    <p>During the nutritional follow-up of hospitalized individuals, the nutritionist has as activities to perform the diagnosis, dietary prescription, supervise the distribution of the diets and evaluate their acceptance, and also perform nutritional counseling so that these individuals understand how a standard specific diet may be more appropriate taking into account their diagnoses and nutritional status. The number of nutritionists issued by Resolution 600 of 2018 of the Federal Council of Nutritionists is 1 professional every 15 beds of high complexity and every 30 beds of medium complexity.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>Lima et al.<xref ref-type="bibr" rid="B2">2</xref> evaluated in an article published in this edition whether nutritional counseling was performed in the hospital environment for patients with Acute Myocardial Infarction (AMI) and the quality of this orientation. The authors found that 57.6% of the individuals hospitalized in the public network and 70.3% private hospitals, both in Sergipe, Brazil, had received in-hospital nutritional counseling. </p>
    <p>One possible cause of this low rate of counseling provided may be the amount of nutritionists available in hospital institutions, which is lower than predicted by the resolution.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>Seta et al.,<xref ref-type="bibr" rid="B3">3</xref> 2010, evaluated 8 public hospitals in 4 Brazilian states, of which none of the nutritionists evaluated reported nutritional counseling.<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>Another problem is the quality of the guidance provided. It should be checked whether it meets the guidelines for preventing the occurrence of new cardiovascular events.</p>
    <p>In the cited article there was a predominance of restrictive guidelines, especially salt and fat. About the insertion of cardioprotective foods, patients from the private network were more benefited, mainly regarding the consumption of fruits and vegetables.</p>
    <p>The diet for prevention after acute myocardial infarction requires caloric adequacy, applied calorie restriction when necessary for the adequacy of nutritional status. It is important that the macronutrients are adequate within normality, taking into account the restriction of saturated fats and balance between the other fats as recommended by the dyslipidemia guideline.<xref ref-type="bibr" rid="B4">4</xref>-<xref ref-type="bibr" rid="B6">6</xref> In Addition, current guidelines on prevention of cardiovascular events recommend a diet similar to the Mediterranean diet, salt intake of &lt; 5 g per day; 30-45 g fibre per day; regular consumption of fruits and vegetables per daily; regular consumption of fish and unsalted nuts daily; limited alcohol intake; and discouraging sugar-sweetened drinks.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref>
		</p>
    <p>The better understanding of the food behavior is indispensable to deepen the knowledge of the determinants of the alimentary behavior, which include a complex range of nutritional, demographic, social, cultural, environmental and psychological factors. Several studies point out that the transthoracic model,<xref ref-type="bibr" rid="B9">9</xref> developed by two US researchers, James O. Prochaska and Carlo DiClemente,<xref ref-type="bibr" rid="B10">10</xref> in the 1980s, can be considered a promising instrument to help understanding health-related behavioral change, and is widely used in research and clinical practice. The transthoracic model of behavior change presents 5 stages. In the pre-contemplation stage, it has not yet been considered by the individual or no changes were made to the behavior and there is no intention to adopt them in the near future. In the stage of contemplation, the individual begins to consider behavioral change. That is, it is intended to change the behavior in the future, but a deadline has not yet been set, therefore.</p>
