    <p>As v&#225;lvulas card&#237;acas biol&#243;gicas s&#227;o cada vez mais utilizadas na pr&#225;tica cl&#237;nica atual. Por outro lado, sua principal limita&#231;&#227;o &#233; o aumento do risco de reopera&#231;&#227;o tardia em virtude da degenera&#231;&#227;o estrutural, o que aumenta progressivamente com o tempo1,2. Considerando que mais pacientes jovens s&#227;o tratados com v&#225;lvulas biol&#243;gicas, torna-se inevit&#225;vel um n&#250;mero crescente de biopr&#243;teses degeneradas que ir&#227;o necessitar de novas trocas valvares2,3. No entanto, reinterven&#231;&#245;es s&#227;o consideradas de alto risco pelo agrupamento de fatores adversos nesse grupo de pacientes4,5.</p>
    <p>Apesar de ser considerada uma indica&#231;&#227;o "fora da bula" e ainda n&#227;o constar em diretrizes, o implante valvar card&#237;aco transcateter do tipo valve-in-valve emerge como alternativa menos invasiva. Essa experi&#234;ncia, entretanto, ainda &#233; limitada a poucos casos e s&#233;ries publicadas6-9.</p>
    <p>Relatamos o caso de um paciente com duas substitui&#231;&#245;es cir&#250;rgicas pr&#233;vias da v&#225;lvula mitral, apresentando-se com nova degenera&#231;&#227;o da biopr&#243;tese, insufici&#234;ncia card&#237;aca grave e alto risco cir&#250;gico, sendo submetido ao implante transcateter com t&#233;cnica valve-in-valve com dispositivo Inovare (Braile Biom&#233;dica, S&#227;o Paulo, Brasil).</p>
    <p>
      Relato do caso
    </p>
    <p>Homem de 74 anos, branco, foi admitido com dispneia progressiva, fadiga e tosse seca h&#225; tr&#234;s dias. Negava dor tor&#225;cica, febre e calafrios. A hist&#243;ria m&#233;dica inclu&#237;a diagn&#243;sticos de prolapso da v&#225;lvula mitral, hiperplasia prost&#225;tica benigna e insufici&#234;ncia renal cr&#244;nica (IRC) n&#227;o dial&#237;tica. H&#225; 13 anos apresentou endocardite infecciosa da v&#225;lvula mitral que necessitou de tratamento cir&#250;rgico com substitui&#231;&#227;o por biopr&#243;tese (St. Jude n&#186; 31). Depois de sete anos apresentou ruptura do folheto p&#243;stero-lateral da biopr&#243;tese necessitando de nova substitui&#231;&#227;o valvar sendo trocada por outra biopr&#243;tese (St. Jude n&#186; 31).</p>
    <p>Ao exame f&#237;sico apresentava regular estado geral, dispneico e afebril. Frequ&#234;ncia card&#237;aca de 105 bpm e frequ&#234;ncia respirat&#243;ria de 24 mrpm. Ausculta card&#237;aca evidenciou sopro holossist&#243;lico de 5+/6+ em foco mitral com irradia&#231;&#227;o para regi&#227;o axilar esquerda. Ausculta pulmonar com estertores crepitantes em ambas as bases pulmonares. Abd&#244;men sem altera&#231;&#245;es. Extremidades com boa perfus&#227;o perif&#233;rica. Eletrocardiograma apresentava taquicardia sinusal com sobrecarga ventricular e atrial esquerda. Exames laboratoriais demonstraram infec&#231;&#227;o do trato urin&#225;rio com agravo da fun&#231;&#227;o renal. Ecodopplercardiograma transtor&#225;cico e transesof&#225;gico demonstraram regurgita&#231;&#227;o mitral severa devido &#224; ruptura do folheto p&#243;stero-lateral da biopr&#243;tese e aus&#234;ncia de vegeta&#231;&#227;o com fra&#231;&#227;o de eje&#231;&#227;o (FE) de 65%.</p>
    <p>O paciente evoluiu com sepse grave de foco urin&#225;rio e piora da insufici&#234;ncia card&#237;aca (IC) necessitando de interna&#231;&#227;o em unidade de terapia intensiva (UTI). Recebeu alta da UTI ap&#243;s cinco dias com melhora da IC e da sepse, por&#233;m evoluindo com IRC agudizada. Creatinina s&#233;rica basal que era de 2,5 mg/dl passou para 4,2 mg/dl com depura&#231;&#227;o da creatinina end&#243;gena (DCE) calculada de 17 ml/min. O paciente foi avaliado por uma equipe multidisciplinar sendo considerado de alto risco cir&#250;rgico para nova substitui&#231;&#227;o da v&#225;lvula mitral com Euroscore log&#237;stico = 17,8% e STS escore = 18,9% para mortalidade. Considerando esse quadro, optou-se pela implante transapical de v&#225;lvula biol&#243;gica transcateter expans&#237;vel com bal&#227;o Inovare (Braile Biom&#233;dica, S&#227;o Paulo, Brasil) com m&#233;todo valve-in-valve.</p>
