			<title>Introdução</title>
			<p>A amiloidose é caracterizada por deposição extracelular de proteínas insolúveis (depósito amilóide) em múltiplos órgãos e disfunção orgânica progressiva. Classifica-se pelo tipo de proteína que se deposita. Entre elas, a amiloidose por transtirretina (ATTR), que pode ocorrer por uma variante genética e pelo envelhecimento.<xref ref-type="bibr" rid="B1">1</xref> Cerca de 120 variantes genéticas são reconhecidamente a causa da ATTR entre elas, uma das mais comuns é a representada pela Val50Met. A apresentação fenotípica da amiloidose TTR pela mutação Val50Met é majoritariamente neurológica, chamada de polineuropatia amiloidótica familiar (PAF), porém o envolvimento cardíaco está cada vez mais sendo reconhecido.<xref ref-type="bibr" rid="B2">2</xref> Casos esporádicos com manifestação estritamente cardiológica, ocasionando hipertrofia da parede ventricular, disfunção diastólica e distúrbios de condução já são descritos na literatura.<xref ref-type="bibr" rid="B3">3</xref></p>
			<p>Neste artigo, relatamos um caso de ATTR pela mutação Val50Met com a expressão fenotípica de acometimento cardíaco, com taquicardia ventricular sustentada como apresentação isolada.</p>

			<title>Relato do Caso</title>
			<p>Brasileiro de 61 anos, hipertenso, filho de pais portugueses, queixando-se de dor torácica seguida de palpitações uma hora antes de chegar ao pronto socorro. Apresentava-se com sudorese intensa, frequência cardíaca elevada (196 batimentos por minuto) e pressão arterial em repouso de 130/80 mmHg. O eletrocardiograma revelou taquicardia ventricular sustentada ( <xref ref-type="fig" rid="f01">Figura 1</xref> ). Foi prontamente submetido à cardioversão elétrica com sucesso e dois dias após foi implantado um cardiodesfibrilador interno (CDI). Na investigação etiológica, foram realizados vários exames descritos a seguir: marcadores de necrose miocárdica mostraram-se elevados, em curva ascendente (pico de CK-MB: 31,3 ug/L e troponina-US: 19.400 ng/ml); cineangiocoronariografia revelou lesão de 30% em artéria circunflexa e área luminal de 8,6 mm2 (ultrassom intracoronariano); ecocardiograma mostrou fração de ejeção de 66%, aumento de espessura de paredes (septo 19 mm e parede posterior 15 mm), aumento da espessura do ventrículo direito (VD) e septo interatrial, ventrículo esquerdo (VE) com 52/33 mm, aumento atrial moderado (volume atrial esquerdo indexado de 45 ml/m2), disfunção diastólica grave (grau III) e pressão sistólica de artéria pulmonar (PSAP) de 45mmHg. Apesar da fração de ejeção normal, o strain longitudinal global evidenciou comprometimento da função sistólica do VE de −9 (valor de referência: −18), poupando a porção apical.</p>

			<p>A ressonância magnética mostrou a maior espessura na parede média inferosseptal do ventrículo esquerdo (22 mm), realce tardio difuso com gadolínio de padrão não isquêmico, circunferencial e edema miocárdico. ( <xref ref-type="fig" rid="f02">Figura 2</xref> ) A pesquisa de cadeias leves, relação kappa/lambda, imunofixação sérica e urinária foram negativas e a cintilografia com pirofosfato demonstrou intensa captação do pirofosfato marcado com tecnécio simétrica nas paredes do VE e de intensidade leve no VD, com contagem do hemitórax esquerdo sobre o hemitórax contralateral de 2,3 (estudo positivo &gt; 1,5). O teste genético evidenciou uma mutação no gene da transtirretina p.Val50Met.</p>

			<p>O paciente evoluiu com perda de peso, síndrome do túnel do carpo bilateral e diminuição discreta de força distal de membros inferiores, sintomas compatíveis com comprometimento do sistema nervoso periférico e do sistema nervoso autônomo. Iniciou tratamento com tafamidis em novembro de 2019. Um ano após, apresentou sintoma de dispneia aos esforços, sendo observada ao exame físico, a presença de hepatomegalia e turgência jugular. Foi medicado com dose baixa de diurético por três dias e foi aumentada a frequência cardíaca do seu cardiodesfibrilador implantável para 80 bpm, com resolução dos sintomas. Na evolução apresentou uma terapia apropriada do CDI por taquicardia ventricular.</p>
			<p>Evoluiu assintomático, referindo apenas lentificação dos seus passos ao subir ladeiras íngremes. Repetiu o ecocardiograma um ano e meio após o início do uso do tafamidis que evidenciou uma fração de ejeção de 56%, espessura septal de 18 mm e de parede posterior de 16 mm, aumento da espessura de VD e septo interatrial, VE 51/36 mm, volume atrial moderado (volume atrial esquerdo indexado de 42ml/m2), disfunção diastólica grau II, PSAP de 23 mmHg, strain longitudinal global de –7,7.</p>

