    <p>We read with interest the article by Bassi et al.,<xref ref-type="bibr" rid="B1">1</xref> titled &#8220;Effects of Coexistence Hypertension and Type II Diabetes on Heart Rate Variability and Cardiorespiratory Fitness&#8221;, published in the issue of July 2018. The authors investigated the influence of systemic hypertension on cardiac autonomic modulation in patients with type 2 diabetes mellitus (T2DM) and assessed the heart rate variability (HRV) on exercise capacity in these patients. They concluded that hypertension negatively affects cardiac autonomic function, with greater impairment in HRV, when compared to normotensive patients with T2DM.</p>
    <p>Several aspects of this study require discussion. As previously reported, numerous factors may have impact on HRV indices, including sex, insulin resistance, body mass index (BMI), hyperlipidemia, hypertension, ischemic and non-ischemic cardiomyopathy, and smoking status.<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> For instance, increased BMI can independently decrease HRV, particularly when central adiposity is present.<xref ref-type="bibr" rid="B5">5</xref> Indeed, the hypertensive group had a higher BMI when compared to the normotensive group (28 &#177; 4.4 vs 31 &#177; 3.8, p = 0.031). Given the lack of control for BMI between the two groups, the conclusions made by the authors should be regarded cautiously.</p>
    <p>Finally, it is known that subclinical myocardial dysfunction is highly prevalent in diabetic patients and is independently associated with cardiac autonomic neuropathy.<xref ref-type="bibr" rid="B6">6</xref> However, the authors only considered medical history consistent with ischemic heart disease for stratification/exclusion of patients for analysis. We believe that a more detailed cardiovascular assessment, including echocardiography to determine left ventricular mass and left ventricular diastolic dysfunction, would be important to better stratify the patients and strengthen their conclusions.</p>

      <p>We appreciate the authors&#8217; interest towards our article, &#8220;Effects of Coexistence Hypertension and Type II Diabetes on Heart Rate Variability and Cardiorespiratory Fitness&#8221;. We also appreciate the opportunity to respond to their comments. Their critique of our study mainly focused on 3 issues: 1) lack of methodological attention in relation to the participants&#8217; gender, BMI and insulin resistance; 2) lack of methodological details about the study population, regarding smoking habits and hyperlipidemia; and 3) lack of consistent investigation on ischemic and non-ischemic cardiomyopathy. We appreciate the authors&#8217; concerns; however, we do not agree with many of their comments.</p>
      <p>The first issue was clearly acknowledged in our paper. Since cardiac variability dynamics differ between genders, with higher parasympathetic activity and overall complexity for women, gender distribution must be considered when investigating heart rate dynamics.<xref ref-type="bibr" rid="B7">1</xref> However, in our study, gender distribution is not significantly different for the investigated groups (Diabetes Mellitus (DM) and DM + Hypertension Systemic arterial hypertension (SAH), p = 0.464). In relation to insulin resistance, we agree with the authors&#8217; comments about differences possibly influencing HRV indices. However, we believe that the difference found between groups could be attributed to different weight and BMI. We kindly invite the authors to read a recent study from our group, demonstrating that obesity per se impairs aerobic-hemodynamic responses to exercise but that, however, metabolic syndrome (obesity, DM and hypertension) in young adults negatively impacts overall HRV, parasympathetic activity and HRV complexity, corroborating our findings.<xref ref-type="bibr" rid="B8">2</xref> In the present study, differences were observed between groups for BMI and weight, with patients in the DM group being overweight and patients in the DM + Hypertension group presenting grade 1 obesity; however, we would like to emphasize that, after age, sex and BMI adjustments, we concluded that these variables did not influence our results.</p>
