  <body>
    <p>Epidemiology has shown the growing importance of prevention as a way of dealing with the cardiovascular risk epidemic. One of the fundamental changes in concepts in recent decades was the shift in the prevention paradigm to ever earlier stages, both in the disease development, affecting an increasing number of individuals that have yet to develop a pathological condition or those who are in the early and treatable stages, as of the life line itself, looking for the origin of chronic diseases in intrauterine life.<xref ref-type="bibr" rid="B1">1</xref> Thus, the concept of primordial prevention arose, that is, the prevention of the actual risk factors.</p>
    <p>Ideally, preventing the establishment of risk factors in a population would keep it disease-free. Although utopian, the scenario described is useful to guide prevention strategies. One of the main current objectives is to reach the greatest possible number of individuals before they develop a risk profile.</p>
    <p>In this sense, the revolution of wearables, or wearable devices, has great potential to contribute to primary prevention, by participating in the daily lives of individuals with an all-inclusiveness that is impossible for isolated interventions based solely on health services to achieve.</p>
    <p>To understand the wearables, one must recall how the computer ceased to be something detached to become something that is always close to our bodies. The computational device formats have evolved since the late 1970s, with the introduction of the concept of personal machines, or personal computers - the PCs. The term "personal" refers to a computing device attached to the daily lives of individuals, not corporate life. If they conquered the homes in the 1980s, in the middle of the 1990s, there were devices of several sizes and formats powered by batteries that allowed user mobility that was unheard of. While wireless networks (data or voice) expanded their reach, the telephone started to be carried along by people and even steal the attention that was previously given to the radio and watch.</p>
    <p>This infrastructure allowed a radical change to occur in the interface of the common user and computing, also resulting in the birth of a mobile application ecosystem in this new form of Internet access - the main one, for many individuals. This mobile connection status is "always-on" and hyperpersonal,<xref ref-type="bibr" rid="B2">2</xref> in addition to allowing new forms of communication in the metropolis environment. The logical evolution would be the intense use of mobile communication, as well as the miniaturization of components and devices, which are used attached to the body and not in pockets anymore.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
		</p>
    <p>Thus, wearables can be not only always close to the individual, but also collect, through sensors, the body's own information, such as counting steps or measuring heart rate. Because of the very small screens, or without displays, they are at the same time, the smartphone extensions and information entry points through voice.<xref ref-type="bibr" rid="B5">5</xref>
		</p>
    <p>At this initial phase, they take familiar forms, such as glasses (Google Glass) or watches (Apple Watch and Pebble, among others). In both cases, there is an adaptation of the original shapes of glasses and watches to process information. Watches would potentially have lower resistance, as they always provided some kind of extra information in addition to the hour. As for glasses, they were always perceived as intended for the amplification and correction of optical realities, which makes their transformation less acceptable socially at this historical moment.</p>
    <p>At the moment, what seems clear is that people have different points of information in their bodies, thus making the perception of connection more natural and "invisible".</p>
