    <p>Cardiovascular diseases (CVD) are the leading cause of death in the world,<xref ref-type="bibr" rid="B1">1</xref> with increasing numbers of cases in low and middle-income countries. In Brazil, it is estimated that 350,000 patients die each year due to CVD.<xref ref-type="bibr" rid="B2">2</xref> The relationship between acute coronary syndrome (ACS) and peripheral atherosclerotic disease (PAD) is well established.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>Cross-sectional studies carried out in countries with genetic characteristics different from ours help us to evaluate the possible relationship between the characteristics of the patients evaluated and the increase in cardiovascular risk, and, despite their limitations, are good hypothesis generators.</p>
    <p>The present study<xref ref-type="bibr" rid="B5">5</xref> analyzed the characteristics of patients with ACS and PAD, showing that advanced age, diabetes, worse lipid profile and multiarterial disease were more prevalent in patients with ACS and PAD than in patients with ACS and without PAD. In addition, it suggested that patients with such an association have a worse prognosis.</p>
    <p>Limitations of the present study were the facts of its retrospective analysis and the exclusion of patients at higher risk (prior acute myocardial infarction, acute myocardial infarction caused by thrombus detachment, intravascular surgery, patients with cardiogenic shock and post-cardiac arrest and gastrointestinal bleeding on admission), which could have shown an even higher risk in such patients, considering that generally patients with associated PAD have greater complications and worse prognosis. It is also worth noting that it was carried out in a single center.</p>
    <p>However, the present study has great value for showing once again, as previous studies have demonstrated,<xref ref-type="bibr" rid="B6">6</xref> the worsening of the prognosis of patients with ACS and PAD and which risk factors are most prevalent in this population, making possible the detection of subgroups of patients with ACS and PAD are more susceptible to a worse outcome and in this way emphasize their strict control. It is also necessary to offer the best treatment evidenced in the literature to such patients since real-life studies have shown that despite the higher risk of patients with ACS and PAD, they frequently receive less drugs with established benefit.<xref ref-type="bibr" rid="B7">7</xref>

      <p>As doen&#231;as cardiovasculares (DCVs) s&#227;o a principal causa de morte em todo o mundo,<xref ref-type="bibr" rid="B1">1</xref> com n&#250;meros crescentes de casos em pa&#237;ses de baixa e m&#233;dia renda. No Brasil, estima-se que 350 mil pacientes morrem a cada ano por DCVs.<xref ref-type="bibr" rid="B2">2</xref> A rela&#231;&#227;o entre s&#237;ndrome coronariana aguda (SCA) e doen&#231;a arterial perif&#233;rica (DAP) est&#225; bem estabelecida.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>Estudos transversais realizados em pa&#237;ses com popula&#231;&#245;es de caracter&#237;sticas gen&#233;ticas diferentes das nossas ajudam a avaliar a possibilidade da rela&#231;&#227;o entre as caracter&#237;sticas dos pacientes avaliados e o aumento do risco cardiovascular e, apesar de suas limita&#231;&#245;es, esses estudos s&#227;o bons geradores de hip&#243;teses.</p>
      <p>O presente estudo<xref ref-type="bibr" rid="B5">5</xref> analisou caracter&#237;sticas de pacientes com SCA e DAP e mostrou que idade avan&#231;ada, diabetes, perfil lip&#237;dico desfavor&#225;vel e doen&#231;a arterial eram mais prevalentes em pacientes com SCA e DAP que em pacientes com SCA e sem DAP. Al&#233;m disso, os resultados sugeriram que pacientes com tal associa&#231;&#227;o apresentam um pior progn&#243;stico.</p>
      <p>Limita&#231;&#245;es do presente estudo foram sua an&#225;lise retrospectiva e a exclus&#227;o de pacientes em alto risco - pacientes com infarto agudo do mioc&#225;rdio (IAM) pr&#233;vio, IAM causado por deslocamento de trombo, cirurgia intravascular, pacientes com choque cardiog&#234;nico, parada card&#237;aca, e pacientes com sangramento gastrointestinal na admiss&#227;o. A n&#227;o exclus&#227;o desses pacientes poderia ter demonstrado um risco ainda maior nesses pacientes, considerando que, em geral, os pacientes com DAP associada apresentam mais complica&#231;&#245;es e pior progn&#243;stico. Vale ressaltar ainda que o estudo foi realizado em um &#250;nico centro.</p>
      <p>No entanto, o estudo tem grande valor ao mostrar, mais uma vez, conforme outros estudos j&#225; demonstraram,<xref ref-type="bibr" rid="B6">6</xref> o pior progn&#243;stico dos pacientes com SCA e DAP, e quais fatores de risco s&#227;o mais prevalentes nessa popula&#231;&#227;o, tornando poss&#237;vel detectar quais subgrupos de pacientes com SCA e DAP estariam mais suscept&#237;veis a um pior desfecho e, assim, enfatizar seu controle. Tamb&#233;m &#233; necess&#225;rio oferecer o melhor tratamento evidenciado na literatura para esses pacientes, uma vez que estudos na vida real mostraram que, apesar de pacientes com SCA e DAP apresentarem maior risco, frequentemente recebem menos medicamentos com benef&#237;cio j&#225; estabelecido.<xref ref-type="bibr" rid="B7">7</xref>