      <p>As for the second issue, current smokers were excluded from our study, as previous evidence showed that tobacco use represents an important cardiovascular risk and leads to HRV impairment.<xref ref-type="bibr" rid="B9">3</xref> Even though it was not detailed in the exclusion criteria section, Table 1 clearly shows that none of the participants were current smokers. As for dyslipidemia, we agree with the authors that it may influence HRV; however, after adjustments for this variable, we concluded that dyslipidemia did not significantly influence our results (p = 1.000).</p>
      <p>The last issue is related to the lack of consistent investigation on ischemic and non-ischemic cardiomyopathy in the present study. The authors have criticized that a simple clinical evaluation may not be sufficient to determine the presence of ischemic conditions; however, clinical investigation may indicate the need for further exams, aimed at detecting ischemic and non-ischemic heart disease. In addition, the absence of effort-induced ischemic signs was evident during the cardiopulmonary test. Even though this was not clearly stated in the text, we emphasized that all participants underwent a thorough clinical evaluation, consisting of physical examination, resting electrocardiogram and maximal incremental exercise. We kindly invite the authors to read a relevant study about screening procedures for this kind of patients.<xref ref-type="bibr" rid="B10">4</xref> Our patients presented no signs or symptoms of suspected ischemic disease, neither at rest, nor during effort. Thus, according to the most recent guidelines for investigation of ischemic patients with DM,<xref ref-type="bibr" rid="B11">5</xref> no further examination was needed through echocardiography or other exams to investigate the presence of myocardial dysfunction. </p>
      <p>Finally, it is well known that hypertension per se negatively affects HRV;<xref ref-type="bibr" rid="B12">6</xref> however, no previous studies investigated linear and non-linear HRV indices in DM + SAH coexistence. Thus, we believe that our article provides a relevant contribution to the understanding of HRV alterations in pathological conditions.</p>
      <p>In consideration of the fact that HRV is highly influenced by a number of variables, including demographic and anthropometric characteristics, the presence of obesity, associated comorbidities and cardiovascular risk factors, future, robust studies are needed to further investigate the influence of specific variables on linear and non-linear HRV indices, in order to confirm the preliminary findings of our study.</p>

      <p>Lemos com interesse o artigo de Bassi et al.,<xref ref-type="bibr" rid="B1">1</xref> intitulado &#8220;Efeitos da Coexist&#234;ncia de Diabetes Tipo 2 e Hipertens&#227;o sobre a Variabilidade da Frequ&#234;ncia Card&#237;aca e Capacidade Cardiorrespirat&#243;ria&#8221;, publicado na edi&#231;&#227;o de julho de 2018. Os autores investigaram a influ&#234;ncia da hipertens&#227;o arterial sist&#234;mica na modula&#231;&#227;o auton&#244;mica card&#237;aca em pacientes com diabetes mellitus tipo 2 (DM2) e avaliaram a variabilidade da frequ&#234;ncia card&#237;aca (VFC) na capacidade de exerc&#237;cio nesses pacientes. Eles conclu&#237;ram que a hipertens&#227;o afeta negativamente a fun&#231;&#227;o auton&#244;mica card&#237;aca, com maior comprometimento da VFC, quando comparada aos pacientes normotensos com DM2.</p>
