      <title>Introduction</title>
      <p>Vector transmission of Trypanosoma cruzi has declined in Latin America, which has been attributed to better epidemiological control of this form of transmission, estimated at 8 to 10 million chronic cases, in addition to reducing the number of new cases.<xref ref-type="bibr" rid="B1">1</xref>,<xref ref-type="bibr" rid="B2">2</xref> However, there has been an increase in the incidence of acute cases, predominantly by oral transmission due to the ingestion of food contaminated with feces of triatomids, both in isolated cases and in family micro-outbreaks.<xref ref-type="bibr" rid="B3">3</xref>

      <p>Necropsy studies that describe myocarditis in the acute phase of Chagas' disease are scarce and the existing reports in the literature are of studies carried out in the past decades and involve vector transmission in endemic areas.</p>
      <p>Cardiac involvement in the acute phase may have varied aspects, especially in relation to myocardial lesion, from an undetectable one, to the evolution to acute heart failure with severe myocarditis and death. Cardiac involvement is present in 90% of the cases, manifesting with myocarditis and pericardial effusion.<xref ref-type="bibr" rid="B4">4</xref>,<xref ref-type="bibr" rid="B5">5</xref> The analysis of a series of acute myocarditis cases showed a mortality of 5.6%.<xref ref-type="bibr" rid="B6">6</xref>

      <title>Case Report</title>
      <p>A 34-year-old African-descendant male patient, born and raised in the urban area of the municipality of Bragan&#231;a (state of Para, Brazil), an area considered endemic for Chagas' disease was assessed.</p>
      <p>He had had fever for 30 days, associated with chills, holocranial headache and myalgia. Three weeks before admission, he noticed dark-colored urine and dyspnea, which was progressive and quickly developed to dyspnea at minimal effort, accompanied by abdominal pain, nausea and vomiting, jaundice and facial edema. On admission, he had severe dyspnea, cold extremities, mild dehydration and mucocutaneous pallor. He was neither diabetic nor dyslipidemic. He denied hypertension and smoking. The patient regularly consumed acai juice during meals. He lived in a brick house.</p>
      <p>On clinical examination, the heart sounds were muffled and he had tachycardia with gallop rhythm. Blood pressure was 74/40 mmHg. The lungs had decreased sounds bilaterally and the abdomen was flaccid and distended, painful to deep palpation, with no visceromegalies. Pulses were palpable, with poor peripheral perfusion and cyanosis.</p>
      <p>Biochemical assessment on the first day of hospitalization were: TB of 3.19 mg/dL; DB of 0.18 mg/dL; IB of 2,51 mg/dL; CK of 537 U/L; CKmb of 139 U/L; Hemoglobin (Hb) of 12 g; Hematocrit (Htc) of 32%; leukocytes of 15,600; lymphocytes of 30,000; platelets of 161,000/mm&#179;; TGO of 860 U/l; TGP of 421 U/L; PT of 25; APTT of 53.</p>
      <p>ECG showed junctional tachycardia, low-voltage complexes in classical leads and diffuse ventricular repolarization alterations.</p>
      <p>Chest x-ray showed significant cardiomegaly with left pleural effusion, and the Doppler echocardiogram showed LVDD of 58 mm, LVSD of 39 mm, IVS of 10 mm, LVPW of 10 mm. The EF was estimated at 23% LA of 41 mm; with diffuse hypocontractility. The valves were normal and there was mild mitral, aortic and tricuspid regurgitation. Severe pericardial effusion was also observed.</p>
      <p>The thick smear examination was positive forT. Cruzi.</p>
      <p>On the second day of hospitalization, he developed fever (38 C), with worsening of dyspnea and required pericardial effusion drainage. He underwent endotracheal intubation and developed septic and cardiogenic shock and ventricular arrhythmias. He was treated with benznidazole, Angiotensin Converting Enzyme (ACE) inhibitors, beta-blockers, vasoactive amines, ceftriaxone and amiodarone. He went into cardiac arrest, which was refractory to resuscitation maneuvers and died.</p>
      <p>The cause of death was septic shock and cardiogenic shock. The anatomopathological diagnosis of the necropsy study was described as heart failure due to acute Chagas myocarditis.</p>

