	<body>
		<p>Pathogenic variants of the transtiretin gene (TTRv) result in the production of amyloidogenic fibrils that deposit in the extracellular space of many tissues, resulting in amyloidogenic multisystemic disease (ATTRv). Several tissues may be affected in different combinations, including the peripheral nerves and the cardiac system, which are usually the main targets, but also the kidneys, eyes, gastrointestinal system, central nervous system and musculoskeletal structures.<xref ref-type="bibr" rid="B1">1</xref></p>
		<p>ATTRv is a progressive disease<xref ref-type="bibr" rid="B2">2</xref> that results in death around 8 to 15 years after the onset of the disease, even more rapidly when the cardiac system is involved early.<xref ref-type="bibr" rid="B3">3</xref> However, the natural history of the disease has changed with the introduction of treatments that, by different mechanisms, decrease the production of amyloid fibrils.<xref ref-type="bibr" rid="B4">4</xref></p>
		<p>Such a complex disease demands coordinated multiprofessional interaction in order to provide the best care for patients. Considering medical specialties, well trained cardiologists, neurologists, ophthalmologists, gastroenterologists, nephrologists and geneticists should work in close collaboration both to treat patients who have already manifested the disease and to follow asymptomatic carriers, in order to detect the disease onset, when treatment is much more effective. For the best care, however, in addition to professionals who are trained in the field of amyloidosis, it is necessary that an institution is able to provide a complex set of prepared investigations, including neurophysiology (including small fiber evaluation), skin biopsy, other types of biopsy, magnetic resonance imagine, and scintigraphy.</p>
		<p>For these reasons, we believe these patients should be followed in tertiary reference centers that should interact closely with local physicians, in order to offer the best care at the best time to patients with this complex disease.</p>

		<body>
			<p>Variantes patogênicas do gene da transtiretina (TTRv) resultam na produção de fibrilas amiloidogênicas que se depositam no espaço extracelular de muitos tecidos, resultando em uma doença multissistêmica amiloidogênica (ATTRv). Podem ser afetados diversos tecidos em diferentes combinações, incluindo os nervos periféricos e o sistema cardíaco, que tipicamente são os principais alvos, mas também os rins, olhos, sistema gastrointestinal, sistema nervoso central e estruturas musculoesqueléticas.<xref ref-type="bibr" rid="B1">1</xref></p>
			<p>A ATTRv é uma doença progressiva<xref ref-type="bibr" rid="B2">2</xref> que resulta em óbito, aproximadamente, num período de 8 a 15 anos após o início da doença, ainda mais rapidamente quando o sistema cardíaco é envolvido precocemente.<xref ref-type="bibr" rid="B3">3</xref> Porém, a história natural da doença tem mudado com a introdução de tratamentos que, por meio de diferentes mecanismos, diminuem a produção de fibrilas amiloides.<xref ref-type="bibr" rid="B4">4</xref></p>
			<p>Uma doença tão complexa exige uma interação multiprofissional coordenada para fornecer o melhor atendimento aos pacientes. Considerando as especialidades médicas, cardiologistas, neurologistas, oftalmologistas, gastroenterologistas, nefrologistas e geneticistas bem treinados devem trabalhar em estreita colaboração tanto no tratamento de pacientes que já manifestaram a doença quanto no acompanhamento de portadores assintomáticos, com a finalidade de detectar o início da doença e quando o tratamento será muito mais eficaz. No entanto, para o melhor atendimento, além de profissionais treinados na área de amiloidose, é necessário que haja uma instituição capaz de fornecer um conjunto complexo de investigações preparadas, incluindo neurofisiologia (com avaliação de pequenas fibras), biópsia de pele, outras biópsias, imagem de ressonância magnética e cintilografia.</p>
			<p>Por esses motivos, nós acreditamos que esses pacientes devam ser acompanhados em centros de referência terciários que devem interagir de perto com os médicos locais, a fim de oferecer o melhor atendimento no melhor momento aos pacientes com essa complexa doença.</p>
