  <body>
    <p>The Rheumatic fever (RF) and the rheumatic heart disease (RHD) remain important health problems among populations of developing countries<xref ref-type="bibr" rid="B1">1</xref>. The number of studies is scarce, and public and private investments are practically inexistent. In addition, the cases of rheumatic fever are underdiagnosed, and are recognized only following permanent valve damage through the manifestation of carditis, more frequently represented by mitral stenosis.<xref ref-type="bibr" rid="B2">2</xref>
		</p>
    <p>In high-income countries, a tonsillitis is only a "throat infection". However, in developing countries, or even those of average income, as it is the case of Brazil, it may be a harbinger of rheumatic heart disease, a disease that kills approximately 233,00 to 500,000 of RF/RHD sufferers per year globally.<xref ref-type="bibr" rid="B2">2</xref> According to a projection by the World Health Organization (WHO) and the latest census by the Brazilian Institute of Geography and Statistics ("Instituto Brasileiro de Geografia e Estat&#237;stica", IBGE), it is estimated that annually in Brazil 10 million streptococcal pharygotonsillitis occur, totaling thirty Thousand new RF cases, of which up to half may evolve with cardiac involvement.<xref ref-type="bibr" rid="B3">3</xref>
		</p>
    <p>The great villain is the streptococcus from the A group (Streptococcus pyogenes ), which triggers a series of pathological reactions, usually starting with tonsillitis and is followed, in certain cases, by acute rheumatic fever, whose outcome may result in the degeneration of the heart valve.<xref ref-type="bibr" rid="B2">2</xref> According to Srinath Redd, the former president of the World Heart Federation (WHF), the rheumatic heart disease is a marker of inequality, social injustice and abandonment of countless populations living in poverty. In the last years, defense groups, including the WHF, are making efforts to rectify this unequality.<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>The negligence is manifested in the relative lack of engagement in the control of the disease on the behalf of governments, society and fostering agencies. The number of academic publications between 1996 and 2006 were 66% lower than between 1966 and 1976.<xref ref-type="bibr" rid="B5">5</xref>
		</p>
    <p>The RF was responsible for 5.1 million potential disability-adjusted life years ("DALYs"), resulting from 280,000 deaths, in 2004, and it was the seventh and eighth causes of mortality and morbidity due to neglected diseases, respectively. However, the G-Finder report on research and development for neglected diseases identified only four organizations that invested in this infirmity in the last 4 years.<xref ref-type="bibr" rid="B5">5</xref> The funding is disproportionate when compared with the other neglected diseases. Public research institutes contributed with 91.4% of the funds, and philanthropic organizations, with 8.6%. On the other hand, funding from the pharmaceutical industry was insignificant.<xref ref-type="bibr" rid="B5">5</xref>
		</p>
    <p>Late diagnosis is due to the fact that the stethoscope is the only noninvasive tool available to doctors in remote places of low social and economic level where RF and RHD are more prevalent. However, the detection frequency of rheumatic carditis through cardiac auscultation is usually low, and depends on factors such as the experience of the examiner.<xref ref-type="bibr" rid="B6">6</xref> The echodopplercardiogram was shown to be more sensible and specific than cardiac auscultation in the early detection of RF and RHD,<xref ref-type="bibr" rid="B7">7</xref> but it is still little available in low-income settings.</p>
