    <p>This issue of Arquivos Brasileiros de Cardiologia brings a paper entitled &#8220;The Usefulness of Admission Plasma NT-pro BNP Level to Predict Left Ventricular Aneurysm Formation after Acute ST-Segment Elevation Myocardial Infarction&#8221;.<xref ref-type="bibr" rid="B1">1</xref> The authors bring a cohort of 1,519 post-acute ST-segment elevation myocardial infarction (STEMI) who were followed-uP for at least six months. Despite its observational and retrograde design, the authors were straightforward in looking for predictive variables that could foresee the occurrence of left ventricular aneurysms (LVA). Among other major clinical aspects such as previous coronary artery bypass graft, post-MI heart failure, younger age, smoking and no-reflow phenomenon; authors highlighted the importance of high NT-proBNP at admission as a predictor of LVA formation after acute STEMI.</p>
    <p>I would probably highlight one weakness and a potentially positive aspect of their work.</p>
    <p>The weakness is that a LVA will never be diagnosed by a NT-Pro-BNP level and will always be found, confirmed and/or followed by an image test (Echo, CMR, etc.). NT-ProBNP usually and reliably identifies patients who are sicker or more congested, either in acute,<xref ref-type="bibr" rid="B2">2</xref> or in chronic heart failure,<xref ref-type="bibr" rid="B3">3</xref> or even without heart failure.<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>The potentially positive one was, interestingly, what the authors have considered their limitation: that the NT-ProBNP values have been collected at admission. Having a high natriuretic peptide level at the admission of a STEMI patient could be a predictive variable of a clinical event, such as LVA formation, in six months. It was there, on the &#8220;Limitations&#8221; section, the best and most clinically relevant information.</p>

      <p>Esta edi&#231;&#227;o dos Arquivos Brasileiros de Cardiologia traz um artigo intitulado &#8220;Utilidade do N&#237;vel de NT-Pro-BNP no Plasma &#224; Interna&#231;&#227;o em Predizer a Forma&#231;&#227;o de Aneurisma do Ventr&#237;culo Esquerdo ap&#243;s Infarto Agudo do Mioc&#225;rdio com Eleva&#231;&#227;o do Segmento ST&#8221;.<xref ref-type="bibr" rid="B1">1</xref> Os autores trazem uma coorte de 1.519 pacientes com infarto agudo do mioc&#225;rdio com supradesnivelamento do segmento ST (IAMCSST) que foram acompanhados por pelo menos seis meses.</p>
      <p>Apesar de seu desenho observacional e retr&#243;grado, os autores foram diretos na busca de vari&#225;veis preditivas que pudessem prever a ocorr&#234;ncia de aneurismas do ventr&#237;culo esquerdo (AVE). Entre outros aspectos cl&#237;nicos importantes, como revasculariza&#231;&#227;o do mioc&#225;rdio pr&#233;via, insufici&#234;ncia card&#237;aca p&#243;s-IM, idade mais jovem, tabagismo e fen&#244;meno de no-reflow, os autores destacaram a import&#226;ncia de n&#237;veis altos de NT-proBNP na hospitaliza&#231;&#227;o como preditor da forma&#231;&#227;o de AVE ap&#243;s IAMCSST.</p>
      <p>Eu gostaria de destacar um ponto fraco e um aspecto potencialmente positivo do trabalho deles.</p>
      <p>O ponto fraco &#233; que um AVE nunca ser&#225; diagnosticado pelo n&#237;vel de NT-Pro-BNP e sempre ser&#225; encontrado, confirmado e /ou acompanhado por um teste de imagem (eco, RM card&#237;aca, etc.). O NT-ProBNP geralmente identifica de maneira confi&#225;vel os pacientes mais doentes ou mais congestionados, seja na insufici&#234;ncia card&#237;aca aguda<xref ref-type="bibr" rid="B2">2</xref> ou cr&#244;nica,<xref ref-type="bibr" rid="B3">3</xref> ou mesmo na aus&#234;ncia da insufici&#234;ncia card&#237;aca.<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>O potencial positivo foi, curiosamente, o que os autores consideraram sua limita&#231;&#227;o: o fato de que os valores de NT-ProBNP foram coletados &#224; hospitaliza&#231;&#227;o. Apresentar um alto n&#237;vel de pept&#237;deo natriur&#233;tico na hospitaliza&#231;&#227;o de um paciente com IAMCSST pode ser uma vari&#225;vel preditiva de um evento cl&#237;nico, como a forma&#231;&#227;o de AVE, em seis meses. Estavam l&#225;, na se&#231;&#227;o &#8220;Limita&#231;&#245;es&#8221;, as melhores e mais clinicamente relevantes informa&#231;&#245;es.</p>
