  <body>
    <p>A 60-year-old Caucasian female with a history of rheumatic mitral stenosis, permanent atrial fibrillation and chronic lymphocytic leukemia was admitted due to decompensated chronic heart failure. The transthoracic echocardiogram depicted a severe mitral stenosis (anatomic valve area of 0.9 cm2), mild mitral regurgitation, aneurysmatic left atrium and mildly compromised left ventricular ejection fraction. Given the indication for mitral valve replacement, coronary angiography was performed, revealing an abnormal vascularized mass at the level of the left atrium beyond normal coronary arteries (Panel A). For better characterization an angio-CT was requested. A well-delimited, 7x4x3cm left atrial homogeneous, slight hyperdense mass was observed along the lateral portion of the atrial roof (Panel B and C). The patient underwent both surgical mass resection and mitral valve replacement with an uneventful recovery. The pathological analysis showed a multifocal left atrial wall and pericardial fat infiltration with CD20+, CD5 +, bcl-2+, cyclin D1+, CD10- and CD23- lymphoid cells, in addition to a left atrial adherent thrombus (Panels D-I). These findings were compatible with lymphocytic lymphoma/chronic lymphocytic leukemia and the patient remains clinically stable. </p>
    <p>Secondary or metastatic tumors are much more common than primary tumors of the heart. A recent necropsy study revealed that cardiac metastases in patients with leukemia and lymphomas may be present in 25% of patients.<xref ref-type="bibr" rid="B1">1</xref> Despite being mostly clinically silent, cardiac imaging improvements and availability has led to increased incidental recognition and awareness.</p>

      <p>Uma mulher branca de 60 anos de idade, com hist&#243;ria de estenose mitral reum&#225;tica, fibrila&#231;&#227;o atrial permanente e leucemia linfoc&#237;tica cr&#244;nica foi internada devido &#224; insufici&#234;ncia card&#237;aca cr&#244;nica descompensada. O ecocardiograma transtor&#225;cico mostrou estenose mitral grave (&#225;rea valvar anat&#244;mica de 0,9 cm2), insufici&#234;ncia mitral leve, &#225;trio esquerdo aneurism&#225;tico e fra&#231;&#227;o de eje&#231;&#227;o do ventr&#237;culo esquerdo levemente comprometida. Dada a indica&#231;&#227;o de substitui&#231;&#227;o valvar mitral, uma angiografia coronariana foi realizada, revelando a presen&#231;a de uma massa vascularizada anormal ao n&#237;vel do &#225;trio esquerdo al&#233;m das art&#233;rias coron&#225;rias normais (Painel A). Para melhor caracteriza&#231;&#227;o, uma angiotomografia foi solicitada. Foi identificada uma massa discreta hiperdensa, homog&#234;nea, de 7x4x3cm, bem delimitada, ao longo da por&#231;&#227;o lateral do teto atrial (Pain&#233;is B e C). A paciente foi submetida &#224; ressec&#231;&#227;o cir&#250;rgica da massa e troca valvar mitral, apresentando recupera&#231;&#227;o sem intercorr&#234;ncias. A an&#225;lise anatomopatol&#243;gica mostrou parede atrial esquerda multifocal e infiltra&#231;&#227;o de gordura peric&#225;rdica com c&#233;lulas linfoides CD20+, CD5+, bcl-2+, ciclina D1+, CD10 e CD23-, al&#233;m de trombo aderente atrial esquerdo (Pain&#233;is D-I). Esses achados foram compat&#237;veis com linfoma linfoc&#237;tico / leucemia linfoc&#237;tica cr&#244;nica e a paciente permanece clinicamente est&#225;vel.</p>
      <p>Os tumores secund&#225;rios ou metast&#225;ticos s&#227;o muito mais comuns que os tumores prim&#225;rios do cora&#231;&#227;o. Um recente estudo de necropsia revelou que met&#225;stases card&#237;acas em pacientes com leucemia e linfomas podem estar presentes em 25% dos pacientes.<xref ref-type="bibr" rid="B1">1</xref> Embora a maioria desses tumores seja clinicamente silenciosa, a melhora e a disponibilidade de imagens card&#237;acas levaram ao aumento da identifica&#231;&#227;o incidental e conscientiza&#231;&#227;o.</p>
