  <body>
    <p>Notwithstanding the decline which has been observed in recent years, circulatory diseases continue to be the leading cause of death in Brazil.<xref ref-type="bibr" rid="B1">1</xref> In spite of recent advances in clinical and interventional treatment,<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> ischemic heart disease (IHD) was responsible for 116,333 deaths in 2016, of which 80% were due to acute manifestations, principally in the form of acute myocardial infarction (AMI).<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>In treating AMI patients, especially those with ST-segment elevation, the pre-hospital phase of care plays a crucial role in short- and long-term prognosis. Two data related to this phase deserve our attention. First, we may observe delays in reaching healthcare services. In 80% of cases, these delays last for more than two hours starting from the moment symptoms begin to manifest.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Second, it stands out that 50% of deaths resulting from AMI were recorded precisely during the pre-hospital phase.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>Whether these delays in reaching pre-hospital care are predominantly patient-related, including, among other factors, difficulties in recognizing and interpreting symptoms owing to socioeconomic status and/or cultural factors, or whether they are associated with a lack of efficiency within the healthcare system, for example in transporting patients from the place where symptoms onset to the final destination, i.e. the hospital, has yet to be established in the literature.<xref ref-type="bibr" rid="B4">4</xref> It is also important to highlight that a recent study indicated that there are sex-specific differences, with greater delays being observed in women, mainly owing to atypical symptoms which lead to delays in the decision to seek healthcare.<xref ref-type="bibr" rid="B5">5</xref>
		</p>
    <p>Several factors have been associated with delays during the pre-hospital phase, including non-white ethnicity; low socioeconomic status; cultural factors; previous history of angina, diabetes, or hypertension; sociodemographic and situational factors, for example, distance to treatment centers or medical consultation conducted by spouses or relatives; lack of knowledge regarding the meaning of symptoms; anxiety caused by symptoms; access to public and private healthcare systems; the time of day/night when symptoms onset; previous infarction, and associated symptoms, such as profuse sweating, arterial hypotension, and intensity of precordial pain severity.<xref ref-type="bibr" rid="B6">6</xref>-<xref ref-type="bibr" rid="B8">8</xref>
		</p>
    <p>In Brazil, Rodrigues et al. published a study on predictors of late presentation in 1,297 patients with AMI in a referral center in the country&#8217;s South Region, which is able to perform primary angioplasty 24 hours a day, seven days a week.<xref ref-type="bibr" rid="B9">9</xref> Approximately 25% (n = 302) of the total patients attended between December 2009 and November 2014 presented a delay of more than six hours, with a significantly higher mortality rate. The independent predictors of late presentation were: black ethnicity, low income level (less than five times minimum wage), and diabetes mellitus. The following variables lost statistical significance after adjusting for multiple logistic regression analysis: female sex, less than eight years of schooling, and occurrence of chronic renal failure. Patients with all of the independent predictors of late presentation took twice as long to reach the hospital as other patients. History of previous heart disease, AMI, or myocardial revascularization were protective factors,<xref ref-type="bibr" rid="B9">9</xref> likely owing to the early recognition of a new event and, thus, to reduced delays in seeking medical treatment.</p>
    <p>Unfortunately, the authors of this study did not record the distance between the place where symptoms onset and the referral center; they also excluded transfer patients in order to evaluate spontaneous demand.<xref ref-type="bibr" rid="B9">9</xref> These two factors also influence mortality related to the pre-hospital phase, even though they are dependent on the healthcare system. On the other hand, they also did not analyze the following confounding factors which are related both to distance between place of symptoms onset and referral center and to transfer patients: ventricular function, time taken to implement mechanical or drug reperfusion therapy, reperfusion therapy success rate, associated procedures, and implementation of adjuvant therapy recommended in the guidelines.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>It is also noteworthy that overall mortality differed significantly between the two groups studied, there being no differences observed regarding subgroups, even with major cardiovascular events. This leads us to suppose that other factors that were not analyzed influenced 30-day mortality rate, for instance, mortality related to the performance of highly complex procedures.</p>
