  <body>
    <p>Endomyocardial biopsy (EBM) is the gold standard method for the diagnosis of rejection after cardiac transplantation. Complications associated with the procedure are rare but can occur in about 8% of the cases. We describe a case of an uncommon complication probably associated with multiple EBMs in a transplanted patient. A 54-year-old male patient underwent orthotopic cardiac transplantation due to idiopathic dilated cardiomyopathy. Over the next ten months, the patient was submitted to eight EMBs and required pulse therapy on two occasions. In the routine outpatient evaluation, continuous systo diastolic murmur was observed in the left lower parasternal border and significant worsening of renal function. A transthoracic echocardiogram was performed, showing high velocity, right-sided apical flow on the color flow mapping and a significant dilation of the anterior descending coronary artery (ADA). The ADA presented significant dilatation with signs of communication with the right ventricle in the apical region, suggesting the diagnosis of coronary fistula (<xref ref-type="fig" rid="f1">Figure 1A</xref> and <xref ref-type="fig" rid="f1">B</xref>). Coronary angiography revealed aneurysmal ADA and vascular remodeling due to hyperflow, confirming the diagnosis of coronary-cavitary fistula (<xref ref-type="fig" rid="f1">Figure 1C</xref>). The patient was submitted to percutaneous closure of the coronary fistula with Coil Vortex-18 device and presented good clinical evolution and improvement of renal function. It is important to consider the risks associated with EBM. Echocardiography is a valuable method for the evaluation of patients with suspected complications after the procedure. This case describes a rare complication after EBM and the diagnosis of which was possible by echocardiographic evaluation.</p>

      <p>A bi&#243;psia endomioc&#225;rdica (BEM) &#233; o m&#233;todo padr&#227;o ouro no diagn&#243;stico de rejei&#231;&#227;o ap&#243;s transplante card&#237;aco. Complica&#231;&#245;es associadas ao procedimento s&#227;o raras, mas podem ocorrer em cerca de 8% dos casos. Descrevemos um caso de complica&#231;&#227;o incomum provavelmente associada a m&#250;ltiplas BEMs em um paciente transplantado. Paciente de 54 anos, sexo masculino, foi submetido a transplante card&#237;aco ortot&#243;pico por miocardiomiopatia dilatada idiop&#225;tica. Nos dez meses seguintes, o paciente foi submetido a oito BEMs necessitando de pulsoterapia em duas ocasi&#245;es. Em avalia&#231;&#227;o ambulatorial de rotina, foi observado ao exame f&#237;sico, sopro cont&#237;nuo sistodiast&#243;lico em regi&#227;o paraesternal esquerda baixa al&#233;m de piora significativa da fun&#231;&#227;o renal. Foi realizado ecocardiograma transtor&#225;cico que evidenciou fluxo em regi&#227;o apical do ventr&#237;culo direito, sistodiast&#243;lico, de alta velocidade ao mapeamento do fluxo em cores, al&#233;m de dilata&#231;&#227;o significativa da art&#233;ria coron&#225;ria descendente anterior (ADA). A ADA apresentava dilata&#231;&#227;o importante e sinais de comunica&#231;&#227;o com o ventr&#237;culo direito na regi&#227;o apical, sugerindo o diagn&#243;stico de f&#237;stula coronariana (<xref ref-type="fig" rid="f2">Figura 1A</xref> e <xref ref-type="fig" rid="f2">B</xref>). A cineangiocoronariografia evidenciou ADA aneurism&#225;tica e remodelamento vascular decorrente de hiperfluxo al&#233;m de confirmar o diagn&#243;stico de f&#237;stula coron&#225;rio-cavit&#225;ria (<xref ref-type="fig" rid="f2">Figura 1C</xref>). O paciente foi submetido ao fechamento percut&#226;neo da f&#237;stula coron&#225;ria com dispositivo Coil Vortex-18 e apresentou boa evolu&#231;&#227;o cl&#237;nica al&#233;m de melhora da fun&#231;&#227;o renal. &#201; importante considerar os riscos associados &#224; BEM. A ecocardiografia &#233; um m&#233;todo valioso para avalia&#231;&#227;o de pacientes com suspeita de complica&#231;&#245;es ap&#243;s o procedimento. Este caso descreve uma complica&#231;&#227;o rara ap&#243;s BEM, cujo diagn&#243;stico foi poss&#237;vel pela avalia&#231;&#227;o ecocardiogr&#225;fica.</p>