        <title>Anatomopathological Heart Study</title>
        <p>At the macroscopic analysis, the heart was globular, pale-gray, flaccid and congested. The presence of pericardial fluid was above normal. The assessment of the cardiac chambers showed significant chamber dilation (with predominantly dilated left chambers, mainly the left ventricle). The myocardial study showed a pinkish-gray and shiny aspect; the parietal endocardium was smooth and shiny. The heart valves were normal. The epicardium was smooth and shiny. Following the trajectory of the coronary branches, a number of small nodules was observed, described in previous studies as "rosary beads" (moliniform epicarditis),<xref ref-type="bibr" rid="B7">7</xref> as shown in <xref ref-type="fig" rid="f1">Figure 1A</xref>.</p>

        <p>The microscopic study showed severe acute inflammation of the epicardium and myocardium, preserving the endocardium and normal coronary arteries.</p>
        <p>The myocardium showed intense and diffuse inflammatory activity, extensive cardiac fiber dissociation and massive destruction of the entire cardiac tissue (<xref ref-type="fig" rid="f1">Figure 1B</xref>).</p>
        <p>The histological sections, which were stained with hematoxylin-eosin and Giemsa, disclosed the parasite-containing amastigotes (<xref ref-type="fig" rid="f2">Figure 2</xref>).</p>

        <title>Comments</title>
        <p>The case described here reports on a young patient who acquired Chagas disease by oral transmission in a family micro-outbreak attributed to acai juice consumption. The case was considered as oral transmission due to the epidemiological evidence of the acai consumption and the fact that two relatives had concomitant symptoms without entry signs, considered classical of vector transmission.<xref ref-type="bibr" rid="B8">8</xref> In the Amazon region, the incidence of acute Chagas disease has increased since 1996, especially in the state of Para. Over a thousand acute cases were documented, caused by family micro-outbreaks, between the years 2000-2010, and most (71 %) of these cases were attributed to probable contamination of food with triatomid feces.<xref ref-type="bibr" rid="B9">9</xref> Acute cases of the disease occur throughout the year, but a higher incidence is observed in the second semester, especially in the months of September and October, which coincides with the time of acai harvest in the region.<xref ref-type="bibr" rid="B5">5</xref>
				</p>
        <p>The possible mechanisms of transmission of T. cruzi by oral route are attributed to the ingestion of food contaminated with feces of triatomids or urine of marsupials, as well as eating infected undercooked meat of wild animals. These conditions are facilitated by the fact that the parasite survives at temperatures similar to that of the vector, especially in tropical regions, with high humidity and environment temperature, favoring this form of transmission.<xref ref-type="bibr" rid="B10">10</xref> Acai is the most often consumed fruit, and its palm tree is found both in the vicinity of residences, as well as in forested areas. The combination of poor hygiene facilitates this route of infection, despite the presence of the classic vector responsible for the usual form of transmission.<xref ref-type="bibr" rid="B11">11</xref>
				</p>
        <p>Generally, oral T. cruzi infection has an incubation period ranging from 3 to 21 days and, when symptoms are apparent, it starts with fever, which is usually prolonged, lasting on average 19 days. This form of transmission is considered the most severe one, a fact observed in our patient.<xref ref-type="bibr" rid="B8">8</xref> Animal models of acute infection by T. cruzi, correlated the aggressiveness of clinical manifestations, mainly the cardiac abnormalities, with a high parasite load and to high inoculum.<xref ref-type="bibr" rid="B12">12</xref> Consistent with these studies, we considered the possibility that our patient ingested a large inoculum and, thus, had a more severe clinical evolution when compared to his relatives.</p>
        <p>Acute myocarditis by T. cruzi can be fatal in 3-5% of cases and is the leading cause of death after meningoencephalitis.<xref ref-type="bibr" rid="B13">13</xref> The junctional rhythm, which develops into ventricular fibrillation, significant ventricular dysfunction and pericardial effusion, manifested by the patient, is similar to the finding described in myocarditis by other etiologies. Marques et al.<xref ref-type="bibr" rid="B14">14</xref> reported the presence of arrhythmias in 26.5% of cases of oral infection by T. cruzi. Ventricular repolarization alterations and low-voltage complexes are common in acute Chagas myocarditis and are present in the initial electrocardiogram.<xref ref-type="bibr" rid="B15">15</xref>
				</p>
        <p>We report the finding of inflammatory nodules in the coronary trajectory, described in rare literature records as moliniform epicarditis or "rosary beads", which usually appear after 30 days of infection, coinciding with our patient's disease evolution stage and in accordance with reports in the literature regarding the stage of the disease.<xref ref-type="bibr" rid="B7">7</xref>
				</p>
        <p>An interesting aspect of the histological study was the massive destruction of cardiac cells, a fact that was not described in histological studies of vector transmission, which leads to the assumption about the severity of the oral transmission route.<xref ref-type="bibr" rid="B15">15</xref> We stress the finding of ruptured cysts of amastigotes. Torres et al.<xref ref-type="bibr" rid="B16">16</xref> had already described finding these cysts, containing intracellular forms of amastigotes of Leishmania, in 1948, present in the cardiac tissue during the acute phase of T. cruzi infection.</p>
        <p>In this case, the aggressiveness of the acute cardiac lesion involving the oral route of transmission is quite evident. We conclude that myocarditis can be extremely severe and fatal, and the implementation of public policies directed at food hygiene strategies must be implemented immediately in order to minimize the globalization of disease by this route of transmission.</p>
        <p>The association between acai consumption and Chagas disease have been increasing annually in Para, especially in cities where there is a significant number of acai sales points, which results in increased supply of the drink throughout the year and especially during the time of the harvest, making these points a "common source", that is, the single interrelated factor among the cases.</p>