    <p>One recent study suggests that higher mortality in women due to delays during the pre-hospital phase may be due to the fact that women appear to be more vulnerable to prolonged untreated ischemia.<xref ref-type="bibr" rid="B10">10</xref> The longest delays in this study were related to the healthcare system. The authors stress that mortality was even higher in those who arrived at the hospital with more than twelve hours&#8217; delay, as they did not receive any form of reperfusion therapy. In the study carried out by Rodrigues et al., these patients were excluded, thus making it impossible to establish a comparison.<xref ref-type="bibr" rid="B9">9</xref>
		</p>
    <p>Cultural differences regarding attitudes toward AMI symptoms are also relevant patient-dependent factors. A recent study carried out in Japan showed that patients who were men, were elderly, had lower levels of schooling and had lower self-confidence regarding their understanding of AMI would present delays in seeking medical treatment.<xref ref-type="bibr" rid="B11">11</xref> These patient-related factors were also absent from Rodrigues et al.<xref ref-type="bibr" rid="B9">9</xref>
		</p>
    <p>It is important to highlight that the results of the study carried out by Rodrigues et al. come from a single center whose conditions are quite rare in Brazil, which demonstrates that the continuous availability of mechanical reperfusion therapy was not sufficient to reduce the 30-day AMI mortality rate of about 10% in patients who arrived at the hospital with more than six hours&#8217; delay following onset of symptoms. This is an additional conclusion to the data presented by the authors.</p>
    <p>It is necessary to invest not only in the availability of excellent mechanical reperfusion therapy, but also in equal access to healthcare systems, both by improving the population&#8217;s socio-economic and cultural conditions and by implementing thrombolytic therapy close to the place where the patient is located during the onset of symptoms or in pre-hospital transport. Only then will we be able to make the mortality rates we observe in our clinical practice match the ones described in the clinical trials of the guidelines.</p>

      <p>As doen&#231;as do aparelho circulat&#243;rio s&#227;o a principal causa de &#243;bito no Brasil, apesar do decl&#237;nio observado nos &#250;ltimos anos, principalmente relacionado &#224;s doen&#231;as cerebrovasculares.<xref ref-type="bibr" rid="B1">1</xref> Mesmo com os avan&#231;os recentes nos tratamentos cl&#237;nico e intervencionista,<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> as doen&#231;as isqu&#234;micas do cora&#231;&#227;o foram respons&#225;veis por 116.333 &#243;bitos em 2016, sendo 80% devido &#224;s apresenta&#231;&#245;es agudas, representadas pelo infarto agudo do mioc&#225;rdico (IAM).<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>Na abordagem do paciente com IAM, especialmente com eleva&#231;&#227;o do segmento ST, a fase pr&#233;-hospitalar tem papel fundamental no progn&#243;stico de curto e de longo prazo. Nessa fase, dois dados devem ser destacados. Em primeiro lugar, podemos observar retardos na chegada aos servi&#231;os de sa&#250;de, que em 80% das vezes tem dura&#231;&#227;o m&#233;dia superior a 2 horas, a contar do in&#237;cio da apresenta&#231;&#227;o dos sintomas.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Em segundo, ressalta-se que 50% dos &#243;bitos resultantes de IAM foram registrados exatamente na fase pr&#233;-hospitalar.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Ainda n&#227;o est&#225; estabelecido na literatura se os atrasos no atendimento pr&#233;-hospitalar decorrem, predominantemente, de retardos relacionados aos pacientes, como &#224;s dificuldades de reconhecimento e interpreta&#231;&#227;o dos sintomas devido ao baixo n&#237;vel socioecon&#244;mico e cultural dos doentes, entre outros; ou se s&#227;o associados a inefici&#234;ncia do sistema de sa&#250;de, tais como no transporte do local onde se deram o in&#237;cio dos sintomas at&#233; seu destino final, o hospital.<xref ref-type="bibr" rid="B4">4</xref> &#201; importante ressaltar que estudo recente apontou diferen&#231;as entre os sexos, com maiores retardos observados no feminino, principalmente, devido aos sintomas at&#237;picos, e a um tempo maior para tomar a decis&#227;o de procurar o sistema de sa&#250;de.<xref ref-type="bibr" rid="B5">5</xref>
			</p>
      <p>Diversos fatores foram associados com retardos no atendimento pr&#233;-hospitalar como etnia n&#227;o branca, baixo n&#237;vel socioecon&#244;mico e cultural, hist&#243;ria pr&#233;via de angina, diabetes e hipertens&#227;o, fatores sociodemogr&#225;ficos e situacionais, como dist&#226;ncia at&#233; o centro de refer&#234;ncia e consulta a c&#244;njuge e parentes, falta de conhecimento do significado dos sintomas, ansiedade experimentada com os sintomas, ter ou n&#227;o acesso aos sistemas p&#250;blico e privado de sa&#250;de, hor&#225;rio de aparecimento dos sintomas, infarto pr&#233;vio e sintomas associados como sudorese profusa, hipotens&#227;o arterial e intensidade da dor precordial.<xref ref-type="bibr" rid="B6">6</xref>-<xref ref-type="bibr" rid="B8">8</xref>
			</p>
