  <body>
    <p>An 84-years-old woman with hypertension and dyslipidemia was admitted in the emergency room with acute abdominal pain; patient complained of chest pain three weeks before hospital admission. On physical examination, she was hypotensive, tachycardic (with arrhythmic pulse), tachypneic and with diffuse abdominal pain. Electrocardiogram showed atrial fibrillation with rapid ventricular response, complete left bundle branch block and inferior Q waves. Abdominal computed tomography (CT) revealed a thrombus in the superior mesenteric artery (<xref ref-type="fig" rid="f1">Figure 1</xref>, white asterisk). The patient showed a clinical course with congestive heart failure and low cardiac output. Transthoracic echocardiogram (<xref ref-type="fig" rid="f4">Videos 1</xref>-<xref ref-type="fig" rid="f5">2</xref>) showed mild left ventricular dilatation with a mild dysfunction and a pseudoaneurysm of the basal half of the posterior and inferior walls with left-to-right shunt, confirmed by color Doppler imaging (<xref ref-type="fig" rid="f2">Figure 2 A</xref>-<xref ref-type="fig" rid="f2">D</xref>). Cardiac CT (<xref ref-type="fig" rid="f6">Video 3</xref>) revealed contained myocardial rupture, located at the basal segments of the inferior and posterior septal walls, extending to the free wall of the right ventricle, forming a pseudo-cavity, which communicates with the true cavity of the right ventricle (<xref ref-type="fig" rid="f3">Figure 3</xref>). Despite vasopressor and inotropic support and proposal for cardiac surgery, the patient had an unfavorable course.</p>

    <p>This case illustrates an interesting entity - pseudoaneurysm, with left-to-right shunt and contained myocardial rupture, which extended to the right ventricle, leading to a dismal prognosis.</p>

      <p>Uma mulher de 84 anos, hipertensa e dislipid&#234;mica, deu entrada na emerg&#234;ncia com dor abdominal aguda. A paciente queixava-se de dor no peito tr&#234;s semanas antes da interna&#231;&#227;o. Ao exame f&#237;sico, a paciente apresentava hipotens&#227;o, taquicardia (pulso arr&#237;tmico), taquipneia e dor abdominal difusa. Eletrocardiograma revelou fibrila&#231;&#227;o atrial com r&#225;pida resposta ventricular, bloqueio completo do ramo esquerdo e ondas Q inferiores. Pela tomografia computadorizada (TC) abdominal, detectou-se trombo na art&#233;ria mesent&#233;rica superior (<xref ref-type="fig" rid="f7">Figura 1</xref>, asterisco branco). A paciente evoluiu com insufici&#234;ncia card&#237;aca congestiva e baixo d&#233;bito card&#237;aco. Ecocardiografia transtor&#225;cica (<xref ref-type="fig" rid="f10">V&#237;deos 1</xref>-<xref ref-type="fig" rid="f11">2</xref>) mostrou dilata&#231;&#227;o leve do ventr&#237;culo esquerdo, com leve disfun&#231;&#227;o, e um pseudoaneurisma na metade inferior da parede posterior e inferior, com deriva&#231;&#227;o (shunt) da esquerda para a direita, confirmada pelo exame com Doppler colorido (<xref ref-type="fig" rid="f8">Figura 2 A</xref>-<xref ref-type="fig" rid="f8">D</xref>). A TC card&#237;aca (<xref ref-type="fig" rid="f12">V&#237;deo 3</xref>) revelou ruptura do mioc&#225;rdio contida, localizada nos segmentos basais da parede inferior e posterior do septo, estendendo-se &#224; parede livre do ventr&#237;culo, formando uma pseudocavidade, que se comunica com a cavidade real do ventr&#237;culo direito (<xref ref-type="fig" rid="f9">Figura 3</xref>). Apesar do suporte vasopressor e inotr&#243;pico, e proposta de cir&#250;rgica card&#237;aca, a paciente apresentou evolu&#231;&#227;o desfavor&#225;vel.</p>
