  <body>
    <p>The search for response markers to Cardiac Resynchronization Therapy (CRT) remains intensive. Currently, the main criteria for CRT indication are the QRS morphology and the absence of myocardial fibrosis.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>The electrocardiogram remains an important tool for selecting CRT candidates, and new parameters, such as the PR interval, are interesting to discriminate the prognosis in this population. On this issue, we have a meta-analysis study<xref ref-type="bibr" rid="B2">2</xref> concluding that the presence of prolonged PR interval is a marker of poor prognosis at baseline.</p>
    <p>In clinical practice, these data may surprise clinicians. The common sense is that it is much easier to make adjustments of the atrioventricular interval to obtain the best hemodynamic response,<xref ref-type="bibr" rid="B3">3</xref> as well as to ensure a higher rate of effective atriobiventricular resynchronization.<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>The pathophysiological hypotheses that could justify this worse prognosis remain a challenge for medicine.</p>
    <p>However, a critical view of these data is needed. The question of strong clinical interest is &#8220;Can the PR interval be used as a selection criterion for CRT indication?&#8221;</p>
    <p>This doubt cannot be clarified yet, focusing on findings of this systematic review and meta-analysis. The reason is very clear: the analysis did not include a control group with prolonged PR interval in patients not undergoing CRT, to assess its actual benefit.</p>
    <p>Therefore, this meta-analysis adds scientific collaboration, but we still have much more to study!</p>

      <p>A busca por marcadores de resposta &#224; Terapia de Ressincroniza&#231;&#227;o Card&#237;aca (TRC) permanece intensiva. Atualmente, os principais crit&#233;rios para indica&#231;&#227;o de TRC s&#227;o a morfologia do QRS e a aus&#234;ncia de fibrose mioc&#225;rdica.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>O eletrocardiograma continua sendo uma ferramenta importante para a sele&#231;&#227;o de candidatos &#224; TRC, e novos par&#226;metros, como o intervalo PR, s&#227;o interessantes para discriminar o progn&#243;stico nessa popula&#231;&#227;o. Sobre esta quest&#227;o, temos um estudo de metan&#225;lise<xref ref-type="bibr" rid="B2">2</xref> que concluiu que a presen&#231;a de intervalo PR prolongado &#233; um marcador de mau progn&#243;stico em momento basal.</p>
      <p>Na pr&#225;tica cl&#237;nica, esses dados podem surpreender os m&#233;dicos. O senso comum &#233; que &#233; muito mais f&#225;cil fazer ajustes do intervalo atrioventricular para obter a melhor resposta hemodin&#226;mica,<xref ref-type="bibr" rid="B3">3</xref> assim como garantir maior taxa de ressincroniza&#231;&#227;o atriobiventricular efetiva.<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>As hip&#243;teses fisiopatol&#243;gicas que poderiam justificar esse pior progn&#243;stico permanecem um desafio para a medicina.</p>
      <p>No entanto, uma vis&#227;o cr&#237;tica desses dados &#233; necess&#225;ria. A quest&#227;o do forte interesse cl&#237;nico &#233;: &#8220;O intervalo PR pode ser usado como um crit&#233;rio de sele&#231;&#227;o para indica&#231;&#227;o de TRC?&#8221;</p>
      <p>Essa d&#250;vida ainda n&#227;o pode ser esclarecida, concentrando-se nos achados desta revis&#227;o sistem&#225;tica e metan&#225;lise. A raz&#227;o &#233; muito clara: a an&#225;lise n&#227;o incluiu um grupo controle com intervalo PR prolongado em pacientes n&#227;o submetidos &#224; TRC para avaliar seu benef&#237;cio real.</p>
      <p>Portanto, esta metan&#225;lise acrescenta colabora&#231;&#227;o cient&#237;fica, mas h&#225; muito ainda o que estudar!</p>
