    <p>A 34-year-old man was referred to our institution after an echocardiography performed at another center because of tachycardia (atrial flutter), which showed heterogeneous pericardial mass infiltrating the right chambers. Thoracic computed tomography (CT) and cardiac magnetic resonance (MR) imaging were performed for more accurate assessment of the exact tumor location, size, and potential infiltration of other cardiac and mediastinal structures. CT (<xref ref-type="fig" rid="f1">Figure 1A</xref>) and MR imaging (<xref ref-type="fig" rid="f1">Figure 1B</xref>) confirmed an 8-cm ill-defined heterogeneous enhancing pericardial mass infiltrating the anterior and superior walls of the right atrium and extending to lateral and inferior walls of the right ventricle, consistent with cardiac angiosarcoma. The patient was deemed inoperable, as the mass also invaded the superior vena cava, aortic root, and epicardial fat. CT-guided core needle biopsy of the cardiac mass was the method of choice for histological verification of tentative diagnosis. Once the patient signed the informed consent and was put in supine position, an 18-gauge biopsy needle was driven between the left thoracic internal arteries and the left border of the sternal body (<xref ref-type="fig" rid="f1">Figure 1C</xref>) and a tissue specimen was safely obtained from the beating heart without adverse events. The procedure was performed by an experienced interventional thoracic radiologist under local anesthesia and in the presence of a thoracic surgeon. CT images obtained immediately after biopsy showed no post-procedural complications. Preliminary histological analysis performed on-site by a pathologist determined the adequacy of the tissue specimen. Final histopathologic diagnosis was high-grade cardiac angiosarcoma. To the best of our knowledge, only one case of a CT-guided core needle biopsy of a cardiac angiosarcoma involving the right chambers has been previously reported in English-language scientific literature.</p>

    <body>
      <p>Um homem de 34 anos foi encaminhado &#224; nossa institui&#231;&#227;o ap&#243;s uma ecocardiografia realizada em outro centro devido a uma taquicardia (flutter atrial), que apresentou massa peric&#225;rdica heterog&#234;nea infiltrando-se nas c&#226;maras direitas. Foram realizadas tomografia computadorizada (TC) e resson&#226;ncia magn&#233;tica (RM) card&#237;acas para uma avalia&#231;&#227;o mais precisa sobre a localiza&#231;&#227;o exata do tumor, o seu tamanho, e outras potenciais infiltra&#231;&#245;es de outras estruturas card&#237;acas e mediastinais. As imagens de TC (<xref ref-type="fig" rid="f2">Figura 1A</xref>) e RM (<xref ref-type="fig" rid="f2">Figura 1B</xref>) confirmaram uma massa peric&#225;rdica heterog&#234;nea pouco definida de 8 cm infiltrando-se nas paredes anterior e superior do &#225;trio direito e estendendo-se &#224;s paredes lateral e inferior do ventr&#237;culo direito, consistente com o angiossarcoma card&#237;aco. O paciente foi considerado inoper&#225;vel, considerando que a massa tamb&#233;m invadiu a veia cava superior, a raiz a&#243;rtica, e a gordura epic&#225;rdica. A bi&#243;psia da massa card&#237;aca por agulha grossa guiada por TC foi o m&#233;todo escolhido para a verifica&#231;&#227;o histol&#243;gica de diagn&#243;stico de tentativa. Uma vez que o paciente assinou o termo de consentimento livre e esclarecido e foi colocado em posi&#231;&#227;o supinada, uma agulha calibre 18 foi conduzida entre as art&#233;rias internas tor&#225;cicas esquerdas e a borda esquerda do corpo esternal (<xref ref-type="fig" rid="f2">Figura 1C</xref>) e uma amostra de tecido do cora&#231;&#227;o em batimento foi obtida com seguran&#231;a e sem eventos adversos. O procedimento foi realizado por um radiologista tor&#225;cico intervencionista experiente sob anestesia local e na presen&#231;a de um cirurgi&#227;o tor&#225;cico. As imagens de TC obtidas imediatamente ap&#243;s a bi&#243;psia n&#227;o apresentaram complica&#231;&#245;es p&#243;s-procedimento. A an&#225;lise histol&#243;gica preliminar realizada no local pelo patologista determinou a adequa&#231;&#227;o da amostra de tecido. O diagn&#243;stico histopatol&#243;gico final foi angiossarcoma card&#237;aco de alto grau. At&#233; onde sabemos, apenas um caso de bi&#243;psia de angiossarcoma card&#237;aco por agulha grossa guiada por TC envolvendo as c&#226;maras direitas foi anteriormente relatado na literatura cient&#237;fica em l&#237;ngua inglesa.</p>
