        <title>Abstract</title>
        <sec>
          <title>Background:</title>
          <p>The improvement in surgical techniques has contributed to an increasing number of childbearing women with complex congenital heart disease (CCC). However, adequate counseling about pregnancy in this situation is uncertain, due to a wide variety of residual cardiac lesions.</p>
        </sec>
        <sec>
          <title>Objectives:</title>
          <p>To evaluate fetal and maternal outcomes in pregnant women with CCC and to analyze the predictive variables of prognosis.</p>
        </sec>
        <sec>
          <title>Methods:</title>
          <p>During 10 years we followed 435 consecutive pregnancies in patients (pts) with congenital heart disease. Among of them, we selected 42 pregnancies in 40 (mean age of 25.5 &#177; 4.5 years) pts with CCC, who had been advised against pregnancy. The distribution of underlying cardiac lesions were: D-Transposition of the great arteries, pulmonary atresia, tricuspid atresia, single ventricle, double-outlet ventricle and truncus arteriosus. The surgical procedures performed before gestation were: Fontan, Jatene, Rastelli, Senning, Mustard and other surgical techniques, including Blalock, Taussing, and Glenn. Eight (20,0%) pts did not have previous surgery. Nineteen 19 (47.5%) pts had hypoxemia. The clinical follow-up protocol included oxygen saturation recording, hemoglobin and hematocrit values; medication adjustment to pregnancy, anticoagulation use, when necessary, and hospitalization from 28 weeks, in severe cases. The statistical significance level considered was p &lt; 0.05.</p>
        </sec>
        <sec>
          <title>Results:</title>
          <p>Only seventeen (40.5%) pregnancies had maternal and fetal uneventful courses. There were 13 (30.9%) maternal complications, two (4.7%) maternal deaths due to hemorrhage pos-partum and severe pre-eclampsia, both of them in women with hypoxemia. There were 7 (16.6%) stillbirths and 17 (40.5%) premature babies. Congenital heart disease was identified in two (4.1%) infants. Maternal and fetal complications were higher (p &lt; 0.05) in women with hypoxemia.</p>
        </sec>
        <sec>
          <title>Conclusions:</title>
          <p>Pregnancy in women with CCC was associated to high maternal and offspring risks. Hypoxemia was a predictive variable of poor maternal and fetal outcomes. Women with CCC should be advised against pregnancy, even when treated in specialized care centers.</p>

      <title>Introduction</title>
      <p>In the last decade, the continuous and progressive improvement in the surgical and late postoperative treatment has allowed an increased number of children with complex congenital heart disease (CCC) to reach childbearing age.</p>
      <p>The registry of 1000 cases of pregnant women followed at the Heart Institute of S&#227;o Paulo (InCor) <xref ref-type="bibr" rid="B1">1</xref> between 1989 and 1999 showed that during that 10-year period, congenital heart diseases corresponded to 19.2% of the cases, which represented the second most frequent structural cardiac lesion. Among them, less than 1% were congenital complex lesions.</p>
      <p>However, over the last 50 years, there has been a significant tendency in our country to an increase in the percentage of CCC during pregnancy, as observed throughout the world, as shown by the European Registry for Cardiac Diseases in Pregnancy ( ROPAC) , in which 20% of 66% of pts with congenital heart disease had complex heart defects.<xref ref-type="bibr" rid="B2">2</xref> This setting required a risk stratification scheme to predict adverse outcomes in pregnant women with congenital heart diseases seeking guidance to conceive. The modified World Health Organization (WHO) <xref ref-type="bibr" rid="B3">3</xref>-<xref ref-type="bibr" rid="B4">4</xref> classification is the most well-accepted risk stratification model for pregnancy in congenital heart disease patients, and it considers CCC as risk III, which means medical advice against pregnancy.</p>
      <p>Despite this advice, both desired and unplanned pregnancy rates have been gradually rising, thus increasing the number of pregnant women with CCC. The scarcity of publications on pregnancy evolution in these women motivated this study.</p>

      <p>Regarding the newborn, the variables considered were: gestational age at the delivery, fetal loss classified as: miscarriage (&lt; 20 weeks), stillbirth (between 20 and 36 weeks) and neonatal death (up to 30 days after delivery) and malformations related to the maternal heart disease.</p>
      <p>The conditions: hypoxemia, prior heart surgery and univentricular anatomy were studied as predictive variables of maternal and fetal outcomes.</p>

        <title>Statistical analysis</title>
        <p>The categorical variables were considered in the tables containing absolute (n) and relative (%) frequencies. The association of the categorical data was evaluated using the chi-square method and, when necessary, Fisher's exact test. The distribution of the quantitative variables regarding normality was evaluated with the Kolmogorov-Smirnov test. The variables with normal distribution were presented as mean and standard deviation and compared using Student&#8217;s t-test for independent samples. The non-parametric variables were presented as median and interquartile interval and compared using the Mann-Whitney test. The values of p &lt; 0.5 were considered significant. The SPSS software version 18.0 was used for the statistic calculations.</p>
