  <body>
    <p>Atrial fibrillation (AF) episodes have been traditionally observed in up to 20% of the patients who suffer an acute myocardial infarction (AMI); 5-10% of them, as a &#8220;first episode&#8221;, during hospital admission.<xref ref-type="bibr" rid="B1">1</xref> The etiology of AF, in this phase of AMI, includes several factors, such as elevated atrial pressure resulting from acute ventricular dysfunction, associated atrial ischemia (more common in inferior wall AMI), secondary inflammatory reaction, changes in autonomic nervous system behavior and in the neurohumoral pattern related to the pathophysiology of AMI, especially in those with ventricular dysfunction.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>A recent study published in the Arquivos Brasileiros de Cardiologia by Congo et al.<xref ref-type="bibr" rid="B4">4</xref> suggest that, in the last years, the incidence of AF in the acute phase of AMI may be decreasing, due to the greater access of patients to early reperfusion and to better clinical treatment, associated with the more frequent use of angiotensin converting enzyme inhibitors (ACE inhibitors), beta blockers, statins and antiplatelet therapy. In addition, the study corroborates previous observations that new onset AF is associated to poorer clinical evolution and raises awareness of the clinical management of these patients, in particular in relation to the non-use of oral anticoagulants at hospital discharge.</p>
    <p>The study was performed using data obtained from a Portuguese national registry on AMI, which included 6,325 patients from 2010 to 2017, out of whom 365 (5.8%) presented an initial episode or first incident AF in the acute phase of AMI, most of them in the first day of hospitalization. Patients admitted due to AMI and ST-segment elevation who had (first episode) AF were compared with patients who did not develop AF during hospitalization.</p>
    <p>Acute reperfusion rates and strategies were similar in both groups (without AF = 83.2%, with AF = 82.9%), apparently with no impact on the occurrence of AF. However, it was observed that patients with AF had longer door-to-balloon time, less prescription of beta blockers, ACE inhibitors and angiotensin receptor blockers. Factors like advanced age, previous stroke, complete atrioventricular blockage and inferior infarction were independent predictors for the occurrence of AF.</p>
    <p>The occurrence of AF was associated with longer hospitalization time, higher incidence of complications and mortality (13.4%) compared with the control group (3.8%; p &lt; 0.001). However, the occurrence of AF was not an independent predictor of in-hospital mortality, which suggests that it may be only an indicator of greater disease severity.</p>
    <p>Unfortunately, the study does not provide information on the clinical presentation of AF, detailed clinical management of patients during hospitalization and AF recurrence during evolution. Two-thirds of the patients who developed new onset AF received amiodarone during hospitalization, which was maintained in 26% of patients at hospital discharge. In addition, only 20.6% were discharged on oral anticoagulants, with or without double platelet aggregation. These observation may reflect the lack of information on the literature in how to manage these patients. Then, more robust data on the decisions concerning heart rate (HR) control or rhythm control and the need for maintenance of anticoagulation after hospital discharge are still required.</p>