			<title>Discussão</title>
			<p>Taquiarritmia ventricular em paciente idoso com elevação em curva dos níveis de troponina é altamente indicativo de quadros de síndrome coronariana aguda.<xref ref-type="bibr" rid="B4">4</xref> No presente caso, esta foi a primeira hipótese diagnóstica considerada, no entanto, a cinecoronariografia realizada afastou esta possibilidade. O ecocardiograma ao demonstrar a presença de aumento de espessura importante tanto em septo como em parede lateral, nos desvia do diagnóstico de quadros inflamatórios pela magnitude da hipertrofia e nos leva ao diagnóstico de um grupo de doenças que se apresentam com aumento de espessura de parede, a cardiomiopatia hipertrófica, a mais prevalente após a hipertensiva, a doença de Fabry e a amiloidose cardíaca.<xref ref-type="bibr" rid="B5">5</xref> O aumento de espessura de parede associado aos outros achados encontrados no ecocardiograma como disfunção diastólica grave, apical sparing e redução significativa do strain longitudinal, típicos de amiloidose, nos aponta na direção deste diagnóstico. A ressonância de coração, além de confirmar estes achados, acrescenta mais informações características desta doença, o realce tardio circunferencial e edema miocárdico. O achado de edema miocárdico tem sido relacionado à doença aguda, é mais comum na amiloidose de cadeias leves e está associado à ocorrência de complicações graves.<xref ref-type="bibr" rid="B6">6</xref> No presente caso, a ocorrência de taquicardia ventricular pode ser relacionada a este substrato estrutural.</p>
			<p>O implante de CDI nos pacientes com amiloidose tem indicações conflitantes nos estudos realizados antes do início da terapêutica específica, modificadora de sobrevida. O caráter progressivo da doença, com perda evolutiva da função ventricular pode justificar a não demonstração do benefício do implante. No entanto, agora com a introdução do tratamento específico, principalmente se o paciente se encontra numa fase mais precoce de evolução da doença, em classe funcional I/II da NYHA é provável que esta recomendação seja modificada em diretrizes, por ser possível a expectativa de sobrevida razoável.<xref ref-type="bibr" rid="B1">1</xref></p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusão</title>
			<p>A Val50Met é uma mutação patogênica caracterizada, principalmente, por ser clinicamente expressa por PAF. Cardiomiopatia, nefropatia e opacificação do humor vítreo podem estar presentes, mas a PAF é a apresentação principal. Quando a cardiomiopatia está presente, geralmente insuficiência cardíaca, mas sem arritmia ventricular, é a apresentação principal. No paciente descrito, a arritmia ventricular era a única manifestação e amiloidose nem mesmo foi considerada no diagnóstico diferencial, até a ressonância magnética. Este caso destaca o risco de morte cardíaca súbita em pacientes com ATTR hereditária. Embora a associação entre ATTR hereditária e morte súbita seja bem reconhecida, os estudos não mostraram nenhum benefício de sobrevida com o implante de desfibriladores implantáveis, talvez devido ao atraso no diagnóstico. A ATTR hereditária como causa da taquicardia ventricular sustentada deve então ser considerada.</p>

				<title>Introduction</title>
				<p>Amyloidosis is characterized by extracellular deposition of insoluble proteins (amyloid deposition) in multiple organs and progressive organ dysfunction. It is classified by the type of protein that is deposited, including transthyretin amyloidosis (ATTR), which can occur due to a genetic variant and aging.<xref ref-type="bibr" rid="B1">1</xref> Approximately 120 genetic variants have been recognized to cause ATTR; one of the most common is Val50Met. The phenotypic presentation of ATTR due to the Val50Met mutation is mostly neurological, known as familial amyloidotic polyneuropathy (FAP), but cardiac involvement is being increasingly recognized.<xref ref-type="bibr" rid="B2">2</xref> Sporadic cases with strictly cardiac manifestation, causing ventricular wall hypertrophy, dysfunction diastolic, and conduction disturbances have already been described in the literature.<xref ref-type="bibr" rid="B3">3</xref></p>
				<p>In this article, we report a case of ATTR due to the Val50Met mutation, with the phenotypic expression of cardiac involvement, with sustained ventricular tachycardia as an isolated presentation.</p>

				<title>Case Report</title>
				<p>A 61-year-old Brazilian man with hypertension, whose parents were Portuguese, complained of chest pain, followed by palpitations 1 hour before arriving at the emergency room. He presented with intense sweating, elevated heart rate (196 beats per minute) and resting blood pressure of 130/80 mmHg. Electrocardiogram revealed sustained ventricular tachycardia ( <xref ref-type="fig" rid="f01001">Figure 1</xref> ). He was promptly submitted to successful electrical cardioversion, and, 2 days later, an internal cardiac defibrillator (ICD) was implanted. During etiological investigation, several tests were performed, as described subsequently. Myocardial necrosis markers were shown to be elevated, in an ascending curve (peak CK-MB: 31.3 ug/L and ultra-sensitive troponin: 19,400 ng/ml); coronary cineangiography revealed a 30% lesion in the circumflex artery and a luminal area of 8.6 mm2(intracoronary ultrasound); echocardiogram showed ejection fraction of 66%, increased wall thickness (septum 19 mm and posterior wall 15 mm), increased thickness of the right ventricle (RV) and interatrial septum, left ventricle (LV) with 52/33 mm, moderate atrial enlargement (indexed left atrial volume of 45 ml/m2) , severe diastolic dysfunction (grade III), and pulmonary artery systolic pressure (PASP) of 45 mmHg. In spite of the normal ejection fraction, global longitudinal strain showed impairment of the LV systolic function of −9 (reference value: −18), sparing the apical portion.</p>

