      Sir Editor,
    </p>
    <p>The recent report on "angiotensin-converting enzyme (ACE) genetic polymorphism." is very interesting. Albuquerque et al.<xref ref-type="bibr" rid="B01">1</xref> concluded that "the DD genotype was independently associated with worse echocardiographic outcome, while the DI genotype, with best echocardiographic profile." In fact, the polymorphism of ACE is different in different countries and the relationship to clinical disorder is widely mentioned<xref ref-type="bibr" rid="B02">2</xref>. The study of ACE polymorphism has proved to be useful in management of cardiac patients, especially for selection of proper cardiac drug<xref ref-type="bibr" rid="B03">3</xref>. However, the relationship to cardiac pathology is still controversial. Similar to the present study, the ventricular function abnormality determined by EKG was proposed for interrelationship with ACE polymorphism by Yu Jin et al<xref ref-type="bibr" rid="B04">4</xref>. Nevertheless, the small sample size in the published studies<xref ref-type="bibr" rid="B01">1</xref>, <xref ref-type="bibr" rid="B04">4</xref> is the main obstacle for conclusions. The effect of race in clinical relationship of ACE polymorphism requires a very large study in order to lead to conclusions<xref ref-type="bibr" rid="B02">2</xref>.</p>

      <p>Indeed, the sample size limitation is a universal bias in all of the genetic polymorphisms in heart failure and the ACE-genotype was no different. The metanalysis by Bai et al.<xref ref-type="bibr" rid="B05">1</xref>. with 17 different studies included 2.453 cases (i.e. heart failure patients) from all over the world with only 60.3% being white individuals. The author concluded that there was no association between ACE DD genotype and heart failure risk. The different ethnic background of these populations is one of the reasons for the lack of the association and when analyzed individually, some of these studies<xref ref-type="bibr" rid="B06">2</xref>,<xref ref-type="bibr" rid="B07">3</xref> did find a relation with ACE genotype and heart failure risks. Even more important is the relationship between the surrogate markers of disease progression - Echocardiogram parameters - and the genotypes since clinical outcomes will be very hard to prove with the sample size of these populations (which would have to be in thousands not hundreds). Andersson and Sylv&#233;n<xref ref-type="bibr" rid="B08">4</xref> for example did find a relationship between echocardiographic parameteres and the ACE genetic polymorphisms. In conclusion, it looks like all these papers combined do not have statistical power to answer in a definitive way the relationship between Heart Failure Prognosis and ACE Genetic Polymorphisms. But it looks like we are on the right track and we will have to proceed with a lot more patients.</p>

      <p>O relat&#243;rio recente sobre " o polimorfismo gen&#233;tico da enzima conversora de angiotensina ( ECA)" &#233; muito interessante. Albuquerque e cols.<xref ref-type="bibr" rid="B01">1</xref> conclu&#237;ram que "o gen&#243;tipo DD foi independentemente associado &#224; pior evolu&#231;&#227;o ecocardiogr&#225;fica, enquanto o gen&#243;tipo DI, com melhor perfil de ecocardiografia". Na verdade, o polimorfismo da ECA &#233; diferente em diferentes pa&#237;ses e a rela&#231;&#227;o com dist&#250;rbio cl&#237;nico &#233; amplamente mencionada<xref ref-type="bibr" rid="B02">2</xref>. O estudo dos polimorfismos da ECA provou ser &#250;til no tratamento de pacientes card&#237;acos, especialmente para a sele&#231;&#227;o de drogas card&#237;acas adequadas<xref ref-type="bibr" rid="B03">3</xref>. No entanto, a rela&#231;&#227;o com a patologia card&#237;aca ainda &#233; controversa. &#192; semelhan&#231;a do presente estudo, a anormalidade da fun&#231;&#227;o ventricular determinada pelo ECG foi proposta para inter-rela&#231;&#227;o com o polimorfismo da ECA por Yu Jin et al<xref ref-type="bibr" rid="B04">4</xref>. No entanto, o reduzido tamanho da amostra dos estudos publicados<xref ref-type="bibr" rid="B01">1</xref>,<xref ref-type="bibr" rid="B04">4</xref> &#233; o principal obst&#225;culo para uma conclus&#227;o . O efeito da ra&#231;a na rela&#231;&#227;o cl&#237;nica do polimorfismo da ECA exige um estudo muito mais amplo para conduzir a conclus&#245;es<xref ref-type="bibr" rid="B02">2</xref>.</p>

        <p>Realmente, a limita&#231;&#227;o do tamanho da amostra &#233; um vi&#233;s universal em todos os polimorfismos gen&#233;ticos na insufici&#234;ncia card&#237;aca, e no gen&#243;tipo ECA n&#227;o foi diferente. A metan&#225;lise por Bai e cols.<xref ref-type="bibr" rid="B05">1</xref> com 17 estudos diferentes incluiu 2.453 casos (i.e. pacientes com insufici&#234;ncia card&#237;aca ) de todo o mundo, com apenas 60,3 % de indiv&#237;duos de ra&#231;a branca. O autor concluiu que n&#227;o houve associa&#231;&#227;o entre o gen&#243;tipo da ECA DD e o risco de falha card&#237;aca. A origem &#233;tnica diferente dessas popula&#231;&#245;es &#233; uma das raz&#245;es para a falta de associa&#231;&#227;o e, quando analisados individualmente, alguns desses estudos<xref ref-type="bibr" rid="B06">2</xref>,<xref ref-type="bibr" rid="B07">3</xref> encontraram uma rela&#231;&#227;o entre o gen&#243;tipo da ECA e o risco de falha card&#237;aca. Ainda mais importante &#233; a rela&#231;&#227;o entre os marcadores substitutos de progress&#227;o da doen&#231;a - par&#226;metros do ecocardiograma - e os gen&#243;tipos j&#225; que os resultados cl&#237;nicos ser&#227;o muito dif&#237;ceis de provar com o tamanho da amostra dessas popula&#231;&#245;es (que teria que ser de milhares e n&#227;o centenas). Andersson e cols.<xref ref-type="bibr" rid="B08">4</xref>, por exemplo, encontraram uma rela&#231;&#227;o de par&#226;metros ecocardiogr&#225;ficos e os polimorfismos gen&#233;ticos da ECA. Em conclus&#227;o, parece que todos esses estudos combinados n&#227;o t&#234;m poder estat&#237;stico para responder de forma definitiva &#224; rela&#231;&#227;o entre o progn&#243;stico de insufici&#234;ncia card&#237;aca e os polimorfismos gen&#233;ticos da ECA. Contudo, parece que estamos no caminho certo e vamos ter que continuar nele com um n&#250;mero muito superior de pacientes.</p>