    <p>For instance, in the VALsartan In Acute myocardial iNfarcTion (VALIANT) trial,<xref ref-type="bibr" rid="B5">5</xref> 1,131 patients who presented with AF after AMI and heart failure were assessed; 371 were treated with rhythm control (amiodarone 87.3%) and 760 patients with HF control strategy. After adjustment to the clinical profile of patients, rhythm control was associated with increased mortality in the first 45 days (HR: 1.9, 95% CI 1.2-3.0, p = 0.004), but not with late mortality. In this study, mortality cannot be attributed to the use of Class I antiarrhythmics (AA), since most of deaths occurred in the rhythm control group receiving amiodarone (95.6%). The relation between amiodarone and mortality was also observed in the Global Use of Strategies to Open Occluded Coronary Arteries (GUSTO-3) trial.<xref ref-type="bibr" rid="B6">6</xref> For rhythm control, 132 patients (12%) used Class I AA agents, 55 (5%) sotalol and 168 (15%) amiodarone. The sinus rhythm was restored in 72% of the patients who received Class I drugs, in 67% of those receiving sotalol, and in 79% of the patients receiving amiodarone. There was a tendency to less mortality with the use of Class I AA drugs and sotalol in relation to the patients receiving amiodarone, which the authors attributed to the probable option for Class I AA drugs and sotalol in the case of patients with better clinical condition.</p>
    <p>The Global Utilization of Streptokinase and TPA for Occluded Coronary Arteries (GUSTO-I) trial,<xref ref-type="bibr" rid="B7">7</xref> on the other hand, which included 40,891 patients with AMI, registered an increased incidence of stroke in patients with AF in the acute phase of AMI (3.1%), compared with those who maintained the sinus rhythm (1.3%) - (p = 0.0001). Ischemic strokes were observed in 1.8% and 0.5% of patients with or without AF, respectively. In addition, the this study did not provide data on the use of anticoagulants after hospital discharge. Hence, Siu et al.<xref ref-type="bibr" rid="B8">8</xref> analyzed 431 consecutive patients with inferior wall AMI and preserved ventricular function. They found that transient new-onset AF during hospitalization was observed in 59 patients (13.9%). Over a mean follow-up period of one year, the incidence of AF was of 22% in patients with transitory AF in admission, compared with 1.3% (p = 0.01) in the group without AF and, even more importantly, the incidence of ischemic stroke was of 10.2% in patients with transitory AF and of 1.8% in those without AF (p = 0.01). It is worth highlighting that the patients with AF did not receive anticoagulants, only antiplatelet agents. Similar findings were obtained in the Optimal Trial in Myocardial Infarction with the Angiotensin II Antagonist Losartan (OPTIMAAL) trial<xref ref-type="bibr" rid="B9">9</xref> in patients with AMI and ventricular dysfunction, in which the presence of new onset AF was also associated with increased risk of stroke in the first 30 days (HR 14.6, p &lt; 0.001) and during the whole period of observation of the study (HR 2.29, p &lt; 0.001).</p>
    <p>Current guidelines recommend that patients with acute coronary syndrome and AF should receive anticoagulant therapy (warfarin or NOACs), associated with aspirin and/or clopidogrel for at least 6 months, except if they are absolutely contraindicated.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref> However, the studies that support it are based on the existence of known AF or AF prior to the coronary event.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B17">17</xref>
		</p>
    <p>In the absence of randomized clinical trials, it seems reasonable that this recommendation should also be applied to the patients who presented with a first-episode AF in the acute phase of AMI, particularly in those with high-risk predictors of recurrence and with elevated CHA2DS2-VASc score<xref ref-type="bibr" rid="B18">18</xref>.</p>

