    <p>The study by Silva et al.<xref ref-type="bibr" rid="B1">1</xref> presented the results of a series of cases from a single-center registry of implantable cardiac devices, including 28 implantations of isolated cardioverter defibrillators (ICD) and 14 associated to resynchronization (CRT-ICD), in addition to procedures for generator replacement and handling of electrode cable.<xref ref-type="bibr" rid="B1">1</xref> ICD and CRT-ICD implantations were considered the strongest predictors of re-admission into the hospital during a six-month period after the surgical procedure, although it is not clear whether there was adjustment by functional class or left ventricular ejection fraction.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>We have recently published the results of short, medium, and long term follow-ups of all ICD and CRT-ICD implantations funded by The Brazilian Unified Healthcare System (SUS) in all of Brazil, between 2001 and 2007, including 3,295 ICD implantations in 85 hospitals and 681 CRT-ICD implantations in 50 hospitals.<xref ref-type="bibr" rid="B2">2</xref>
		</p>
    <p>In our study, when compared to the ICD implantation group, patients who underwent CRT-ICD implantation had a worse mortality relative to the procedure and also a worse survival in the medium and long run.<xref ref-type="bibr" rid="B2">2</xref> However, in the study by Silva et al.<xref ref-type="bibr" rid="B1">1</xref>, there apparently was no worse short-term prognosis of patients who underwent CRT-ICD with relation to those who underwent ICD implantation, at least in regards to re-admission.<xref ref-type="bibr" rid="B1">1</xref> In relation to survival time, it would be interesting to consider stratification by device type, since the presented 6-month survival<xref ref-type="bibr" rid="B1">1</xref> seemed low in relation to our finding,<xref ref-type="bibr" rid="B2">2</xref> especially considering most implantations were for pacemakers.<xref ref-type="bibr" rid="B1">1</xref> The percentage of double-chamber ICD would also be interesting information, considering some studies point to an association with poorer results.<xref ref-type="bibr" rid="B2">2</xref>
		</p>
    <p>We have observed in the national multi-center study that the type of technique used for CRT-ICD implantation affected short-term mortality, and there was an important decrease in survival, approximately four years after the initial implantation in this group, possibly related to complications associated to the need for intervention.<xref ref-type="bibr" rid="B2">2</xref> It would be interesting to know the magnitude of re-interventions in the short period of follow-up of the single-center registry and the possible impacts of implantation techniques in the outcomes.</p>
    <p>The greater representativeness of Chagas heart disease, in the group with complications in the study by Silva et al.<xref ref-type="bibr" rid="B1">1</xref>, may be related to the type of implanted device. In our study, Chagas heart disease patients did not have a worse prognosis when compared to those with ischemic heart disease,<xref ref-type="bibr" rid="B2">2</xref> similarly to another study recently published in Arquivos Brasileiros de Cardiologia.<xref ref-type="bibr" rid="B3">3</xref>

    <body>
      <p>The care with patients with implantable electronic cardiac devices (IECD) demands more and more the attention to the different clinical presentations of these patients. Such differences are becoming more prominent due to two main factors: an increase in the population longevity, which has increased comorbidities, and the increasing frequency of patients with cardiomyopathy and ventricular dysfunction.</p>
      <p>We believe that the main message of this publication is to prove, through data derived from real clinical practice, that older patients, just as individuals with severe left ventricular dysfunction, are more prone to clinical events, related or not to cardiac disease, that require readmission to the hospital and that, therefore, must be observed more closely in order to be avoided.</p>