    <p>The worldwide sales of wearables should reach the figure of 200 million units by 2019.<xref ref-type="bibr" rid="B6">6</xref> Considering today's scenario, we must think of wearables not only as a wearable accessory, but as an information accessory that can help the public or by capturing or reporting data. These two possibilities and their combinations allow more than just uploading data at times when people are not in front of the screens, in addition to the new useful characteristic that information on the wrist, for instance, can carry.</p>
    <p>Simple data capture devices, such as straps with motion and heart rate sensors, can generate a huge volume of information, which allows not only individual patient monitoring, but also the planning of collective prevention strategies. This type of Big Data can be useful, for instance, to evaluate ecological interventions, such as changing the urban layout, to promote physical activity in a city's population.<xref ref-type="bibr" rid="B7">7</xref>
		</p>
    <p>Additionally, the monitoring itself can help change behaviors, as it furthers individuals' knowledge about themselves and encourages them to take the leading role when caring for their own health.<xref ref-type="bibr" rid="B8">8</xref>
		</p>
    <p>This is another example of the need for new paradigms in research on prevention. To be able to follow the evolution of concepts in prevention and the change in lifestyles at a time of mobile information, research also needs to be innovative, transdisciplinary and agile.</p>
    <p>The Brazilian Archives of Cardiology, in its role of stimulating innovative research in cardiology, proposes to carry out intervention studies in our country, so that technological innovations can in fact contribute to the improvement of health outcomes.</p>

      <p>A epidemiologia tem apontado para a import&#226;ncia crescente da preven&#231;&#227;o como forma de lidar com a epidemia de risco cardiovascular. Uma das mudan&#231;as fundamentais de conceitos nas &#250;ltimas d&#233;cadas foi o deslocamento do paradigma da preven&#231;&#227;o para etapas cada vez mais precoces, tanto do desenvolvimento da doen&#231;a, atingindo um n&#250;mero cada vez maior de pessoas que ainda n&#227;o desenvolveram uma condi&#231;&#227;o patol&#243;gica ou que se encontram em seus est&#225;gios iniciais e trat&#225;veis, como da pr&#243;pria linha da vida, buscando, na vida intrauterina, a origem de doen&#231;as cr&#244;nicas.<xref ref-type="bibr" rid="B1">1</xref> Assim, surgiu o conceito de preven&#231;&#227;o primordial, ou seja, a preven&#231;&#227;o dos fatores de risco propriamente ditos.</p>
      <p>Em uma situa&#231;&#227;o ideal, evitar a instala&#231;&#227;o dos fatores de risco em uma popula&#231;&#227;o a manteria livre de doen&#231;as. Apesar de ut&#243;pico, o cen&#225;rio descrito &#233; &#250;til para guiar estrat&#233;gias de preven&#231;&#227;o. Um dos principais objetivos atuais &#233; alcan&#231;ar o maior n&#250;mero poss&#237;vel de pessoas antes que elas desenvolvam um perfil de risco.</p>
      <p>Nesse sentido, a revolu&#231;&#227;o dos wearables, ou dispositivos vest&#237;veis, tem grande potencial para contribuir com a preven&#231;&#227;o primordial, por participar do dia a dia das pessoas em uma abrang&#234;ncia imposs&#237;vel para interven&#231;&#245;es isoladas baseadas somente nos servi&#231;os de sa&#250;de.</p>
      <p>Para entender os wearables, &#233; preciso recordar como o computador deixou de ser algo frio para estar sempre colado ao corpo. Os formatos de dispositivos computacionais evoluem desde o final dos anos 1970, com a introdu&#231;&#227;o do conceito de m&#225;quinas pessoais, ou personal computers - os PC. Esse termo "pessoal" designa um artefato de computa&#231;&#227;o dilu&#237;do na vida cotidiana das pessoas, n&#227;o corporativo. Se, nos anos 1980, eles conquistaram os lares, no meio da d&#233;cada de 1990, surgiram dispositivos de diversos tamanhos e formatos alimentados por baterias que permitiam uma mobilidade de uso at&#233; ent&#227;o in&#233;dita. Enquanto redes wireless (de dados ou voz) expandiram seus alcances, o telefone passou a andar com as pessoas e at&#233; mesmo roubar a aten&#231;&#227;o que antes era dada ao r&#225;dio e ao rel&#243;gio.</p>