      <p>Alguns aspectos deste estudo requerem discuss&#227;o. Como relatado anteriormente, v&#225;rios fatores podem ter impacto nos &#237;ndices de VFC, incluindo sexo, resist&#234;ncia &#224; insulina, &#237;ndice de massa corporal (IMC), hiperlipidemia, hipertens&#227;o, cardiomiopatia isqu&#234;mica e n&#227;o isqu&#234;mica e tabagismo.<xref ref-type="bibr" rid="B2">2</xref>-<xref ref-type="bibr" rid="B4">4</xref> Por exemplo, o aumento do IMC pode diminuir a VFC, particularmente quando a adiposidade central est&#225; presente.<xref ref-type="bibr" rid="B5">5</xref> De fato, o grupo hipertenso apresentou um IMC maior quando comparado ao grupo normotenso (28 &#177; 4,4 vs 31 &#177; 3,8, p = 0,031). Dada a falta de controle do IMC entre os dois grupos, as conclus&#245;es dos autores devem ser consideradas com cautela.</p>
      <p>Por fim, sabe-se que a disfun&#231;&#227;o mioc&#225;rdica subcl&#237;nica &#233; altamente prevalente em pacientes diab&#233;ticos e est&#225; independentemente associada &#224; neuropatia auton&#244;mica card&#237;aca.<xref ref-type="bibr" rid="B6">6</xref> No entanto, os autores consideraram apenas a hist&#243;ria cl&#237;nica consistente com doen&#231;a card&#237;aca isqu&#234;mica para estratifica&#231;&#227;o/exclus&#227;o de pacientes para an&#225;lise. Acreditamos que uma avalia&#231;&#227;o cardiovascular mais detalhada, incluindo ecocardiografia para determinar a massa ventricular esquerda e a disfun&#231;&#227;o diast&#243;lica do ventr&#237;culo esquerdo, seria importante para melhor estratificar os pacientes e fortalecer suas conclus&#245;es.</p>

          Caro Editor,
        </p>
        <p>Agradecemos o interesse dos autores em rela&#231;&#227;o ao nosso artigo, &#8220;Efeitos da Coexist&#234;ncia de Diabetes Tipo 2 e Hipertens&#227;o sobre a Variabilidade da Frequ&#234;ncia Card&#237;aca e Capacidade Cardiorrespirat&#243;ria&#8221;. N&#243;s tamb&#233;m apreciamos a oportunidade de responder aos seus coment&#225;rios. Suas cr&#237;ticas ao nosso estudo concentraram-se principalmente em 3 quest&#245;es: 1) falta de aten&#231;&#227;o metodol&#243;gica em rela&#231;&#227;o ao g&#234;nero, IMC e resist&#234;ncia &#224; insulina dos participantes; 2) falta de detalhes metodol&#243;gicos sobre a popula&#231;&#227;o do estudo, quanto aos h&#225;bitos de fumar e hiperlipidemia; e 3) falta de investiga&#231;&#227;o consistente sobre cardiomiopatia isqu&#234;mica e n&#227;o isqu&#234;mica. Agradecemos as preocupa&#231;&#245;es dos autores; no entanto, n&#227;o concordamos com muitos dos seus coment&#225;rios.</p>
        <p>A primeira quest&#227;o foi claramente reconhecida em nosso trabalho. Como a din&#226;mica da variabilidade card&#237;aca difere entre os g&#234;neros, com maior atividade parassimp&#225;tica e complexidade geral para as mulheres, a distribui&#231;&#227;o de g&#234;nero deve ser considerada quando se investiga a din&#226;mica da frequ&#234;ncia card&#237;aca.<xref ref-type="bibr" rid="B13">1</xref> Entretanto, em nosso estudo, a distribui&#231;&#227;o por sexo n&#227;o &#233; significativamente diferente para os grupos investigados (Diabetes Mellitus (DM) e DM + Hipertens&#227;o Arterial Sist&#234;mica (HAS), p = 0,464). Em rela&#231;&#227;o &#224; resist&#234;ncia &#224; insulina, concordamos com os coment&#225;rios dos autores sobre as diferen&#231;as que possivelmente influenciam os &#237;ndices de VFC. No entanto, acreditamos que a diferen&#231;a encontrada entre os grupos poderia ser atribu&#237;da a diferentes pesos e IMC. N&#243;s gentilmente convidamos os autores a lerem um estudo recente do nosso grupo, demonstrando que a obesidade per se prejudica as respostas aer&#243;bico-hemodin&#226;micas ao exerc&#237;cio, mas que a s&#237;ndrome metab&#243;lica (obesidade, DM e hipertens&#227;o) em adultos jovens afeta negativamente a VFC, atividade parassimp&#225;tica. e a complexidade da VFC, corroborando nossos achados.