      <p>Undoubtedly, ICD and CRT incorporation to the list of artificial cardiac stimulation procedures has brought a considerable contingent of patients with severe heart failure, which did not use to be as frequent when the only implanted devices were pacemakers. It is also not likely that this is the reason for the association mentioned in the commentary. With regards specifically to the relation between Chagas disease and mortality, our study was not designed with this objective nor would it have the sample power for that.</p>
      <p>To make any comparison between the results of the presently published study and those of the publication of the comment's authors does not seem possible. While our study is a prospective analysis of primary data collected from a population of patients with all types of implantable devices, treated in one single, highly specialized, center, actively monitored by the researchers, the study by Migowski et al. is a retrospective analysis of secondary data obtained from administrative bases of SUS, from a population with cardio-defibrillators implanted at various centers with a varied level of specialization. Furthermore, the follow-up information is clearly incomplete, especially considering the authors of the study themselves have informed they censored all deaths from causes not related to heart disease or treatment.</p>
      <p>We believe an attempt to make this kind of comparison would be like trying to compare the wine from a single producer to water from a container whose samples come from different sources.</p>

Os implantes de CDI e TRC-CDI foram considerados os mais fortes preditores de readmiss&#227;o hospitalar durante per&#237;odo de seguimento de 6 meses ap&#243;s o procedimento cir&#250;rgico, embora n&#227;o tenha ficado claro se houve ajuste pelas vari&#225;veis classe funcional ou fra&#231;&#227;o de eje&#231;&#227;o do ventr&#237;culo esquerdo.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>Recentemente publicamos os resultados de seguimento de curto, m&#233;dio e longo prazos de todos os implantes de CDI e TRC-CDI pagos pelo SUS em todo o Brasil entre 2001 e 2007, incluindo 3.295 implantes de CDI em 85 hospitais e 681 de TRC-CDI em 50 hospitais.<xref ref-type="bibr" rid="B2">2</xref>
			</p>
      <p>Em nosso estudo, quando comparados ao grupo com implante de CDI, os pacientes submetidos a implante de TRC-CDI tiveram pior mortalidade realtiva ao procedimento e tamb&#233;m pior sobrevida no m&#233;dio e longo prazos.<xref ref-type="bibr" rid="B2">2</xref> J&#225; no estudo de Silva et al.,<xref ref-type="bibr" rid="B1">1</xref> aparentemente n&#227;o houve pior progn&#243;stico de curto prazo dos pacientes submetidos &#224; TRC-CDI com rela&#231;&#227;o aos submetidos a implante de CDI, ao menos no que se refere &#224; readmiss&#227;o hospitalar. Com rela&#231;&#227;o ao tempo de sobrevida, seria interessante a estratifica&#231;&#227;o por tipo de dispositivo, uma vez que a sobrevida em 6 meses apresentada<xref ref-type="bibr" rid="B1">1</xref> pareceu baixa com rela&#231;&#227;o aos nossos achados,<xref ref-type="bibr" rid="B2">2</xref> especialmente considerando que a maioria dos implantes foi de marca-passo.<xref ref-type="bibr" rid="B1">1</xref> O percentual de CDI dupla c&#226;mara tamb&#233;m seria uma informa&#231;&#227;o interessante, tendo em vista que alguns estudos apontam a associa&#231;&#227;o com piores resultados.<xref ref-type="bibr" rid="B2">2</xref>
			</p>