        <p>This research was approved by the Institutional Review Board of Hospital das Cl&#237;nicas of the School of Medicine of the University of S&#227;o Paulo - SDC protocol 4563/17/063.</p>

        <p>Maternal and fetal success was considered in 17 (40.5%) cases, when the mother and healthy newborn were discharged from the hospital after delivery without complications. <xref ref-type="fig" rid="f1">Figure 1</xref>shows the maternal-fetal evolution and complications. Heart failure occurred in cases 5, 6, 15, 24 and 41 (<xref ref-type="table" rid="t4">Table 4</xref>) and was treated with hospitalization, strict hygiene-dietetic measures, furosemide, carvedilol or metoprolol associated or not to digitalis, when indicated. Electrical cardioversion was needed for atrial flutter treatment in case 21 (<xref ref-type="table" rid="t4">Table 4</xref>). The hospitalization duration for the treatment of complications or for childbirth planning varied between 21 and 68 days (average of 45 days). There were two maternal deaths (4.7%) related to obstetric complications: hemorrhage after delivery and preeclampsia, cases 34 and 38, respectively (<xref ref-type="table" rid="t4">Table 4</xref>).</p>
        <p>The obstetric complications are shown in <xref ref-type="fig" rid="f1">Figure 1</xref>. The fetal losses correspond to miscarriages in cases 12, 17 and 37, stillbirth in case 34 and neonatal death in cases 14, 16 (premature babies) and 35 (<xref ref-type="table" rid="t4">Table 4</xref>). The delivery occurred on average at 37 weeks of gestation; 24 (54.5%) were Caesarian section due to obstetric raisons or progressive maternal clinical worsening. Among the live newborns, there were two (4.7%) cases of congenital heart disease: one with recurrence of maternal heart disease (case 25) and the other with tetralogy of Fallot (case 32) ; neither of the cases were preterm babies.</p>
        <p>Among of the predictive variables of maternal and fetal outcomes, hypoxemia showed a significant correlation with worse pregnancy prognosis, while prior surgery (whether the mother had been submitted to previous surgery or not) and univentricular function showed no correlation with the maternal and fetal outcomes (<xref ref-type="table" rid="t5">Table 5</xref>).</p>

      <title>Discussion</title>
      <p>This study included one substancial series of pregnant CCC patients, submitted to the multidisciplinary protocol at the Heart Disease and Pregnancy tertiary care center. This studied group represented 9.6% of 435 pregnancies in women with congenital heart diseases included in the InCor-Registry during the last decade. It is undeniable that higher post-operative survival of these patients will result in an increasing number of pregnancies in women with CCC in the near future.</p>
      <p>The CCC considered in this study were included in the WHO risk category III,<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> which means pregnancy is discouraged, justified by the rates of 25.5% of maternal complications and 70% of poor fetal outcome. These considerations are according to the results of this study, which recorded only 40% of successful pregnancies, i.e., healthy mothers and newborns without complications. The high rates of maternal events (36%) and fetal events (43%) are the bases for the WHO guideline that advises against pregnancy in this group of patients.</p>
      <p>However, the global experience in this clinical situation is increasing and it represents a major challenge for clinicians. Occasionally, women become pregnant without prior counselling or sometimes they desire a pregnancy despite the advice against it.<xref ref-type="bibr" rid="B7">7</xref> The diversity of the anatomical and functional conditions of the heart defects in CCC restrict the creation of management protocols for eventual complications during pregnancy, delivery and postpartum.<xref ref-type="bibr" rid="B7">7</xref>
			</p>
      <p>However, knowledge of the most common complications that occur in the late postoperative period of CCC helps in the management of pregnancy in these patients. In this regard, a study about the causes of death in patients with CCC showed that heart failure, sudden death, ischemic heart disease and infective endocarditis were the most common ones. In addition, the most significant anatomical lesions (except for Eisenmenger's syndrome) were the transposition of the great arteries and the Fontan circulation.<xref ref-type="bibr" rid="B8">8</xref>
			</p>
      <p>A study of 120 necropsies of congenital heart disease patients<xref ref-type="bibr" rid="B9">9</xref> confirmed heart failure as the main cause of death, since the ventricular remodeling in response to volumetric and pressure overload during life favors fibrosis, hypertrophy and a reduced number of myocardial interstitial capillaries. Thromboembolism, which is the second cause of death, was significantly associated with histological signs of pulmonary hypertension, also detected in chronic hypoxemia. The third cause of death recorded was infective endocarditis, which reinforced the recommendation of antibiotic prophylaxis during delivery in our protocol.</p>
      <p>The routine elective hospitalization as of the 28th week of pregnancy for patients at likely higher risk situation, regardless of the functional condition, was based on the fact that the third trimester is critical for the mother, due to the hemodynamic overload and higher prothrombotic activity, as well as for the fetus, due to the high incidence of prematurity and intrauterine growth restriction, which are characteristics of CCC.</p>