    <p>The decision-making individual, also called the preparation, intends to change his behavior in the near future, as in the next month. Generally, after overcoming previous attempts frustrated, small changes are made and a plan of action is adopted, still not making a serious commitment to the same. Already the individuals in action correspond to those who have in fact altered their behavior, their experiences or their environment so as to overcome barriers previously perceived. Such changes are visible and have occurred recently, as in the last six months. In the maintenance stage, the individual already changed his behavior and kept him more than six months.<xref ref-type="bibr" rid="B10">10</xref>
		</p>
    <p>The study by Vieira et al.,<xref ref-type="bibr" rid="B11">11</xref> carried out with individuals after angioplasty at a hospital specialized in cardiology, in S&#227;o Paulo, identified the stages of behavior change in which they were. 36% were in maintenance, 26% in preparation, 17% in pre-contemplation, 12% in action and 9% in contemplation. It is necessary for the nutrition team to create adequate food education strategies for the individuals at each stage, in order to promote adherence to a more favorable food plan and the adequacy of nutritional status.</p>
    <p>A study with individuals from the northern region of Paran&#225; after AMI or angioplasty aimed at identifying changes in attitudes and habits in these survivors. The main changes identified were an increase in the number of meals, an increase in fruit consumption, a reduction in the consumption of fats and fried foods, and the use of the salt shaker on the table. The number of patients who did not perform physical activity decreased.<xref ref-type="bibr" rid="B12">12</xref>
		</p>
    <p>It is very important to evaluate the achievemente and the quality of nutritional counseling to allows the implementation of appropriate actions, since the moment after the recent cardiovascular event may favor the adoption of favorable dietary changes for these individuals.</p>

      <p>Durante o acompanhamento nutricional de indiv&#237;duos hospitalizados, o nutricionista realiza o diagn&#243;stico, prescri&#231;&#227;o diet&#233;tica, supervisiona a distribui&#231;&#227;o das dietas e avalia sua aceita&#231;&#227;o, al&#233;m de realizar aconselhamento nutricional para que os pacientes compreendam como um padr&#227;o diet&#233;tico espec&#237;fico pode ser mais adequada tendo em conta os seus diagn&#243;sticos e estado nutricional. Segundo a Resolu&#231;&#227;o 600 de 2018 do Conselho Federal de Nutricionistas, o n&#250;mero de profissionais necess&#225;rios &#233; 1 a cada 15 leitos de alta complexidade e a cada 30 leitos de m&#233;dia complexidade.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>Lima et al.,<xref ref-type="bibr" rid="B2">2</xref> em um artigo publicado nesta edi&#231;&#227;o, avaliaram se era realizado aconselhamento nutricional no ambiente hospitalar para pacientes que haviam sofrido Infarto Agudo do Mioc&#225;rdio (IAM) e a qualidade dessa orienta&#231;&#227;o. Segundo eles, 57,6% dos indiv&#237;duos internados na rede p&#250;blica e 70,3% na rede privada, em Sergipe, receberam aconselhamento nutricional intra-hospitalar.</p>
      <p>Uma poss&#237;vel causa dessa baixa taxa de aconselhamento pode ser a quantidade de nutricionistas dispon&#237;veis nas institui&#231;&#245;es hospitalares, que &#233; mais baixa do que o previsto pela resolu&#231;&#227;o.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>Seta et al.,<xref ref-type="bibr" rid="B3">3</xref> avaliaram, em 2010, oito hospitais p&#250;blicos em quatro estados brasileiros, dos quais nenhum nutricionista relatou aconselhamento nutricional.<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Outro problema &#233; a qualidade da orienta&#231;&#227;o. &#201; preciso verificar se ela atende &#224;s diretrizes para prevenir a ocorr&#234;ncia de novos eventos cardiovasculares.</p>
      <p>Nesse artigo, houve predom&#237;nio de diretrizes restritivas, principalmente quanto ao sal e gorduras. Os pacientes da rede privada foram mais beneficiados quanto &#224; inser&#231;&#227;o de alimentos cardioprotetores, principalmente frutas e hortali&#231;as.</p>