        <p>Paciente masculino afrodescendente de 34 anos, natural e procedente da zona urbana do munic&#237;pio de Bragan&#231;a (PA), regi&#227;o considerada end&#234;mica para doen&#231;a de Chagas.</p>
        <p>Apresentava febre h&#225; 30 dias, associada a calafrios, cefaleia holocraniana e mialgia. Tr&#234;s semanas antes da interna&#231;&#227;o, notou urina de cor escura, com dispneia, que foi progressiva e evoluiu rapidamente aos m&#237;nimos esfor&#231;os, acompanhada de dor abdominal, n&#225;useas e v&#244;mitos, icter&#237;cia e edema de face. Na admiss&#227;o, encontrava-se muito dispneico, com extremidades frias, desidrata&#231;&#227;o leve e palidez cut&#226;neo-mucosa. N&#227;o era diab&#233;tico e nem dislipid&#234;mico. Negava hipertens&#227;o e tabagismo. Fazia uso regular de suco de a&#231;a&#237; durante as refei&#231;&#245;es. Morava em casa de alvenaria.</p>
        <p>Ao exame cl&#237;nico, as bulhas eram hipofon&#233;ticas e taquic&#225;rdicas com ritmo de galope. A press&#227;o arterial era de 74/40 mmHg. Os pulm&#245;es apresentavam murm&#250;rio vesicular diminu&#237;do bilateralmente e abdome fl&#225;cido, globoso, doloroso &#224; palpa&#231;&#227;o profunda, sem visceromegalias. Os pulsos eram palp&#225;veis, com m&#225; perfus&#227;o perif&#233;rica e cianose.</p>
        <p>Exames bioqu&#237;micos do primeiro dia de interna&#231;&#227;o foram: BT de 3,19 mg/dL; BD de 0,18 mg/dL; BID de 2,51 mg/dL; CK de 537 U/L; CKmb de 139 U/L; Hemoglobina (Hb) de 12 g; Hemat&#243;crito (Htc) de 32%; leuc&#243;citos de 15.600; linf&#243;citos de 30.000; plaquetas de 161.000/mm&#179;; TGO de 860 U/L; TGP de 421U/L; TP de 25; TTPA de 53.</p>
        <p>Eletrocardiograma indicava taquicardia juncional. Complexos de baixa voltagem nas deriva&#231;&#245;es cl&#225;ssicas e altera&#231;&#245;es difusas da repolariza&#231;&#227;o ventricular.</p>
        <p>O raio X de t&#243;rax mostrava cardiomegalia importante com derrame pleural &#224; esquerda, e o ecoDopplercardiograma apresentava DDVE de 58 mm, DSVE de 39 mm, SIV de 10 mm, PPVE de 10 mm. A FEJ foi estimada em 23%, AE de 41 mm; hipocontratilidade difusa. As valvas apresentavam-se normais e havia refluxo mitral, a&#243;rtico e tric&#250;spide de grau leve. Derrame peric&#225;rdico importante.</p>
        <p>A pesquisa de T. cruzi por exame de gota espessa foi positivo.</p>
        <p>No segundo dia de interna&#231;&#227;o, evolui febril (38&#186; C), com piora da dispneia, e derrame peric&#225;rdico, necessitando de drenagem. Foi submetido &#224; entuba&#231;&#227;o orotraqueal, evoluindo com choque s&#233;ptico e cardiog&#234;nico, e arritmias ventriculares. Tratado com benzonidazol, Inibidores da Enzima Conversora da Angiotensina (IECA), betabloqueadores, aminas vasoativas, cefitriaxona e amiodarona. Apresentou parada cardiorrespirat&#243;ria refrat&#225;ria a manobras de ressuscita&#231;&#227;o e foi a &#243;bito.</p>
        <p>A causa do &#243;bito foi choque s&#233;ptico e choque cardiog&#234;nico. O diagn&#243;stico anatomopatol&#243;gico do estudo de necr&#243;psia foi descrito como insufici&#234;ncia card&#237;aca por miocardite chag&#225;sica aguda.</p>