    <p>Marijon et al.<xref ref-type="bibr" rid="B7">7</xref> performed portable echocardiography in schoolchildren in Mozambique and Cambodia and observed a detection rate ten times greater than that from the auscultation. The prevalence of the subclinical carditis then was documented in various studies throughout the world. The greatest prevalence mentioned was among school age children in Togo (3.3%), followed by Mozambique (3.0%), Cambodia (2.2%), Aboriginal Australians (2.2%), Zaire (1.4%) and Namibia (1.2%). The prevalence of 6.7% was estimated in Vietnam.<xref ref-type="bibr" rid="B8">8</xref> The echocardiographic criteria for diagnosis of rheumatic carditis were unified by WHF in 2012, aiming to standardize the publications.<xref ref-type="bibr" rid="B9">9</xref>
		</p>
    <p>The echocardiographic findings are divided into three categories: definitive RHD, borderline RHD and normal<xref ref-type="bibr" rid="B9">9</xref>. The natural history of the patients deemed as having definitive RHD without previous history of acute and borderline rheumatic fever are uncertain. If they are variants of the rheumatic heart disease, it is not known for certain if the secondary prophylaxis shall prevent the progression of the disease as it occurs with patients diagnosed through the "traditional" method. The false positive cases may have a detrimental impact on the individuals and their families, as well as being a burden for the health system.<xref ref-type="bibr" rid="B10">10</xref>
		</p>
    <p>The definition of subclinical rheumatic heart disease, according to the WHF, is the functional or structural alteration of the heart to the echocardiogram, consistent with the rheumatic heart disease in the absence of a pathological heart murmur.<xref ref-type="bibr" rid="B11">11</xref>
		</p>
    <p>The prognostic of the RHD is directly linked to the gravity of the initial carditis and recurrent attacks. Approximately 60% of the patients that develop cardiac insufficiency during the first outbreak will have some valvar disease in the next 10 years. However, even these patients have a great chance of improvement and even regression of the cardiac injury is appropriate secondary prophylaxis is performed.</p>
    <p>Most of the patients that are enrolled in health programs are symptomatic individuals with advanced disease, indicating they had silent or undetectable acute RF attacks; therefore, they go undiagnosed in the initial outbreaks.</p>
    <p>The cost of performing an echodopplercardiogram and the difficulty to access this procedure makes the disease to be underdiagnosed in its initial stage, progressing towards irreversible valve damages with need for surgery for valve replacement.<xref ref-type="bibr" rid="B12">12</xref>
		</p>
    <p>However, the expenses generated by assistance to RF and carditis patients in Brazil are much more significant. Thus, in 2007, approximately R$150 million in admissions resulting from the RF were used by the Unified Health System (SUS), in interventional procedures, surgery and percutaneous valvotomy due to cardiac sequelae from the RF<xref ref-type="bibr" rid="B3">3</xref>.</p>
    <p>Additionally, the difficulty to diagnose the acute RF is due to various other factors, such as the absence of specific laboratory markers, atypical displays of articular involvement, little availability of echodopplercardiogram in areas where it is most needed, as well as the low sensibility of the Jones criteria. They all contribute towards the underdiagnosis of this pathology<xref ref-type="bibr" rid="B13">13</xref>.</p>
