    <p>I have read the article entitled "Recommendation of Early Surgery in Primary Mitral Regurgitation: Pros and Cons" by Rosa et al.<xref ref-type="bibr" rid="B1">1</xref> with great interest, recently published in Arquivos Brasileiros de Cardiologia 2016; 107: 173-5. The investigators reported that currently, the recommendation of mitral surgery for asymptomatic patients is very controversial, since the indication of valvular intervention for symptoms, left ventricular dysfunction and dilatation, recent onset atrial fibrillation or pulmonary arterial hypertension is well consolidated in literature.<xref ref-type="bibr" rid="B1">1</xref>
		</p>
    <p>Quantifying the mitral regurgitation (MR) in echocardiographic quantification is used primarily to aid grading as mild, moderate, and severe regurgitation. Cardiovascular magnetic resonance (CMR) is able to quantify MR with high accuracy and reproducibility using a combination of left ventricle (LV) volumetric measurements and aortic flow quantification. Myerson et al.<xref ref-type="bibr" rid="B2">2</xref> reported that quantifying MR with CMR showed a strong association with the future need for surgery over the subsequent 5 years, demonstrating the potential value of this approach. Previous studies also suggest only moderate agreement between CMR and echocardiography<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> and limited reproducibility for quantitative echocardiographic grading.<xref ref-type="bibr" rid="B5">5</xref> Evaluation of MR with CMR showed a significant relation with the future need for mitral valve surgery and was superior to CMR-derived LV volume and echocardiographic grading of regurgitation. These CMR parameters might prove useful for identifying suitable patients for early mitral valve repair replacement.<xref ref-type="bibr" rid="B2">2</xref>
		</p>
    <p>In the light of these knowledge, CMR parameters might be beneficial to determine suitable patients for early mitral valve repair/replacement.</p>

      <p>Thank you so much for you interest in our Article "Indica&#231;&#227;o Cir&#250;rgica Precoce na Insufici&#234;ncia Mitral Prim&#225;ria: Pr&#243;s e Contras", published in Arquivos Brasileiros de Cardiologia.</p>
      <p>Cardiovascular magnetic resonance (CMR) is a method of great value, primarily regarding the anatomical diagnostic of valvular diseases. Its main application, to the present day, occurs in patients in which a clinical and echocardiographic dissociation, namely cases in which propaedeutics indicates an important valvar disease and the echocardiogram describes such injury as moderate, or vice-versa. In these cases, CMR is a better choice instead of cardiac catheterization together with ventriculography, as it seems less invasive and able to quantify more precisely the cardiac chambers volumes and functions.<xref ref-type="bibr" rid="B6">1</xref>
			</p>
      <p>Myerson et al.<xref ref-type="bibr" rid="B7">2</xref> and Enriquez-Sarano et al.<xref ref-type="bibr" rid="B8">3</xref> articles show how extremely important is to demonstrate the existence of patient subgroups which could have benefited from an early mitral valvular surgery. Nevertheless, the parameters evaluated in this article need validation in future studies due to the work's pattern under discussion (observational study, non-blinded, surgical recommendation based on the patient's doctor opinion).</p>
      <p>Thus, our opinion is that one must pay attention when recommending intervention based solely on the CMR parameters. However, we are in agreement as to such data is able to, in connection with other risk markers (BPF, effective regurgitant orifice, among others), add information for the Heart Team clinical decision.</p>