          <p>Na an&#225;lise macrosc&#243;pica, o cora&#231;&#227;o apresentou-se globoso p&#225;lido-acinzentado, fl&#225;cido e congesto. Contatou-se presen&#231;a de l&#237;quido peric&#225;rdico acima do normal. A explora&#231;&#227;o das cavidades card&#237;acas demonstrou dilata&#231;&#227;o importante das cavidades (predominando a dilata&#231;&#227;o das cavidades esquerdas, principalmente do ventr&#237;culo esquerdo). O estudo do mioc&#225;rdio demonstrou aspecto r&#243;seo-cinzento e brilhante; o endoc&#225;rdio parietal apresentou aspecto liso e brilhante. As v&#225;lvulas card&#237;acas eram normais. O epic&#225;rdico apresentou-se com aspecto liso e brilhante.           <title>Coment&#225;rios</title>
          <p>O caso aqui descrito trata de um paciente jovem, que adquiriu a doen&#231;a de Chagas por transmiss&#227;o oral em um surto de microepidemia familiar, atribu&#237;do &#224; ingest&#227;o de suco de a&#231;a&#237;. Foi considerado transmiss&#227;o oral pela evid&#234;ncia epidemiol&#243;gica do consumo de a&#231;a&#237; e pelo fato de dois familiares apresentarem sintomas concomitantes sem sinais de porta de entrada, considerados cl&#225;ssicos da transmiss&#227;o vetorial.<xref ref-type="bibr" rid="B8">8</xref> Na regi&#227;o Amaz&#244;nica, a incid&#234;ncia da doen&#231;a de Chagas aguda tem se tornado crescente desde 1996, especialmente no Estado do Par&#225;. Foram documentados mais de mil casos agudos por surtos de microepidemia familiar, entre os anos de 2000 a 2010, e grande parte (71%) desses casos foi atribu&#237;da &#224; prov&#225;vel contamina&#231;&#227;o alimentar com fezes de triatom&#237;neo.<xref ref-type="bibr" rid="B9">9</xref> Os casos agudos da doen&#231;a ocorrem durante todo o ano, mas &#233; no segundo semestre que observamos uma maior incid&#234;ncia, principalmente nos meses de setembro e outubro, coincidindo com a &#233;poca de maior safra de a&#231;a&#237; na regi&#227;o.<xref ref-type="bibr" rid="B5">5</xref>
					</p>
          <p>Os prov&#225;veis mecanismos de transmiss&#227;o do T. cruzi pela via oral s&#227;o atribu&#237;dos &#224; ingest&#227;o de alimentos contaminados com fezes de triatom&#237;neos ou com a urina de marsupiais, assim como a ingest&#227;o de carne mal cozida de animais silvestres infectados. Essas condi&#231;&#245;es s&#227;o facilitadas pelo fato de o protozo&#225;rio sobreviver em temperaturas semelhantes &#224;s do vetor, especialmente em regi&#245;es tropicais, com alta umidade e temperatura ambiental, favorecendo essa forma de transmiss&#227;o.<xref ref-type="bibr" rid="B10">10</xref> O a&#231;a&#237; &#233; o fruto consumido com maior frequ&#234;ncia, e sua palmeira est&#225; distribu&#237;da tanto nas vizinhan&#231;as das resid&#234;ncias, quanto em &#225;reas florestais. A associa&#231;&#227;o de m&#225;s condi&#231;&#245;es de higiene facilita essa via de infec&#231;&#227;o, apesar da presen&#231;a dos vetores cl&#225;ssicos respons&#225;veis pela transmiss&#227;o habitual.<xref ref-type="bibr" rid="B11">11</xref>

