    <p>I have read with great interest the article entitled "Early Markers of Atherosclerotic Disease in Individuals with Excess Weight and Dyslipidemia" by Menti et al.<xref ref-type="bibr" rid="B1">1</xref> recently published in Arquivos Brasileiros de Cardiologia 2016; 106: 457-63. The investigators reported that fibrinogen is associated with subclinical atherosclerosis in individuals with excess weight.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>Several studies have shown that high serum levels of fibrinogen are strongly associated with coronary artery disease.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> High serum levels of fibrinogen may contribute to vascular disease by increasing blood viscosity, stimulating fibrin formation, or increasing platelet-platelet interaction.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>Catena et al.<xref ref-type="bibr" rid="B4">4</xref> reported that plasma homocysteine (Hcy) levels were directly correlated with age, waist circumference, fasting glucose, triglyceride, uric acid, and fibrinogen levels, and inversely correlated with creatinine clearance and high-density lipoprotein cholesterol, vitamin B12, and folate levels. Low vitamin B12 concentration and hyperhomocysteinemia are common, and might affect serum fibrinogen levels.</p>
    <p>25-hydroxyvitamin D [25(OH)D] deficiency with increased risks of cardiovascular disease and venous thromboembolism may relate to adverse hemostatic and inflammatory responses. Blondon et al.<xref ref-type="bibr" rid="B5">5</xref> reported that low levels of serum [25(OH)D] were cross-sectionally associated with higher levels of interleukin-6, homocysteine, total tissue factor pathway inhibitor and plasminogen activator inhibitor-1.</p>
    <p>In light of these findings, it might be beneficial to evaluate serum levels of vitamin B12, Hcy, and [25(OH)D] because of their close association with fibrinogen levels.</p>

      <p>We were honored by your interest in our article entitled "Early Markers of Atherosclerotic Disease in Individuals with Excess Weight and Dyslipidemia" published in Arquivos Brasileiros de Cardiologia.</p>
      <p>The finding of high fibrinogen levels in patients with atherosclerotic disease has generated a large volume of clinical evidences in the past few decades, and has been considered a risk marker for cardiovascular events. High fibrinogen levels are also known to promote atherosclerotic disease by increasing blood viscosity, stimulating fibrin formation and increasing platelet aggregation. In the setting of an inflammatory status such as that seen in individuals with excess weight, the higher hepatic production of fibrinogen regulated by inflammatory cytokines may be an important link in the progression of the atherosclerotic disease in its different subclinical and clinical stages. Our study had a small sample size, however large enough to bring this association to light.<xref ref-type="bibr" rid="B6">1</xref>
			</p>
      <p>As regards the assessment of serum levels of vitamin B12 and vitamin D, in a recent study with a small sample, Baser et al<xref ref-type="bibr" rid="B7">2</xref> observed the association of vitamin D deficiency with high fibrinogen levels and pro-oxidative serum markers. Vitamin B12 deficiency, by hyperhomocysteinemia induction, also plays a role in the development of cardiovascular disease. Every 5-mcmol/L increase above 10 mcmol/L in serum levels of homocystein is associated with a 20% increase in the risks of circulatory disorders.<xref ref-type="bibr" rid="B8">3</xref>
			</p>
      <p>Although the assessment of changes in endothelial function in patients with vitamin B12 and vitamin D deficiency had not been included in this study, it is a promising research field. In a study with a small sample assessing endothelial function using flow-mediated brachial artery dilatation in patients with vitamin B12 deficiency, increased dilatation after proper vitamin B12 replacement was observed.<xref ref-type="bibr" rid="B9">4</xref> A similar finding was observed in a sample of individuals undergoing hemodialysis after vitamin D replacement.<xref ref-type="bibr" rid="B10">5</xref>
			</p>
      <p>We thank Dr. Cerit's remarks and are satisfied to stimulate discussion regarding this important field of clinical cardiology, which has been a frequent concern in daily practice, in view of the growing incidence of obesity among our society.</p>

      <p>Li, com grande interesse, o artigo intitulado "Marcadores Precoces de Doen&#231;a Ateroscler&#243;tica em Indiv&#237;duos com Excesso de Peso e Dislipidemia", de Menti et al.,<xref ref-type="bibr" rid="B1">1</xref> recentemente publicado nos Arquivos Brasileiros de Cardiologia 2016;106:457-63. Os investigadores relataram que o fibrinog&#234;nio est&#225; associado a aterosclerose subcl&#237;nica em indiv&#237;duos com excesso de peso.</p>
      <p>V&#225;rios estudos mostraram que n&#237;veis aumentados de fibrinog&#234;nio s&#233;rico est&#227;o fortemente associados a doen&#231;a arterial coronariana.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref> Altos n&#237;veis de fibrinog&#234;nio podem contribuir para doen&#231;as vasculares atrav&#233;s do aumento da viscosidade sangu&#237;nea, do est&#237;mulo &#224; forma&#231;&#227;o de fibrina, ou do aumento da intera&#231;&#227;o plaqueta-plaqueta.<xref ref-type="bibr" rid="B2">2</xref>,<xref ref-type="bibr" rid="B3">3</xref>