<xref ref-type="bibr" rid="B14">2</xref> No presente estudo, foram observadas diferen&#231;as entre os grupos para IMC e peso, sendo os pacientes do grupo DM com excesso de peso e os pacientes do grupo DM + Hipertens&#227;o apresentando obesidade grau 1; no entanto, gostar&#237;amos de enfatizar que, ap&#243;s ajustes de idade, sexo e IMC, conclu&#237;mos que essas vari&#225;veis n&#227;o influenciaram nossos resultados.</p>
        <p>Quanto &#224; segunda quest&#227;o, os fumantes atuais foram exclu&#237;dos de nosso estudo, pois evid&#234;ncias pr&#233;vias mostraram que o tabagismo representa um grande risco cardiovascular e leva ao comprometimento da VFC.<xref ref-type="bibr" rid="B15">3</xref> Embora n&#227;o tenha sido detalhada na se&#231;&#227;o de crit&#233;rios de exclus&#227;o, a Tabela 1 mostra claramente que nenhum dos participantes era fumante. Quanto &#224; dislipidemia, concordamos com os autores que ela pode influenciar a VFC; entretanto, ap&#243;s ajustes para essa vari&#225;vel, conclu&#237;mos que a dislipidemia n&#227;o influenciou significativamente nossos resultados (p = 1,000).</p>
        <p>A &#250;ltima quest&#227;o est&#225; relacionada &#224; falta de investiga&#231;&#227;o consistente sobre cardiomiopatia isqu&#234;mica e n&#227;o isqu&#234;mica no presente estudo. Os autores criticaram que uma simples avalia&#231;&#227;o cl&#237;nica pode n&#227;o ser suficiente para determinar a presen&#231;a de condi&#231;&#245;es isqu&#234;micas; entretanto, a investiga&#231;&#227;o cl&#237;nica pode indicar a necessidade de novos exames com o objetivo de detectar cardiopatia isqu&#234;mica e n&#227;o isqu&#234;mica. Al&#233;m disso, a aus&#234;ncia de sinais isqu&#234;micos induzidos pelo esfor&#231;o foi evidente durante o teste cardiopulmonar. Embora isso n&#227;o tenha sido claramente indicada no texto, enfatizamos que todos os participantes foram submetidos a uma avalia&#231;&#227;o cl&#237;nica completa, consistindo em exame f&#237;sico, eletrocardiograma de repouso e exerc&#237;cio incremental m&#225;ximo. Convidamos os autores a lerem um estudo relevante sobre procedimentos de triagem para esse tipo de paciente.<xref ref-type="bibr" rid="B16">4</xref> Nossos pacientes n&#227;o apresentavam sinais ou sintomas de suspeita de doen&#231;a isqu&#234;mica, nem em repouso, nem durante o esfor&#231;o. Assim, de acordo com as diretrizes mais recentes para investiga&#231;&#227;o de pacientes isqu&#234;micos com DM,<xref ref-type="bibr" rid="B17">5</xref> nenhum exame adicional foi necess&#225;rio por meio de ecocardiografia ou outros exames para investigar a presen&#231;a de disfun&#231;&#227;o mioc&#225;rdica.</p>
        <p>Por fim, &#233; bem conhecido que a hipertens&#227;o per se afeta negativamente a VFC;<xref ref-type="bibr" rid="B18">6</xref> no entanto, nenhum estudo anterior investigou &#237;ndices de VFC lineares e n&#227;o-lineares na coexist&#234;ncia de DM + HAS. Assim, acreditamos que nosso artigo fornece uma contribui&#231;&#227;o relevante para o entendimento das altera&#231;&#245;es da VFC em condi&#231;&#245;es patol&#243;gicas.</p>
        <p>Considerando-se o fato da VFC ser altamente influenciada por uma s&#233;rie de vari&#225;veis, incluindo caracter&#237;sticas demogr&#225;ficas e antropom&#233;tricas, a presen&#231;a de obesidade, comorbidades associadas e fatores de risco cardiovasculares, estudos futuros e mais aprimorados s&#227;o necess&#225;rios para investigar a influ&#234;ncia de vari&#225;veis espec&#237;ficas nos &#237;ndices de VFC lineares e n&#227;o lineares, a fim de confirmar as conclus&#245;es preliminares do nosso estudo.</p>