          <p>Geralmente, a infec&#231;&#227;o oral por T. cruzi apresenta um per&#237;odo de incuba&#231;&#227;o que varia de 3 a 21 dias e, quando com sintomas aparentes, inicia-se com quadro de febre que, normalmente, &#233; prolongada, durando em m&#233;dia 19 dias. Essa forma de transmiss&#227;o &#233; considerada de maior gravidade, fato observado em nosso paciente.<xref ref-type="bibr" rid="B8">8</xref> Modelos animais de infec&#231;&#227;o aguda por T. cruzi, correlacionaram a agressividade das manifesta&#231;&#245;es cl&#237;nicas, principalmente as altera&#231;&#245;es card&#237;acas, com a alta, devido &#224; alta carga parasit&#225;ria e a um alto inoculum.<xref ref-type="bibr" rid="B12">12</xref> Em analogia com estes estudos, aventamos a possibilidade do nosso paciente ter ingerido um grande inoculum e, dessa maneira, ter apresentado evolu&#231;&#227;o cl&#237;nica com maior gravidade em rela&#231;&#227;o a seus familiares.</p>
          <p>A miocardite aguda por T. cruzi pode ser fatal em 3 a 5% dos casos e representa a principal causa de &#243;bito ap&#243;s a meningoencefalite.<xref ref-type="bibr" rid="B13">13</xref> O ritmo juncional, evoluindo para fibrila&#231;&#227;o ventricular, disfun&#231;&#227;o ventricular importante e derrame peric&#225;rdico, manifestados pelo paciente, &#233; similar ao achado descrito nas miocardites por outras etiologias. Marques et al.<xref ref-type="bibr" rid="B14">14</xref> observaram a presen&#231;a de arritmias em 26,5% dos casos de infec&#231;&#227;o oral por T. cruzi. As altera&#231;&#245;es da repolariza&#231;&#227;o ventricular e os complexos de baixa voltagem s&#227;o frequentes na cardite chag&#225;sica aguda e est&#227;o presentes no eletrocardiograma inicial.<xref ref-type="bibr" rid="B15">15</xref>

          <p>Relatamos o encontro de n&#243;dulos inflamat&#243;rios no trajeto das coron&#225;rias descritos em raros registros da literatura como epicardite moliniforme ou em ros&#225;rio, que, em geral, surgem ap&#243;s 30 dias de infec&#231;&#227;o, coincidindo com a fase evolutiva em que se encontrava o paciente e de acordo com relatos da literatura quanto &#224; fase evolutiva.<xref ref-type="bibr" rid="B7">7</xref>

          <p>Um aspecto interessante do estudo histol&#243;gico foi a destrui&#231;&#227;o maci&#231;a de c&#233;lulas card&#237;acas, fato n&#227;o descrito em estudos histol&#243;gicos de transmiss&#227;o vetorial, o que leva ao racioc&#237;nio da gravidade da forma de transmiss&#227;o oral.<xref ref-type="bibr" rid="B15">15</xref> Real&#231;amos o encontro de cistos rotos de amastigotas. Torres et al.<xref ref-type="bibr" rid="B16">16</xref> j&#225; tinham descrito, em 1948, o encontro desses cistos contendo formas intracelulares de amastigotas do tipo leishmania, presentes no tecido card&#237;aco durante a fase aguda da infec&#231;&#227;o por T cruzi.</p>
          <p>Neste caso, &#233; bem evidente a agressividade da les&#227;o card&#237;aca aguda envolvendo a via oral. Conclu&#237;mos que a miocardite pode ser extremamente grave e fatal, al&#233;m de ser necess&#225;ria a implementa&#231;&#227;o urgente de pol&#237;ticas p&#250;blicas direcionadas para higiene de alimentos, para minimizar a globaliza&#231;&#227;o da doen&#231;a por essa forma de transmiss&#227;o.</p>
          <p>A rela&#231;&#227;o entre o a&#231;a&#237; e doen&#231;a de Chagas tem sido exuberante anualmente no Par&#225;, especialmente nas cidades onde existe um n&#250;mero expressivo de pontos de vendas de a&#231;a&#237;, que resulta em maior oferta da bebida durante todo ano e, principalmente, na safra, convertendo estes pontos em "fonte comum", ou seja, o &#250;nico fator inter-relacionado entre os casos.</p>
