    <p>
      Dear Editor,
    </p>
    <p>We read with great interest the article entitled as &#8220;Strain Analysis of Left Ventricular Function in the Association of Hypertrophic Cardiomyopathy and Systemic Arterial Hypertension&#8221;. In this paper, the authors evaluated the global longitudinal strain (GLS) of the left ventricle (LV) in two distinct groups: patients with hypertrophic cardiomyopathy (HCM) and patients with HCM and systemic arterial hypertension (SAH) and demonstrated that GLS was lower in the second group. This important finding may indicate greater impairment of LV function in patients with SAH and HCM.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>However, despite these findings, it is important to notice the impact of the LV mass on the GLS, unfortunately not reported by the authors. A previous study by Soufi Taleb Bendiab et al.<xref ref-type="bibr" rid="B2">2</xref> showed that hypertensive patients with LV hypertrophy had a reduced GLS.<xref ref-type="bibr" rid="B2">2</xref> Moreover, in the study of L&#243;pes-Candales et al.<xref ref-type="bibr" rid="B3">3</xref> patients with increased LV mass had a significant reduced GLS.<xref ref-type="bibr" rid="B3">3</xref> From a mechanical point of view of a physiological heart model validated by echocardiography, the volume of heart tissue is constant throughout the cardiac cycle, since it is incompressible.<xref ref-type="bibr" rid="B4">4</xref> In that way, patients with increased LV thickness would have less longitudinal myocardial deformation.<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>Since the population of this study were patients with hypertrophic cardiomyopathy with or without hypertension, the measurement of LV mass becomes even more important to report an accurate multivariate analysis and to measure the impact of this important confounder on the study&#8217;s conclusions.</p>

        Caro Editor,
      </p>
      <p>N&#243;s lemos, com grande interesse, o artigo intitulado &#8220;Avalia&#231;&#227;o da Fun&#231;&#227;o Ventricular Esquerda na Associa&#231;&#227;o de Cardiomiopatia Hipertr&#243;fica e Hipertens&#227;o Arterial Sist&#234;mica pela T&#233;cnica de Strain&#8221;. Nesse trabalho, os autores avaliaram o strain longitudinal global (SLG) do ventr&#237;culo esquerdo (VE) em dois grupos distintos: pacientes portadores de cardiomiopatia hipertr&#243;fica (CMH) e pacientes portadores de CMH com hipertens&#227;o arterial sist&#234;mica (HAS), e demonstraram que o SLG foi menor no segundo grupo. Essa importante descoberta pode indicar um maior comprometimento da fun&#231;&#227;o do VE nos pacientes com HAS e CMH.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>Entretanto, apesar dessas descobertas, &#233; importante observar o impacto da massa do VE sobre o SLG, infelizmente, n&#227;o relatado pelos autores. Um estudo anterior desenvolvido por Soufi Taleb Bendiab et al.,<xref ref-type="bibr" rid="B2">2</xref> mostrou que os pacientes hipertensos com hipertrofia do VE apresentaram uma redu&#231;&#227;o do SLG.<xref ref-type="bibr" rid="B2">2</xref> Al&#233;m disso, no estudo de L&#243;pes-Candales et al.,<xref ref-type="bibr" rid="B3">3</xref> os pacientes com aumento da massa do VE tiveram uma redu&#231;&#227;o significativa o SLG.<xref ref-type="bibr" rid="B3">3</xref> Do ponto de vista mec&#226;nico de um modelo card&#237;aco fisiol&#243;gico validado pelo ecocardiograma, o volume do tecido card&#237;aco &#233; constante ao longo do ciclo card&#237;aco, uma vez que ele &#233; incompress&#237;vel.<xref ref-type="bibr" rid="B4">4</xref> Desse modo, os pacientes com aumento da espessura do VE apresentariam menor deforma&#231;&#227;o mioc&#225;rdica longitudinal.<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Uma vez que a popula&#231;&#227;o desse estudo foi composta por pacientes portadores de cardiomiopatia hipertr&#243;fica com ou sem hipertens&#227;o, o c&#225;lculo da massa do VE se torna ainda mais importante para relatar uma an&#225;lise multivariada e mensurar o impacto desse importante fator confundidor nas conclus&#245;es do estudo.</p>