      <p>Furthermore, elective hospitalization improved maternal and fetal monitoring, allowed intermittent oxygen therapy to be applied, individualized anticoagulation, and optimized therapy for possible complications and delivery planning.</p>
      <p>Our results showed similar rates of cardiac (14.2%) and obstetric complications (16.6%). In both cases, maternal deaths were associated with obstetric causes (pre-eclampsia and postpartum hemorrhage). This result allows a reassessment of the severe cardiac reserve limitation in these patients, who do not tolerate the events inherent to pregnancy and postpartum, regardless of the baseline functional condition.</p>
      <p>Pre-eclampsia, one of the causes of death in this study, is responsible for 15% of maternal deaths in Brazil, with an incidence of around 10% in the pregnant population.<xref ref-type="bibr" rid="B10">10</xref> Early diagnosis and an effective prenatal care, although they do not prevent the disease, can improve maternal mortality in healthy women. However, in patients with complex heart disease, the prognosis of pre-eclampsia is much worse, due to both systemic endothelial dysfunction, inherent to the disease and the circulatory overload caused by arterial hypertension.</p>
      <p>Postpartum hemorrhage, the reason for the second death in our study, is considered an important obstetric cause of maternal death in women with heart disease, especially in hypoxemic patients. This fact was documented in a study of 366 primiparous women with congenital heart diseases, which recorded 21% of postpartum hemorrhage. That study identified early Caesarean section, general anesthesia and use of low-molecular-weight heparin at both prophylactic and therapeutic doses, as variables of higher correlation with postpartum bleeding. Furthermore, women with Fontan circulation had the highest blood loss and the difference remained significant after correcting for the other variables.<xref ref-type="bibr" rid="B11">11</xref>
			</p>
      <p>In our study, the association of hypoxemia and postpartum hemorrhage associated with Caesarian delivery resulted in the second maternal death. Actually, the highest maternal and fetal morbidities associated with premature Caesarean delivery is due to maternal clinical instability and intrauterine growth restriction, common in complex heart situations.</p>
      <p>A brief emphasis should be placed on the most common heart disease in this cohort, such as the transposition of the great arteries and the Fontan circulation. The promising evolution after the correction of the transposition of the great arteries through atrial inversion (Senning&#8217;s procedure or Mustard&#8217;s technique) or arterial inversion (Jatene&#8217;s technique) has allowed the development of pregnancy.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B13">13</xref> However, there are expected events in the late postoperative period that may be unfavorable to the success of pregnancy. Supraventricular arrhythmias and ventricular dysfunction in adulthood may occur in about 40% of patients after surgical correction using Mustard&#8217;s technique or Senning&#8217;s procedure. On the other hand, neo-aortic valve regurgitation, present in 1-2% and coronary complications observed in 3-11% of cases, may both occur in the long term after Jatene&#8217;s technique. Regarding Rastelli's surgery, the late evolution depends on the type of tissue used, which can determine different degrees of calcification and progressive occlusion of the graft.<xref ref-type="bibr" rid="B14">14</xref>
			</p>
      <p>Based on these considerations, during pregnancy, one must be prepared for the treatment of heart failure and arrhythmias when there is right ventricular dysfunction and tricuspid regurgitation after Mustard&#8217;s and Senning&#8217;s techniques; heart failure and low output due to calcified conduits after the Rastelli procedure, of valve dysfunctions and/or coronary complications, when Jatene&#8217;s technique had been used.<xref ref-type="bibr" rid="B14">14</xref>
			</p>
      <p>This study showed that, with the exception of one patient who was not operated whose pregnancy ended in miscarriage (case 12), the other pregnant women with transposition of the great vessels of the base showed favorable maternal and fetal evolutions, regardless of the type of surgical correction. It is worth emphasizing that the expected complications were controlled with hospitalization and constant monitoring of the mother and fetus.</p>
      <p>Fontan circulation has allowed the survival of 70% of the patients with univentricular heart disease up to childbearing age.<xref ref-type="bibr" rid="B15">15</xref> However, in the late postoperative period, complications such as atrial tachycardia, thromboembolism (related to hepatic and venous system stasis) , heart failure, liver failure and, protein-losing enteropathy. The abnormal connection - vena cava and pulmonary circulation - despite the improvement in cyanosis, ventricular overload reserve and pulmonary circulation capacity, may be threatened by the variations in the central venous pressure and by the negative intrathoracic pressure induced by hyperventilation and changes in cardiac output during pregnancy.</p>
