  <body>
    <p>A 56-year-old Caucasian male, came to our hospital complaining of thoracic oppression at exertion and sometimes occurring at rest, lasting for a few minutes. The patient was an active smoker, with a moderate alcohol consumption habit and had had an episode of unstable angina two months earlier. At that episode, two drug-eluting stents were implanted, one in the distal anterior descending artery and the other in the proximal first diagonal artery. The ECG showed mild ST-elevation in V1-V3 and a T-wave inversion in V3-V5. There was a slight increase in Troponin I up to 0.24 ng/mL but the blood tests were otherwise unremarkable. The patient was admitted at the coronary unit and was scheduled for urgent coronary angiogram. The exam revealed severe and diffuse stenosis in the territories of the right and left coronary arteries with slow flow (TIMI 1-2), sparing only the stented segments (picture/<xref ref-type="fig" rid="m01">video 1</xref>). The administration of 2 mg of intracoronary isosorbite dinitrate reverted all the stenosis but slow flow (TIMI 2) was still observed in the left coronary artery. Hence, the diagnosis of vasospastic angina was made. The patient was successfully controlled with calcium antagonists and has remained asymptomatic.</p>
    <p>Vasospastic angina is commonly misinterpreted as acute coronary syndrome. Although its pathophysiology is not fully understood, it usually has a favorable long-term prognosis, although coronary artery spasms may have an important role in arrhythmia generation and subsequent cardiac arrest.</p>

      <p>Um homem caucasiano de 56 anos veio ao nosso hospital com queixas de opress&#227;o tor&#225;cica relacionada a esfor&#231;os e ocorrendo por vezes em repouso, com dura&#231;&#227;o de alguns minutos. O paciente era fumante ativo, com h&#225;bitos etilistas moderados e apresentou um epis&#243;dio de angina inst&#225;vel dois meses antes. Nesse epis&#243;dio foram implantados dois stents farmacol&#243;gicos, um na art&#233;ria descendente anterior distal e outro na art&#233;ria diagonal proximal. O ECG mostrou eleva&#231;&#227;o leve do segmento ST em V1-V3 e invers&#227;o de onda T em V3-V5. Verificou-se um ligeiro aumento da Troponina I at&#233; 0,24 ng/mL, embora os exames de sangue tenham sido normais. O paciente foi internado na unidade coronariana e uma angiografia coronariana de urg&#234;ncia foi programada. O exame revelou estenose grave e difusa nos territ&#243;rios das art&#233;rias coron&#225;rias direita e esquerda com fluxo lento (TIMI 1-2), com apenas os segmentos dos stents preservados (foto/<xref ref-type="fig" rid="m02">v&#237;deo 1</xref>). A administra&#231;&#227;o de 2 mg de dinitrato de isossorbida intracoron&#225;rio reverteu toda a estenose, mas o fluxo lento (TIMI 2) ainda foi observado na art&#233;ria coron&#225;ria esquerda. Dessa forma, foi feito o diagn&#243;stico de angina vasoesp&#225;stica. O paciente foi controlado com sucesso com antagonistas de c&#225;lcio e permanece assintom&#225;tico.</p>
      <p>A angina vasosp&#225;stica &#233; comumente mal interpretada como s&#237;ndrome coronariana aguda. Embora sua fisiopatologia n&#227;o seja totalmente compreendida, geralmente tem um progn&#243;stico favor&#225;vel em longo prazo, ainda que os espasmos da art&#233;ria coron&#225;ria possam ter um papel importante na gera&#231;&#227;o de arritmias e subsequente parada card&#237;aca.</p>
