A 67-year-old woman who underwent pacemaker implantation 4 days ago (A) consulted the emergency department with complaints of perception of muscle contractions on the right side of her chest. She did not have chest pain, dyspnea, or palpitations. Physical examination did not reveal any relevant findings. Electrocardiogram showed an absence of sensing or stimulation of the right atrium and right ventricle. Chest X-ray showed the displacement of the leads into the right atrium and the superior vena cava (B). Thus, she was referred to the cardiology department for lead repositioning.

A few months later, the patient presented with the same symptoms. A repeat chest X-ray showed a similar displacement of both the leads (C).

Paciente do sexo feminino, 67 anos de idade, com implante de marca-passo feito há 4 dias atrás (A), apresentou-se ao departamento de emergência relatando a percepção de contrações musculares no lado direito do tórax. A paciente não apresentava dor torácica, dispnéia ou palpitações e o exame físico não demonstrou nenhuma alteração. O eletrocardiograma mostrou ausência sensorial e na estimulação atrial e ventrícular direita. O raio X mostrou deslocamento dos eletrodos projetados no átrio direito e veia cava superior (B) a razão pela qual a paciente foi admitida no serviço de cardiologia, para que os cabos-eletrodos fossem reposicionados.

Poucos meses depois, a paciente apresentou os mesmos sintomas e a radiografia mostrou, novamente, deslocamento de ambos os eletrodos (C).

Radiografia de tórax após o implante de marca-passo (A); quatro dias depois, a radiografia mostrou deslocamento dos cabos-eletrodos no átrio direito e veia cava superior e ambos estavam enrolados ao redor do gerador de marca-passo (B). Alguns meses depois, a radiografia mostrou deslocamento dos eletrodos novamente (C).