      <p>Observamos no estudo multic&#234;ntrico nacional que o tipo de t&#233;cnica utilizada para o implante de TRC-CDI influenciou na mortalidade no curto prazo, e que houve uma queda importante de sobrevida em torno do quarto ano ap&#243;s o implante inicial nesse grupo, possivelmente relacionada a complica&#231;&#245;es associadas &#224; necessidade de reinterven&#231;&#227;o.<xref ref-type="bibr" rid="B2">2</xref> Seria interessante saber a magnitude das reinterven&#231;&#245;es no curto per&#237;odo de seguimento do registro unic&#234;ntrico e os poss&#237;veis impactos das t&#233;cnicas de implante nos desfechos.</p>
      <p>A maior representatividade de pacientes com cardiopatia chag&#225;sica no grupo com complica&#231;&#245;es no estudo de Silva et al.<xref ref-type="bibr" rid="B1">1</xref> pode ter rela&#231;&#227;o com o tipo de dispositivo implantado. Em nosso estudo, os pacientes com cardiopatia chag&#225;sica n&#227;o tiveram pior progn&#243;stico quando comparados &#224;queles com cardiopatia isqu&#234;mica,<xref ref-type="bibr" rid="B2">2</xref> de forma semelhante a outro estudo publicado recentemente em Arquivos Brasileiros de Cardiologia.<xref ref-type="bibr" rid="B3">3</xref>

      <body>
        <p>Os cuidados com portadores de dispositivos card&#237;acos eletr&#244;nicos implant&#225;veis (DCEI) exige, cada vez mais, a aten&#231;&#227;o &#224;s diferentes apresenta&#231;&#245;es cl&#237;nicas desses pacientes. Tais diferen&#231;as t&#234;m se acentuado em fun&#231;&#227;o de dois fatores principais: aumento da longevidade da popula&#231;&#227;o, que fez aumentar as comorbidades, e a frequ&#234;ncia cada vez maior de pacientes com miocardiopatia e disfun&#231;&#227;o ventricular.</p>
        <p>Acreditamos que a principal mensagem desta publica&#231;&#227;o &#233; a comprova&#231;&#227;o, por meio de dados derivados da pr&#225;tica cl&#237;nica real, de que os pacientes mais idosos, assim como os portadores de disfun&#231;&#227;o ventricular esquerda grave, est&#227;o mais propensos a apresentar eventos cl&#237;nicos relacionados ou n&#227;o &#224; doen&#231;a card&#237;aca, que exigem readmiss&#245;es hospitalares e que, portanto, devem ser observados mais de perto, com o objetivo de evit&#225;-los.</p>
        <p>Sem sombra de d&#250;vida, a incorpora&#231;&#227;o dos CDIs e da TRC ao rol de procedimentos da estimula&#231;&#227;o card&#237;aca artificial trouxe um contingente consider&#225;vel de pacientes com insufici&#234;ncia card&#237;aca grave, o que n&#227;o era t&#227;o frequente quando os &#250;nicos dispositivos implantados eram os marca-passos. E &#233; muito prov&#225;vel que este seja o motivo da associa&#231;&#227;o citada no coment&#225;rio. No que diz respeito especificamente &#224; rela&#231;&#227;o da doen&#231;a de Chagas com os &#243;bitos, nosso estudo n&#227;o foi desenhado com essa finalidade e nem teria poder de amostra para esse fim.</p>
        <p>Tecer qualquer tipo de compara&#231;&#227;o entre os resultados do estudo presentemente publicado e os da publica&#231;&#227;o dos autores do coment&#225;rio, entretanto, nos parece imposs&#237;vel. Enquanto nosso estudo &#233; uma an&#225;lise prospectiva, de dados prim&#225;rios colhidos de uma popula&#231;&#227;o de pacientes com todos os tipos de dispositivos implant&#225;veis, tratada em um &#250;nico centro altamente especializado, acompanhada ativamente pelos pr&#243;prios pesquisadores; o estudo de Migowski et al &#233; uma an&#225;lise retrospectiva, de dados secund&#225;rios obtidos de bases administrativas do SUS, de uma popula&#231;&#227;o de pacientes com cardio-desfibrilador implantado em v&#225;rios centros, com n&#237;vel de especializa&#231;&#227;o vari&#225;vel. Al&#233;m disso, as informa&#231;&#245;es de seguimento s&#227;o evidentemente incompletas, at&#233; porque, os pr&#243;prios autores do estudo informam que censuraram todos os &#243;bitos de causa n&#227;o relacionada &#224; cardiopatia ou ao tratamento realizado.</p>
        <p>Parece-nos que tentar fazer esse tipo de compara&#231;&#227;o, seria como comparar o vinho de uma garrafa de um &#250;nico produtor com a &#225;gua de um recipiente cujas amostras s&#227;o oriundas de diferentes fontes.</p>
