiovascular de extrema relev&#226;ncia devido &#224; sua crescente incid&#234;ncia, preval&#234;ncia e alta morbimortalidade associadas.<xref ref-type="bibr" rid="B1">1</xref> Estima-se que a sua preval&#234;ncia esteja variando entre 1% a 2% nos pa&#237;ses desenvolvidos, atingindo &gt;10% nas pessoas acima de 70 anos de idade;<xref ref-type="bibr" rid="B2">2</xref> ademais, &#233; considerada a principal causa cardiovascular de interna&#231;&#227;o em indiv&#237;duos acima de 60 anos.<xref ref-type="bibr" rid="B3">3</xref></p>
    <p>Entre os principais sintomas caracter&#237;sticos da IC, destacam-se dispneia e fadiga, os quais est&#227;o intimamente associados ao comprometimento da capacidade funcional e, consequentemente, da qualidade de vida desses indiv&#237;duos.<xref ref-type="bibr" rid="B4">4</xref> O treinamento f&#237;sico, por sua vez, ganhou um espa&#231;o de destaque nas &#250;ltimas d&#233;cadas, com o intuito de melhoria deste cen&#225;rio, embasado por um crescente n&#250;mero de comprova&#231;&#245;es s&#243;lidas; sendo assim, ele tem a capacidade de impactar favoravelmente no ganho da capacidade funcional e qualidade de vida dos pacientes com IC.<xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref> Dentre os diversos tipos de treinamento f&#237;sico direcionados a esta popula&#231;&#227;o, o treinamento muscular inspirat&#243;rio (TMI) &#233; conhecido por sua f&#225;cil aplicabilidade e potenciais benef&#237;cios neste cen&#225;rio.</p>

      <title>Treinamento muscular inspirat&#243;rio na insufici&#234;ncia card&#237;aca</title>
      <p>H&#225; evid&#234;ncias robustas sugerindo que a fraqueza dos m&#250;sculos inspirat&#243;rios &#233; um dos principais fatores que levam &#224; baixa toler&#226;ncia ao exerc&#237;cio nos pacientes com IC.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref> De fato, ensaios cl&#237;nicos randomizados comprovaram in&#250;meros benef&#237;cios do TMI em portadores desta s&#237;ndrome, a citar: melhora significativa na efici&#234;ncia na capta&#231;&#227;o do oxig&#234;nio,<xref ref-type="bibr" rid="B9">9</xref> capacidade funcional e escores de qualidade de vida.<xref ref-type="bibr" rid="B10">10</xref> Estes resultados foram confirmados por metan&#225;lises, como aquela conduzida por Smart et al.<xref ref-type="bibr" rid="B11">11</xref> Quando comparados ao grupo controle, os pacientes submetidos a TMI alcan&#231;aram uma melhora importante no consumo m&#225;ximo de oxig&#234;nio (VO2m&#225;x): 1,83 ml.kg-1.min-1 (IC95%, 1,33 para 2,32 ml.kg-1.min-1, p &lt;0,00001), bem como no teste de caminhada de 6 minutos: 34,35 m (IC95%, 22,45 para 46,24 m, p &lt;0,00001). Por sua vez, a for&#231;a muscular inspirat&#243;ria parece ter uma correla&#231;&#227;o significativa com o VO2m&#225;x, o qual &#233; um preditor independente de sobrevida nos indiv&#237;duos com IC.<xref ref-type="bibr" rid="B12">12</xref> O TMI, deve, portanto, ser parte integrante dos cuidados destes pacientes sempre que poss&#237;vel.</p>
      <p>Nesta edi&#231;&#227;o dos Arquivos Brasileiros de Cardiologia, um ensaio cl&#237;nico randomizado, placebo e controlado<xref ref-type="bibr" rid="B13">13</xref> avaliou as implica&#231;&#245;es de uma sess&#227;o aguda de diferentes intensidades de TMI sobre a resposta hemodin&#226;mica central (RHC) de indiv&#237;duos com IC, utilizando um m&#233;todo n&#227;o invasivo de monitoriza&#231;&#227;o. Para tal, foram inclu&#237;dos 20 pacientes com fra&#231;&#227;o de eje&#231;&#227;o reduzida (37,2% &#177; 6,3%), idade m&#233;dia de 65 anos, e na grande maioria em classe funcional II da New York Heart Association (NYHA). O protocolo do TMI se resumiu em 3 sess&#245;es durante 15 minutos cada. Todos os participantes realizaram o treinamento com intensidade em 30% e 60% da press&#227;o inspirat&#243;ria m&#225;xima (PIm&#225;x), al&#233;m de interven&#231;&#227;o sham (placebo), com washout de 1 hora entre elas. Foi observado que a RHC se comportou de forma heterog&#234;nea entre as intensidades. Por exemplo, houve um aumento da frequ&#234;ncia