      <p>The inability of patients with Fontan circulation to adapt to physiology of the pregnancy and postpartum period was documented in our study that showed worsening of the functional class in all the patients. Heart failure occurs because the abnormal anatomical and functional ventricle is unable to adjust to the increased cardiac output. However, there was no maternal death due to a good responses were obtained to the clinical treatment with the use of diuretic and beta-blocker in patients who developed heart failure (cases 15 and 41), and to electric cardioversion in case of atrial flutter (case 21).</p>
      <p>On the other hand, the evolution unfavorable to the fetus was documented in six Fontan cases, resulting in a miscarriage and five premature deliveries. Review of the literature that included six studies with 255 pregnancies and 133 women showed 137 (69%) fetal loss, and 68 (59%) of the 115 live newborns were premature and six (5,2%) evolved to neonatal death. The causes of prematurity were not detailed, particularly in those induced by the anticipation of the delivery due to maternal reasons. However, the premature rupture of the amniotic membranes and placental premature detachment occurred in 6.2% and 10.9%, respectively.<xref ref-type="bibr" rid="B16">16</xref>
			</p>
      <p>The poor fetal prognosis was confirmed by the multicenter study of the United Kingdom that included 50 women, 124 pregnancies, showed an incidence of 68 (54.8%) miscarriages and, among the 56 (45.2%) live newborns, four died due to extreme prematurity (delivery with gestational age below 32 weeks). On the other hand, the maternal complications (heart failure in 13.5%, arrhythmias in 11.3% and pulmonary thromboembolism in 1.19% of the cases) do not result in maternal death.<xref ref-type="bibr" rid="B17">17</xref>
			</p>
      <p>The full anticoagulation routine in patients with Fontan circulation should be considered by reasons of the high risk of thromboembolism peculiar to this setting and the hypercoagulable state of the pregnancy and postpartum. This present study showed three cases of maternal thromboembolism, two of which were in a non-anticoagulated patient.</p>
      <p>Patients with Fontan circulation should advise against pregnancy specially in patients with depressed ventricular function, cyanosis, important mitral valve insufficiency or protein-losing enteropathy.<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B17">17</xref>
			</p>
      <p>This study showed that hypoxemia was the presumable variables of the worst maternal-fetal prognosis, as previous report.<xref ref-type="bibr" rid="B18">18</xref> The unusual result of the previous surgical correction did not show any difference in the evolution of the pregnancy, possibly was related to a small number of the cases and some cases had cardiac lesion with an anatomy favorable to survival during childbearing age. Therefore, another study that analyzed 102 necropsies of congenital heart failure patients verified that the average age of cases not operated was higher than those operated and presented also less serious anatomical defects.<xref ref-type="bibr" rid="B9">9</xref>
			</p>
      <p>Another highlight of our study is the record of two newborns (5%) with of congenital heart disease which equivalent to six times more than the 0.8% estimated for the general population. This rate is still above the value by Oliveira et al. reporter which identifies 3 (3.2%) of cardiac malformations in children from 100 pregnant women with congenital heart diseases followed at InCor.<xref ref-type="bibr" rid="B19">19</xref> The recurrence of heart disease in the babies of mothers with congenital heart diseases should be considered in counselling before pregnancy in response to the questions about hereditary as well as indication of routine fetal echocardiography study.</p>
      <sec>
        <title>Study limitations</title>
        <p>Both the small number of patients and the wide heterogeneity of the anatomical defects contribute to a limitation of the accurate statistical analysis. However, it should be considered that this sample of patients was exclusively from the high risk group, in which pregnancy is not advised and constitutes a great guidance dilemma regarding family planning provided to women with CCC. The character of the study (retrospective and observational, restricted to a single center) can also influence the appropriate conclusions.</p>
      </sec>
      <sec>
        <title>Final comments</title>
        <p>Congenital heart diseases affect approximately 0.8% of all live newborns and the survival rate of 86% are highlighted in international records. It is estimated that there are currently more adults with congenital heart diseases than children, which naturally provides a considerable number of women at childbearing age.</p>
        <p>The qualification of the multidisciplinary team is fundamental in the counselling of young women with heart disease regarding pregnancy, including advice such as alternatives to a safe and effective childbirth. Despite the stratification of the WHO risk III that allows the advice against pregnancy, the provision contained in the Brazilian Legislation should be considered:</p>
        <p>
          Furthermore, article 226 should be considered: Based on the principles of human dignity and responsible parenthood, family planning is a free decision made by the couple, and it is the State&#8217;s responsibility to provide educational and scientific resources to exercise this right, prohibiting any enforcement by official or private institutions&#8221; (our emphasis). This standard applies to other institutes: a) human dignity (article 1, III) and b) right to freedom (article 5, caput)

      <title>Conclusions</title>
