      Ao Editor,
    </p>
    <p>Lemos com grande interesse o artigo publicado na edi&#231;&#227;o anterior relatado por Clemente e cols.1sobre a an&#225;lise de alguns par&#226;metros de repolariza&#231;&#227;o ventricular em pacientes diab&#233;ticos (PD). No estudo, os autores d&#227;o novas informa&#231;&#245;es sobre um t&#243;pico relevante e atual. O risco de desenvolver arritmias ventriculares e, posteriormente, morte s&#250;bita card&#237;aca (MSC) em PD tem sido demonstrado em v&#225;rios estudos. A associa&#231;&#227;o referida, pelo menos, em parte, &#233; devida &#224; presen&#231;a de aterosclerose coron&#225;ria, doen&#231;a microvascular e neuropatia auton&#244;mica, que s&#227;o muito frequentes em pacientes com essa condi&#231;&#227;o. O substrato arritmog&#234;nico, nesses casos de doen&#231;as card&#237;acas isqu&#234;micas, inclui reentrada no mioc&#225;rdio devido &#224; forma&#231;&#227;o de cicatriz, hipertrofia compensat&#243;ria em mioc&#225;rdio n&#227;o infartado, remodelamento ventricular progressivo e anormalidades neuro-hormonais2.</p>
    <p>Esse estudo comparou as medidas de alguns preditores de arritmia ventricular entre PD e controles. A maioria dos preditores mostrou ser mais prolongada no primeiro. Esses resultados significam que esses pacientes t&#234;m uma maior predisposi&#231;&#227;o para desenvolver arritmias ventriculares malignas, devido a um aumento na dispers&#227;o espacial e transmural. Em um trabalho anterior, Cardoso e cols.3 observaram que os par&#226;metros de intervalo QT prolongado estavam associados com um risco aumentado de MSC em pacientes com Diabetes Mellitus. Ambos os estudos mostraram resultados semelhantes. No entanto, a caracter&#237;stica marcante do estudo de Clemente e cols.1 &#233; a avalia&#231;&#227;o de alguns par&#226;metros de repolariza&#231;&#227;o ventricular como Tpeak-Tend e jTpeak-jTend e suas dispers&#245;es. Embora anteriormente a Tpeak-Tend tenha mostrado ser um preditor de arritmias ventriculares em outra situa&#231;&#227;o4, essas medidas s&#227;o bastante novas na pr&#225;tica cl&#237;nica e mais estudos s&#227;o necess&#225;rios para avaliar sua real utilidade. Por esse motivo, a sua caracteriza&#231;&#227;o nesse estudo &#233; um avan&#231;o nesse campo. Se futuras investiga&#231;&#245;es definitivamente demonstrarem a import&#226;ncia desses marcadores em prever MSC em PD, os m&#233;dicos ter&#227;o uma nova ferramenta para estabelecer estrat&#233;gias e melhorar o progn&#243;stico desses pacientes.</p>
    <p>Uma limita&#231;&#227;o do artigo &#233; que os pacientes n&#227;o foram acompanhados e, assim, alguns dos resultados mostrados s&#227;o incertos. No entanto, o estudo de Clemente e cols.1 fornece novas informa&#231;&#245;es que aumentam o nosso conhecimento sobre um tema t&#227;o interessante. Outros estudos prospectivos com maior n&#250;mero de indiv&#237;duos devem ser realizados para identificar popula&#231;&#245;es em risco de MSC no futuro.</p>

        To the Editor,
      </p>
      <p>We have read with great interest the article published in the previous issue reported by Clemente et al1 on the analysis of some ventricular repolarization parameters in diabetic patients (DP). In the study, the authors give new information about a relevant and contemporary topic. The risk to develop ventricular arrhythmias and subsequently sudden cardiac death (SCD) in DP has been demonstrated in several studies. The mentioned association is at least partly due to the presence of coronary atherosclerosis, microvascular disease and autonomic neuropathy, which are very frequent in patients with this condition. The arrhythmogenic substrate in these cases of ischemic heart diseases includes myocardial re-entry due to scar formation, compensatory hypertrophy in non-infarcted myocardium, progressive ventricular remodeling and neurohormonal abnormalities2.</p>
      <p>This study compared the measures of some ventricular arrhythmia predictors between DP and controls. Most predictors were more prolonged in the first. These outcomes mean that these patients have a more predisposition to develop malignant ventricular arrhythmias due to an increase in spatial and transmural dispersion. In a previous work, Cardoso et al. observed that prolonged QT interval parameters were associated with an increased risk of SCD in patients with Diabetes Mellitus3. Both studies showed similar outcomes. However, the outstanding feature of the study by Clemente et al1 is the evaluation of some ventricular repolarization parameters such as Tpeak-Tend and jTpeak-jTend and its dispersions. Although previously the Tpeak-Tend showed to be a predictor of ventricular arrhythmias in other setting4, these measures are quite new in clinical practice and further investigations are necessary to assess their real usefulness. For that reason, their characterization in this study is an advance in this field. If further investigations definitely demonstrate the importance of these markers to predict SCD in DP, physicians will have a new tool to design strategies to improve the prognosis of these patients.</p>
      <p>A limitation of the article is that the patients were not followed and thus, some of the results shown are uncertain. However, the study by Clemente et al1 provides novel information that increases our knowledge on such interesting topic. Further prospective investigations with a larger number of subjects should be carried out to identify populations at risk of SCD in the future.</p>