      <p>A dieta preventiva ap&#243;s IAM requer adequa&#231;&#227;o cal&#243;rica e restri&#231;&#227;o cal&#243;rica aplicada, quando necess&#225;rio, para adequa&#231;&#227;o do estado nutricional. &#201; importante que os macronutrientes sejam adequados dentro da normalidade, levando-se em conta a restri&#231;&#227;o de gorduras saturadas e o equil&#237;brio entre as outras gorduras, conforme recomendado pelas diretrizes sobre dislipidemia.<xref ref-type="bibr" rid="B4">4</xref>-<xref ref-type="bibr" rid="B6">6</xref> Al&#233;m disso, as recomenda&#231;&#245;es atuais de preven&#231;&#227;o de eventos cardiovasculares englobam uma dieta semelhante &#224; mediterr&#226;nica: ingest&#227;o &lt;5 g de sal por dia; 30-45 g de fibra por dia; consumo regular de frutas e legumes por dia; consumo di&#225;rio regular de peixe e nozes sem sal, ingest&#227;o limitada de &#225;lcool, e desencorajamento do consumo bebidas ado&#231;adas com a&#231;&#250;car.<xref ref-type="bibr" rid="B7">7</xref>,<xref ref-type="bibr" rid="B8">8</xref>

      <p>A compreens&#227;o do comportamento alimentar &#233; indispens&#225;vel para aprofundar o conhecimento dos fatores determinantes do comportamento alimentar, que incluem uma gama complexa de fatores nutricionais, demogr&#225;ficos, sociais, culturais, ambientais e psicol&#243;gicos. Diversos estudos apontam que o modelo transte&#243;rico,<xref ref-type="bibr" rid="B9">9</xref> desenvolvido na d&#233;cada de 1980 por dois pesquisadores norte-americanos, James O. Prochaska e Carlo DiClemente,<xref ref-type="bibr" rid="B10">10</xref> pode ser considerado um instrumento promissor para auxiliar a compreender a mudan&#231;a comportamental relacionada &#224; sa&#250;de e &#233; amplamente utilizado em pesquisa e pr&#225;tica cl&#237;nica. O modelo transte&#243;rico de mudan&#231;a de comportamento tem cinco etapas. No est&#225;gio de pr&#233;-contempla&#231;&#227;o, ainda n&#227;o foi considerada pelo indiv&#237;duo ou nenhuma mudan&#231;a foi feita, n&#227;o havendo inten&#231;&#227;o de adot&#225;-la em um futuro pr&#243;ximo. No est&#225;gio da contempla&#231;&#227;o, o indiv&#237;duo come&#231;a a considerar a mudan&#231;a comportamental, isto &#233;, pretende mudar o comportamento no futuro, mas um prazo ainda n&#227;o foi definido.</p>
      <p>O indiv&#237;duo que toma a decis&#227;o, na fase chamada de prepara&#231;&#227;o, pretende mudar seu comportamento no futuro pr&#243;ximo, por exemplo, no pr&#243;ximo m&#234;s. Geralmente, depois de superar tentativas anteriores frustradas, pequenas mudan&#231;as s&#227;o feitas e um plano de a&#231;&#227;o &#233; adotado, ainda n&#227;o havendo um compromisso s&#233;rio com o mesmo. J&#225; os indiv&#237;duos em a&#231;&#227;o de fato alteraram seu comportamento, suas experi&#234;ncias ou seu ambiente para superar as barreiras previamente percebidas. Essas mudan&#231;as s&#227;o vis&#237;veis e ocorreram recentemente, nos &#250;ltimos seis meses, por exemplo. Na fase de manuten&#231;&#227;o, o indiv&#237;duo j&#225; mudou seu comportamento e o manteve por mais de seis meses.<xref ref-type="bibr" rid="B10">10</xref>
			</p>
      <p>O estudo de Vieira et al.,<xref ref-type="bibr" rid="B11">11</xref> realizado com pacientes que passaram por angioplastia em um hospital especializado em cardiologia de S&#227;o Paulo, identificou os est&#225;gios de mudan&#231;a de comportamento em que se encontravam: 36% estavam em manuten&#231;&#227;o, 26% em prepara&#231;&#227;o, 17% em pr&#233;-contempla&#231;&#227;o, 12% em a&#231;&#227;o e 9% em contempla&#231;&#227;o. &#201; necess&#225;rio que a equipe de nutri&#231;&#227;o crie estrat&#233;gias adequadas de educa&#231;&#227;o alimentar para os indiv&#237;duos em cada uma dessas etapas, a fim de promover a ades&#227;o a um plano alimentar mais favor&#225;vel e a adequa&#231;&#227;o do estado nutricional.Um estudo buscou identificar mudan&#231;as de atitudes e h&#225;bitos em indiv&#237;duos da regi&#227;o norte do Paran&#225;, ap&#243;s IAM ou angioplastia. As principais mudan&#231;as identificadas foram: aumento do n&#250;mero de refei&#231;&#245;es, aumento no consumo de frutas, redu&#231;&#227;o no consumo de gorduras e frituras e uso do saleiro na mesa. O n&#250;mero de pacientes que n&#227;o realizavam atividade f&#237;sica diminuiu.<xref ref-type="bibr" rid="B12">12</xref>
			</p>
      <p>&#201; muito importante avaliar o alcance e a qualidade da orienta&#231;&#227;o nutricional de forma a permitir a implementa&#231;&#227;o de a&#231;&#245;es adequadas, j&#225; que o momento seguinte ao evento cardiovascular pode ser ben&#233;fico para se adotar mudan&#231;as diet&#233;ticas favor&#225;veis.</p>
