  <body>
    <p>Diabetes mellitus may be associated with a specific form of cardiomyopathy independent of other comorbidities. Left ventricular hypertrophy is the main pathological change described in cardiomyopathy of patients with diabetes mellitus. Different mechanisms may be operative in the pathogenesis of the hypertrophy: metabolic derangements, inflammation and among other factors, oxidative stress.<xref ref-type="bibr" rid="B1">1</xref> Oxidative stress may lead to cellular damage by free radical-induced oxidation of DNA, proteins and lipids.<xref ref-type="bibr" rid="B2">2</xref>
		</p>
    <p>Reactive oxygen species may lead to consequences in cardiomyocytes including hypertrophy, apoptosis and fibrosis; in this regard a cellular detoxification mechanism may be missing for attenuating severity of damage induced by some reactive oxygen species.<xref ref-type="bibr" rid="B3">3</xref> Hence, interventions that might be protective would be worth to be investigated, including drug therapy.</p>
    <p>Trimetazidine (1-[2,3,4-trimethoxybenzyl] piperazine dihydrochloride)<xref ref-type="bibr" rid="B4">4</xref> is one of the drugs that may be used in combination with other drugs for the treatment of patients with angina pectoris.<xref ref-type="bibr" rid="B5">5</xref> It is a piperazine derivative<xref ref-type="bibr" rid="B6">6</xref> characterized as a metabolic modulator<xref ref-type="bibr" rid="B5">5</xref> that reduces long-chain fatty acid (3-ketoactyl CoA thiolase) oxidation.<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>In this issue,<xref ref-type="bibr" rid="B7">7</xref> an experimental study of alloxan induced diabetes in Sprague-Dawley rats tested the hypothesis that the administration of trimetazidine might prevent pathologic changes induced in the heart of the studied animals, including QT interval, heart weight relative to body weight, myocardial contractility indices and antioxidant enzyme activities (superoxide dismutase, catalase and glutathione peroxidase). The authors found that the administration of trimetazidine reduced the modifications in the studied variables induced by diabetes in the rats. Thus, additional experimental findings were added to current knowledge about the interaction between diabetes, cardiomyopathy and drug therapy in experimental animals, rats in this specific study. In the event of progress in accumulating knowledge together with other studies, evidences may evolve to deserving clinical studies.</p>

      <p>O diabetes mellitus pode estar associado a uma forma espec&#237;fica de cardiomiopatia independente de outras comorbidades. A hipertrofia ventricular esquerda &#233; a principal altera&#231;&#227;o patol&#243;gica descrita na cardiomiopatia de pacientes com diabetes mellitus. Diferentes mecanismos podem agir na patog&#234;nese da hipertrofia: desarranjos metab&#243;licos, inflama&#231;&#227;o e, entre outros fatores, estresse oxidativo.<xref ref-type="bibr" rid="B1">1</xref> O estresse oxidativo pode levar a danos celulares pela oxida&#231;&#227;o de DNA, prote&#237;nas e lip&#237;dios induzida por radicais livres.<xref ref-type="bibr" rid="B2">2</xref>

      <p>Esp&#233;cies reativas de oxig&#234;nio podem levar a consequ&#234;ncias nos cardiomi&#243;citos, incluindo hipertrofia, apoptose e fibrose; a esse respeito, pode estar faltando um mecanismo de desintoxica&#231;&#227;o celular para atenuar a gravidade do dano induzido por algumas esp&#233;cies reativas de oxig&#234;nio.<xref ref-type="bibr" rid="B3">3</xref> Portanto, valeria a pena investigar interven&#231;&#245;es que poderiam ser protetoras, incluindo terapia medicamentosa.</p>
      <p>A trimetazidina (dicloridrato de 1- [2,3,4-trimetoxibenzil] piperazina)<xref ref-type="bibr" rid="B4">4</xref> &#233; um dos f&#225;rmacos que pode ser utilizado em combina&#231;&#227;o com outros para o tratamento de pacientes com angina.<xref ref-type="bibr" rid="B5">5</xref> &#201; um derivado da piperazina,<xref ref-type="bibr" rid="B6">6</xref> caracterizado como um modulador metab&#243;lico<xref ref-type="bibr" rid="B5">5</xref> que reduz a oxida&#231;&#227;o de &#225;cidos graxos de cadeia longa (3-Cetoacil CoA Tiolase).<xref ref-type="bibr" rid="B4">4</xref>

      <p>Nesta edi&#231;&#227;o,<xref ref-type="bibr" rid="B7">7</xref> um estudo experimental de diabetes induzido por aloxana em ratos Sprague-Dawley testou a hip&#243;tese de que a administra&#231;&#227;o de trimetazidina pode prevenir as altera&#231;&#245;es patol&#243;gicas induzidas no cora&#231;&#227;o dos animais estudados, incluindo o intervalo QT, peso card&#237;aco em rela&#231;&#227;o ao peso corporal, &#237;ndices de contratilidade mioc&#225;rdica e atividades de enzimas antioxidantes (super&#243;xido dismutase, catalase e glutationa peroxidase).</p>
      <p>Os autores constataram que a administra&#231;&#227;o de trimetazidina reduziu as modifica&#231;&#245;es nas vari&#225;veis estudadas induzidas por diabetes nos ratos. Assim, achados experimentais suplementares foram adicionados ao conhecimento atual sobre a intera&#231;&#227;o entre diabetes, cardiomiopatia e terapia medicamentosa em animais experimentais, ratos neste estudo espec&#237;fico. Em caso de progresso no ac&#250;mulo de conhecimento em conjunto com outros estudos, as evid&#234;ncias podem evoluir para fazer jus a estudos cl&#237;nicos.</p>