      <p>The strict care protocol during pregnancy, delivery, and puerperium did not prevent maternal deaths, prematurity or miscarriage in patients with CCC. Hypoxemia was a poor prognostic factor and maternal evolution was unsatisfactory, but the fetal outcome was worse.. Although the autonomy of intention to conceive should be respected, women with CCC should still be advised against getting pregnant.</p>

          <title>Fundamento:</title>
          <p>A cont&#237;nua habilidade na conduta das cardiopatias cong&#234;nitas complexas (CCC) tem permitido o alcance da idade f&#233;rtil. Contudo, a heterogeneidade das les&#245;es card&#237;acas na idade adulta limita a estimativa do progn&#243;stico da gravidez.</p>
        </sec>
        <sec>
          <title>Objetivo:</title>
          <p>Estudar a evolu&#231;&#227;o materno-fetal das gestantes portadoras de CCC e analisar as vari&#225;veis presum&#237;veis de progn&#243;stico.</p>
        </sec>
        <sec>
          <title>M&#233;todo:</title>
          <p>No per&#237;odo de 10 anos, 435 gestantes portadoras de cardiopatias cong&#234;nitas foram consecutivamente inclu&#237;das no Registro do Instituto do Cora&#231;&#227;o (Registro-InCor). Dentre elas, foram selecionadas 42 gesta&#231;&#245;es em 40 mulheres com CCC (24,5 &#177; 3,4 anos) que haviam sido desaconselhadas a engravidar. As cardiopatias de base distribu&#237;ram-se em: transposi&#231;&#227;o das grandes art&#233;rias, atresia pulmonar, atresia tric&#250;spide, ventr&#237;culo &#250;nico, dupla via de sa&#237;da de ventr&#237;culo direito, dupla via de entrada de ventr&#237;culo esquerdo e outras les&#245;es estruturais. As cirurgias realizadas foram Rastelli, Fontan, Jatene, Senning, Mustard e outros procedimentos combinados, como tuneliza&#231;&#227;o, Blalock Taussing e Glenn. Oito pacientes (20%) n&#227;o haviam sido operadas, e 19 (47,5%) apresentavam hipoxemia. O protocolo de atendimento incluiu: registro da satura&#231;&#227;o de oxig&#234;nio, hemoglobina s&#233;rica, hemat&#243;crito, ajuste das medica&#231;&#245;es, anticoagula&#231;&#227;o individualizada e hospitaliza&#231;&#227;o a partir de 28 semanas de gesta&#231;&#227;o, em face da gravidade do quadro cl&#237;nico e obst&#233;trico. Na an&#225;lise estat&#237;stica, o n&#237;vel de signific&#226;ncia adotado foi de 0,05.</p>
        </sec>
        <sec>
          <title>Resultado:</title>
          <p>Somente 17 gesta&#231;&#245;es (40,5%) n&#227;o tiveram complica&#231;&#245;es maternas nem fetais. Houve 13 problemas maternos (30,9%) e 2 mortes (4,7%) causadas por hemorragia p&#243;s-parto e pr&#233;-ecl&#226;mpsia grave, ambas em pacientes que apresentavam hipoxemia. Houve 7 perdas fetais (16,6%), 17 beb&#234;s prematuros (40,5%) e 2 rec&#233;m-nascidos (4,7%) com cardiopatia cong&#234;nita. As complica&#231;&#245;es materno-fetais foram significativamente maiores em pacientes que apresentavam hipoxemia (p &lt; 0,05).</p>
        </sec>
        <sec>
          <title>Conclus&#227;o:</title>
          <p>O alcance da idade reprodutiva em pacientes com CCC &#233; crescente; contudo, a m&#225; evolu&#231;&#227;o materno-fetal desaconselha a gravidez, particularmente nas pacientes que apresentam hipoxemia.</p>

        <title>Introdu&#231;&#227;o</title>
        <p>Nas &#250;ltimas d&#233;cadas, a cont&#237;nua e progressiva habilidade no tratamento cir&#250;rgico e a condu&#231;&#227;o do p&#243;s-operat&#243;rio imediato e tardio t&#234;m possibilitado que crian&#231;as portadoras de CCC alcancem a idade reprodutiva. O Registro de 1.000 casos de gestantes acompanhadas no InCor<xref ref-type="bibr" rid="B1">1</xref> entre 1989 e 1999 mostrou que naqueles 10 anos as cardiopatias cong&#234;nitas corresponderam a 19,2% dos casos, ocupando o segundo lugar como les&#227;o card&#237;aca estrutural, ressaltando-se que menos de 1% era de les&#245;es complexas.</p>
        <p>Contudo, atualmente, a experi&#234;ncia brasileira vivencia uma tend&#234;ncia expressiva ao aumento do percentual de CCC durante a gravidez, outrora relatada em pa&#237;ses europeus, como mostra o Registro Europeu de Cardiopatias e Gravidez (ROPAC), em que 20% dos 66% de casos de cardiopatias cong&#234;nitas eram complexas.<xref ref-type="bibr" rid="B2">2</xref> Esse cen&#225;rio exigiu a elabora&#231;&#227;o de um escore de riscos para as mulheres com cardiopatias cong&#234;nitas de modo geral, a fim de que houvesse orienta&#231;&#227;o &#224; concep&#231;&#227;o. <p>Apesar dessa orienta&#231;&#227;o, tanto a gravidez desejada como a n&#227;o planejada vem gradativamente crescendo, o que aumenta, portanto, o n&#250;mero de gestantes portadoras de CCC. A escassez de publica&#231;&#245;es sobre a evolu&#231;&#227;o da gesta&#231;&#227;o nessas mulheres motivou tal estudo.</p>

        <p>Este estudo corresponde a uma das maiores s&#233;ries de gesta&#231;&#227;o em portadora de CCC acompanhada sob protocolo multiprofissional em centro de refer&#234;ncia em cardiopatia e gravidez - Instituto do Cora&#231;&#227;o e Maternidade do Hospital das Cl&#237;nicas da FMUSP. Representou 9,6% das 435 gesta&#231;&#245;es em mulheres com cardiopatias cong&#234;nitas do Registro-InCor da &#250;ltima d&#233;cada. Diante disso, &#233; indiscut&#237;vel que a maior sobrevida p&#243;s-operat&#243;ria tardia dessas pacientes resultar&#225; em n&#250;mero crescente de gesta&#231;&#245;es no futuro pr&#243;ximo.</p>
