      Dear Editor
    </p>
    <p>We would like to congratulate the authors for the publication of their article "Subclinical Ventricular Dysfunction Detected by Speckle-Tracking Two Years after Use of Anthracycline", considering the great practical applicability of the theme. Cardiotoxicity secondary to chemotherapy drugs is a reality that imposes, on cardiologists and oncologists, the challenge of prevention and/or early detection of this complication, which has high morbidity and mortality<xref ref-type="bibr" rid="B01">1</xref>.</p>
    <p>In this regard, we read with interest the abovementioned article, which highlights the usefulness of speckle-tracking to attain an early diagnosis of subclinical ventricular dysfunction, although this finding does not directly imply in implementing treatment due to the lack of current scientific evidence, which makes studies in this area even more important.</p>
    <p>However, we would like to point out some aspects to add to the scientific information brought on by this article. In the Results section, the authors demonstrate that almost 80% of patients had also received cyclophosphamide as a chemotherapy drug, an alkylating agent that may be associated with ventricular dysfunction rates of up to 25% of the cases<xref ref-type="bibr" rid="B02">2</xref>,<xref ref-type="bibr" rid="B03">3</xref>, which cannot be minimized in the Discussion and Conclusion sections of this study. This same rationale can be applied to the more than 50% of patients receiving radiotherapy in the mediastinal region, regardless of the treated hemithorax, since the incidence of coronary heart disease in these patients is a side effect of significant incidence<xref ref-type="bibr" rid="B04">4</xref>.</p>
    <p>We also observed a high rate of hypertension in both groups of patients and controls and that the systolic and diastolic BP levels were higher in the latter than in the first group. We would like to know if there was any difference between the groups regarding the class of antihypertensive drug used, as data in the literature suggest some protective effect of ACE inhibitors and beta-blockers on the incidence of ventricular dysfunction and major clinical outcomes<xref ref-type="bibr" rid="B05">5</xref>.</p>

      <p>
        Dear authors
      </p>
      <p>We appreciate your interest and comments about our article.</p>
      <p>Cyclophosphamide-associated cardiotoxicity presents as a syndrome of heart failure, myocarditis, pericarditis, or their association and can lead to death. It has an acute onset, with signs and symptoms occurring within one to ten days after the first dose, lasting for approximately one week<xref ref-type="bibr" rid="B06">1</xref>. Delayed cardiotoxicity development (&gt; 3 weeks) is very rare in patients that survive the initial event<xref ref-type="bibr" rid="B07">2</xref>-<xref ref-type="bibr" rid="B09">4</xref>. In our sample, patients were assessed after a median of two years after the end of chemotherapy, making it unlikely that the obtained results can be attributed to the use of cyclophosphamide.</p>
      <p>The observation about the increased incidence of coronary artery disease (CAD), as a side effect of radiotherapy in patients with breast cancer is absolutely pertinent. This increase is proportional to the number of cardiovascular risk factors the patients have and the mean dose of radiation to the heart. Previous data demonstrated that CAD development after radiation therapy occurred after a longer period of follow-up: 82 months, on average<xref ref-type="bibr" rid="B10">5</xref>.</p>