    <p>O procedimento foi realizado em sala cir&#250;rgica h&#237;brida, sob anestesia geral, sem necessidade de circula&#231;&#227;o extracorp&#243;rea. Por meio de minitoracotomia lateral esquerda no 5&#186; espa&#231;o intercostal foi exposto o &#225;pice card&#237;aco. Confeccionada sutura em bolsa com fios de prolene 4.0 com refor&#231;o de peric&#225;rdio bovino para controlar a hemorragia e implantado fio de marca-passo epic&#225;rdico. Puncionado o &#225;pice e avan&#231;ado fio-guia 0,35" com aux&#237;lio de cateter JR dentro da veia pulmonar superior direita. Administrada heparina e introduzido bainha 24 F atrav&#233;s do &#225;pice sobre o fio-guia stiff. Realizado medida do di&#226;metro interno da biopr&#243;tese mitral com ecocardiograma transesof&#225;gico intraoperat&#243;rio igual a 27 mm. Uma biopr&#243;tese bovina expans&#237;vel com bal&#227;o n&#186;28, Inovare (Braile Biom&#233;dica, S&#227;o Paulo, Brasil) foi introduzida pela bainha 24 F e posicionada sobre o &#226;nulo mitral.</p>
    <p>Realizado o posicionamento da pr&#243;tese transcateter da v&#225;lvula mitral sobre a biopr&#243;tese degenerada utilizando fluoroscopia e ecocardiografia simultaneamente. Com a utiliza&#231;&#227;o de rapid pacing foi expandido o bal&#227;o e liberada a v&#225;lvula (<xref ref-type="fig" rid="a18fig01">Figura 1</xref>). N&#227;o foi necess&#225;ria a utiliza&#231;&#227;o de contraste iodado. Fios, cateteres e introdutor foram retirados, e o &#225;pice fechado. Posicionado um dreno de t&#243;rax no espa&#231;o pleural esquerdo.</p>

    </p>
    <p>O paciente foi encaminhado para unidade de terapia p&#243;s-operat&#243;ria, acordado e extubado. N&#227;o apresentou d&#233;ficit neurol&#243;gico nem necessitou de transfus&#227;o sangu&#237;nea. Ecocardiograma transesof&#225;gico imediato p&#243;s-implante demonstrou biopr&#243;tese normofuncionante sem qualquer leak paravalvular ou valvular com gradiente diast&#243;lico de pico igual a 16,4 e m&#233;dio de 8 mmHg (<xref ref-type="fig" rid="a18fig02">Figura 2</xref>) e &#225;rea valvar efetiva de 2,6 cm2.</p>

    </p>
    <p>O paciente evoluiu bem e foi transferido para o quarto no terceiro dia. Apresentou pneumonia aspirativa, que foi tratada com antibioticoterapia de amplo espectro. Recebeu alta depois de 13 dias do procedimento sem sinais de IC e com melhora parcial da fun&#231;&#227;o renal.</p>
    <p>
      Discuss&#227;o
    </p>
    <p>A reopera&#231;&#227;o e substitui&#231;&#227;o da v&#225;lvula mitral pode estar associada &#224; maior mortalidade em pacientes idosos com comorbidades5. Diretriz recente recomenda substitui&#231;&#227;o da v&#225;lvula mitral em pacientes com regurgita&#231;&#227;o grave sintom&#225;ticos mesmo com FE ventricular esquerda preservada10. Deve-se salientar, entretanto, que na insufici&#234;ncia mitral, pelo fato de a eje&#231;&#227;o tamb&#233;m se dar para uma c&#226;mara de baixa press&#227;o como o &#225;trio esquerdo, pode haver um superestima&#231;&#227;o da fun&#231;&#227;o ventricular.</p>
    <p>Em virtude do risco cir&#250;rgico aumentado e da crescente evolu&#231;&#227;o do implante valvular a&#243;rtico transcateter os procedimento de valve-in-valve s&#227;o aplicados nesses pacientes de alto risco. No entanto, pela aus&#234;ncia de dados referentes &#224; durabilidade dos dispositivos transcateter e os &#243;timos resultados em longo prazo com a cirurgia valvar convencional, essa nova abordagem ainda demanda considera&#231;&#245;es especiais a respeito dos crit&#233;rios de sele&#231;&#227;o dos pacientes. Particularmente pacientes idosos com perfil de alto risco cir&#250;rgico e indica&#231;&#227;o de reopera&#231;&#227;o se beneficiariam dessa t&#233;cnica. O paciente relatado apresentava agravo da fun&#231;&#227;o renal ap&#243;s um epis&#243;dio de sepse urin&#225;ria e tinha indica&#231;&#227;o de reinterven&#231;&#227;o na v&#225;lvula mitral. O acesso transapical foi de f&#225;cil obten&#231;&#227;o e permitiu uma rota curta e reta at&#233; o plano mitral. N&#227;o ocorreram complica&#231;&#245;es hemorr&#225;gicas. Faz-se necess&#225;rio uma cuidadosa medida ecocardiogr&#225;fica do di&#226;metro interno da biopr&#243;tese para definir o tamanho do dispositivo a ser implantado. Fundamental, tamb&#233;m, &#233; a certifica&#231;&#227;o de que a regurgita&#231;&#227;o pela v&#225;lvula degenerada &#233; central ou transvalvar, pois essa t&#233;cnica n&#227;o se presta para a corre&#231;&#227;o de vazamentos perivalvares. O posicionamento e a libera&#231;&#227;o devem ser sempre auxiliados pelo ecocardiograma transesof&#225;gico. Nesse caso, por causa da inj&#250;ria renal, n&#227;o foi utilizado contraste radiol&#243;gico, o que &#233; recomend&#225;vel e poss&#237;vel.</p>