        <p>As CCC consideradas na casu&#237;stica est&#227;o classificadas pela OMS na categoria de risco III,<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> o que significa desaconselhamento &#224; gravidez, justificada pelos &#237;ndices de 25,5% de complica&#231;&#245;es maternas e de 70% de insucesso para o concepto. As altas taxas de problemas maternos (36%) e fetais (43%) fundamentam a orienta&#231;&#227;o da OMS que desaconselha a gravidez nesse grupo de pacientes.</p>
        <p>Entretanto, a experi&#234;ncia mundial frente a essa situa&#231;&#227;o cl&#237;nica &#233; crescente e se deve a mulheres que engravidam sem orienta&#231;&#227;o pr&#233;via ou &#224;quelas que desejam a gravidez a despeito dos esclarecimentos no planejamento familiar.<xref ref-type="bibr" rid="B7">7</xref> A diversidade do quadro anat&#244;mico e funcional dos problemas card&#237;acos nas CCC restringem a elabora&#231;&#227;o de protocolos para o tratamento de eventuais complica&#231;&#245;es que possam ocorrer durante a gravidez, o parto e o puerp&#233;rio. Al&#233;m disso, a maioria dos casos n&#227;o permite propostas eficazes ou de interven&#231;&#245;es fact&#237;veis no tratamento das les&#245;es card&#237;acas residuais.<xref ref-type="bibr" rid="B7">7</xref>

        <p>No entanto, o conhecimento das complica&#231;&#245;es mais frequentes que ocorrem no p&#243;s-operat&#243;rio tardio das CCC auxilia nas estrat&#233;gias para preveni-las durante a gravidez. Nesse sentido, um estudo sobre as causas de morte em pacientes com CCC mostrou que insufici&#234;ncia card&#237;aca, morte s&#250;bita, doen&#231;a card&#237;aca isqu&#234;mica e endocardite infecciosa foram as mais frequentes. Ademais, entre as les&#245;es anat&#244;micas, excluindo-se a s&#237;ndrome de Eisenmenger, a transposi&#231;&#227;o das grandes art&#233;rias e a circula&#231;&#227;o de Fontan foram as mais importantes.<xref ref-type="bibr" rid="B8">8</xref>

        <p>O artigo sobre 120 necropsias em portadoras de cardiopatias cong&#234;nitas no InCor<xref ref-type="bibr" rid="B9">9</xref> corroborou a insufici&#234;ncia card&#237;aca como principal causa de morte, uma vez que o remodelamento ventricular em resposta &#224; sobrecarga volum&#233;trica e press&#243;rica ao longo da vida favorece a fibrose e a hipertrofia e redu&#231;&#227;o do n&#250;mero de capilares intersticiais do mioc&#225;rdio. O tromboembolismo, segunda causa de morte, foi significativamente associado a sinais histol&#243;gicos de hipertens&#227;o pulmonar e tamb&#233;m detectado na hipoxemia cr&#244;nica. A terceira causa de morte registrada no estudo foi a endocardite infecciosa, o que refor&#231;ou a recomenda&#231;&#227;o, em nosso protocolo, da antibioticoprofilaxia por ocasi&#227;o do parto.</p>
        <p>A hospitaliza&#231;&#227;o eletiva como rotina a partir da 28&#170; semana de gesta&#231;&#227;o para as pacientes em situa&#231;&#227;o de presum&#237;vel risco mais elevado, independentemente do quadro funcional, fundamentou-se no fato de que o terceiro trimestre &#233; cr&#237;tico, tanto para a m&#227;e, devido &#224; sobrecarga hemodin&#226;mica e &#224; atividade pr&#243;-tromb&#243;tica (que alcan&#231;am os limites m&#225;ximos), como para o feto, pela prematuridade e restri&#231;&#227;o de crescimento intrauterino, que s&#227;o apan&#225;gio das CCC.</p>
        <p>A hospitaliza&#231;&#227;o eletiva auxiliou no refinamento do monitoramento materno-fetal, na oxigenoterapia intermitente, na anticoagula&#231;&#227;o individualizada, na otimiza&#231;&#227;o da terap&#234;utica para as poss&#237;veis complica&#231;&#245;es e no planejamento do parto.</p>
        <p>O estudo tamb&#233;m mostrou similaridade na incid&#234;ncia das complica&#231;&#245;es card&#237;acas (14,2%) e obst&#233;tricas (16,6%), sendo que, em ambas, as mortes maternas foram decorrentes de causas obst&#233;tricas (pr&#233;-ecl&#226;mpsia e hemorragia p&#243;s-parto). Isso permite considerar que a reserva card&#237;aca extremamente limitada dessas pacientes n&#227;o suporta as intercorr&#234;ncias inerentes ao ciclo grav&#237;dico-puerperal.</p>
        <p>A pr&#233;-ecl&#226;mpsia, uma das causas de &#243;bito neste estudo, &#233; respons&#225;vel por 15% das mortes maternas no Brasil, com incid&#234;ncia pr&#243;xima a 10% na popula&#231;&#227;o em geral.<xref ref-type="bibr" rid="B10">10</xref> Por isso, um diagn&#243;stico precoce e um pr&#233;-natal efetivo, embora n&#227;o a evitem, podem mudar o quadro em mulheres saud&#225;veis. Contudo, nas portadoras de cardiopatias complexas, o progn&#243;stico &#233; reservado devido &#224; enigm&#225;tica disfun&#231;&#227;o endotelial sist&#234;mica, caracter&#237;stica da doen&#231;a, e &#224; sobrecarga circulat&#243;ria determinada pela hipertens&#227;o arterial.</p>
        <p>A hemorragia p&#243;s-parto, raz&#227;o do segundo &#243;bito em nossa casu&#237;stica, &#233; considerada importante causa obst&#233;trica de morte materna em mulheres cardiopatas, e particularmente expressiva nas pacientes hipox&#234;micas. Esse fato foi documentado no relato de 366 parturientes nul&#237;paras portadoras de cardiopatias cong&#234;nitas, o qual registrou o percentual de 21% de hemorragia obst&#233;trica e identificou a ces&#225;rea prematura, a anestesia geral e o uso de heparina profil&#225;tica como vari&#225;veis de maior correla&#231;&#227;o com sangramento no p&#243;s-parto. Al&#233;m disso, entre as cardiopatias cong&#234;nitas, a circula&#231;&#227;o de Fontan foi a situa&#231;&#227;o cl&#237;nica de maior risco.<xref ref-type="bibr" rid="B11">11</xref>
				</p>
