    <p>An 80-year old woman was admitted to the emergency room due to severe dyspnea. She had no known history of cardiovascular disease, but in last month she complained to increasing fatigue and shortness of breath. Physical examination was notable for diffuse lungs crackles. The heart sounds were normal and no murmur could be heard. The 12-lead electrocardiogram demonstrated sinus rhythm with non-specific ST-segment changes. The chest radiograph showed congested lung and there was also a round shadow behind the heart (<xref ref-type="fig" rid="f1">figure 1-A</xref>). The patient was admitted to Cardiology department for further investigation.</p>
    <p>

          <title>A) The chest radiography showed congested lung and a round shadow behind the heart; B) Transthoracic echocardiography showing echolucent mass severely compressing the left atrium; C) Spiral computed tomography of thorax showed a large hiatus hernia with intrathoracic extension.</title>

    </p>
    <p>In the next day, a two-dimension transthoracic echocardiography was performed and showed normal left ventricular function, no valvular disease, but the left atrium was severely compressed by an extrinsic amorphous, echolucent mass (<xref ref-type="fig" rid="f1">Figure 1-B</xref>). Spiral computed tomography of thorax showed a large hiatus hernia with intrathoracic extension (<xref ref-type="fig" rid="f1">Figure 1-C</xref>). The hernia was located behind the left atrium causing anterior shift of the heart. The intrathoracic migration of a large part of the stomach was confirmed by upper gastrointestinal barium examination, which was performed after consulting a surgeon, to further assess the extent of the hernia and the potential need for surgical treatment.</p>
    <p>In the first few months, the patient was successfully treated with conservative measures, but the symptoms returned and she was admitted again with acute pulmonary edema. One week later, the corrective surgery was performed and the patient had no further recurrence of acute pulmonary edema in the subsequent 6 months.</p>
    <p>In the light of these knowledges, FFR might be a useful tool to evaluate moderate coronary lesions with regard to revascularization appropriateness.</p>

      <p>Uma mulher de 80 anos foi admitida ao pronto-socorro devido &#224; dispneia intensa. Ela n&#227;o tinha hist&#243;rico conhecido de doen&#231;a cardiovascular, mas no &#250;ltimo m&#234;s, queixou-se de aumento de fadiga e falta de ar. No exame f&#237;sico foram observadas crepita&#231;&#245;es difusas nos pulm&#245;es. Os sons card&#237;acos eram normais e n&#227;o havia sopros. O eletrocardiograma de 12 deriva&#231;&#245;es demonstrou ritmo sinusal com altera&#231;&#245;es n&#227;o-espec&#237;ficas do segmento ST. A radiografia de t&#243;rax mostrou pulm&#245;es congestionados, e tamb&#233;m uma mancha de forma arredondada atr&#225;s do cora&#231;&#227;o (<xref ref-type="fig" rid="f2">Figura 1-A</xref>). A paciente foi internada no Servi&#231;o de Cardiologia para posterior investiga&#231;&#227;o.</p>

      </p>
      <p>No dia seguinte, foi realizada uma ecocardiografia transtor&#225;cica bidimensional que demonstrou fun&#231;&#227;o ventricular esquerda normal, sem doen&#231;a valvular, mas o &#225;trio esquerdo estava severamente comprimido por uma massa extr&#237;nseca amorfa, ecoluscente (<xref ref-type="fig" rid="f2">Figura 1-B</xref>). A tomografia computadorizada espiral de t&#243;rax mostrou uma grande h&#233;rnia de hiato com extens&#227;o intrator&#225;cica (<xref ref-type="fig" rid="f2">Figura 1-C</xref>). A h&#233;rnia estava localizada atr&#225;s do &#225;trio esquerdo, causando desvio anterior do cora&#231;&#227;o. A migra&#231;&#227;o intrator&#225;cica de uma grande parte do est&#244;mago foi confirmada por teste de b&#225;rio digestivo superior, o qual foi realizado ap&#243;s consulta com um cirurgi&#227;o, para avaliar a extens&#227;o da h&#233;rnia e a potencial necessidade de tratamento cir&#250;rgico.</p>
      <p>Nos primeiros meses, a paciente foi tratada com sucesso com medidas conservadoras, mas os sintomas voltaram e ela foi internada novamente com edema pulmonar agudo. Uma semana mais tarde, a cirurgia corretiva foi realizada e a paciente n&#227;o apresentou mais recorr&#234;ncia do edema agudo de pulm&#227;o nos 6 meses subsequentes.</p>