      <p>Epis&#243;dios de fibrila&#231;&#227;o atrial (FA) t&#234;m sido observados tradicionalmente em at&#233; 20% dos pacientes que sofrem infarto agudo do mioc&#225;rdico (IAM); 5-10% desses, como primeiro epis&#243;dio, durante a interna&#231;&#227;o hospitalar.<xref ref-type="bibr" rid="B1">1</xref> A etiologia da FA, nessa fase do IAM tem v&#225;rios fatores, incluindo aumento da press&#227;o atrial pela disfun&#231;&#227;o ventricular aguda, isquemia atrial associada (mais comum no IAM inferior), rea&#231;&#227;o inflamat&#243;ria secund&#225;ria, mudan&#231;as no comportamento do sistema nervoso aut&#244;nomo e no padr&#227;o neuro-humoral relacionados &#224; fisiopatologia do IAM, principalmente naqueles com disfun&#231;&#227;o ventricular.<xref ref-type="bibr" rid="B1">1</xref>-<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Estudo recente, publicado nos Arquivos Brasileiros de Cardiologia por Congo et al.,<xref ref-type="bibr" rid="B4">4</xref> sugere que nos &#250;ltimos anos a incid&#234;ncia de FA na fase aguda do IAM pode estar diminuindo, devido ao maior acesso dos pacientes &#224; reperfus&#227;o precoce e ao melhor tratamento cl&#237;nico, com uso mais frequente de inibidores da enzima conversora de angiotensina (IECA), beta bloqueadores estatinas e terapia antiplaquet&#225;ria. Adicionalmente, o estudo confirma observa&#231;&#245;es anteriores de que o surgimento de FA associa-se a pior evolu&#231;&#227;o cl&#237;nica hospitalar e alerta em rela&#231;&#227;o ao manuseio cl&#237;nico desses pacientes, em particular da n&#227;o utiliza&#231;&#227;o de anticoagulantes orais na alta hospitalar.</p>
      <p>O estudo foi realizado a partir de dados obtidos de um registro nacional portugu&#234;s de IAM, que incluiu 6.325 pacientes no per&#237;odo de 2010 a 2017, dos quais 365 (5,8%) apresentaram epis&#243;dio inicial ou primeiro registro de FA na fase aguda do IAM, a maioria j&#225; no primeiro dia de interna&#231;&#227;o. Foram comparados pacientes internados por IAM e supradesnivelamento do segmento ST que tiveram FA (primeiro epis&#243;dio), com pacientes que n&#227;o desenvolveram FA durante a interna&#231;&#227;o hospitalar.</p>
      <p>A taxa e estrat&#233;gias de reperfus&#227;o aguda foram semelhantes nos dois grupos (sem FA = 83,2%, com FA = 82,9%), aparentemente sem impacto na ocorr&#234;ncia de FA. Mas, observaram que os pacientes com FA tiveram tempo porta bal&#227;o mais prolongado, menor prescri&#231;&#227;o de betabloqueadores, IECA e bloqueadores dos receptores da angiotensina. Fatores como idade avan&#231;ada, acidente vascular cerebral (AVC) pr&#233;vio, bloqueio atrioventricular completo e o infarto inferior foram preditores independentes da ocorr&#234;ncia de FA.</p>
      <p>A ocorr&#234;ncia de FA associou-se &#224; interna&#231;&#245;es mais prolongadas, maior incid&#234;ncia de complica&#231;&#245;es e maior mortalidade (13,4%) em compara&#231;&#227;o com o grupo controle (3,8%; p &lt; 0,001). Entretanto, a ocorr&#234;ncia de FA n&#227;o foi preditor independente de mortalidade hospitalar, indicando que pode ser apenas um indicador de maior gravidade da doen&#231;a.</p>
      <p>Infelizmente, o estudo n&#227;o fornece informa&#231;&#227;o sobre a apresenta&#231;&#227;o cl&#237;nica da FA, o manuseio cl&#237;nico detalhado dos pacientes durante a interna&#231;&#227;o e a taxa de recorr&#234;ncia na evolu&#231;&#227;o. Dois ter&#231;os dos pacientes com FA de in&#237;cio recente receberam amiodarona durante a interna&#231;&#227;o, que foi mantida em 26% dos pacientes na alta hospitalar. Adicionalmente, apenas 20,6% tiveram alta sob anticoagula&#231;&#227;o oral, com ou sem dupla agrega&#231;&#227;o plaquet&#225;ria. Nesse sentido, h&#225; car&#234;ncia na literatura sobre a melhor conduta a ser tomada e dados mais robustos sobre decis&#245;es em rela&#231;&#227;o ao controle da frequ&#234;ncia card&#237;aca (FC) ou o controle do ritmo e a necessidade de manuten&#231;&#227;o da anticoagula&#231;&#227;o ap&#243;s a alta hospitalar, ainda s&#227;o necess&#225;rios.</p>