        <p>Em nosso estudo, a associa&#231;&#227;o de hipoxemia e hemorragia p&#243;s-parto ces&#225;reo ocorreu em um caso, que resultou na segunda morte materna. Na verdade, a maior morbidade materno-fetal associada &#224; ces&#225;rea prematura se deve aos casos de instabilidade cl&#237;nica materna e de restri&#231;&#227;o ao crescimento intrauterino, situa&#231;&#245;es habituais nestes casos graves.</p>
        <p>Um breve destaque deve ser dado &#224;s cardiopatias mais frequentes em nosso estudo, como a transposi&#231;&#227;o das grandes art&#233;rias e a circula&#231;&#227;o de Fontan. A evolu&#231;&#227;o promissora do p&#243;s-operat&#243;rio tardio da corre&#231;&#227;o da transposi&#231;&#227;o das grandes art&#233;rias, seja a n&#237;vel atrial (t&#233;cnicas de Senning ou Mustard) seja a n&#237;vel arterial (t&#233;cnica de Jatene), tem possibilitado o desenvolvimento de gravidez.<xref ref-type="bibr" rid="B12">12</xref>,<xref ref-type="bibr" rid="B13">13</xref> Entretanto, existem eventos previstos no p&#243;s-operat&#243;rio tardio que devem ser desfavor&#225;veis durante a gesta&#231;&#227;o. Assim, na corre&#231;&#227;o de Mustard ou Senning, estima-se que 40% das pacientes apresentam arritmias supraventriculares e disfun&#231;&#227;o ventricular na idade adulta, enquanto, na t&#233;cnica de Jatene, a insufici&#234;ncia da valva neoa&#243;rtica est&#225; presente em 1 a 2% dos casos, e as complica&#231;&#245;es coronarianas ocorrem em 3 a 11% entre os eventos tardios. Com respeito &#224; cirurgia de Rastelli, a evolu&#231;&#227;o tardia depende do tipo de tecido utilizado na confec&#231;&#227;o do conduto, que pode determinar diferentes graus de calcifica&#231;&#227;o e a oclus&#227;o progressiva desse enxerto.<xref ref-type="bibr" rid="B14">14</xref>
				</p>
        <p>Com base nessas considera&#231;&#245;es, por ocasi&#227;o da gravidez, &#233; preciso estar preparado para o tratamento da insufici&#234;ncia card&#237;aca e de arritmias quando h&#225;: disfun&#231;&#227;o ventricular direita e insufici&#234;ncia tric&#250;spide ap&#243;s as t&#233;cnicas de Mustard e Senning; insufici&#234;ncia card&#237;aca e baixo d&#233;bito frente aos condutos calcificados ap&#243;s o procedimento de Rastelli; disfun&#231;&#245;es valvares quando tiver sido usada a t&#233;cnica de Jatene.<xref ref-type="bibr" rid="B14">14</xref>
				</p>
        <p>O estudo mostrou ainda que, &#224; exce&#231;&#227;o de uma paciente n&#227;o operada que evoluiu para aborto espont&#226;neo (caso 12), as demais 14 gestantes com transposi&#231;&#227;o dos grandes vasos da base apresentaram evolu&#231;&#227;o favor&#225;vel materno-fetal, independentemente do tipo de corre&#231;&#227;o cir&#250;rgica. Vale ressaltar que as complica&#231;&#245;es previstas foram muito bem controladas com hospitaliza&#231;&#227;o e monitoramento constante da m&#227;e e do feto.</p>
        <p>A t&#233;cnica de Fontan tem proporcionado a sobrevida de 70% das pacientes com cardiopatia univentricular at&#233; a idade f&#233;rtil.<xref ref-type="bibr" rid="B15">15</xref> Entretanto, no p&#243;s-operat&#243;rio tardio, s&#227;o esperadas complica&#231;&#245;es como: taquicardia atrial, tromboembolismo (estase hep&#225;tica e do sistema venoso) , insufici&#234;ncia card&#237;aca, fal&#234;ncia hep&#225;tica e enteropatia perdedora de prote&#237;na. A conex&#227;o anormal (veia cava e circula&#231;&#227;o pulmonar), apesar de corrigir a cianose, minimizar a sobrecarga ventricular e ajustar a circula&#231;&#227;o pulmonar, pode estar prejudicada pelas varia&#231;&#245;es da press&#227;o venosa central e pela press&#227;o negativa intrator&#225;cica induzida por hiperventila&#231;&#227;o e oscila&#231;&#245;es do d&#233;bito card&#237;aco durante a gravidez.</p>
        <p>A incapacidade de adapta&#231;&#227;o das pacientes com circula&#231;&#227;o de Fontan &#224; fisiologia do ciclo grav&#237;dico-puerperal foi documentada em nosso estudo, que mostrou piora da classe funcional em todas as pacientes. A insufici&#234;ncia card&#237;aca ocorre porque o ventr&#237;culo anat&#244;mico e funcional anormal &#233; incapaz de se ajustar ao d&#233;bito card&#237;aco aumentado. Contudo, n&#227;o houve morte materna face &#224;s saud&#225;veis, porque foram obtidas boas respostas ao tratamento cl&#237;nico com uso de diur&#233;tico e betabloqueador nas pacientes que desenvolveram insufici&#234;ncia card&#237;aca (casos 15 e 41) e &#224; cardiovers&#227;o el&#233;trica no caso do flutter atrial (caso 21).</p>
        <p>A evolu&#231;&#227;o desfavor&#225;vel para o feto foi documentada em seis casos de Fontan, resultando em um aborto espont&#226;neo e cinco partos prematuros. Uma revis&#227;o da literatura que incluiu seis estudos com 255 gesta&#231;&#245;es em 133 mulheres mostrou 137 perdas fetais (69%), e dentre os 115 rec&#233;m-nascidos vivos, 68 (59%) foram prematuros e 6 (5,2%) evolu&#237;ram para &#243;bito neonatal. As causas da prematuridade n&#227;o foram detalhadas, particularmente naquelas induzidas pela antecipa&#231;&#227;o do parto devido a raz&#245;es maternas. Todavia, a rotura prematura da bolsa amni&#243;tica e o descolamento prematuro de placenta foram as intercorr&#234;ncias associadas em 6,2% e 10,9%, respectivamente.<xref ref-type="bibr" rid="B16">16</xref>
				</p>
        <p>O mau progn&#243;stico fetal foi corroborado pelo estudo multic&#234;ntrico do Reino Unido, que incluiu 50 mulheres e 124 gesta&#231;&#245;es. Ele mostrou incid&#234;ncia de 68 abortos espont&#226;neos (54,8%) ; dentre os 56 rec&#233;m-nascidos vivos (45,2%), quatro morreram devido a prematuridade extrema (parto com idade gestacional abaixo de 32 semanas). Por outro lado, as complica&#231;&#245;es maternas, como insufici&#234;ncia card&#237;aca em 13,5%, arritmias em 11,3% e tromboembolismo pulmonar em 1,19% dos casos, n&#227;o acarretaram morte da m&#227;e.<xref ref-type="bibr" rid="B17">17</xref>