    <p>The appearance of echocardiographic screening programs revealed that the rheumatic heart disease might be considerably more prevalent than imagined among school-aged children in developing countries. These epidemiological evidences show a clear relation between the incidence of acute RF fever and RHD. However, there are few reliable estimations on the incidence and prevalence of acute Rheumatic Fever in developing countries, including in Brazil. The high prevalence of rheumatic heart disease mentioned in various studies leads us to speculate that the incidence of acute RF is much greater than what was previously estimated<xref ref-type="bibr" rid="B14">14</xref>.</p>
    <p>The framework presented above pictures the need to create governmental programs with support from private initiative to establish the prevalence of the rheumatic fever in our population and develop study and combat groups against this devastating pathology. This initiative is being undertaken in Australia, with international support from the World Heart Federation and the World Health Organization with great success. </p>
    <p>In this manner, we shall begin a study to assess the rheumatic carditis and subclinical rheumatic heart disease, in a representative probability sample with children from 5 to 15 years of age, living in the outskirts of the southern region of the city of S&#227;o Paulo, in a partnership between the Heart Institute from the Clinical Hospital of the University of S&#227;o Paulo ("Instituto do Cora&#231;&#227;o do Hospital das Cl&#237;nicas da Universidade de S&#227;o Paulo", InCor), the Institute of Education and Research from the Albert Einstein Israelite Hospital ("Instituto de Ensino e Pesquisa do Hospital Israelita Albert Einstein") and the center of studies and research Dr. Jo&#227;o Amorim (CEJAM). In this study, the children shall be clinically assessed, and evaluated by portable echocardiography in accordance with criteria by the WHF. Three groups shall be selected: control (without evidence of rheumatic activity), possible rheumatic activity ('borderline') and definitive rheumatic activity. Genetic studies, HLA analysis and/or immunologic susceptibility shall be compared in the three groups, in order to identify what is the susceptibility standard for the development of rheumatic disease following infection by the Streptococcus pyogenes.</p>
    <p>Finally, the vaccine for rheumatic fever is under development in certain countries<xref ref-type="bibr" rid="B15">15</xref>. Brazil contributes with the possibility of a vaccine through development studies of the specific peptide, in execution for approximately 20 years at InCor, which, in turn, resulted in a clinical study of phase I/Iia, starting in 2017. The success of a vaccine in this scenario represents a great hope for our less privileged population.</p>

      <p>A Febre reum&#225;tica (FR) e a cardiopatia reum&#225;tica (CR) permanecem como problemas importantes de sa&#250;de nas popula&#231;&#245;es de pa&#237;ses em desenvolvimento.<xref ref-type="bibr" rid="B1">1</xref> O n&#250;mero de estudos &#233; escasso, e o investimento p&#250;blico e privado s&#227;o praticamente inexistentes.       <p>A neglig&#234;ncia se manifesta na relativa falta de engajamento no controle da doen&#231;a por parte dos governos, sociedade e ag&#234;ncias de fomento. No entanto, o relat&#243;rio G-Finder em pesquisa e desenvolvimento para doen&#231;as negligenciadas identificou apenas quatro organiza&#231;&#245;es que investiram nesta enfermidade nos &#250;ltimos 4 anos.<xref ref-type="bibr" rid="B5">5</xref> O financiamento &#233; desproporcional quando comparado com as demais doen&#231;as negligenciadas.       <p>O diagn&#243;stico tardio se deve ao fato de que o estetosc&#243;pio &#233; a &#250;nica ferramenta n&#227;o invasiva dispon&#237;vel aos m&#233;dicos em locais remotos de baixo n&#237;vel socioecon&#244;mico onde a FR e a CR s&#227;o mais prevalentes. Entretanto, a frequ&#234;ncia de detec&#231;&#227;o de cardite reum&#225;tica atrav&#233;s da ausculta card&#237;aca &#233; usualmente baixa e dependente de fatores como experi&#234;ncia do examinador.<xref ref-type="bibr" rid="B6">6</xref> O ecodopplercardiograma mostrou ser mais sens&#237;vel e especifico do que a ausculta card&#237;aca na detec&#231;&#227;o precoce de FR e CR,<xref ref-type="bibr" rid="B7">7</xref> mas encontra-se ainda pouco dispon&#237;vel em locais de baixa renda.</p>
