  <body>
    <p>There is growing evidence that platelets play a key role in vascular atherothrombosis associated with endothelium and inflammation.<xref ref-type="bibr" rid="B1">1</xref> Platelet activation leads to abnormalities in the surface glycoprotein expression of these cells, with subsequent aggregation and degranulation, which greatly contribute to thrombus formation.<xref ref-type="bibr" rid="B2">2</xref> Corroborating these facts, the role played by antiplatelet agents in the treatment of atherosclerotic vascular disease<xref ref-type="bibr" rid="B3">3</xref> is of note. In this context, a study by Ding et al.<xref ref-type="bibr" rid="B4">4</xref> shows the association between hematological parameters, with emphasis on leukocyte and platelet indicators, and stable coronary artery disease (SCAD). The authors evaluated platelet count, total platelet mass, mean platelet volume (MPV), platelet amplitude and white blood cell and neutrophil counts of 34 patients diagnosed with SCAD and compared two groups: 50 healthy patients (control) and 31 patients with acute myocardial infarction (AMI). The diagnostic criteria used for SCAD were those established by the European Society of Cardiology guidelines from 2013.<xref ref-type="bibr" rid="B4">4</xref> The authors mainly highlighted the association between higher values of MPV and the presence of SCAD compared with the control group.<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>Some previous studies have outlined the relationship between MPV and the presence of cardiovascular risk factors or acute myocardial infarction.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref> The findings are not always consistent. In a systematic review of 2010, evaluating 16 cross-sectional studies, 14 presented a positive association between higher MPV values in patients with AMI compared with groups without AMI.<xref ref-type="bibr" rid="B7">7</xref> The association between high platelet volume value and coronary artery disease (CAD) was also shown in another meta-analysis on this matter in 2014. In this last systematic review, the authors found that patients with MPV values greater than 7.3 were more likely to present CAD than those with lower values.<xref ref-type="bibr" rid="B8">8</xref> On the other hand, Wada et al.<xref ref-type="bibr" rid="B9">9</xref> found an association between lower MPV values and the presence of stable CAD.<xref ref-type="bibr" rid="B9">9</xref>
		</p>
    <p>Another reason for discussion among the researchers is the use of MPV as a parameter of platelet function, as MPV is not associated with platelet aggregation by turbidimetry. This latter method is considered a gold standard for evaluating platelet aggregation.<xref ref-type="bibr" rid="B10">10</xref>
		</p>
    <p>The pursuit for new risk markers and prognosis for coronary disease is ongoing. In this context, MPV can be added to high-sensitivity C-reactive protein, to coronary calcium score and to the carotid intima-media thickness. However, in different countries, MPV may be a measure with a better profile, since it is inexpensive, technically easy and little invasive.<xref ref-type="bibr" rid="B9">9</xref> The new study by Ding et al.<xref ref-type="bibr" rid="B4">4</xref> adds more data to assist in the choice of MPV as a prognostic method in one of the CAD categories.</p>