card&#237;aca com as intensidades de 30% e 60% da PIm&#225;x (64 &#177; 15 para 69 &#177; 15 batimentos por minuto; e 67 &#177; 14 para 73 &#177; 14 batimentos por minuto, respectivamente). Em rela&#231;&#227;o ao volume sist&#243;lico, houve tend&#234;ncia &#224; redu&#231;&#227;o com a carga de 30% da PIm&#225;x (73 &#177; 26 ml para 64 &#177; 20 ml). J&#225; o d&#233;bito card&#237;aco aumentou apenas no grupo de maior intensidade (4,6 &#177; 1,5 l/min para 5,3&#177;1,7 l/min), comportamento que foi semelhante em rela&#231;&#227;o &#224; resposta da press&#227;o arterial sist&#243;lica. De fato, o aumento do d&#233;bito card&#237;aco observado na maior intensidade aplicada pode ser explicado, parcialmente, pelo aumento da frequ&#234;ncia card&#237;aca neste grupo. Estes achados n&#227;o devem ser ignorados, uma vez que os pacientes com IC tendem a apresentar fluxo sangu&#237;neo prejudicado para os m&#250;sculos em atividade, secund&#225;rio &#224;s redu&#231;&#245;es do d&#233;bito card&#237;aco e da capacidade vasodilatadora perif&#233;rica. Estas altera&#231;&#245;es s&#227;o prejudiciais, causando importante intoler&#226;ncia ao esfor&#231;o, estando associadas &#224; capacidade vasodilatadora reduzida e aumento da estimula&#231;&#227;o simp&#225;tica comuns nesses indiv&#237;duos.<xref ref-type="bibr" rid="B14">14</xref></p>
      <p>Recentemente, atrav&#233;s de uma revis&#227;o sistem&#225;tica com metan&#225;lise, nosso grupo mostrou que o TMI de alta intensidade (&#8805; 60% da PIm&#225;x) pode ser uma estrat&#233;gia eficiente para melhorar a capacidade funcional e a for&#231;a muscular inspirat&#243;ria desta mesma classe de pacientes (leia-se: IC e fra&#231;&#227;o de eje&#231;&#227;o reduzida).<xref ref-type="bibr" rid="B15">15</xref> Por sua vez, acreditamos que o estudo em quest&#227;o<xref ref-type="bibr" rid="B13">13</xref> adiciona importantes conhecimentos &#224; literatura, mostrando diferen&#231;as nas repercuss&#245;es hemodin&#226;micas observadas no TMI em diferentes intensidades, uma &#225;rea pouco explorada at&#233; o momento. Estes achados podem abrir novos horizontes e perspectivas, influenciando a realiza&#231;&#227;o de maiores pesquisas explorando as respostas hemodin&#226;micas do TMI em pacientes com IC. A falta de uma estreita correla&#231;&#227;o entre a hemodin&#226;mica central (como hiperativa&#231;&#227;o do sistema nervoso simp&#225;tico) e a toler&#226;ncia ao exerc&#237;cio fortalecem a import&#226;ncia dos resultados deste estudo.</p>

      <p>Heart failure (HF) is a complex entity and usually has a poor prognosis, identified as an extremely relevant cardiovascular disease due to its increasing incidence, prevalence and high associated morbidity and mortality.<xref ref-type="bibr" rid="B1">1</xref> It is estimated that its prevalence varies from 1% to 2% in developed countries, reaching &gt;10% in people over 70 years of age;<xref ref-type="bibr" rid="B2">2</xref> moreover, it is considered the main cause of cardiovascular hospitalization in individuals older than 60 years.<xref ref-type="bibr" rid="B3">3</xref></p>
      <p>Among the main symptoms characteristic of HF, dyspnea and fatigue stand out, which are closely associated with impaired functional capacity and, consequently, with these individuals&#8217; quality of life.<xref ref-type="bibr" rid="B4">4</xref> Physical training, in turn, has gained a prominent place in recent decades, aiming at improving this scenario, based on increasing evidence; therefore, it has the ability to favorably impact the gain in functional capacity and quality of life of HF patients.<xref ref-type="bibr" rid="B5">5</xref> , <xref ref-type="bibr" rid="B6">6</xref> Among the several types of physical training targeted at this population, inspiratory muscle training (IMT) is known for being easy to apply and showing potential benefits in this scenario.</p>
      <sec>
        <title>Inspiratory muscle training in heart failure</title>