      <p>Marijon et al.<xref ref-type="bibr" rid="B7">7</xref> realizaram ecocardiografia port&#225;til em escolares de Mo&#231;ambique e Camboja e observaram uma taxa de detec&#231;&#227;o dez vezes superior &#224; ausculta. A preval&#234;ncia da cardite subcl&#237;nica foi, ent&#227;o, documentada em diversos estudos pelo mundo. A maior preval&#234;ncia citada foi entre crian&#231;as em idade escolar no Tongo (3,3%), seguida por Mo&#231;ambique (3,0%), Camboja (2,2%), abor&#237;genes Australianos (2,2%), Zaire (1,4%) e Nam&#237;bia (1,2%). A preval&#234;ncia de 6,7% foi estimada no Vietnam.<xref ref-type="bibr" rid="B8">8</xref> Os crit&#233;rios ecocardiogr&#225;ficos para diagn&#243;stico de cardite reum&#225;tica foram unificados pela WHF em 2012 com intuito de uniformiza&#231;&#227;o das publica&#231;&#245;es.<xref ref-type="bibr" rid="B9">9</xref>

      <p>Os achados ecocardiogr&#225;ficos se dividem em 3 categorias: CR definitiva, CR lim&#237;trofe e normais.<xref ref-type="bibr" rid="B9">9</xref> A hist&#243;ria natural dos pacientes classificados como tendo CR definitiva sem hist&#243;ria pr&#233;via de febre reum&#225;tica aguda e lim&#237;trofe s&#227;o incertas. Caso eles sejam variantes da cardiopatia reum&#225;tica, n&#227;o se sabe ao certo se a profilaxia secund&#225;ria ir&#225; prevenir a progress&#227;o da doen&#231;a como ocorre com os pacientes diagnosticados pelo m&#233;todo "tradicional". Os casos falsos positivos podem ter um impacto prejudicial nos indiv&#237;duos e suas fam&#237;lias, al&#233;m de serem um &#244;nus para o sistema de sa&#250;de.<xref ref-type="bibr" rid="B10">10</xref>

      <p>A defini&#231;&#227;o de cardiopatia reum&#225;tica subcl&#237;nica segundo a WHF &#233; altera&#231;&#227;o funcional ou estrutural do cora&#231;&#227;o ao ecocardiograma, consistente com cardiopatia reum&#225;tica na aus&#234;ncia de um sopro card&#237;aco patol&#243;gico.<xref ref-type="bibr" rid="B11">11</xref>
			</p>
      <p>O progn&#243;stico da CR &#233; diretamente associado com a gravidade da cardite inicial e ataques recorrentes. Aproximadamente 60% dos pacientes que desenvolvem insufici&#234;ncia card&#237;aca durante o primeiro surto ter&#227;o alguma doen&#231;a valvar nos pr&#243;ximos 10 anos. Entretanto, mesmo estes pacientes t&#234;m uma grande chance de melhora e at&#233; mesmo regress&#227;o da les&#227;o card&#237;aca se for realizada profilaxia secund&#225;ria adequada.</p>
      <p>A maioria dos pacientes que se matriculam em programas de sa&#250;de s&#227;o indiv&#237;duos sintom&#225;ticos com doen&#231;a avan&#231;ada, indicando que eles tiveram ataques silenciosos ou indetect&#225;veis de FR aguda, portanto, n&#227;o diagnosticados nos surtos iniciais.</p>
      <p>O custo da realiza&#231;&#227;o de ecodopplercardiograma e a dificuldade de acesso a este procedimento faz com que a doen&#231;a seja subdiagnosticada em seu est&#225;gio inicial, progredindo para danos valvares irrevers&#237;veis com necessidade de cirurgia para troca valvar.<xref ref-type="bibr" rid="B12">12</xref>
			</p>
      <p>Entretanto, os gastos gerados pela assist&#234;ncia aos pacientes com FR e cardite no Brasil s&#227;o muito mais significativos. Assim, em 2007, foram destinados pelo Sistema &#218;nico de Sa&#250;de (SUS) cerca de R$150 milh&#245;es em interna&#231;&#245;es decorrentes de FR. Foram tamb&#233;m relatados gastos de R$94 milh&#245;es, em 2005 e de R$100 milh&#245;es, em 2007, em procedimentos intervencionistas, cirurgias e valvotomias percut&#226;neas por sequelas card&#237;acas da FR.<xref ref-type="bibr" rid="B3">3</xref>