      <p>H&#225; evid&#234;ncias crescentes de que as plaquetas apresentam papel fundamental no processo de aterotrombose vascular, associadas ao endot&#233;lio e &#224; inflama&#231;&#227;o.<xref ref-type="bibr" rid="B1">1</xref> A ativa&#231;&#227;o plaquet&#225;ria leva a altera&#231;&#245;es na express&#227;o de glicoprote&#237;nas de superf&#237;cie dessas c&#233;lulas, com posterior agrega&#231;&#227;o e degranula&#231;&#227;o, processos que contribuem de modo essencial para a forma&#231;&#227;o do trombo.<xref ref-type="bibr" rid="B2">2</xref> Corroborando esses fatos, destaca-se o papel exercido pelos antiplaquet&#225;rios no tratamento da doen&#231;a vascular ateroscler&#243;tica.<xref ref-type="bibr" rid="B3">3</xref> Dentro desse contexto, situa-se o artigo de Ding et al.,<xref ref-type="bibr" rid="B4">4</xref> que objetiva mostrar a associa&#231;&#227;o entre par&#226;metros hematol&#243;gicos, com destaque para indicadores leucocit&#225;rios e plaquet&#225;rios, e doen&#231;a arterial coronariana est&#225;vel (DACE). Os autores avaliaram a contagem de plaquetas, a massa total de plaquetas, o volume plaquet&#225;rio m&#233;dio (VPM), a amplitude de distribui&#231;&#227;o de plaquetas e a contagem de gl&#243;bulos brancos e neutr&#243;filos de 34 pacientes diagnosticados com DACE e fizeram compara&#231;&#227;o entre dois grupos: 50 pacientes saud&#225;veis (controle) e 31 pacientes com infarto agudo do mioc&#225;rdio (IAM). Os crit&#233;rios diagn&#243;sticos utilizados para DACE foram os estabelecidos pela diretriz da Sociedade Europeia de Cardiologia de 2013.<xref ref-type="bibr" rid="B4">4</xref> Os autores destacaram principalmente a associa&#231;&#227;o entre maiores valores de VPM e presen&#231;a de DACE em compara&#231;&#227;o com o grupo controle.<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>Alguns estudos anteriores j&#225; haviam tra&#231;ado a rela&#231;&#227;o entre VPM e a presen&#231;a de fatores de risco cardiovasculares ou infarto agudo do mioc&#225;rdio.<xref ref-type="bibr" rid="B5">5</xref>,<xref ref-type="bibr" rid="B6">6</xref> Os achados n&#227;o s&#227;o sempre concordantes. Em revis&#227;o sistem&#225;tica de 2010 com avalia&#231;&#227;o de 16 estudos transversais, 14 apresentaram resultados de associa&#231;&#227;o positiva entre maiores valores de VPM em pacientes com IAM em compara&#231;&#227;o com grupos sem IAM A associa&#231;&#227;o entre alto valor de volume plaquet&#225;rio e a doen&#231;a arterial coronariana (DAC) tamb&#233;m foi mostrada em outra metan&#225;lise sobre o tema em 2014.<xref ref-type="bibr" rid="B7">7</xref> Nessa &#250;ltima revis&#227;o sistem&#225;tica, os autores observaram que pacientes com valores de VPM maiores que 7,3 tinham maior chance de apresentar DAC do que aqueles com menores valores.<xref ref-type="bibr" rid="B8">8</xref> Por outro lado, Wada et al.,<xref ref-type="bibr" rid="B9">9</xref> encontraram uma associa&#231;&#227;o entre menores valores de VPM e a presen&#231;a de DAC est&#225;vel.<xref ref-type="bibr" rid="B9">9</xref>
			</p>
      <p>Outro motivo de discuss&#227;o entre os pesquisadores &#233; sobre o uso do VPM como par&#226;metro de fun&#231;&#227;o plaquet&#225;ria, uma vez que a VPM n&#227;o apresenta associa&#231;&#227;o com agrega&#231;&#227;o plaquet&#225;ria por turbidimetria. Esse &#250;ltimo m&#233;todo &#233; considerado como padr&#227;o-ouro para avaliar a agrega&#231;&#227;o plaquet&#225;ria.<xref ref-type="bibr" rid="B10">10</xref>
			</p>
      <p>A busca por novos marcadores de risco e progn&#243;sticos para doen&#231;a coronariana &#233; cont&#237;nua. Neste &#226;mbito, o VPM pode-se somar &#224; prote&#237;na C reativa de alta sensibilidade, ao escore de c&#225;lcio coronariano e &#224; espessura m&#233;dio-intimal da car&#243;tida. Por&#233;m, diferentes desses, a VPM pode ser uma medida com melhor perfil, uma vez que se apresenta de baixo custo, de facilidade t&#233;cnica e pouco invasiva.<xref ref-type="bibr" rid="B9">9</xref> O estudo novo de Ding et al.,<xref ref-type="bibr" rid="B4">4</xref> contribui ao trazer mais dados para auxiliar na escolha do VPM como m&#233;todo progn&#243;stico em uma das categorias de DAC.</p>