        <p>There is robust evidence suggesting that the weakness of inspiratory muscles is one of the main factors that lead to low exercise tolerance in patients with HF.<xref ref-type="bibr" rid="B7">7</xref> , <xref ref-type="bibr" rid="B8">8</xref> In fact, randomized clinical trials have shown several benefits of IMT in patients with this syndrome, namely: significant improvement in oxygen uptake efficiency,<xref ref-type="bibr" rid="B9">9</xref> functional capacity and quality of life scores.<xref ref-type="bibr" rid="B10">10</xref> These results were confirmed by meta-analyses, such as that carried out by Smart et al.<xref ref-type="bibr" rid="B11">11</xref> When compared to the control group, patients undergoing IMT achieved an important improvement in maximum oxygen consumption (VO2max): 1.83 mL.kg-1.min-1(95%CI, 1.33 for 2.32 mL.kg-1.min-1, p &lt;0.00001), as well as in the 6-minute walking test: 34.35 m (95%CI, 22.45 to 46.24 m, p &lt;0.00001). In turn, inspiratory muscle strength seems to have a significant correlation with VO2max, which is an independent predictor of survival in individuals with HF.<xref ref-type="bibr" rid="B12">12</xref> IMT should therefore be an integral part of these patients&#8217; care whenever possible.</p>
        <p>In this issue of the Arquivos Brasileiros de Cardiologia , a randomized, placebo-controlled trial<xref ref-type="bibr" rid="B13">13</xref> evaluated the implications of an acute session of different intensities of IMT on the central hemodynamic response (CHR) of individuals with HF, using a non-invasive monitoring method. For this purpose, 20 patients with reduced ejection fraction (37.2% &#177; 6.3%), a mean age of 65 years, and the vast majority in New York Heart Association (NYHA) functional class II were included in the study. The IMT protocol consisted of 3 sessions lasting 15 minutes each. All participants underwent the training with an intensity of 30% and 60% of maximum inspiratory pressure (MIP), in addition to sham intervention (placebo), with a 1-hour washout between them. It was observed that CHR behaved in a heterogeneous way between intensities. For instance, there was an increase in heart rate with intensities of 30% and 60% of MIP (64 &#177; 15 to 69 &#177; 15 beats per minute; and 67 &#177; 14 to 73 &#177; 14 beats per minute, respectively). Regarding stroke volume, there was a tendency to decrease with a 30% load of MIP (73 &#177; 26 mL to 64 &#177; 20 mL). The cardiac output increased only in the group with the highest intensity (4.6 &#177; 1.5 L / min to 5.3 &#177; 1.7 L / min), a behavior that was similar in relation to the systolic blood pressure response. In fact, the increase in cardiac output observed at the highest applied intensity can be partially explained by the increase in heart rate in this group. These findings should not be ignored, since patients with HF tend to have impaired blood flow to the active muscles, secondary to reductions in cardiac output and peripheral vasodilator capacity. These changes are harmful, causing important intolerance to effort, being associated with reduced vasodilator capacity and increased sympathetic stimulation, common in these individuals.<xref ref-type="bibr" rid="B14">14</xref></p>
        <p>Recently, in a systematic review with meta-analysis, our group showed that high-intensity IMT (&#8805; 60% of MIP) can be an efficient strategy to improve functional capacity and inspiratory muscle strength in this same class of patients (that is: HF and reduced ejection fraction).<xref ref-type="bibr" rid="B15">15</xref> In turn, we believe that the referred study<xref ref-type="bibr" rid="B13">13</xref> adds important knowledge to the literature, showing differences in the hemodynamic repercussions observed in the IMT at different intensities, an area little explored so far. These findings may open new horizons and perspectives, influencing further research exploring the hemodynamic responses of IMT in patients with HF. The lack of a close correlation between central hemodynamics (such as the sympathetic nervous system hyperactivation) and exercise tolerance strengthens the importance of the results of this study.</p>