			</p>
      <p>Al&#233;m disto, a dificuldade em diagnosticar a FR aguda se deve a diversos outros fatores, tais como aus&#234;ncia de marcadores laboratoriais espec&#237;ficos, apresenta&#231;&#245;es cl&#237;nicas at&#237;picas de envolvimento articular, baixa disponibilidade de ecodopplercardiograma nas &#225;reas onde ele &#233; mais necess&#225;rio, al&#233;m da baixa sensibilidade dos crit&#233;rios de Jones. Todos estes contribuem para o subdiagn&#243;stico desta patologia.<xref ref-type="bibr" rid="B13">13</xref>
			</p>
      <p>O surgimento de programas de triagem ecocardiogr&#225;fica revelou que a cardiopatia reum&#225;tica pode ser consideravelmente mais prevalente do que se imaginava entre crian&#231;as em idade escolar nos paises em desenvolvimento. Estas evid&#234;ncias epidemiol&#243;gicas mostram claramente um relacionamento entre a incid&#234;ncia de febre FR aguda e CR. Entretanto, h&#225; poucas estimativas confi&#225;veis da incid&#234;ncia e preval&#234;ncia de Febre Reum&#225;tica aguda nos paises em desenvolvimento, inclusive no Brasil. A elevada preval&#234;ncia de cardiopatia reum&#225;tica citada em diversos estudos nos leva a especular que a incid&#234;ncia de FR aguda &#233; muito maior do que a anteriormente estimada.<xref ref-type="bibr" rid="B14">14</xref>
			</p>
      <p>O quadro apresentado acima retrata a necessidade da cria&#231;&#227;o de programas governamentais com apoio da iniciativa privada para estabelecer a preval&#234;ncia da febre reum&#225;tica em nossa popula&#231;&#227;o e desenvolver grupos de estudos e combate desta patologia devastadora. Esta iniciativa est&#225; sendo realizada na Australia com apoio internacional da "World Heart Federation" e Organiza&#231;&#227;o Mundial da Sa&#250;de com grande sucesso. </p>
      <p>Assim, iniciaremos um estudo para avalia&#231;&#227;o da cardite reum&#225;tica e cardiopatia reum&#225;tica subcl&#237;nica em amostra probabil&#237;stica representativa de crian&#231;as de 5 a 15 anos, moradoras da periferia da zona Sul, na cidade de S&#227;o Paulo, numa parceria entre o Instituto do Cora&#231;&#227;o do Hospital das Cl&#237;nicas da Universidade de S&#227;o Paulo (InCor), Instituto de Ensino e Pesquisa do Hospital Israelita Albert Einstein e o centro de estudos e pesquisas Dr. Jo&#227;o Amorim (CEJAM). Neste estudo, as crian&#231;as ser&#227;o avaliadas clinicamente e por ecocardiografia port&#225;til de acordo com os crit&#233;rios da WHF. Ser&#227;o selecionados tr&#234;s grupos: controle (sem evid&#234;ncia de atividade reum&#225;tica), poss&#237;vel atividade reum&#225;tica ('borderline') e atividade reum&#225;tica definitiva. Estudos de gen&#233;tica, an&#225;lise de HLA e/ou perfil de susceptibilidade imunol&#243;gica ser&#227;o comparados nos tr&#234;s grupos, a fim de identificarmos qual o padr&#227;o de susceptibilidade ao desenvolvimento de doen&#231;a reum&#225;tica ap&#243;s infec&#231;&#227;o pelo Streptococcus pyogenes.</p>
      <p>Finalmente, a vacina para febre reum&#225;tica est&#225; em desenvolvimento em alguns pa&#237;ses.<xref ref-type="bibr" rid="B15">15</xref> O Brasil contribui com a possibilidade de uma vacina, atr&#225;ves de estudos de desenvolvimento do pept&#237;deo espec&#237;fico em execu&#231;&#227;o h&#225; aproximadamente 20 anos no InCor, que, por sua vez, resultou em projeto de estudo cl&#237;nico de fase I/IIa, com &#237;nicio em 2017. O sucesso de uma vacina neste cen&#225;rio representa grande esperan&#231;a para a nossa popula&#231;&#227;o menos privilegiada.</p>