      <p>Li o artigo intitulado "Recommendation of Early Surgery in Primary Mitral Regurgitation: Pros and Cons" de Rosa et al.<xref ref-type="bibr" rid="B1">1</xref> com grande interesse, recentemente publicado em Arquivos Brasileiros de Cardiologia 2016;107:173-5. Os pesquisadores reportaram que, atualmente, a indica&#231;&#227;o de cirurgia mitral para pacientes assintom&#225;ticos &#233; bastante controversa, uma vez que a indica&#231;&#227;o de interven&#231;&#227;o valvar com base nos sintomas, disfun&#231;&#227;o e dilata&#231;&#227;o ventricular esquerda, in&#237;cio de fibrila&#231;&#227;o atrial recente ou hipertens&#227;o arterial pulmonar &#233; bastante consolidada na literatura.<xref ref-type="bibr" rid="B1">1</xref>
			</p>
      <p>Quantificar a insufici&#234;ncia mitral (IM) em quantifica&#231;&#227;o ecocardiogr&#225;fica &#233; uma t&#233;cnica utilizada principalmente para auxiliar a classifica&#231;&#227;o da insufici&#234;ncia como leve, moderada e severa. Resson&#226;ncia magn&#233;tica card&#237;aca (RMC) &#233; capaz de quantificar IM com uma alta precis&#227;o e reprodutibilidade, utilizando uma combina&#231;&#227;o de medidas volum&#233;tricas do ventr&#237;culo esquerdo (VE) e quantifica&#231;&#227;o do fluxo a&#243;rtico. Myerson et al.<xref ref-type="bibr" rid="B2">2</xref> reportaram que, quantificar IM a partir de RMC demonstrou uma forte associa&#231;&#227;o com a necessidade futura de cirurgia pelos 5 anos subsequentes, demonstrando o potencial valor desta abordagem. Estudos anteriores tamb&#233;m sugeriram apenas uma concord&#226;ncia moderada entre RMC e ecocardiografia<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref> e reprodutibilidade limitada para classifica&#231;&#227;o ecocardiogr&#225;fica quantitativa.<xref ref-type="bibr" rid="B5">5</xref> Avalia&#231;&#227;o de IM a partir de RMC demonstrou uma rela&#231;&#227;o significativa com a futura necessidade de cirurgia valvar mitral e foi melhor do que o volume do (VE) proveniente da RMC e da classifica&#231;&#227;o ecocardiogr&#225;fica da regurgita&#231;&#227;o. Esses par&#226;metros provenientes da RMC podem ser &#250;teis na identifica&#231;&#227;o de pacientes adequados para uma cirurgia precoce de restabelecimento/substitui&#231;&#227;o da v&#225;lvula mitral.<xref ref-type="bibr" rid="B2">2</xref>

      <p>De acordo com esses princ&#237;pios, os par&#226;metros da RMC podem ser ben&#233;ficos para a determina&#231;&#227;o de pacientes adequados para uma cirurgia precoce de restabelecimento/substitui&#231;&#227;o da v&#225;lvula mitral.</p>

        <p>Agradecemos o interesse no nosso Ponto de Vista "Indica&#231;&#227;o Cir&#250;rgica Precoce na Insufici&#234;ncia Mitral Prim&#225;ria: Pr&#243;s e Contras", publicado nos Arquivos Brasileiros de Cardiologia.</p>
        <p>A resson&#226;ncia magn&#233;tica card&#237;aca (RMC) &#233; um m&#233;todo de grande valor, principalmente no diagn&#243;stico anat&#244;mico das doen&#231;as valvares. At&#233; o presente momento, sua principal indica&#231;&#227;o se faz nos pacientes em que h&#225; uma dissocia&#231;&#227;o cl&#237;nico-ecocardiogr&#225;fica, ou seja, casos em que a proped&#234;utica indica uma doen&#231;a valvar importante e o ecocardiograma descreve tal les&#227;o como moderada, ou vice-versa. Nestas situa&#231;&#245;es, a RMC &#233; preferida em rela&#231;&#227;o ao cateterismo card&#237;aco com ventriculografia por ser menos invasiva e quantificar com maior precis&#227;o os volumes e fun&#231;&#227;o das c&#226;maras card&#237;acas.<xref ref-type="bibr" rid="B6">1</xref>
				</p>
        <p>O artigo de Myerson et al.,<xref ref-type="bibr" rid="B7">2</xref> assim como Enriquez-Sarano et al.,<xref ref-type="bibr" rid="B8">3</xref> &#233; de extrema import&#226;ncia em demonstrar que existem subgrupos de pacientes que poderiam ter benef&#237;cio com a cirurgia valvar mitral precoce ("Early Surgery"). Entretanto, os par&#226;metros avaliados neste artigo precisam de valida&#231;&#227;o em estudos futuros devido ao desenho do trabalho em quest&#227;o (estudo observacional, n&#227;o-cego, indica&#231;&#227;o cir&#250;rgica baseada na opini&#227;o do m&#233;dico do paciente).</p>
        <p>Dessa maneira, somos da opini&#227;o que se deve ter cautela na indica&#231;&#227;o de interven&#231;&#227;o com os par&#226;metros da RMC isoladamente. Entretanto concordamos que tais dados podem, associados a outros marcadores de risco (BNP, orif&#237;cio regurgitante efetivo, dentre outros), agregar informa&#231;&#245;es para a decis&#227;o cl&#237;nica do Heart Team.</p>