				</p>
        <p>A conduta de anticoagula&#231;&#227;o plena em pacientes com circula&#231;&#227;o de Fontan considerou o risco alto de tromboembolismo peculiar a essa t&#233;cnica e o estado de hipercoagulabilidade da gravidez e do puerp&#233;rio. No entanto, n&#227;o se pode desconsiderar o estudo com 59 gesta&#231;&#245;es em 37 pacientes, o qual destacou que a anticoagula&#231;&#227;o plena foi associada a pior evolu&#231;&#227;o neonatal. Contudo, houve tr&#234;s casos de tromboembolismo materno, dois deles em pacientes n&#227;o anticoaguladas.</p>
        <p>Diante dos dilemas da conduta durante a gravidez de pacientes submetidas &#224; cirurgia de Fontan, a maioria dos especialistas considera que fun&#231;&#227;o ventricular deprimida, cianose, insufici&#234;ncia valvar mitral importante ou enteropatia perdedora de prote&#237;na s&#227;o fatores que desaconselham a gravidez.<xref ref-type="bibr" rid="B15">15</xref>-<xref ref-type="bibr" rid="B17">17</xref>
				</p>
        <p>Ao analisar as vari&#225;veis presum&#237;veis de progn&#243;stico materno e fetal, o presente estudo destacou a pior evolu&#231;&#227;o em pacientes hipox&#234;micas. O fato de o inusitado resultado das vari&#225;veis, univentricular e cirurgia pr&#233;via, n&#227;o terem demonstrado diferen&#231;as na evolu&#231;&#227;o da gravidez, possivelmente se deve ao pequeno n&#250;mero da casu&#237;stica e &#224; anatomia da les&#227;o card&#237;aca, que foi favor&#225;vel &#224; sobrevida durante a idade reprodutiva. Nesse sentido, o estudo que analisou 102 necr&#243;psias de portadores de cardiopatias cong&#234;nitas verificou que a m&#233;dia de idade dos casos n&#227;o operados era maior quando comparada &#224; dos operados, al&#233;m de essas pacientes apresentarem problemas anat&#244;micos menos graves.<xref ref-type="bibr" rid="B9">9</xref>
				</p>
        <p>Outro destaque do estudo desenvolvido &#233; o registro de dois casos (5%) de cardiopatia cong&#234;nita no neonato, o que corresponde a seis vezes mais que os 0,8% estimados para a popula&#231;&#227;o geral. Esse percentual &#233; ainda superior ao do relato de Oliveira et al., que identificaram tr&#234;s casos (3,2%) de malforma&#231;&#245;es card&#237;acas em filhos de 100 gestantes portadoras de cardiopatia cong&#234;nita acompanhadas no InCor.<xref ref-type="bibr" rid="B19">19</xref> A recorr&#234;ncia de cardiopatia nos beb&#234;s de m&#227;es com cardiopatias cong&#234;nitas deve ser considerada no planejamento familiar, em resposta aos questionamentos sobre a heran&#231;a familiar, e na indica&#231;&#227;o de ecocardiograma fetal de rotina para o rastreamento intrauterino durante a gesta&#231;&#227;o nessas pacientes.</p>
        <sec>
          <title>Limita&#231;&#245;es do estudo</title>
          <p>O n&#250;mero pequeno e a heterogeneidade dos defeitos anat&#244;micos limitam a an&#225;lise estat&#237;stica acurada. Contudo, h&#225; de se considerar que a amostra de pacientes foi exclusiva ao grupo de alto risco, no qual a gravidez &#233; desaconselhada, e constitui o grande dilema de orienta&#231;&#227;o no planejamento familiar. O car&#225;ter do estudo - retrospectivo e observacional, restrito a um &#250;nico centro - pode tamb&#233;m influenciar as conclus&#245;es adequadas.</p>
        </sec>
        <sec>
          <title>Considera&#231;&#245;es finais</title>
          <p>As cardiopatias cong&#234;nitas afetam aproximadamente 0,8% de todos os rec&#233;m-nascidos vivos, e as taxas de sobrevida de 86% s&#227;o destaques nos registros internacionais. Estima-se que, atualmente, existam mais adultos com cardiopatias cong&#234;nitas do que crian&#231;as, o que, naturalmente, proporciona um n&#250;mero consider&#225;vel de mulheres em idade reprodutiva.</p>
          <p>A capacita&#231;&#227;o da equipe multiprofissional &#233; fundamental no planejamento familiar da jovem cardiopata, sobretudo no aconselhamento &#224; gravidez, como as alternativas &#224; anticoncep&#231;&#227;o segura e efetiva. Apesar da estratifica&#231;&#227;o do risco da OMS, que permite desaconselhar a gravidez, deve-se considerar o que consta na legisla&#231;&#227;o brasileira:</p>
          <p>&#8220;Ademais h&#225; de se considerar o artigo 226: Fundado nos princ&#237;pios da dignidade da pessoa humana e da paternidade respons&#225;vel, o planejamento familiar &#233; livre decis&#227;o do casal, competindo ao Estado propiciar recursos educacionais e cient&#237;ficos para o exerc&#237;cio desse direito, vedada qualquer forma coercitiva por parte de institui&#231;&#245;es oficiais ou privadas&#8221; (grifo dos autores).</p>
          <p>Essa norma remete a outros institutos: o da dignidade da pessoa humana (artigo 1&#186;, III) e o do direito &#224; liberdade (artigo 5&#176;, caput).<xref ref-type="bibr" rid="B20">20</xref>)</p>

        <title>Conclus&#245;es</title>
        <p>O r&#237;gido protocolo de atendimento durante a gravidez, o parto e o puerp&#233;rio n&#227;o evitou mortes maternas, prematuridade e perdas fetais em portadoras de CCC. A hipoxemia foi um fator de mau progn&#243;stico, e se a evolu&#231;&#227;o materna foi insatisfat&#243;ria, pior foi a parte reservada ao concepto. Embora a autonomia da inten&#231;&#227;o &#224; concep&#231;&#227;o precise ser respeitada, a gravidez ainda deve ser desaconselhada em mulheres com CCC.</p>
