    <p>
      Caro Editor,
    </p>
    <p>Hoje em dia a abordagem invasiva desempenha um papel crucial no tratamento de s&#237;ndromes coron&#225;rias agudas, de acordo com as recomenda&#231;&#245;es das diretrizes de pr&#225;tica cl&#237;nica. Essa tend&#234;ncia se refletiu em alguns artigos publicados na Revista Espanhola de Cardiologia, em 2011 e 2012. Com rela&#231;&#227;o &#224;s s&#237;ndromes coron&#225;rias agudas com eleva&#231;&#227;o do segmento ST, os programas regionais em angioplastia coron&#225;ria prim&#225;ria foram desenvolvidos em toda a Espanha. O tempo at&#233; a reperfus&#227;o, no entanto, continua a ser a principal desvantagem desses programas.</p>
    <p>Por exemplo, a experi&#234;ncia de Badalona mostra que em apenas 27% dos pacientes transferidos de outros hospitais para angioplastia prim&#225;ria, a art&#233;ria coron&#225;ria estava aberta dentro dos prazos recomendados pelas diretrizes, ou seja, menos de 2 horas a partir do primeiro contato m&#233;dico1.</p>
    <p>Portanto, a fibrin&#243;lise n&#227;o deve ser descartada como tratamento alternativo em alguns casos. Al&#233;m disso, os dados de resson&#226;ncia magn&#233;tica card&#237;aca n&#227;o evidenciaram diferen&#231;as significativas nos volumes e fun&#231;&#227;o ventricular esquerda entre pacientes tratados com angioplastia prim&#225;ria ou estrat&#233;gia f&#225;rmaco-invasiva (fibrin&#243;lise inicial, seguido de Angioplastia Coron&#225;ria de rotina 24 horas depois), em um &#250;nico registro hospitalar2. A previs&#227;o do progn&#243;stico &#233; um motivo de preocupa&#231;&#227;o. Os quatro escores de progn&#243;sticos mais conhecidos (TIMI, PAMI, Cadillac e GRACE) foram comparados em pacientes tratados tanto com a angioplastia prim&#225;ria quanto a de resgate3. Os quatro escores (particularmente TIMI, Cadillac e GRACE) mostraram excelente precis&#227;o para predizer a mortalidade em 30 dias e 1 ano; a previs&#227;o de reinfarto ou nova revasculariza&#231;&#227;o, no entanto, foram ruins com qualquer escore.</p>
    <p>O tratamento invasivo foi estendido a popula&#231;&#245;es anteriormente exclu&#237;das desse tratamento, como em pacientes idosos. Em um estudo retrospectivo em pacientes muito idosos (<underline>&gt;</underline> 85 anos) com s&#237;ndrome coronariana aguda sem eleva&#231;&#227;o do segmento ST, a abordagem invasiva reduziu a mortalidade e qualquer evento isqu&#234;mico em tr&#234;s anos, em compara&#231;&#227;o com uma popula&#231;&#227;o pareada tratada com uma estrat&#233;gia conservadora4. Apesar dos resultados favor&#225;veis da estrat&#233;gia invasiva em qualquer tipo de s&#237;ndrome coronariana aguda, a preven&#231;&#227;o secund&#225;ria n&#227;o deve ser menosprezada.</p>
    <p>Nesse sentido, a abertura de um laborat&#243;rio de cateterismo card&#237;aco, e o consequente aumento de procedimentos de interven&#231;&#227;o coron&#225;ria para infarto do mioc&#225;rdio, aumentou a mortalidade de 30 dias, mas n&#227;o entre 30 dias e dois anos ap&#243;s o ajuste para tratamento com inibidor de ECA, beta-bloqueadores e estatinas5.</p>

        Dear Editor,
      </p>
      <p>Nowadays, the invasive approach plays a crucial role in the management of acute coronary syndromes, according to the recommendations in clinical practice guidelines. This tendency was reflected in some papers published in Revista Espa&#241;ola de Cardiolog&#237;a in 2011 and 2012. Regarding ST-segment elevation acute coronary syndromes, regional programs on primary coronary angioplasty have been developed across Spain. Time delay until reperfusion, however, remains the main drawback of these programs. For instance, Badalona's experience shows that in only 27% of the patients transferred from other hospitals for primary angioplasty the coronary artery was opened within the time limits recommended in the guidelines, i.e. in less than 2 hours from the first medical contact1. Therefore, fibrinolysis should not be ruled out as an alternative treatment in some cases. Furthermore, data from cardiac magnetic resonance imaging did not evidence significant differences in left ventricular volumes and function between patients treated with primary angioplasty or pharmaco-invasive strategy (initial fibrinolysis followed by routine coronary angioplasty 24 hours later) in a single hospital registry2. Prediction of prognosis is a matter of concern. The 4 most known prognostic scores (TIMI, PAMI, CADILLAC and GRACE) were compared in patients managed with either primary or rescue coronary angioplasty3. All 4 scores (particularly TIMI, CADILLAC and GRACE) had an excellent accuracy to predict mortality at 30 days and 1 year; prediction of reinfarction or new revascularization, however, was very poor with any score.</p>
      <p>The invasive management has been extended to populations previously excluded from this treatment, such as elderly patients. In a retrospective study on very old patients (<underline>&gt;</underline> 85 years) with non-ST-segment elevation acute coronary syndrome, the invasive approach reduced mortality and any ischemic event at 3 years compared with a matched population managed with a conservative strategy4. Despite the favorable results of the invasive strategy in any type of acute coronary syndrome, secondary prevention should not be overlooked. In this sense, the opening of a cardiac catheterization laboratory and the subsequent increase of coronary intervention procedures for myocardial infarction, improved mortality at 30 days but not between 30 days and 2 years after adjusting for ACE inhibitor, beta-blocker and statin treatment5.</p>