      <p>Essa infraestrutura viabilizou uma mudan&#231;a radical na rela&#231;&#227;o do usu&#225;rio comum com a computa&#231;&#227;o, tamb&#233;m provocando o nascimento de um ecossistema de aplicativos m&#243;veis nessa nova forma de acesso &#224; internet - a principal, para muitos. Esta conex&#227;o em estado de mobilidade &#233; "always-on" e hiperpessoal,<xref ref-type="bibr" rid="B2">2</xref> al&#233;m de permitir novas formas de comunica&#231;&#227;o no ambiente das metr&#243;poles. A evolu&#231;&#227;o l&#243;gica seria o uso intenso da comunica&#231;&#227;o m&#243;vel, bem como a miniaturiza&#231;&#227;o de componentes e aparelhos usados anexados ao corpo, e n&#227;o mais nos bolsos.<xref ref-type="bibr" rid="B3">3</xref>,<xref ref-type="bibr" rid="B4">4</xref>
			</p>
      <p>Os wearables podem, ent&#227;o, n&#227;o s&#243; estar sempre presentes com o indiv&#237;duo como tamb&#233;m coletar, por meio de sensores, informa&#231;&#245;es do pr&#243;prio corpo, como medir passos ou batimentos card&#237;acos. Em virtude das telas muito pequenas, ou mesmo sem displays, s&#227;o, ao mesmo tempo, extens&#245;es do smartphone e pontos de entrada de informa&#231;&#227;o por meio da voz.<xref ref-type="bibr" rid="B5">5</xref>
			</p>
      <p>Nessa fase inicial, eles assumem formas conhecidas, como um &#243;culos (Google Glass) ou um rel&#243;gio (Apple Watch e Pebble, entre outros). Em ambos os casos, h&#225; uma adapta&#231;&#227;o dos formatos originais de &#243;culos e rel&#243;gios para processar informa&#231;&#245;es. Rel&#243;gios teriam potencialmente uma resist&#234;ncia menor, pois sempre tiveram algum tipo de informa&#231;&#227;o extra em rela&#231;&#227;o &#224;s horas. J&#225; os &#243;culos sempre foram percebidos para a amplifica&#231;&#227;o e corre&#231;&#227;o de realidades &#243;ticas, o que torna sua transforma&#231;&#227;o menos aceit&#225;vel socialmente nesse momento hist&#243;rico.</p>
      <p>No momento, o que parece claro &#233; que as pessoas t&#234;m diferentes pontos de informa&#231;&#227;o em seu corpo, tornando a percep&#231;&#227;o de conex&#227;o mais natural e "invis&#237;vel". A venda global de wearables deve chegar &#224; casa dos 200 milh&#245;es de unidades at&#233; 2019.<xref ref-type="bibr" rid="B6">6</xref> Diante do cen&#225;rio atual, &#233; preciso pensar nos wearables n&#227;o apenas como um acess&#243;rio de vestir, mas como um acess&#243;rio informacional, que pode auxiliar o p&#250;blico capturando dados ou informando. Essas duas possibilidades e suas combina&#231;&#245;es permitem colocar mais do que dados em momentos que as pessoas n&#227;o est&#227;o diante de telas, al&#233;m do novo car&#225;ter de utilidade que informa&#231;&#245;es no pulso, por exemplo, podem carregar.</p>
      <p>Aparelhos simples de captura de dados, como as pulseiras com sensores de movimenta&#231;&#227;o e frequ&#234;ncia card&#237;aca, podem gerar um volume de informa&#231;&#245;es enorme, que permite n&#227;o s&#243; o monitoramento do paciente individual, mas tamb&#233;m o planejamento de estrat&#233;gias coletivas de preven&#231;&#227;o. Esse tipo de Big Data pode ser &#250;til, por exemplo, para avaliar interven&#231;&#245;es ecol&#243;gicas, como a mudan&#231;a do tra&#231;ado urbano, para estimular a atividade f&#237;sica da popula&#231;&#227;o de uma cidade.<xref ref-type="bibr" rid="B7">7</xref>
			</p>
      <p>Al&#233;m disso, a pr&#243;pria monitoriza&#231;&#227;o pode ajudar na mudan&#231;a de comportamento, pois aprofunda o conhecimento do indiv&#237;duo sobre si mesmo e o estimula a assumir o protagonismo no cuidado com sua sa&#250;de.<xref ref-type="bibr" rid="B8">8</xref>
			</p>
      <p>Este &#233; mais um exemplo da necessidade de novos paradigmas na pesquisa em preven&#231;&#227;o. Para que seja poss&#237;vel acompanhar a evolu&#231;&#227;o dos conceitos em preven&#231;&#227;o e a pr&#243;pria mudan&#231;a dos estilos de vida em tempos de mobilidade da informa&#231;&#227;o, a pesquisa tamb&#233;m necessita ser inovadora, transdisciplinar e &#225;gil. Os Arquivos Brasileiros de Cardiologia, em seu papel de estimular a pesquisa inovadora em Cardiologia, prop&#245;em a realiza&#231;&#227;o de estudos de interven&#231;&#227;o em nosso meio, para que as inova&#231;&#245;es tecnol&#243;gicas possam, de fato, contribu&#237;rem para a melhora dos desfechos em sa&#250;de.</p>