				<p>Magnetic resonance imaging showed the greatest thickness in the middle inferoseptal wall of the left ventricle (22 mm), diffuse late enhancement with non-ischemic, circumferential gadolinium pattern, and myocardial edema ( <xref ref-type="fig" rid="f02001">Figure 2</xref> ). The investigation of light chains, kappa:lambda ratio, serum and urinary immunofixation were negative, and pyrophosphate scintigraphy showed intense uptake of pyrophosphate labeled with symmetrical technetium in the LV walls and light intensity in the RV, with a left hemithorax to contralateral hemithorax count ratio of 2.3 (positive study &gt; 1.5). Genetic testing revealed a mutation in the p.Val50Met transthyretin gene.</p>

				<p>The patient evolved with weight loss, bilateral carpal tunnel syndrome, and a slight decrease in the distal strength of the lower limbs; these symptoms are compatible with impairment of the peripheral and autonomic nervous systems. He started treatment with tafamidis in November 2019. One year later, he showed symptoms of dyspnea upon exertion; upon physical examination, hepatomegaly and jugular swelling were observed. He was given a low dose of a diuretic for 3 days, and his implantable defibrillator heart rate was increased to 80 bpm, which resolved the symptoms. During evolution, he showed appropriate ICD therapy for ventricular tachycardia.</p>
				<p>The patient evolved to become asymptomatic, reporting only that his steps slowed when climbing steep slopes. The echocardiogram was repeated 1.5 years after starting the use of tafamidis, revealing ejection fraction of 56%, septal thickness of 18 mm, posterior wall of 16 mm, increased RV and interatrial septum thickness, VE 51/36 mm, moderate atrial volume (indexed left atrial volume 42 ml/m2), grade II diastolic dysfunction, PASP of 23 mmHg, and global longitudinal strain of –7.7.</p>

				<title>Discussion</title>
				<p>Ventricular tachyarrhythmia in an elderly patient with curved elevation of troponin levels is highly indicative of acute coronary syndrome.<xref ref-type="bibr" rid="B4">4</xref> In the present case, this was the first diagnostic hypothesis considered; however, coronary cineangiography ruled out this possibility. The echocardiogram, by demonstrating the presence of a significant increase in thickness in both the septum and the lateral wall, guides us away from the diagnosis of inflammatory conditions due to the magnitude of hypertrophy and leads us toward diagnosis of a group of diseases that present with increased wall thickness, such as hypertrophic cardiomyopathy, which is the most prevalent after hypertensive cardiomyopathy, Fabry disease, and cardiac amyloidosis.<xref ref-type="bibr" rid="B5">5</xref> Increased wall thickness, associated with other findings on echocardiography, such as severe diastolic dysfunction, apical sparing, and significantly reduced longitudinal strain, which are typical of amyloidosis, point us toward this diagnosis. Cardiac resonance, in addition to confirming these findings, added more information that is characteristic of this disease, namely, circumferential delayed enhancement and myocardial edema. The finding of myocardial edema has been related to the acute disease; it is more common in light chain amyloidosis, and it is associated with the occurrence of severe complications.<xref ref-type="bibr" rid="B6">6</xref> In this case, the occurrence of ventricular tachycardia can be related to this structural substrate.</p>
				<p>ICD implantation in patients with amyloidosis has conflicting indications in studies conducted before the start of specific therapy, which modifies survival. The progressive character of the disease, with progressive loss of ventricular function, may explain why the benefit of the implantation has not been demonstrated. However, currently, with the introduction of specific treatment, especially if the patient is at an earlier stage of disease evolution, in NYHA functional class I/II, it is likely that this recommendation will be modified in the guidelines, given that it is possible to expect reasonable survival.<xref ref-type="bibr" rid="B1">1</xref></p>

				<title>Conclusion</title>
				<p>Val50Met is a pathogenic mutation that is characterized, mainly, as being clinically expressed by FAP. Cardiomyopathy, nephropathy, and vitreous humor opacity may be present, but PAF is the main presentation. When cardiomyopathy is present, the main presentation is generally heart failure, but without ventricular arrhythmia. In the patient whose case has been described, ventricular arrhythmia was the only manifestation, and amyloidosis was not even considered in the differential diagnosis, until magnetic resonance was performed. This case highlights the risk of sudden cardiac death in patients with hereditary ATTR. Although the association between hereditary ATTR and sudden death has been well recognized, studies have not shown any survival benefit from defibrillator implantation, perhaps due to delayed diagnosis. Hereditary ATTR should thus be considered as a cause of sustained ventricular tachycardia.</p>