      <p>Recently, Darby et al6 showed an increase of 16.3% (per Gray of radiation) in the rate of major coronary events in the first four years after radiation therapy in women with breast cancer6. This increase begins in the first five years after radiotherapy and persists for at least 20 years<xref ref-type="bibr" rid="B11">6</xref>. The development of new technologies in the field of radiation therapy has shown to be favorable to reduce this side effect<xref ref-type="bibr" rid="B12">7</xref>. In our study, however, none of the patients had coronary event during the study period.</p>
      <p>Although there was no difference in the percentage of participants considered hypertensive in both groups (p = 0.71), the controls had higher levels of systolic and diastolic BP at the time of evaluation. As stated in the Discussion section, we attribute this fact to greater adherence to antihypertensive therapy among patients who were undergoing more stringent medical follow-up in the post-chemotherapy period. The increase in blood pressure levels tends to compromise strain values<xref ref-type="bibr" rid="B13">8</xref>. However, despite these higher blood pressure levels in the control group, the strain values were more compromised in the group using doxorubicin (DOX), which reinforces the importance of this drug as an independent predictor of reduced &#949;LL and &#949;CC in our patients.</p>
      <p>There was no significant difference between the groups regarding the class of antihypertensive drug used: fourteen patients (34%) from the control group used angiotensin&#8209;converting enzyme inhibitors or Angiotensin II&#8209;receptor blockers vs. ten patients (25%) in the group treated with DOX (p = 0.367). One (2.4%) participant from the control group used a beta-blocker vs. five (12.5%) in the DOX group (p = 0.109).</p>
      <p>
        Sincerely,

A cardiotoxicidade secund&#225;ria aos quimioter&#225;picos &#233; uma realidade que imp&#245;e, a cardiologistas e oncologistas, o desafio de prevenir e/ou detectar precocemente essa complica&#231;&#227;o de elevada morbimortalidade<xref ref-type="bibr" rid="B01">1</xref>.</p>
      <p>Nesse aspecto, lemos com interesse o artigo em quest&#227;o, que destaca a utilidade do speckle tracking no diagn&#243;stico precoce de disfun&#231;&#227;o ventricular subcl&#237;nica, mesmo que esse achado n&#227;o implique diretamente em tratamento, por falta de evid&#234;ncia cient&#237;fica atual, o que valoriza ainda mais os estudos na &#225;rea.</p>
      <p>No entanto, gostar&#237;amos de pontuar alguns aspectos, a fim de enriquecer as informa&#231;&#245;es cient&#237;ficas contidas nesse artigo. Em resultados, os autores demonstram que quase 80% dos pacientes tinham recebido, tamb&#233;m, a ciclofosfamida como quimioter&#225;pico, um agente alquilante que pode estar associado a taxas de disfun&#231;&#227;o ventricular em at&#233; 25% dos casos<xref ref-type="bibr" rid="B02">2</xref>,<xref ref-type="bibr" rid="B03">3</xref>, o que n&#227;o pode ser minimizado na discuss&#227;o e na conclus&#227;o desse estudo. Esse mesmo racioc&#237;nio pode ser aplicado nos mais de 50% de pacientes que receberam radioterapia na regi&#227;o mediastinal, independente do hemit&#243;rax tratado, uma vez que a incid&#234;ncia de doen&#231;a coron&#225;ria nesses pacientes &#233; efeito colateral de relevante incid&#234;ncia<xref ref-type="bibr" rid="B04">4</xref>.</p>
      <p>Observamos, ainda, um elevado &#237;ndice de hipertensos tanto no grupo de pacientes como no controle, e que os n&#237;veis de press&#227;o arterial sist&#243;lica e diast&#243;lica foram maiores nesse &#250;ltimo do que naquele grupo. Gostar&#237;amos de saber dos autores se houve diferen&#231;a entre os grupos quanto &#224; classe de anti&#8209;hipertensivos utilizados, uma vez que alguns dados da literatura cogitam algum efeito protetor de inibidores da enzima de convers&#227;o da angiotensina e betabloqueadores na incid&#234;ncia de disfun&#231;&#227;o ventricular e desfechos cl&#237;nicos maiores<xref ref-type="bibr" rid="B05">5</xref>.</p>