    <p>Embora esses achados sejam limitados por representarem a experi&#234;ncia de um &#250;nico paciente, s&#227;o corroborados por descri&#231;&#245;es similares da literatura. Conforme a diretriz atual europeia, essa t&#233;cnica &#233; de exce&#231;&#227;o e emerge como alternativa para o tratamento da disfun&#231;&#227;o de pr&#243;tese biol&#243;gica mitral em pacientes considerados inoper&#225;veis ou de muito alto risco11. Ressalta-se que essa indica&#231;&#227;o deve ser sempre realizada pela avalia&#231;&#227;o minuciosa de uma equipe multidisciplinar refor&#231;ando o conceito e necessidade do Heart Team. Estudos adicionais, com maior poder e desfechos cl&#237;nicos duros, s&#227;o necess&#225;rios para estabelecer o real papel desta abordagem.</p>

        Introduction
      </p>
      <p>Bioprosthetic heart valves are increasingly used in current clinical practice. Their major limitation, however, is the elevated risk of late reoperation due to structural degeneration, which increases progressively with time1,2. Considering that a larger number of young patients are treated with bioprosthetic valves, an increasing number of degenerated bioprostheses requiring replacement is expected2,3. However, valve reinterventions are considered to be highly risky because of the combination of adverse factors in that group of patients4,5.</p>
      <p>Despite being considered an 'off-label' indication and still not being part of guidelines, transcatheter valve-in-valve implantation emerges as a less invasive alternative. That experience, however, is still limited to a few cases and series published6-9.</p>
      <p>We report the case of a male patient with two previous surgical replacements of the mitral valve, who presented with new degeneration of the bioprosthesis, severe heart failure and high surgical risk, being submitted to transcatheter valve-in-valve implantation with the Inovare device (Braile Biom&#233;dica, S&#227;o Paulo, Brazil).</p>
      <p>
        Case report
      </p>
      <p>The patient is a 74-year-old white male, who was admitted complaining of progressive dyspnea, fatigue and dry cough for three days. He denied chest pain, fever and shivering. His medical history included the diagnoses of mitral valve prolapse, benign prostate hyperplasia, and chronic kidney failure (CKF) not requiring dialysis. He reported mitral valve infectious endocarditis 13 years before, which was surgically treated with bioprosthetic replacement (St. Jude n&#186; 31). After seven years, he had rupture of the posterolateral leaflet of the bioprosthesis, requiring new valve replacement with another bioprosthesis (St. Jude n&#186; 31).</p>
      <p>On physical examination, the patient was in regular general condition, dyspneic and had no fever. His heart rate was 105 bpm and his respiratory rate, 24 bpm. His cardiac auscultation evidenced a holosystolic murmur of 5+/6+ on the mitral area, radiating to the left axillary region. His pulmonary auscultation showed crepitant rales on both lung bases. His abdomen had no changes. His extremities showed good peripheral perfusion. His electrocardiogram evidenced sinus tachycardia with left ventricular and atrial overload. His laboratory tests showed urinary tract infection and impaired renal function. His Doppler transthoracic and transesophageal echocardiograms showed: severe mitral regurgitation due to rupture of the posterolateral leaflet of the bioprosthesis; no vegetation; and ejection fraction (EF) of 65%.</p>
      <p>The patient experienced severe sepsis from the urinary focus and aggravation of the heart failure (HF), requiring admission to the intensive care unit (ICU). He was discharged from the ICU after five days with improvement of the HF and sepsis, but experienced acute exacerbation of the CKF. Serum creatinine passed from 2.5 mg/dl (baseline) to 4.2 mg/dl, with endogenous creatinine clearance (ECC) calculated as 17 mL/min. The patient was assessed by a multidisciplinary team and was considered at high surgical risk for a new mitral valve replacement, with logistic EuroSCORE of 17.8% and STS score of 18.9% for mortality. The case was assessed by a multidisciplinary team that chose the transapical transcatheter balloon-expandable bioprosthesis implantation with the Inovare device (Braile Biom&#233;dica, S&#227;o Paulo, Brazil), according to the valve-in-valve method.</p>