      <p>Catena et al.<xref ref-type="bibr" rid="B4">4</xref> relataram que o n&#237;vel de homociste&#237;na plasm&#225;tica (Hcy) estava diretamente correlacionado &#224; idade, circunfer&#234;ncia abdominal e aos n&#237;veis de glicemia de jejum, triglic&#233;rides, &#225;cido &#250;rico e fibrinog&#234;nio, e inversamente correlacionado ao clearance de creatinina e aos n&#237;veis de colesterol lipoprote&#237;na de alta densidade, vitamina B12 e folato. A baixa concentra&#231;&#227;o de vitamin B12 e a hiperhomocisteinemia s&#227;o comuns e podem afetar o n&#237;vel de fibrinog&#234;nio s&#233;rico.</p>
      <p>A defici&#234;ncia de 25-hidroxivitamina D [25(OH)D] com risco aumentado de doen&#231;a cardiovascular e tromboembolismo venoso pode estar associada a respostas hemost&#225;ticas e inflamat&#243;rias adversas. Blondon et al.<xref ref-type="bibr" rid="B5">5</xref> relataram que baixos n&#237;veis de 25(OH)D s&#233;rica estavam transversalmente associados a altos n&#237;veis de interleucina-6, homociste&#237;na, inibidor da via do fator tecidual e inibidor-1 do ativador do plasminog&#234;nio.</p>
      <p>Em vista desses achados, pode ser &#250;til avaliar os n&#237;veis s&#233;ricos de vitamina B12, Hcy e [25(OH)D] devido &#224; sua &#237;ntima associa&#231;&#227;o com o n&#237;vel de fibrinog&#234;nio.</p>

        <p>Ficamos honrados pelo interesse demonstrado pelo nosso artigo entitulado "Marcadores Precoces de Doen&#231;a Ateroscler&#243;tica em Indiv&#237;duos com Excesso de Peso e Dislipidemia" publicado nos Arquivos Brasileiros de Cardiologia.</p>
        <p>A observa&#231;&#227;o de n&#237;veis elevados de fibrinog&#234;nio em paciente com doen&#231;a ateroscler&#243;tica produziu um grande volume de evid&#234;ncias cl&#237;nicas nas &#250;ltimas d&#233;cadas, sendo considerado um marcador de risco para eventos cardiovasculares. Sabe-se, ainda, que n&#237;veis elevados de fibrinog&#234;nio podem promover doen&#231;a ateroscler&#243;tica por aumentar a viscosidade sangu&#237;nea, estimular a forma&#231;&#227;o de fibrina e aumentar a agrega&#231;&#227;o plaquet&#225;ria. Em um status inflamat&#243;rio, como o que ocorre em indiv&#237;duos com excesso de peso, a maior produ&#231;&#227;o hep&#225;tica de fibrinog&#234;nio regulada pelas citocinas inflamat&#243;rias pode ser um importante elo na evolu&#231;&#227;o da doen&#231;a ateroscler&#243;tica em seus diferentes est&#225;gios subcl&#237;nicos e cl&#237;nicos. O nosso estudo contou com uma pequena amostra dessa popula&#231;&#227;o de indiv&#237;duos, mas foi o suficiente para trazer &#224; luz essa associa&#231;&#227;o.<xref ref-type="bibr" rid="B6">1</xref>
				</p>
        <p>Quanto &#224; avalia&#231;&#227;o dos n&#237;veis s&#233;ricos de vitamina B12 e vitamina D, em um pequeno estudo recente, Baser et al.<xref ref-type="bibr" rid="B7">2</xref> observaram a associa&#231;&#227;o entre a defici&#234;ncia de vitamina D com n&#237;veis elevados de fibrinog&#234;nio e marcadores s&#233;ricos pr&#243;-oxidantes. A defici&#234;ncia de vitamina B12, pela indu&#231;&#227;o da hiperhomocisteinemia, tamb&#233;m possui um papel no desenvolvimento da doen&#231;a cardiovascular. Cada aumento de 5 mcmol/L acima de 10 mcmol/L nos n&#237;veis s&#233;ricos de homociste&#237;na est&#225; associado com uma eleva&#231;&#227;o de 20% nos riscos de problemas circulat&#243;rios.<xref ref-type="bibr" rid="B8">3</xref>
				</p>
        <p>Embora n&#227;o tenha sido contemplada em nosso estudo, a avalia&#231;&#227;o das altera&#231;&#245;es da fun&#231;&#227;o endotelial em pacientes com defici&#234;ncia de vitamina B12 e vitamina D &#233; um campo promissor de pesquisas. Em um pequeno estudo que avaliou a fun&#231;&#227;o endotelial atrav&#233;s da dilata&#231;&#227;o da art&#233;ria braquial mediada pelo fluxo em pacientes com defici&#234;ncia de vitamina B12, observou-se acr&#233;scimo da dilata&#231;&#227;o ap&#243;s adequada reposi&#231;&#227;o de vitamina B12.<xref ref-type="bibr" rid="B9">4</xref> O mesmo foi observado em uma amostra de indiv&#237;duos em hemodi&#225;lise ap&#243;s a reposi&#231;&#227;o de vitamina D.<xref ref-type="bibr" rid="B10">5</xref>

        <p>Agradecemos as coloca&#231;&#245;es do Dr. Cerit e sentimo-nos satisfeitos em promover discuss&#227;o nessa &#225;rea t&#227;o importante da cardiologia cl&#237;nica, que tem sido frequente preocupa&#231;&#227;o na pr&#225;tica di&#225;ria, em vista da crescente incid&#234;ncia de obesidade em nossa sociedade.</p>