      <p>Por exemplo, no estudo VALIANT (Valsartan in Acute Myocardial Infarction Trial)<xref ref-type="bibr" rid="B5">5</xref> foram avaliados 1.131 pacientes que apresentaram FA ap&#243;s o IAM e insufici&#234;ncia card&#237;aca; 371 foram tratados com controle do ritmo (amiodarona em 87,3%) e 760 pacientes com a estrat&#233;gia do controle de FC. Ap&#243;s os ajustes para os perfis cl&#237;nicos dos pacientes, o controle do ritmo foi associado ao aumento de mortalidade nos primeiros 45 dias (HR: 1,9, 95% CI 1,2 a 3,0, p = 0,004), mas n&#227;o &#224; mortalidade tardia. Nesse estudo, a mortalidade n&#227;o pode ser atribu&#237;da ao uso de antiarr&#237;tmico (AA) de classe I, j&#225; que a maioria das mortes ocorridas no grupo controle do ritmo receberam amiodarona (95,6%). A correla&#231;&#227;o entre amiodarona e mortalidade foi observada tamb&#233;m no estudo GUSTO-3 (The Global Use of Strategies to Open Occluded Coronary Arteries).<xref ref-type="bibr" rid="B6">6</xref> Para controle do ritmo, 132 pacientes (12%) usaram AA de classe I, 55 (5%) sotalol e 168 (15%) amiodarona. O ritmo sinusal foi restaurado em 72% dos pacientes que receberam f&#225;rmacos de classe I, 67% dos que receberam sotalol, e 79% daqueles que receberam amiodarona. Houve tend&#234;ncia a menor mortalidade com uso de AA da classe I e sotalol em rela&#231;&#227;o aos pacientes com amiodarona, que os autores atribu&#237;ram &#224; prov&#225;vel op&#231;&#227;o pelos f&#225;rmacos AAs de classe I e sotalol nos pacientes com melhor condi&#231;&#227;o cl&#237;nica.</p>
      <p>J&#225; o estudo GUSTO-I (Global Utilization of Streptokinase and TPA for Occluded Coronary Arteries)<xref ref-type="bibr" rid="B7">7</xref> que incluiu 40.891 pacientes com IAM, documentou aumento da incid&#234;ncia de AVC nos pacientes com FA na fase aguda do IAM (3,1%), em compara&#231;&#227;o com aqueles que mantiveram o ritmo sinusal (1,3%) - (p = 0,0001). Acidentes vasculares isqu&#234;micos foram observados em 1,8% e 0,5% nos pacientes com ou sem FA, respectivamente. Tamb&#233;m, esse estudo n&#227;o forneceu dados sobre o uso de anticoagulantes ap&#243;s a alta hospitalar. Nesse sentido, Siu et al.<xref ref-type="bibr" rid="B8">8</xref> analisaram 431 pacientes consecutivos com IAM inferior e fun&#231;&#227;o ventricular preservada. Observaram que FA de in&#237;cio recente e transit&#243;ria durante a interna&#231;&#227;o foi observada em 59 pacientes (13,9%). Durante o seguimento m&#233;dio de um ano, a incid&#234;ncia de FA foi de 22% nos pacientes que tiveram FA transit&#243;ria na interna&#231;&#227;o, em compara&#231;&#227;o com 1,3% (p = 0,01) no grupo sem FA e, mais importante ainda, a incid&#234;ncia de AVC isqu&#234;mico foi de 10,2% nos pacientes com FA transit&#243;ria e de 1,8% naqueles sem FA (p = 0,01). Vale ressaltar que os pacientes com FA n&#227;o receberam anticoagulantes, apenas agentes antiplaquet&#225;rios. Achados semelhantes foram obtidos no estudo OPTIMAAL (The Optimal Trial in Myocardial Infarction with the Angiotensin II Antagonist Losartan)<xref ref-type="bibr" rid="B9">9</xref> em pacientes com IAM e disfun&#231;&#227;o ventricular, no qual a presen&#231;a de FA de in&#237;cio recente tamb&#233;m foi associada ao aumento do risco de AVC nos primeiros 30 dias (HR 14,6, p &lt; 0,001) e durante todo o per&#237;odo da observa&#231;&#227;o do estudo (HR 2,29, p &lt; 0,001).</p>
      <p>As diretrizes atuais recomendam que pacientes com s&#237;ndrome coron&#225;ria aguda e FA devem receber terapia anticoagulante (varfarina ou NOACs), associada a aspirina e/ou clopidogrel por pelo menos 6 meses, exceto se apresentarem contraindica&#231;&#227;o absoluta.<xref ref-type="bibr" rid="B10">10</xref>,<xref ref-type="bibr" rid="B11">11</xref> Entretanto, os estudos que lhe d&#227;o sustenta&#231;&#227;o s&#227;o baseados na exist&#234;ncia de FA conhecida ou pr&#233;via ao advento coronariano.<xref ref-type="bibr" rid="B12">12</xref>-<xref ref-type="bibr" rid="B17">17</xref>
			</p>
      <p>Na aus&#234;ncia de estudos cl&#237;nicos randomizados, parece razo&#225;vel que essa conduta tamb&#233;m se aplique aos pacientes que apresentaram primeiro epis&#243;dio de FA na fase aguda do IAM, em particular naqueles com preditores de alto risco de recorr&#234;ncia e com escore CHA2DS2-VASc elevado.<xref ref-type="bibr" rid="B18">18</xref>