      <p>The procedure was performed in a hybrid operating room, under general anesthesia, with no need for cardiopulmonary bypass. By use of left lateral minithoracotomy over the fifth intercostal space, the cardiac apex was exposed. Purse-string suture was performed with prolene 4.0 thread, bovine pericardial reinforcement was used to control hemorrhage, and an epicardial pacemaker lead was placed. The apex was punctured and a 0.35" guidewire was advanced with the aid of a JR catheter inside the right superior pulmonary vein. Heparin was administered and a 24-F sheath was introduced through the apex over the stiff guidewire. The internal diameter of the mitral bioprosthesis measured by use of intraoperative transesophageal echocardiography was 27 mm. Using the same size estimate for aortic stenosis (10%), an Inovare bovine balloon-expandable bioprosthesis n&#186; 28 (Braile Biom&#233;dica, S&#227;o Paulo, Brazil) was introduced through the 24-F sheath and placed over the mitral annulus.</p>
      <p>The mitral valve was positioned over the degenerated bioprosthesis by simultaneous use of fluoroscopy and echocardiography. By using rapid pacing, the balloon was expanded and the valve deployed (<xref ref-type="fig" rid="en_a18fig01-body2">Figure 1</xref>). No iodine contrast medium was necessary. Guidewires, catheters and introducer were removed, and the apex closed. A chest drain was placed into the left pleural space.</p>

      </p>
      <p>The patient was sent to the postoperative care unit, awakened and extubated. The patient neither had neurological deficit nor required blood transfusion. The transesophageal echocardiogram performed immediately after implantation showed the following: a normal functioning bioprosthesis with neither paravalvular nor valvular leak; peak and mean diastolic gradients of 16.4 and 8 mm Hg, respectively (<xref ref-type="fig" rid="en_a18fig02-body2">Figure 2</xref>); and effective valvular area of 2.6 cm2.</p>

      </p>
      <p>The patient had an uneventful postoperative course, being transferred to a room on the third postoperative day. He had aspiration pneumonia, which was treated with large spectrum antibiotics. The patient was discharged 13 days after the procedure with no signs of HF and partial improvement of his renal function.</p>

        Discussion
      </p>
      <p>Reoperation and mitral valve replacement might be associated with higher mortality in elderly patients with comorbidities5. A recent guideline recommends mitral valve replacement in symptomatic patients with severe regurgitation even with preserved left ventricular EF10. However, it is worth emphasizing that, in mitral regurgitation, because the ejection occurs to a low pressure chamber, such as the left atrium, the ventricular function can be overestimated.</p>
      <p>Because of the increased surgical risk and the growing evolution of transcatheter aortic valve implantation, valve-in-valve procedures are used in such high-risk patients. However, because of the lack of data on the durability of transcatheter devices and the excellent long-term results with conventional valve surgery, that new approach still requires special considerations on the criteria for selecting patients. Elderly patients at high surgical risk and with indication for reoperation would benefit from that technique. Our patient had impaired renal function after an episode of urinary sepsis and had indication for mitral valve reintervention. The transapical access was easily obtained, allowing a short and straight route until the mitral plane. No hemorrhagic complications occurred. Careful echocardiographic measurement of the internal diameter of the bioprosthesis is required to define the size of the device to be implanted. It is also fundamental to make sure that the regurgitation through the degenerated valve is either central or transvalvular, because that technique is not suitable for perivalvular leaks. Positioning and deployment should always be guided by transesophageal echocardiography. In the present case, because of the renal injury, no radiological contrast medium, which is recommended whenever possible, was used.</p>
      <p>Although our findings are limited because they correspond to the experience with one single patient, they are corroborated by similar descriptions in the literature. According to the current European guideline, that is an exception technique, emerging as an alternative for the treatment of mitral bioprosthesis dysfunction in patients considered inoperable or at very high risk11. It is worth emphasizing that such technique should always be indicated via the careful assessment of a multidisciplinary team, strengthening the concept and necessity of the Heart Team. Further studies, with greater power and hard clinical outcomes, are required to establish the actual role of that approach.</p>