          Prezados autores,
        </p>
        <p>Agradecemos o interesse e os coment&#225;rios sobre nosso artigo.</p>
        <p>A cardiotoxicidade relacionada ao uso da ciclofosfamida apresenta-se como uma s&#237;ndrome de insufici&#234;ncia card&#237;aca, miocardite, pericardite ou associa&#231;&#227;o destas, e pode levar &#224; morte. O in&#237;cio &#233; agudo, com sinais e sintomas ocorrendo dentro de 1 a 10 dias ap&#243;s a primeira dose, perdurando por cerca de 1 semana<xref ref-type="bibr" rid="B06">1</xref>. &#201; muito raro o desenvolvimento da cardiotoxicidade tardia (&gt; 3 semanas) nos pacientes que sobrevivem ao evento inicial<xref ref-type="bibr" rid="B07">2</xref>-<xref ref-type="bibr" rid="B09">4</xref>. Em nossa amostra, as pacientes foram avaliadas ap&#243;s uma mediana de 2 anos do final da quimioterapia, tornando pouco prov&#225;vel que os resultados obtidos pudessem ser atribu&#237;dos ao uso da ciclofosfamida.</p>
        <p>A observa&#231;&#227;o do aumento da incid&#234;ncia de Doen&#231;a Arterial Coron&#225;ria (DAC) como efeito colateral da radioterapia em pacientes com tumor de mama &#233; absolutamente pertinente. Esse aumento &#233; proporcional ao n&#250;mero de fatores de risco cardiovasculares apresentados pelos pacientes e &#224; dose m&#233;dia de radia&#231;&#227;o para o cora&#231;&#227;o. Dados anteriores demonstravam que o desenvolvimento de DAC ap&#243;s radioterapia ocorria ap&#243;s um per&#237;odo mais longo de acompanhamento: 82 meses, em m&#233;dia<xref ref-type="bibr" rid="B10">5</xref>. Recentemente, Darby e cols.<xref ref-type="bibr" rid="B11">6</xref> demonstraram um aumento de 16,3% (por Gray de radia&#231;&#227;o), na taxa de eventos coronarianos maiores nos primeiros 4 anos p&#243;s-radioterapia em mulheres com c&#226;ncer de mama<xref ref-type="bibr" rid="B11">6</xref>. Esse aumento come&#231;a nos primeiros 5 anos ap&#243;s a radioterapia e continua por, pelo menos, 20 anos<xref ref-type="bibr" rid="B11">6</xref>. O desenvolvimento de novas tecnologias no campo da radioterapia tem se mostrado favor&#225;vel &#224; redu&#231;&#227;o desse efeito colateral<xref ref-type="bibr" rid="B12">7</xref>. Em nosso estudo, entretanto, nenhuma paciente apresentou evento coronariano no per&#237;odo avaliado.</p>
        <p>Apesar de n&#227;o haver diferen&#231;a no porcentual de participantes considerados hipertensos nos dois grupos (p = 0,71), os controles apresentavam n&#237;veis mais elevados da press&#227;o arterial sist&#243;lica e diast&#243;lica no momento da avalia&#231;&#227;o. Conforme consta na discuss&#227;o, atribu&#237;mos isso &#224; maior ader&#234;ncia &#224; terapia anti&#8209;hipertensiva entre as pacientes que faziam acompanhamento m&#233;dico mais rigoroso no per&#237;odo p&#243;s-quimioterapia. A eleva&#231;&#227;o dos n&#237;veis press&#243;ricos tende a comprometer os valores do strain<xref ref-type="bibr" rid="B13">8</xref>. No entanto, apesar desses n&#237;veis press&#243;ricos mais elevados no grupo controle, os valores do strain mostraram&#8209;se mais comprometidos no grupo que usou a Doxorrubicina (DOX), o que refor&#231;a a import&#226;ncia desta droga como preditora independente da redu&#231;&#227;o do &#949;LL e do &#949;CC em nossos pacientes. N&#227;o houve diferen&#231;a significativa entre os grupos quanto &#224; classe dos anti&#8209;hipertensivos utilizados. Usavam Inibidor da Enzima Conversora de Angiotensina ou Bloqueador dos Canais de C&#225;lcio 14 (34%) participantes do grupo controle versus 10 pacientes (25%) no grupo tratado com DOX (p = 0,367). Uma (2,4%) participante no grupo controle usava um betabloqueador contra cinco (12,5%) no grupo DOX (p = 0,109).</p>