      <p>No Brasil, Rodrigues et al.<xref ref-type="bibr" rid="B9">9</xref> publicaram estudo sobre os preditores de apresenta&#231;&#227;o tardia em 1.297 pacientes com IAM de um centro de refer&#234;ncia na Regi&#227;o Sul do pa&#237;s, com disponibilidade de realizar angioplastia prim&#225;ria nas 24 horas dos 7 dias da semana.<xref ref-type="bibr" rid="B9">9</xref> Do total de atendimentos entre dezembro de 2009 e novembro de 2014, 302 pacientes, cerca de 25%, tiveram retardo superior a 6 horas com registro de mortalidade significativamente maior. Os preditores independentes de apresenta&#231;&#227;o tardia foram: etnia negra, baixa renda (menor que 5 sal&#225;rios m&#237;nimos), e diabetes melito. O sexo feminino, a escolaridade menor que 8 anos de estudo, e a ocorr&#234;ncia de insufici&#234;ncia renal cr&#244;nica, ap&#243;s ajuste por an&#225;lise de regress&#227;o log&#237;stica m&#250;ltipla, perderam signific&#226;ncia estat&#237;stica. Os pacientes que apresentaram todos os preditores de apresenta&#231;&#227;o tardia tiveram o dobro do tempo de chegada ao hospital em rela&#231;&#227;o aos demais. Hist&#243;ria de doen&#231;a card&#237;aca pr&#233;via, IAM ou revasculariza&#231;&#227;o mioc&#225;rdica foram fatores protetores,<xref ref-type="bibr" rid="B9">9</xref> provavelmente porque levaram ao reconhecimento precoce de um novo evento, reduzindo o tempo de procura por atendimento m&#233;dico.</p>
      <p>Infelizmente, os autores n&#227;o registram as dist&#226;ncias entre o local de in&#237;cio dos sintomas e o referido centro, e exclu&#237;ram os pacientes transferidos por pretenderem avaliar a demanda espont&#226;nea.<xref ref-type="bibr" rid="B9">9</xref> Esses dois fatores tamb&#233;m influenciam a mortalidade relacionada ao per&#237;odo pr&#233;-hospitalar, ainda que sejam dependentes do sistema de sa&#250;de. Por outro lado, n&#227;o foram analisados outros fatores de confundimento relacionados com ambos, como a fun&#231;&#227;o ventricular, o tempo de institui&#231;&#227;o da terapia de reperfus&#227;o mec&#226;nica ou medicamentosa, a taxa de sucesso da terapia de reperfus&#227;o, os procedimentos associados, bem como a implementa&#231;&#227;o da terapia adjuvante recomendada nas diretrizes.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Chama a aten&#231;&#227;o que a mortalidade dita geral diferiu significativamente entre os dois grupos, n&#227;o sendo observada diferen&#231;as quanto aos subgrupos, mesmo para eventos cardiovasculares maiores. Isso nos faz supor que outros fatores n&#227;o analisados influenciaram a mortalidade em 30 dias, como por exemplo, a mortalidade relacionada aos procedimentos de alta complexidade realizados.</p>
      <p>Estudo recente sugere que a maior mortalidade relacionada ao sexo feminino, por atraso no tratamento pr&#233;-hospitalar, poderia ser devido ao fato de que as mulheres seriam mais vulner&#225;veis a isquemia prolongada n&#227;o tratada.<xref ref-type="bibr" rid="B10">10</xref> Nesse estudo os maiores atrasos foram relacionados com os sistemas de sa&#250;de. Os autores ressaltam que a mortalidade foi ainda maior para os que chegaram ao hospital com mais de 12 horas, n&#227;o sendo submetidos a nenhuma terapia de reperfus&#227;o. No estudo de Rodrigues et al.,<xref ref-type="bibr" rid="B9">9</xref> esses pacientes foram exclu&#237;dos n&#227;o sendo poss&#237;vel compara&#231;&#227;o.<xref ref-type="bibr" rid="B9">9</xref>
			</p>
      <p>Diferen&#231;as culturais em rela&#231;&#227;o a atitude a ser tomada frente aos sintomas de IAM s&#227;o igualmente fatores relevantes dependentes dos pacientes. Estudo recente realizado no Jap&#227;o demonstrou que homens mais velhos, com baixa escolaridade e baixa autoconfian&#231;a em sua compreens&#227;o do que estava lhes acontecendo poderiam retardar sua procura por atendimento m&#233;dico.<xref ref-type="bibr" rid="B11">11</xref> Esses fatores relacionados ao paciente tamb&#233;m n&#227;o foram abordados no estudo de Rodrigues et al.<xref ref-type="bibr" rid="B9">9</xref>
			</p>
      <p>&#201; importante ressaltar que os achados do estudo realizado por Rodrigues et al. derivam de um &#250;nico centro com condi&#231;&#245;es que pouco se reproduzem em nosso pa&#237;s, demonstrando que a disponibilidade de terapia de reperfus&#227;o mec&#226;nica em todos os hor&#225;rios da semana n&#227;o bastaram para diminuir a mortalidade em 30 dias por IAM, em torno de 10%, nos pacientes que chegaram ao hospital com mais de 6 horas do in&#237;cio dos sintomas. Essa &#233; uma conclus&#227;o adicional aos dados apresentados pelos autores.</p>
      <p>Portanto, &#233; preciso investir n&#227;o s&#243; na disponibiliza&#231;&#227;o de reperfus&#227;o mec&#226;nica de excel&#234;ncia, mas tamb&#233;m na igualdade de acesso aos sistemas de sa&#250;de, quer melhorando o n&#237;vel socioecon&#244;mico e cultural da popula&#231;&#227;o, quer implementado terapia fibrinol&#237;tica pr&#243;xima ao local onde o paciente se encontra no in&#237;cio dos sintomas, ou ainda no transporte pr&#233;-hospitalar. Assim, conseguiremos igualar as taxas de mortalidade descritas nos ensaios cl&#237;nicos das diretrizes com aquelas observadas em nossa pr&#225;tica cl&#237;nica.</p>
